HLA sensitization is associated with an increased risk of primary graft dysfunction after heart transplantation.

UNLABELLED: Primary graft dysfunction (PGD) is a leading cause of early morbidity and mortality following heart transplantation (HT). We sought to determine the association between pretransplant human leukocyte antigen (HLA) sensitization, as measured using the calculated panel reactive antibody (cPRA) value, and the risk of PGD. METHODS: Consecutive adult HT recipients (n = 596) from 1/2015 to 12/2019 at 2 US centers were included. Severity of PGD was based on the 2014 International Society for Heart and Lung Transplantation consensus statement. For each recipient, unacceptable HLA antigens

Plasma Levels of SERCA2a as a Noninvasive Biomarker of Primary Graft Dysfunction After Heart Transplantation.

BACKGROUND: Noninvasive detection of primary graft dysfunction (PGD) remains a major challenge. SERCA2a plays an important role in cardiac homeostasis and its dysregulation has been associated with ventricular dysfunction and rejection. This study aimed to determine the potential utility of plasma levels of SERCA2a as a biomarker of PGD. METHODS: One hundred thirty-five plasma samples were collected from adult recipients 2-6 hours before heart transplantation (HT). Plasma concentrations of SERCA2a were determined using a specific sandwich ELISA. Variables related to the recipient, the donor,

Perioperative statin use is associated with decreased incidence of primary graft dysfunction after lung transplantation.

BACKGROUND: Primary graft dysfunction (PGD) is a major cause of early morbidity and mortality after lung transplantation. Statins reduce the risk of chronic rejection after lung transplantation, but their effects on PGD are unknown. We hypothesized that perioperative statin therapy decreases the risk for PGD after lung transplantation. METHODS: We retrospectively reviewed records of all patients undergoing lung transplantation between January 1999 and December 2014 at the University of Virginia Health System. The primary outcome was PGD (grades 1-3). Secondary outcomes included grade 3 PGD, l

Predicted Heart Mass and Severe Primary Graft Dysfunction in Donation After Circulatory Death Heart Transplant.

BACKGROUND: Donation after circulatory death (DCD) has expanded the US heart donor pool yet carries a higher risk of severe primary graft dysfunction (PGD) for recipients compared to donation after brain death (DBD). This study addresses an important knowledge gap of exploring the role of heart size mismatch in contributing to the risk of severe PGD in DCD-HT. METHODS: All heart-only, adult transplants from the united network for organ sharing (UNOS) database were collected from 09/2023-06/2025. Univariable (UV) and multivariable (MV) analyses were used to identify associations between variou

Gene expression analysis of primary graft dysfunction in adult heart transplant recipients: A cohort study.

BACKGROUND: Cardiac transplantation remains the primary treatment for advanced heart failure, with primary graft dysfunction (PGD) being the leading cause of early mortality. While PGD's exact pathogenesis is unclear, ischemia/reperfusion injury is a key factor. Donor hearts undergo multiple stresses, such as brain death, hypothermia, and ischemia, contributing to PGD and reduced graft survival. Many centers are studying PGD's pathophysiology through epidemiological, biomarker, and transcriptome analyses. This study aims to evaluate, via transcriptomic analysis of myocardial biopsies, the gene

Impact of Pre-Heart Transplant Levosimendan Administration on Post-Transplant Vasoplegia and Primary Graft Dysfunction.

INTRODUCTION: Levosimendan, a calcium-sensitizing inotropic agent, is used in patients with advanced heart failure (HF) awaiting heart transplantation (HT). Its prolonged effects, due to an active metabolite, may influence post-transplant vasodilation, particularly when administered shortly before HT. However, its impact on post-HT complications such as vasoplegia and primary graft dysfunction (PGD) remains unclear. This study aimed to evaluate whether preoperative levosimendan affects these outcomes. METHODS: This retrospective, multicenter observational study included adult HT recipients fr

Body mass index and mortality following primary graft dysfunction: A Lung Transplant Outcomes Group study.

