Comparative toxicity of tetracaine, proparacaine and cocaine evaluated with primary cultures of rabbit corneal epithelial cells.

Cocaine was first used as a topical anesthetic for the eye by Dr Carl Koller in 1884. It became evident that this agent produced erosion of the corneal epithelium in high doses or with repeated use. Synthetic local anesthetics such as tetracaine and proparacaine were developed which were more potent and less toxic than cocaine, but still produced corneal epithelium defects if used chronically. This investigation was undertaken to compare and rank the cytotoxicity of the most commonly used ocular local anesthetics, tetracaine, proparacaine and cocaine, with primary cultures of rabbit corneal ep

REPLACEMENT OF LIDOCAINE GEL WITH TOPICAL PROPARACAINE ANESTHESIA FOR ROUTINE INTRAVITREAL INJECTIONS: A Comparative Study.

PURPOSE: Lidocaine gel was suggested to be highly effective in providing anesthesia for intravitreal injections but adverse effects include a possibility of making sterilization of the conjunctiva difficult. Hence, we wished to determine the effect of using 0.5% proparacaine drops alone over the use of 3.5% lidocaine hydrochloride gel anesthesia during office-based intravitreal injections. METHODOLOGY: This was a case-control study in patients who came routinely to the clinic for antivascular endothelial growth factor injections. Eyes were treated with one of two anesthesia modalities. Topical anesthesia during infant eye examinations: does it reduce stress?

We studied the effect of topical anesthesia on infant stress and corneal haze during the routine eye examination for retinopathy of prematurity. Using a double-blind protocol, 55 premature infants weighing less than 1501 g at birth were selected randomly to receive normal saline or proparacaine HCl 0.5% eye drops as a corneal wetting agent at their initial eye examination. Before, during, and after the procedure, infant stress was evaluated by heart rate, respiration rate, blood pressure, and transcutaneous oxygen saturation. Subjective assessment of the infant's cry intensity and corneal haze

An atypical vasopermeability response to leukotrienes in the iris-ciliary body.

The effects of leukotrienes (LTs) on the blood-aqueous barrier in the guinea pig were investigated. Leukotriene B4 (LTB4) and D4 (LTD4) were administered in doses of 1 to 1000 ng into the anterior chamber. Plasma extravasation was determined as extravascular 125I-albumin levels in the iris-ciliary body using 125I-albumin and 51Cr-erythrocytes as an intravascular marker. LTB4 and LTD4 alone did not cause significant plasma extravasation. Although combinations of 100 ng and more of each LT raised extravascular albumin levels in the ocular anterior uvea. These studies suggest a synergistic intera

The effect of single-application topical ophthalmic anesthesia in patients with trigeminal neuralgia. A randomized double-blind placebo-controlled trial.

To evaluate the reported benefit of ipsilateral single-application ophthalmic anesthetic eyedrops in patients with typical trigeminal neuralgia, a randomized double-blind placebo-controlled trial was performed. Forty-seven patients were randomly assigned to receive two drops of either proparacaine (25 cases) or saline placebo (22 cases). The experimental and placebo groups were equivalent in regard to patient age, distribution of trigeminal neuralgia pain, duration of pain, current medication regimens, and number of prior procedures performed. Pain response was assessed at 3, 10, and 30 days a

The effects of topical and general anesthesia on ocular drug levels and intraocular pressure lowering activity of topically applied carbonic anhydrase inhibitors.

Treatment with topical anesthetics was reported to increase corneal permeability and improve ocular drug bioavailability. These changes were attributed to the loss of reflex blinking, reduction of tear secretion and appearance of superficial corneal epithelial lesions. A recent report showed that pretreatment with Ophthetic (0.5% proparacaine HCl) (OPH) in pentobarbital-anesthetized rabbits significantly increased the permeability of the lipophobic carbonic anhydrase inhibitor (CAI) benzolamide (B) and transformed it to a highly active ocular hypotensive drug. This did not affect the permeabil

Allergic contact dermatitis to proparacaine with subsequent cross-sensitization to tetracaine from ophthalmic preparations.

We report 2 cases of allergic contact dermatitis (ACD) to proparacaine and tetracaine. Patient 1 is an ophthalmologist with chronic finger pad dermatitis sensitized to the topical anesthetic proparacaine. Despite discontinuance of proparacaine and substitution with a patch test negative agent, tetracaine, his hand dermatitis persisted. Follow up patch testing documented that acquisition of contact allergy to tetracaine as well as thiuram had taken place. Patient 2 had a periocular eczematous dermatitis with ACD to both proparacaine and tetracaine. Cross sensitization between related topical op

Systemic manifestations in response to mydriasis and physical examination during screening for retinopathy of prematurity.

OBJECTIVE: To determine whether adverse effects manifested via vital sign changes during the screening examination for retinopathy of prematurity (ROP) are due to the pharmacologic properties of the eye drops or to physical manipulation of the eyes. The authors also investigated the relationship between distress during the screening process and the severity of prematurity of the infant. DESIGN AND METHODS: A prospective observational study was designed that enrolled all infants either weighing < or =1500 g or who were < or =32 weeks gestational age at birth who were admitted to the neonatal i

Efficacy of topical anesthetics to reduce pain in premature infants during eye examinations for retinopathy of prematurity.

BACKGROUND: Eye examinations for retinopathy of prematurity (ROP) are stressful and probably painful, but many ophthalmologists do not apply topical anesthetics because their efficacy in reducing pain has not been established. OBJECTIVE: To evaluate the potential benefits of topical anesthetic eye drops in reducing pain during neonatal eye examination for ROP. METHODS: Neonates born at < or =30 weeks' gestation and expected to have at least 2 examinations for ROP were included. Patients were randomly assigned to receive either proparacaine HCl ophthalmic solution 0.5% or NaCl 0.9% (saline) e

Cycloplegia in African-American children.

PURPOSE: The selection of a cycloplegic agent depends on the desired outcome, the characteristics of the patient receiving the drug, and the associated risks. The Orinda Longitudinal Study of Myopia (OLSM) has used 1% tropicamide to assess the ocular components and cycloplegic refractions in a large cohort of predominantly Caucasian children. Although tropicamide has provided adequate cycloplegia and mydriasis for the OLSM cohort, conventional clinical wisdom and scientific investigations have suggested that tropicamide might not produce adequate cycloplegia and mydriasis for subjects with dar

Voltage-dependent sodium channels in synaptoneurosomes: studies with 22Na+ influx and [3H]saxitoxin and [3H]batrachotoxinin-A 20-alpha-benzoate binding. Effects of proparacaine isothiocyanate.

22Na+ influx and binding of [3H]saxitoxin ([3H]STX) and [3H]batrachotoxin-A 20-alpha-benzoate ([3H]BTX-B) were studied in guinea pig cerebral synaptoneurosomes. STX and tetrodotoxin (TTX) completely blocked the stimulation of sodium influx induced by 1 microM BTX. The IC50 values for STX and TTX closely matched the Ki values for inhibition of [3H]STX binding, suggesting that the sites labelled by [3H]STX are associated with a population of BTX-sensitive channels. BTX induced a dose-dependent stimulation of sodium influx in synaptoneurosomes (EC50 280 nM). The potency of BTX for stimulation of
