Cephalic vein dilatation at the elbow by preoperative supervised forearm exercise depends on adherence to therapy: Results from the randomized controlled single-blind PINCH trial.

BACKGROUND: Maturation failure may be as high as 30%-60% of arteriovenous fistulas (AVF). Forearm exercise postoperatively has shown potential in increasing blood vessel size in patients with end-stage renal disease and may promote maturation of AVFs. Little evidence exists on the impact of pre-operative forearm exercises on the increase of vascular diameters. Aim of this study was to evaluate the effect of supervised forearm exercises on vascular diameters and AVF maturation. METHODS: The PINCH trial is a single-blinded, randomized controlled trial on supervised forearm exercises compared to

Optimizing Physical Fitness Before Colorectal Cancer Surgery (CANOPTIPHYS): The Effect of Preoperative Exercise on Pre- and Postoperative Physical Fitness in Older people - A Randomized Controlled Trial.

INTRODUCTION/OBJECTIVE: Surgery-induced losses in physical fitness may have detrimental consequences for patients with low reserve capacity at start. Our objective was to evaluate the effect of preoperative exercise on physical fitness in older patients with low physical fitness scheduled for colorectal cancer surgery. METHODS: In this randomized, controlled trial, patients ≥65 years of age, who were scheduled for colorectal cancer surgery were included if they had a low maximal walking speed. Exercise for 2 to 3 weeks before surgery was home-based, high-intensity, and partly supervised by a

Preoperative diagnosis of coronary artery disease in patients with valvular heart disease using technetium-99m isonitrile tomographic imaging together with high-dose dipyridamole and handgrip exercise.

Forty-seven consecutive patients (mean age 61 +/- 8 years) referred for cardiac catheterization due to moderate to severe aortic (n = 30) or mitral (n = 17) valvular heart disease were examined by technetium-99m isonitrile tomography together with a high-dose dipyridamole infusion (0.7 mg/kg) and handgrip stress. Tomography did not identify coronary artery disease (CAD) in 3 of the 21 patients with angiographically proven disease (sensitivity 86%) and suggested false positive results in 5 of the 26 without the disease (specificity 81% and negative predictive accuracy 88%). No patient without a

Preoperative longitudinal left ventricular function by tissue Doppler echocardiography at rest and during exercise is valuable in timing of aortic valve surgery in male aortic regurgitation patients.

BACKGROUND: The aim of this study was to evaluate if left ventricular (LV) systolic function by tissue Doppler echocardiography at rest and during exercise preoperatively could predict postoperative LV function and thereby be useful in the timing of aortic valve surgery in patients with severe aortic regurgitation. METHODS: In 29 patients (median age, 59 years; interquartile range, 39-64 years), echocardiography, tissue Doppler echocardiography, and radionuclide ventriculography were performed preoperatively and postoperatively at rest and during supine bicycle exercise. RESULTS: Preoperativ

Effect of early supervised progressive resistance training compared to unsupervised home-based exercise after fast-track total hip replacement applied to patients with preoperative functional limitations. A single-blinded randomised controlled trial.

OBJECTIVE: To examine if 2 weekly sessions of supervised progressive resistance training (PRT) in combination with 5 weekly sessions of unsupervised home-based exercise is more effective than 7 weekly sessions of unsupervised home-based exercise in improving leg-extension power of the operated leg 10 weeks after total hip replacement (THR) in patients with lower pre-operative function. METHOD: A total of 73 patients scheduled for THR were randomised (1:1) to intervention group (IG, home based exercise 5 days/week and PRT 2 days/week) or control group (CG, home based exercise 7 days/week). Acceptability of prehabilitation for cancer surgery: a multi-perspective qualitative investigation of patient and 'clinician' experiences.

BACKGROUND: 'Prehabilitation' interventions aim to enhance individuals' physical fitness prior to cancer treatment, typically involve exercise training as a key component, and may continue to support physical activity, strength, and fitness during or after treatment. However, uptake of prehabilitation is variable. This study investigated how patients from diverse socio-economic status groups perceived an exemplar prehabilitation and recovery programme, aiming to understand factors impacting acceptability, engagement and referral. METHODS: This research was conducted in the context of the Preh

Prehabilitation for Vascular Surgery Patients: Challenges and Opportunities.

Global demand for major surgery is rising as a consequence of a growing ageing population, and clinically applicable approaches to perioperative risk reduction have never been more needed. Prehabilitation aims to optimise aerobic capacity and address modifiable risk factors before surgery to improve postoperative outcomes. Given the inherently high-risk nature of vascular surgery and the frequently high-acuity frail vascular surgical population, the potential to introduce an intervention into the surgical pathway that may reduce postoperative complications cannot be overlooked. The aim of this

International consensus on the content of prehabilitation in spine surgery: results of a nominal group technique.

