Ileostomy in extremely low birth weight and premature neonates.

AIM: The establishment of an ileostomy is a surgical option in the treatment of neonatal intestinal diseases, such as necrotizing enterocolitis (NEC) and meconial disease, in premature or extremely-low-birth-weight (ELBW) infants. METHODS: A prospective study was performed between July 2000-April 2011, with in exam all cases of acute abdomen in newborn premature babies. We perfomed a temporary ileostomy with a skin bridge and resection of the necrotic intestine. The temporary ileostomy was followed by anastomosis and the effect of possible confounding factors were assessed on the intestinal c

Skewed birth sex ratio and premature mortality in elephants.

Sex allocation theories predict equal offspring number of both sexes unless differential investment is required or some competition exists. Left undisturbed, elephants reproduce well and in approximately even numbers in the wild. We report an excess of males are born and substantial juvenile mortality occurs, perinatally, in captivity. Studbook data on captive births (CB, n=487) and premature deaths (PD, <5 years of age; n=164) in Asian and African elephants in Europe and North America were compared with data on Myanmar timber (Asian) elephants (CB, n=3070; PD, n=738). Growth in CB was found i

Respiratory distress syndrome and birth order in premature twins.

OBJECTIVE: To determine the effect of birth order on respiratory distress syndrome (RDS) in the outcome of twins in a large premature population managed in a modern neonatal intensive care unit. METHODS: An historical cohort study design was used to analyse the neonatal outcomes of 301 premature liveborn twin sibling pairs of between 23 and 31 weeks gestation from the Australia and New Zealand Neonatal Network 1995 database. RESULTS: Among the 56 twin sibling pairs who were discordant for RDS, the second twin was affected in 41 cases (odds ratio (OR) 2.7, 95% confidence interval (CI) 1.5 to

Neonatal hypothyroxinemia: effects of iodine intake and premature birth.

We have investigated the effects of iodine (I) intake on urinary I excretion in preterm (PT) babies up to 2 months after birth and its effect on serum T4, free T4 (FT4), T3, TSH, and thyroglobulin (Tg) levels compared to those in term (T) newborns. Very premature and very sick infants were in negative I balance for the first weeks after birth. Later, these same infants, as well as the other PT and T newborns, were in positive balance; 75- 80% of the ingested I was not accounted for in the urine. The urinary I levels of PT and T neonates cannot be equated to their I intakes. T4, FT4, and T3 lev

The influence of premature birth on laryngeal development for phonation.

INTRODUCTION: Dysphonia is a known consequence of premature birth, and is usually associated with endotracheal intubation in the neonatal period or surgical ligation of persistent patent ductus arteriosus. Recently, cases of dysphonia, in the absence of these causative factors, have been reported. OBJECTIVES: This review seeks to identify literature pertaining to those aspects of laryngeal development that may potentially be disrupted by premature birth. The purpose of the review is to determine whether there is any possible anatomical or physiological explanation for dysphonia to arose solel

Birth weight, prematurity and accuracy of gestational age.

OBJECTIVE: To investigate commonly used birth weight categories in relation to gestational age assignment by menstrual (LMP) or ultrasound dates. METHODS: A total of 34,249 cases were retrieved from the East Midlands Obstetric Database. Of these, 2281 (6.7%) delivered preterm (< 37 weeks) by ultrasound dating. The percentage of preterm and post-term cases was calculated for birth weight categories from < 1500 g to > 4499 g at 500 g intervals. The incidence of preterm delivery was estimated for birth weights from < 1500 g to 3400 g. RESULTS: For the 'low birth weight' cut-off of 2500 g, only

[A comment about premature births (author's transl)].

Prenatal case histories and labours of 908 mothers who delivered prematurely at the University of Heidelberg, Department of Gynaecology and Obstetrics in Mannheim, between the years 1966-1971 were compared with a freely selected group of 782 mothers who delivered healthy mature infants with birth weights over 2500 g. (Premature infants -- under 2501 g; Stillborn infants -- under 2501 g and a minimum 35 cm crown-heel length). A tendency to premature delivery was observed in primipara, multipara with at least 4 previous deliveries, unmarried mothers, and in women with a preceding miscarriage or

The Association of Birth Defects or Birth Asphyxia with the Risk of Mortality and Morbidity in Premature Infants.