Higher body mass index (BMI) increases the risk of developing primary graft dysfunction (PGD) after lung transplantation; whether BMI is associated with decreased survival after PGD is unknown. We utilized the Lung Transplant Outcomes Group cohort of 1,538 subjects from 2011-2018. We evaluated the association between preoperative BMI and graft survival among subjects with severe PGD using Cox proportional hazards models with linear splines. Models were stratified by center and adjusted for sex, age, Lung Allocation Score, and diagnosis. PGD developed in 383 subjects. Among subjects with PGD, l

Plasma protein biomarkers for primary graft dysfunction after lung transplantation: a single-center cohort analysis.

The clinical use of circulating biomarkers for primary graft dysfunction (PGD) after lung transplantation has been limited. In a prospective single-center cohort, we examined the use of plasma protein biomarkers as indicators of PGD severity and duration after lung transplantation. The study comprised 40 consecutive lung transplant patients who consented to blood sample collection immediately pretransplant and at 6, 24, 48, and 72 h after lung transplant. An expert grader determined the severity and duration of PGD and scored PGD at T0 (6 h after reperfusion), T24, T48, and T72 h post-reperfus

Extracorporeal membrane oxygenation for grade 3 primary graft dysfunction after lung transplantation: Long-term outcomes.

INTRODUCTION: Extracorporeal membrane oxygenation (ECMO) is an efficient and innovative therapeutic tool for primary graft dysfunction (PGD). However, its effect on survival and long-term lung function is not well known. This study evaluated those parameters in patients with PGD requiring ECMO. METHOD: This single-center, retrospective study included patients who underwent LTx at our institute between January 2007 and December 2013. Patients and disease characteristics, survival, and pulmonary function tests were recorded. RESULTS: A total of 309 patients underwent LTx during the study perio

Soluble Adhesion Molecules During Ex Vivo Lung Perfusion Are Associated With Posttransplant Primary Graft Dysfunction.

Ex vivo lung perfusion (EVLP) enables assessment of marginal donor lungs for transplantation. We aimed to discover biomarkers in EVLP perfusate that could predict development of primary graft dysfunction (PGD). From September 2008 to August 2013, 100 clinical EVLPs were performed. Eleven patients developed PGD grade 3 within 72 h after transplant. The non-PGD group consisted of 34 patients without PGD grade 3. Nonbilateral lung transplants or transplant after extracorporeal life support were excluded from analyses. Soluble intercellular adhesion molecule 1 (sICAM-1), soluble VCAM-1 (sVCAM-1),

Unilateral radiographic abnormalities after bilateral lung transplantation: exclusion from the definition of primary graft dysfunction?

OBJECTIVES: Unilateral infiltrates on chest x-ray films are occasionally seen after bilateral lung transplantation. In the primary graft dysfunction grading system, the presence or absence of a radiographic abnormality is crucial in determining the incidence and severity of primary graft dysfunction. However, no consideration is given as to whether unilateral infiltrates have the same impact and relevance as bilateral infiltrates. This study aims to describe the incidence, features, and outcomes of posttransplant unilateral infiltrates and their effect on the novel primary graft dysfunction gr

Targeting the proline-glycine-proline-protease feed-forward loop attenuates primary graft dysfunction after lung transplantation.

INTRODUCTION: Primary graft dysfunction (PGD) is the leading cause of early mortality after lung transplantation, yet no targeted therapy exists. We investigated whether the collagen-derived matrikine proline-glycine-proline (PGP) drives neutrophil-predominant injury in PGD and whether its neutralization confers protection. METHODS: Human mini-bronchoalveolar lavage (BAL) fluid was collected 72 hours post-transplantation from recipients with grade 3 PGD and non-PGD controls. In parallel, a murine orthotopic lung transplantation model incorporating 18 hours of cold ischemia was used to reprodu

Caspase-3 in Brain Death Donors Is Associated with Reduced Primary Graft Dysfunction After Heart Transplantation.

