Biochemical and pharmacological characterization of propofol infusion syndrome risk markers following high-dose propofol.

Propofol infusion syndrome (PRIS) is a rare, severe complication from prolonged high-dose propofol use. Predictive biomarkers, especially in spinal surgery, remain unclear. This study aimed to biochemically and pharmacologically characterize risk markers of PRIS following high-dose propofol infusion in patients undergoing elective spinal surgery. A quasi-experimental interventional study was conducted in 2024 at Allameh Bohlool Gonabadi Hospital. Fifty-four patients aged 20-70 years scheduled for elective spinal surgery and classified as ASA physical status I or II were enrolled via convenienc

Does propofol definitely improve postoperative cognitive dysfunction?-a review of propofol-related cognitive impairment.

Postoperative cognitive dysfunction (POCD) is a common brain function-related complication after surgery. In addition to old age being an independent risk factor, anesthetics are also important predisposing factors. Among them, propofol is the most commonly used intravenous anesthetic in clinical practice. It has a rapid onset, short half-life, and high recovery quality. Many studies report that propofol can attenuate surgery-induced cognitive impairment, however, some other studies reveal that propofol also induces cognitive dysfunction. Therefore, this review summarizes the effects of propof

Comparison of Esketamine/Propofol and Sufentanil/Propofol on Intraoperative Hypoxemia During Bronchoscopy: A Randomized Trial.

PURPOSE: Propofol and sufentanil are the most commonly used anesthetics during bronchoscopy. Esketamine is an s-enantiomer of ketamine racemate and has both sedative and analgesic effects, it does not inhibit respiration and maintains hemodynamic stability. We aimed to compare the intraoperative hypoxemia risk of esketamine/propofol with sufentanil/propofol for patients in bronchoscopy. METHODS: This study was an investigator-initiated, single-center, randomized, double-blind clinical trial. Patients undergoing bronchoscopy were randomly assigned to receive either sufentanil group (n = 33; su

Bronchial mucus transport velocity in patients receiving anaesthesia with propofol and morphine or propofol and remifentanil.

In vitro morphine does not reduce cilia beat frequency, a key factor determining bronchial mucus transport velocity. There are no reports about the effect of remifentanil on bronchial mucus transport. We compared the bronchial mucus transport velocity in patients having total intravenous anaesthesia with either propofol and morphine, or propofol and remifentanil. Twenty patients scheduled for elective surgery were randomly allocated to the two groups. Fifteen minutes after insertion of the laryngeal mask airway, bronchial mucus transport velocity was assessed by fibreoptic observation of the m

Propofol and Clevidipine-induced Hypertriglyceridemia.

Hypertriglyceridemia and related pancreatitis due to the use of lipid emulsions such as propofol has been documented, but less is known about the additive adverse effects of propofol and clevidipine lipid emulsions in the literature. We report an unusual case, highlighting the trend of serum triglyceride and pancreatic enzymes (amylase/lipase) with the administration of propofol and clevidipine for a prolonged period in the neurocritical care setting. We present a case of a 27-year-old male who was admitted to the neuroscience intensive care unit (NSICU) for management of severe subarachnoid h

Fixed drug eruption to propofol.

We present a case of fixed drug eruption to propofol following a series of sedations of a patient for a number of day case procedures. The patient experienced oedema and blistering of his penis, increasing in severity and duration following each subsequent exposure. The diagnosis was confirmed by punch biopsy following an intravenous challenge test with propofol. Whilst reports of fixed drug eruptions to anaesthetic induction agents are uncommon, a number of drugs used commonly by anaesthetists are known triggers. We discuss fixed drug eruptions in relation to anaesthetic practice, aiming to r

Hypertriglyceridemia independent propofol-induced pancreatitis.

CONTEXT: Propofol can cause acute pancreatitis related to hypertriglyceridemia, however, other mechanisms may also exist. CASE REPORT: A 71-year-old male on propofol infusion in the intensive care setting, developed acute pancreatitis as confirmed biochemically and by imaging. He did not have any elevation of triglycerides and on propofol re-challenge, had recurrence of pancreatitis with the absence of triglyceride elevation. CONCLUSION: We report a case of hypertriglyceridemia independent propofol induced pancreatitis possibly related to an idiosyncratic reaction and propose that this drug

We report on a 49-year-old female patient suffering from recurrent carcinoma of the rectum, who underwent a palliative Hartmann operation for an anus praeter reconstruction. After a remifentanil bolus of 90 microg and a propofol bolus of 200 mg, anaesthesia was maintained with 0.25 microg/kg/min remifentanil and 4 mg/kg propofol, and after skin incision with 1.0 microg/kg/min remifentanil and 5 mg/kg/h propofol. Throughout the operation, the patient showed a stable blood pressure of 120-130/80 mmHg but 15 min after skin incision the heart rate suddenly rose to 140 beats/min, so remifentanil wa

LC-MS/MS analysis of propofol in postmortem blood: propofol glucuronide as an indicator of exposure.