BACKGROUND CONTEXT: Spine surgery, like all major surgeries, carries the risk of adverse events and delayed recovery. Prehabilitation programs may mitigate negative prognostic factors to reduce complications and promote faster recovery postoperatively following spine surgery. There is no international consensus or recommendations regarding prehabilitation components in spine surgery. PURPOSE: This study aims to establish international consensus on important modalities of prehabilitation before spine surgery for patients appropriate for prehabilitation using a modified nominal group technique

Prevalence of poor functional capacity in patients undergoing prehabilitation before major surgery: A systematic review and meta-analysis.

INTRODUCTION AND OBJECTIVES: Prehabilitation aims to enhance physiological reserve prior to major surgery; however, the proportion of patients with impaired functional capacity who actually receive these interventions remains unclear. Identifying this subgroup is crucial, as patients with reduced functional capacity are those most likely to benefit from prehabilitation. This systematic review and meta-analysis aimed to estimate the prevalence of poor functional capacity among adult patients undergoing prehabilitation before major surgery. Patient characteristics and preferences for a surgical prehabilitation program design: results from a pilot survey.

PURPOSE: Surgical prehabilitation aims to reduce a decline in the functional capacity thereby optimising health and fitness before surgery. One of the major barriers in successfully implementing a prehabilitation program in hospitals has been poor patient adherence. In our pilot survey on surgical patients, we sought to explore patient preferences regarding the program design, the barriers and enablers to patient participation in a multimodal prehabilitation program. METHODS: The survey was administered to patients undergoing major abdominal surgery in the preoperative period. The first two p

Supervised prehabilitation in patients scheduled for spinal surgery - a scoping review.

BACKGROUND CONTEXT: Prehabilitation before spinal surgery may enhance patients' ability to withstand physical and mental stress during the perioperative period. It has the potential to reduce complications, accelerate recovery, and deliver sustainable, patient-relevant improvements. However, high-quality evidence remains limited, and it is unclear which prehabilitation concepts, training protocols, and outcomes are most effective for different patient groups. PURPOSE: To evaluate the current state of research on preoperative interventions before spinal surgery, including the representation of

Predictors of exercise benefit after operative relief of left ventricular outflow obstruction by the myotomy-myectomy procedure in hypertrophic cardiomyopathy.

To determine predictors of exercise benefit in patients with hypertrophic cardiomyopathy after operative relief of left ventricular (LV) outflow tract obstruction, 30 patients underwent catheterization and exercise testing before and 6 months after operation, and hemodynamic measurements were obtained. The increase in maximal oxygen consumption (VO2max) during treadmill exercise testing was chosen as an index of exercise benefit. Univariate analysis showed a significant positive correlation of operative change in VO2max with preoperative LV end-diastolic and pulmonary arterial wedge pressures,

Efficacy of prehabilitation interventions on exercise capacity of patients undergoing major abdominal and cardiothoracic surgery: A systematic review and network meta-analysis of randomized controlled trials.

BACKGROUND: Prehabilitation has reported improvements in exercise capacity, as measured by the six-minute walking test (6MWT), in people undergoing major abdominal and cardiothoracic surgery, although there is no evidence about which component of prehabilitation is most effective for improving exercise capacity. METHODS: A network meta-analysis (NMA) was performed to determine the effects of different components of prehabilitation on exercise capacity in people undergoing major abdominal or cardiothoracic surgery. A literature search was performed from baseline to September 2025. Randomized c

Perioperative risk assessment - focus on functional capacity.

PURPOSE OF REVIEW: This review examines how functional capacity informs preoperative risk stratification, as well as strengths and limitations of options for estimating functional capacity. RECENT FINDINGS: Functional capacity (or cardiopulmonary fitness) overlaps with other important characteristics, including muscular strength, balance, and frailty. Poor functional capacity is associated with postoperative morbidity, especially noncardiovascular complications. Both patient interviews and exercise tests are used to assess functional capacity. The usual approach of an unstructured patient int

The maintenance and optimization of functional capacities before, during, and after treatment are major challenges for frailty persons as cancer's patients. It is now known that physical activity in prehabilitation plays a crucial role in limiting, among other things, post-operative complications. The benefits have already been demonstrated in various studies, including a decrease in hospitalization duration, an increase in cardiorespiratory endurance, improvement in quality of life, and better fatigue management. It is observed that patients who undergo prehabilitation are those who recover t