Primary graft dysfunction (PGD) remains a major cause of early morbidity and mortality after a heart transplant (HTx). Understanding the donor-related mechanisms involved may help improve organ selection and post-HTx outcomes. This study aimed to explore the association between the donor serum biomarkers of cell death and inflammation and the incidence of PGD and rejection in HTx recipients. We conducted a retrospective, multicenter observational study of brain-dead (DBD) heart donors and corresponding recipients between 2013 and 2019. Donor blood samples were analyzed for inflammatory cytokin

Association between primary graft dysfunction among lung, kidney and heart recipients from the same multiorgan donor.

Even organs from an ideal donor will occasionally develop primary graft dysfunction (PGD) causing a significant morbidity and mortality after transplantation. It is likely that this situation represents subtle undetectable levels of ongoing donor organ dysfunction. The aim of this study is to investigate the association of PGD between lung, kidney and heart recipients from the one donor. From 202 multiorgan donors, contributed 231 consecutive lung transplants at the Alfred Hospital, 378 kidney and 114 heart transplants were subsequently performed at multiple centers across Australia and New Ze

Donor type and organ preservation in heart transplantation: Association with recovery following severe primary graft dysfunction.

BACKGROUND: Primary graft dysfunction (PGD) remains a major cause of post-heart transplant morbidity and mortality. Prior studies have focused on predictors of developing PGD. Less is known about factors associated with post-PGD recovery. METHODS: We queried the Scientific Registry of Transplant Recipients for adult heart transplants between September 2023 and December 2025. Severe PGD was defined as dependence on new extracorporeal membrane oxygenation or ventricular assist device support at 24 h post-transplant. Outcomes were compared across donor type (donation after circulatory death [DCD

Activation of donor-derived eosinophils during static storage contributes to primary graft dysfunction after lung transplantation.

Ischemia-reperfusion injury (IRI) contributes to deleterious outcomes after lung transplantation. Although we have shown a unique protective role for eosinophils in both establishing and maintaining lung allograft tolerance, their role in IRI remains unclear. Based on previous research demonstrating a protective role for eosinophils in liver IRI, we hypothesized that they might play a similar function in the lung. In this study, we show that donor-, but not recipient-derived, eosinophils worsen injury of syngeneic and allogeneic lung grafts rather than protect from it. Eosinophils in the lungs

Metabolomic atlas of ischemia-reperfusion injury in human lung transplants: Mitochondrial stress and primary graft dysfunction.

Ischemia-reperfusion injury drives primary graft dysfunction (PGD) in lung transplantation, yet the metabolic landscape of this process remains unclear. We performed longitudinal untargeted metabolomic studies on lung grafts collected before and after reperfusion, from 39 patients with PGD grade 3 (PGD3) and 40 patients without. The expression levels and pathways of identified metabolites were analyzed and correlated with donor/recipient conditions and early clinical outcomes. A total of 443 metabolites were detected, and metabolites that significantly changed following reperfusion were mapped

L-Alanyl-L-Glutamine Alleviated Ischemia-Reperfusion Injury and Primary Graft Dysfunction in Rat Lung Transplants.

BACKGROUND: Concern of ischemia-reperfusion injury reduces utilization of donor lungs. We hypothesized adding L-alanyl-L-glutamine (L-AG) to preservation solution may protect donor lungs from ischemia-reperfusion injury through its multiple cytoprotective effects. METHODS: A lung transplantation cell culture model was used on human lung epithelial cells and pulmonary microvascular endothelial cells, and the effects of adding different concentrations of L-AG on basic cellular function were tested. Rat donor lungs were preserved at 4 °C with 8 mmol/L L-AG for 12 h followed by 4 h reperfusion or

Biomarkers for primary graft dysfunction after lung transplantation: a review of current evidence and future prospects.

Lung transplantation remains the only effective treatment for end-stage lung disease, offering the potential to significantly prolong survival and enhance quality of life for recipients. However, primary graft dysfunction (PGD)-a severe form of lung injury occurring within the first 72 h post-transplantation-constitutes a major cause of early mortality and presents a substantial barrier to the broader clinical adoption of lung transplantation. Biomarkers, defined as specific molecules, cells, or other biological indicators detectable within or outside the body, can reflect physiological states
