Failure to rescue measure: validation of community- and hospital-acquired complications.

The inclusion of the failure to rescue (FTR) measure as one of the Centers for Medicare and Medicaid Services Inpatient Prospective Payment System measures has raised questions about the characteristics of FTR cases and their outcomes. In this study, we validated 75% of the identified FTR complications using medical record review (n = 461). Nearly half (49.5%) of the complications originated in the community and were present on admission. Acute renal failure, gastrointestinal hemorrhage, and sepsis most often originated in the community. Cardiac arrest/shock, pneumonia, and pulmonary embolism

Powering Value-Based Care with Clinician-Led, Patient-Centered Design: The MyHealth Bundles Program.

Value-based care has long been championed for its potential to align clinical outcomes with financial incentives, but many health systems struggle to implement truly transformative models. Fee-for-service constraints, payer fragmentation, and complex claims processing often limit progress to incremental gains rather than transformation to true value-based arrangements. Herein, the authors present an innovative approach to bundled payments that overcomes many of these hurdles by placing physicians and patients at the center of the redesign process while partnering with self-insured employers. Chronic ventilator dependence in elderly patients.

Long-term ventilator dependence is the need for mechanical ventilation for more than 6 h/d for more than 21 days. Long-term ventilator dependence complicates 9% to 20% of the episodes of mechanical ventilation treated in the intensive care units of acute care hospitals; it is associated with an average mortality rate of 40%. Unlike acute respiratory failure, the risk for which does not increase with age, long-term ventilator dependence falls disproportionately to patients aged 70 years or older. During the past 2 decades, a profusion of care sites for patients with long-term ventilator depende

Medical Devices Applying for Outpatient Medicare Supplemental Payments.

IMPORTANCE: Medicare transitional pass-through payments (TPTPs) provide supplemental reimbursement that is intended to facilitate adoption of new devices in the outpatient setting. The US Centers for Medicare & Medicaid Services (CMS) have historically evaluated manufacturer applications for TPTPs based on newness, cost, and evidence of substantial clinical improvement, ie, the traditional pathway. In 2020, CMS introduced an alternative pathway to allow US Food and Drug Administration (FDA)-designated breakthrough devices to qualify for supplemental reimbursement without demonstrating substant

Nurse and client perceptions of home health wound care effectiveness after a change in Medicare reimbursement.

The purpose of this study was to begin to explore client and nurse perceptions related to how a change in a Medicare reimbursement, Prospective Payment System (PPS), affects home health care. The target population included home health clients with stage III or greater decubitus ulcers, age 65 years or older. Six Virginia home health agencies were randomly selected. From those sites, all 39 charts that met population criteria were accessed and analyzed, and then 16 care recipients were purposely selected. In addition, 26 home health nurses with pre- and post-PPS experience were purposefully sel

Development of a casemix classification to predict costs of home care in the Netherlands: a study protocol.

INTRODUCTION: Compared with fee-for-service systems, prospective payment based on casemix classification is thought to promote more efficient, needs-based care provision. We aim to develop a casemix classification to predict the costs of home care in the Netherlands. METHODS AND ANALYSIS: The research is designed as a multicentre, cross-sectional cohort study using quantitative methods to identify the relative cost predictors of home care and combine these into a casemix classification, based on individual episodes of care. The dependent variable in the analyses is the cost of home care utili

Cost-effectiveness analysis of continuous-flow left ventricular assist devices as destination therapy.

BACKGROUND: Continuous-flow left ventricular assist devices (LVADs) have become the dominant devices for mechanical circulatory support, but their cost-effectiveness is undetermined. This study assessed the cost-effectiveness of continuous-flow devices for destination therapy versus optimal medical management in advanced heart failure and compared the results with previous estimates for pulsatile devices. METHODS AND RESULTS: A Markov model was developed to assess cost-effectiveness. Survival, hospitalization rates, quality of life, and cost data were obtained for advanced heart failure patie

Hospital care for elderly patients with diseases of the circulatory system. A comparison of hospital use in the United States and Canada.

In an effort to determine how lower health care expenditures in Canada as compared with the United States translate into actual hospital services received, we examined discharge rates, lengths of stay, and the case-mix index (calculated from diagnosis-related group weights) for those 65 years of age and older hospitalized with cardiovascular disease in the United States and two Canadian provinces, Manitoba and Ontario, in 1981 and 1985. We expected that the effect of the prospective payment system might be reflected in changes between 1981 and 1985. Discharge rates for medical treatment of car

Immune checkpoint inhibitors in urothelial carcinoma: a practical framework for patient selection, toxicity management, response assessment, and treatment sequencing.