Propofol (PRF) is a short-acting intravenous anesthetic widely used for the induction and maintenance of general anesthesia. However, multiple accidental deaths resulting from the non-therapeutic use of PRF have been reported; therefore, PRF is an important target analyte in forensic investigations. In this study, quantitative analytical methods for PRF and propofol glucuronide (PRG) in postmortem blood were developed using liquid chromatography-tandem mass spectrometry (LC-MS/MS). For quantification, PRF was derivatized with N-(2-(bromomethyl)benzyl)-N,N-diethylethanaminium bromide (CAXB) to

A Case Report of "Propofol Frenzy": Anesthetic Considerations for a Severe Propofol-Induced Neuroexcitation Reaction.

Propofol "frenzy" is considered a severe propofol-induced neuroexcitatory reaction involving nonepileptic spells of extremity thrashing, marked agitation, irregular eye movements, and impaired consciousness. Patients with propofol neuroexcitation present unique challenges for anesthesia providers due to underrecognition, lack of diagnostic tests, and differentiating from other comparable disorders that require medications that can exacerbate symptoms. We present a case of a healthy young patient whose postoperative course was complicated by propofol frenzy and functional limb paralysis followi

Blood concentration of propofol in a patient with delayed emergence from propofol-nitrous oxide anesthesia.

We measured the blood concentration of propofol in a patient with delayed emergence from propofol-nitrous oxide anesthesia. A 78-year-old man underwent subtotal gastrectomy under both epidural and propofol-nitrous oxide anesthesia and did not regain consciousness soon after termination of propofol infusion. Preoperative laboratory examination revealed anemia and a low blood total protein concentration, but there was no evidence of impaired liver function. While the anesthesiologists were waiting for the patient to regain consciousness, a surgeon mentioned that the common hepatic artery might h

A comparison of sevoflurane-propofol versus sevoflurane or propofol for laryngeal mask airway insertion in adults.

In a prospective, randomized study, we investigated the incidence of successful insertion of laryngeal mask airway (LMA) at the first attempt and the incidence of side effects after LMA insertion using the combination of sevoflurane and propofol as compared with either sevoflurane or propofol alone for induction of anesthesia. Eighty-three unpremedicated ASA physical status I-II patients were anesthetized with a single vital capacity breath (VCB) of sevoflurane 8% supplemented with IV propofol 1.5 mg/kg, a single VCB of sevoflurane 8%, or IV propofol 3 mg/kg. The effect on serum lipid concentrations of a prolonged infusion of propofol--hypertriglyceridaemia associated with propofol administration.

Serum concentrations of triglyceride, cholesterol and high density lipoprotein-cholesterol (HDL-cholesterol) were measured in an ICU patient after he had received a 10-day continuous infusion of propofol. No additional parenteral lipid was given in the 72 h prior to initial sampling, but a total of 71 of 10% intralipid had been administered over the remaining 7 days. The total cumulative dose of propofol was 66.1 g (range 0.7-6.4 mg kg-1 h-1). There was no visual appearance of lipaemia. Both the serum triglyceride and cholesterol concentrations increased (triglyceride level increased to 4 time

Propofol inhibits Piezo mechanosensitive channels.

Modulation of ion channels is a key mechanism by which anesthetics exert their effects. Propofol, a widely used anesthetic, has been shown to influence mechanosensitive ion channels, although the details of this interaction remain under investigation. In this study, we show that propofol inhibits Piezo mechanosensitive channels using electrophysiological recordings and calcium imaging in HEK293T cells overexpressing human Piezo1 (hP1) and Piezo2 (hP2) channels. At 50 μM, propofol inhibited hP1 currents across multiple configurations (outside-out, whole cell, and cell attached) with a dissociat

Propofol for sedation during colonoscopy.

RATIONALE: Colonoscopy is a common diagnostic and screening test used in the management of many gastrointestinal diseases. Colonoscopy can be uncomfortable, so there is an impetus to optimize sedative medications for test performance and patient comfort. Traditionally, a combination of opioids and benzodiazepines have been used to increase comfort during colonoscopy. Propofol is a potential alternative sedative agent for use during colonoscopy, though it often leads to deeper sedation than with traditional sedatives used for colonoscopy. Many jurisdictions require anesthesiologists to administ

New perspectives on propofol allergy.

PURPOSE: Propofol is an intravenous sedative used in many patient populations and care settings. Although generally considered safe and effective, the drug has historically been avoided in patients with reported allergies to egg, soy, and/or peanut on the basis of the manufacturer's prescribing information. Concerns exist for potential adverse events, increased medication costs, reduced efficacy, and risk of medication errors when using alternative agents. Here we present a critical examination of the literature concerning cross-reactivity of food allergies with propofol to provide evidence-ba
