Use, location, and timeliness of clinical microbiology testing in Georgia for select infectious diseases.

OBJECTIVE: Although clinical microbiology testing facilitates both public health surveillance of infectious diseases and patient care, research on testing patterns is scant. We surveyed hospital laboratories in Georgia to assess their diagnostic testing practices. METHODS: Using e-mail, all directors of hospital laboratories in Georgia were invited to participate. The survey focused on timing and location of diagnostic testing in 2006 for 6 reportable diseases: giardiasis, legionellosis, meningococcal disease, pertussis, Rocky Mountain spotted fever, and West Nile virus disease. RESULTS: Of

Overcoming geographic isolation.

There are a lot of advantages to living in an isolated area, away from the hustle and bustle of the big city. We work hard to provide services more commonly located in a metropolitan area. There is satisfaction in being self-sufficient and able to spread your wings and start a new venture such as an off-campus diagnostic center or a courier route. We enjoy certain advantages in this environment. Changes in medical care delivery systems get to us slowly, giving us time to plan for the impact. Antibiotic-resistant organisms are slower to move in and are easier to control. The blood donor populat

Follow-up study of the regional quota system of Japanese medical schools and prefecture scholarship programmes: a study protocol.

INTRODUCTION: Given the shortage of physicians, particularly in rural areas, the Japanese government has rapidly expanded the number of medical school students by adding chiikiwaku (regional quotas) since 2008. Quota entrants now account for 17% of all medical school entrants. Quota entrants are usually local high school graduates who receive a scholarship from the prefecture government. In exchange, they temporarily practise in that prefecture, including its rural areas, after graduation. Many prefectures also have scholarship programmes for non-quota students in exchange for postgraduate in-

Rethinking policies for the retention of allied health professionals in rural areas: a social relations approach.

OBJECTIVE: Retaining allied health professionals in rural areas is a recognised problem. Generally the literature has concentrated on three elements: practitioner needs, community needs and organisational needs. There has been little attempt to focus other types of social relations in which health practitioner retention and recruitment takes place. The aim of this paper is to question the present dominant hierarchical approach taken in relation to the retention of allied health professionals in rural localities. METHODS: The data derives from a survey in Southwest Victoria, Australia. The sam

Practice Predictors of Buprenorphine Prescribing by Family Physicians.

INTRODUCTION: Both opioid use disorder and mortality for opioid overdoses are increasing. Family physicians (FPs) can treat opioid use disorder if they are waivered to prescribe buprenorphine. Our objective was to determine personal, practice, and community characteristics associated with FPs prescribing buprenorphine. METHODS: We used data from the 2017 and 2018 American Board of Family Medicine examination registration questionnaire. The questionnaire asked about current prescribing of buprenorphine, as well as about practice size, organization, and location. Logistic regression was used to

Observation and Prevention of Caregiver-Related Medication Errors in Community Pharmacies: A Cross-Sectional Study Across Urban, Town, and Rural Settings.

Background: Medication errors committed by informal caregivers of older adults represent a significant risk to patient safety, particularly in the context of population aging and increasing therapeutic complexity. Community pharmacies could play a key role in detecting and preventing them; furthermore, pharmacy location (urban, town, or rural settings) role in medication errors remains underexplored. Objective: To analyze the types and frequency of caregiver-related medication errors observed by community pharmacists and to determine if differences exist according to pharmacy location. Methods

Estimating the rate of cesarean section by maternal request: anonymous survey of obstetricians in Australia.

BACKGROUND: The rate of cesarean section in Australia now exceeds 30 percent, and evidence from population studies indicates that maternal requests for elective cesarean delivery might make an important contribution. The objective of this study was to explore the rate of such deliveries in Australia, in the absence of a formal investigation. METHODS: An anonymous survey was sent to all 1,239 specialist obstetricians and 317 obstetric specialty trainees in Australia. Specialists were asked the number of elective cesarean deliveries they performed in 2006 that satisfied the National Institutes

The effects of location and race on the performance of cardiac procedures.

This study assessed the impact of patient and facility characteristics on patient outcomes for coronary artery disease. The sample data studied and analyzed (i.e., hospital financial and administrative data) were from patients discharged in 2004, from hospitals in Florida with principal diagnoses codes indicating diseases of the circulatory system. Patient outcomes were measured as principal procedure codes for patients who received coronary arteriography, percutaneous transluminal coronary angioplasty, and coronary artery bypass graft surgeries. Chi-square analysis indicated that both patient

Residents' preferences and preparation for caring for underserved populations.

Access to care by low-income persons and residents of rural and poor inner-city areas is a persistent problem, yet physicians tend to be maldistributed relative to need. The objectives were to describe preferences of resident physicians to locate in underserved areas and to assess their preparedness to provide service to low-income populations. A national survey was made of residents completing their training in eight specialties at 162 US academic health center hospitals in 1998, with 2,626 residents responding. (Of 4,832 sampled, 813 had invalid addresses or were no longer in the residency p

The Anabranch Framework for the Ruralization of Health Professional Education.

Background/Objective: The quality of care afforded to rural, remote, and First Nations Peoples is dependent on access to a health workforce with the capacity to contextualize healthcare and practice to the needs and expectations of these populations. In Australia, the lack of representation of rural health in undergraduate and post graduate health professional education undermines this preparedness and consideration of rural practice uptake and longevity, compounding the inequities confronted by 7 million Australians residing in these locations. Urgent educational reforms are required to addre