INTRODUCTION: Managing urothelial carcinoma with immune checkpoint inhibitors requires addressing issues beyond clinical outcomes. Implementation difficulties affect most patients, predictive biomarkers show limited utility, and treatment discontinuation from toxicity remains problematic. AREAS COVERED: This article integrates clinical trials, systematic reviews, health-system evaluations, and guidelines from 2016 to 2025. We detail limitations of programmed death-ligand 1 (PD-L1) expression, panel-based tumor mutational burden (TMB), clinic-ready toxicity pathways, immune-appropriate respons

Adverse Events in Orthopaedics: Is Trauma More Risky? An Analysis of the NSQIP Data.

OBJECTIVES: As our healthcare system moves toward bundling payments, orthopaedic trauma surgeons will be increasingly benchmarked on perioperative complications. We therefore sought to determine financial risks under bundled payments by identifying adverse event rates for (1) orthopaedic trauma patients compared with general orthopaedic patients and (2) based on anatomic region and (3) to identify patient factors associated with complications. DESIGN: Prospective. SETTING: Multicenter. PATIENTS/PARTICIPANTS: A total of 146,773 orthopaedic patients (22,361 trauma) from 2005 to 2011 NSQIP dat

Prediction of length of stay in an inpatient dual diagnosis unit.

The institution of prospective payment systems, in which flat fees are paid per discharge, raised the concern that hospitals might preferentially admit patients expected to have a short length of stay (LOS). This concern presupposes that intake workers could accurately predict psychiatric hospitalization LOS, but this does not appear to have been empirically demonstrated. Accordingly, we examined the ability of two psychiatrists heading separate treatment teams on an inpatient, dual-diagnosis unit and a program coordinator who worked with both teams to predict LOS for 94 patients consecutively

Cost distribution and Monte Carlo simulation of financial risk under DRG-based prospective payment for laparoscopic cholecystectomy in patients with gallbladder polyps: a single-center retrospective study.

BACKGROUND: Under China's Diagnosis-Related Group (DRG) prospective payment system, hospitals bear the financial risk of costs that exceed the fixed reimbursement standard, making case-level cost distribution and the probability of loss critical to departmental sustainability for high-volume, low-complexity procedures such as laparoscopic cholecystectomy. OBJECTIVE: However, the probability distribution of hospitalization costs and the quantitative financial risk faced by a single institution operating under a fixed DRG payment standard for the group HS10B (laparoscopic cholecystectomy withou

Hospital use patterns for children in the United States, 1983 and 1984.

The hospital discharge rate of children less than 15 years of age in the United States declined 12% from 1983 to 1984. This was the first time in the 20-year history of the National Hospital Discharge Survey that there was a statistically significant decrease in children's hospital discharge rates in a 1-year period. The change occurred during a period when prospective hospital payment systems were introduced and when prepaid group health plans and alternative systems of providing health care were expanding. The unprecedented decrease in children's hospital use was evaluated using data from th

Patient comorbidity score predicting the incidence of perioperative complications: assessing the impact of comorbidities on complications in spine surgery.

OBJECT: Present attempts to control health care costs focus on reducing the incidence of complications and hospital-acquired conditions (HACs). One approach uses restriction or elimination of hospital payments for HACs. Present approaches assume that all HACs are created equal and that payment restrictions should be applied uniformly. Patient factors, and especially patient comorbidities, likely impact complication incidence. The relationship of patient comorbidities and complication incidence in spine surgery has not been prospectively reported. METHODS: The authors conducted a prospective a

Decoding Homelessness: Z-Codes and the Recognition of Homelessness as a Comorbid Condition.

There are an estimated 653,100 people across the United States experiencing homelessness. Homelessness is an important social determinant of health associated with increased morbidity and mortality. The prevalence of homelessness among hospitalized patients is substantially higher than the community prevalence, and people experiencing homelessness (PEH) are more likely to be admitted to the hospital and have longer length of stays and higher healthcare costs relative to stably housed patients. The Centers for Medicare and Medicaid Services (CMS) recently introduced an important policy change r

Nursing home case mix in Wisconsin. Findings and policy implications.

Along with many other states, Wisconsin is considering a case mix approach to Medicaid nursing home reimbursement. To support this effort, a nursing home case mix model was developed from a representative sample of 410 Medicaid nursing home residents from 56 facilities in Wisconsin. The model classified residents into mutually exclusive groups that were homogeneous in their use of direct care resources, i.e., minutes of direct care time (weighted for nurse skill level) over a 7-day period. Groups were defined initially by intense, Special, or Routine nursing requirements. Within these nursing

The effect of competition on nursing home expenditures under prospective reimbursement.

The for-profit nursing home's incentive to minimize costs has been maligned as a major cause of the quality problems that have traditionally plagued the nursing home care industry. Yet, profit-maximizing firms in other industries are able to produce products of adequate quality. In most other industries, however, firms are constrained from reducing costs to the point where quality suffers by the threat of losing business to competing firms. In the nursing home industry, competition for patients often does not exist because of the shortage of nursing home beds. As a result, one would expect tha