Introduction: Preterm birth is a leading international health issue with morbidity and mortality risks. Premature infants frequently present with congenital anomalies or perinatal asphyxia, both of which contribute to prolonged hospitalization and greater need for respiratory support. This research investigates the effects of birth defects or asphyxia among premature infants on survival and recovery. Methods: This retrospective cohort study was carried out in an NICU in Mecca, Saudi Arabia. We examined 120 medical records of premature infants diagnosed with birth defects or birth asphyxia to d

Transcutaneous CO2 Monitoring in Extremely Low Birth Weight Premature Infants.

Extremely low birth weight (ELBW) premature infants are particularly susceptible to hypocarbia and hypercarbia, which are associated with brain and lung morbidities. Transcutaneous CO2 (TcCO2) monitoring allows for continuous non-invasive CO2 monitoring during invasive and non-invasive ventilation and is becoming more popular in the NICU. We aimed to evaluate the correlation and agreement between CO2 levels measured by a TcCO2 monitor and blood gas CO2 (bgCO2) among ELBW infants. This was a prospective observational multicenter study. All infants < 1000 g admitted to the participating NICUs du

[Using Cervical Pessary to Prevent Premature Birth - a Clinical Analysis].

OBJECTIVE: In the event of a cervical shortening < 25 mm before the 34th week of gestation, a cervical pessary can be inserted in accordance with the German AMWF guidelines. However, data concerning the benefit of this intervention in order to avoid premature birth are controversial. METHODS: Between the 20th and 32nd week of gestation, we inserted a cervical pessary in 83 patients and evaluated our data with regard to the extension of gestational age due to this intervention. We also reviewed the literature on this topic. We retrospectively analysed the data in a high-risk group (cervical le

Neighbourhood socio-economic status and spontaneous premature birth in Alberta.

OBJECTIVE: To evaluate a possible association between neighbourhood socio-economic status and spontaneous premature birth in Alberta births. METHODS: The study design was a retrospective cohort of all births in Alberta for the years 2001 and 2006. The primary outcome was spontaneous preterm birth at <37 weeks gestation. Neighbourhood socio-economic status was measured by the Pampalon Material Deprivation Index for each Statistics Canada census dissemination area. Births were linked to dissemination area using maternal postal codes. RESULTS: The analysis comprised 73,585 births, in which t

[New challenges for prevention of premature birth-associated respiratory distress].

Antenatal glucocorticoid decreases the risk of RDS and other serious developmental diseases in infants born preterm. Elective planned term deliveries before full 39 weeks of gestation increase the risk of RDS. While survival of extremely preterm (<28 weeks) infants has increased dramatically, the increased risk of bronchopulmonary dysplasia (BPD) and neuro-sensory disorders (CP, retinopathy of prematurity; ROP, cognitive disorders) remains. Each of these diseases represents a specific disorder in growth and differentiation. Further basic research is required to develop effective prevention of

Long term respiratory outcomes of very premature birth (<32 weeks).

Many very prematurely born infants develop bronchopulmonary dysplasia (BPD), remaining oxygen dependent for many months and requiring frequent rehospitalisations. Troublesome, recurrent respiratory symptoms requiring treatment and lung function abnormalities at follow-up are common. The most severely affected may remain symptomatic with evidence of airways obstruction even as adults. Data from adolescents and adults on the respiratory outcome of extreme prematurity, however, are usually from patients who have had 'classical' BPD with severe respiratory failure in the neonatal period. Nowadays,

Meconium obstruction in the very low birth weight premature infant.

Meconium obstruction of prematurity is a distinct clinical condition that occurs in very low birth weight infants, predisposing them to intestinal perforation and a prolonged hospitalization if not diagnosed and treated promptly. We report a series of 21 infants, including 2 detailed case reports, whose clinical course is indicative of meconium obstruction of prematurity. Specific risk factors are identified along with descriptions of clinical and radiologic findings, disease course, treatment, and outcome. Meconium obstruction of prematurity was more common in infants with a maternal history

[Secondary surfactant deficiencies in extremely low birth weight premature infants].

Primary deficiency of surfactant is responsible for the respiratory distress syndrome and concerns premature neonates born before 33 weeks of gestation. However, newborns may develop respiratory disorders related to a secondary deficiency or dysfunction of surfactant. We report the course of three extremely low birth weight premature infants who experienced clinical respiratory decompensation at two weeks and showed a marked improvement after exogenous natural surfactant administration.
