12-hour psychotherapy.

The two cases cited illustrate how short-term therapy can be used to meet a patient's needs to deal with loss and grief. Although the goals as set out in short-term therapy contracts are not always fully achieved, this treatment modality, as delineated by Mann, is valuable to psychiatry. Short-term therapy is not only a sound therapeutic intervention, but also a means of bringing psychiatric health care to a greater segment of the population.

Benefits of Using Intensive Short-Term Dynamic Psychotherapy in Psychiatric Practice.

Psychiatry residency programs in the United States and Canada currently require trainees to demonstrate competency in psychodynamic therapy. Developed by Habib Davanloo, intensive short-term dynamic psychotherapy (ISTDP) is a brief psychodynamic therapy with emerging evidence to support its efficacy as an evidence-based, cost-effective treatment for various common psychiatric disorders and personality pathologies. Davanloo's metapsychology of the unconscious offers a way to understand the psychodynamic processes that maintain patients' psychiatric symptoms. Some key clinical processes unique t

Efficacy and suitability of adding short-term psychodynamic psychotherapy (STPP) to pharmacotherapy in patients with depressive disorders: a systematic review.

OBJECTIVE: Recent guidelines on depressive disorders suggest a combination of antidepressants and psychotherapy in case of moderate to severe symptomatology. While cognitive behavioral therapy (CBT) and interpersonal therapy (ITP) are the most investigated interventions, psychodynamic psychotherapies have been less explored. The aim of this paper is to systematically review literature data on the efficacy of short-term psychodynamic psychotherapy (STPP) in combination with antidepressants in the treatment of depressive disorders, focusing on both short and long-term results and on potential mo

A Psychoanalytic-Derived Brief Psychotherapeutic Approach in the Treatment of Major Depression: Monotherapy Studies.

Over the years, short term psychodynamic therapy (STPP) has been broadly researched in order to evaluate its efficacy in the treatment of major psychiatric disorders. In particular, a consistent number of studies focused on assessing clinical outcomes of the principal psychodynamic techniques in treating depressive disorders. We conducted a narrative review in which we aimed to evaluate the efficacy of STPP in monotherapy in major depressive disorder and to assess possible features that may correlate with its clinical use. Databases searched were PubMed, Ovid, Scopus, PsycINFO and Cochrane Lib

Psychodynamic psychotherapy for children and adolescents: a meta-analysis of short-term psychodynamic models.

OBJECTIVE: Psychodynamically based brief psychotherapy is frequently used in clinical practice for a range of common mental disorders in children and adolescents. To our knowledge, there have been no meta-analyses to evaluate the effectiveness of these therapies. METHOD: After a broad search, we meta-analyzed controlled outcome studies of short-term psychodynamic psychotherapies (STPP, 40 or fewer sessions). We also performed sensitivity analyses and evaluated the risk of bias in this body of studies. RESULTS: We found 11 studies with a total of 655 patients covering a broad range of conditi

Stepped care: an alternative to routine extended treatment for patients with borderline personality disorder.

This review examined evidence supporting stepped care for borderline personality disorder as an alternative to routine extended treatment. Empirical studies have shown that patients with borderline personality disorder have a heterogeneous course, but symptomatic improvement can sometimes be relatively rapid. Currently, there is no evidence that any long-term treatment is superior to briefer interventions for borderline personality disorder. Long-term therapy may not be necessary for all patients, and its routine use leads to access problems. A stepped-care model, similar to models applied to

Brief group psychoeducation for bulimia nervosa: assessing the clinical significance of change.

Forty-one women with bulimia nervosa completed a brief intervention that was designed to promote symptom management. Cutoff scores for the determination of reliable and clinically significant change were calculated for several psychometric instruments that are commonly used as outcome measures in the field of eating disorders. Using these statistical procedures to assess therapeutic change, this study documents the diversity of outcomes that individuals reported following their participation in the intervention. As expected, there was a differential reporting of clinically significant change i

Short-term psychodynamic psychotherapy for social anxiety disorder: a meta-analysis of randomized controlled trials.

BACKGROUND: Although established treatments for social anxiety disorder (SAD) are effective, a substantial proportion of patients does not respond. This meta-analysis evaluated the efficacy of psychodynamic therapy (PDT) by assessing its short- and long-term effects on social anxiety symptoms, as well as on secondary measures of depressive symptoms. METHODS: A systematic literature search following PICO criteria identified randomized controlled trials (RCTs) of PDT for SAD published since 1980. Study quality was assessed using the Randomized Controlled Trial Psychotherapy Quality Rating Scale

The Emergence of a Stereotypic Movement During Intensive Short-Term Dynamic Psychotherapy in a Patient with Obsessive Compulsive Disorder: A Case Report.

No previous research has investigated the emergence of stereotypic movements in patients during psychotherapeutic sessions, nor have such movements been mentioned in the numerous articles on Habib Davanloo's technique of intensive short-term dynamic psychotherapy. In this article, we present two clinical vignettes from early intensive short-term dynamic psychotherapy sessions, using Davanloo's technique, with a patient who was observed to have an emerging stereotypic movement. These vignettes, extracted from the videotaped sessions using audiovisual technology, concern a young woman who is kno

Which patients benefit specifically from short-term psychodynamic psychotherapy (STPP) for depression? Study protocol of a systematic review and meta-analysis of individual participant data.

INTRODUCTION: Short-term psychodynamic psychotherapy (STPP) is an empirically supported treatment that is often used to treat depression. However, it is largely unclear if certain subgroups of depressed patients can benefit specifically from this treatment method. We describe the protocol for a systematic review and meta-analysis of individual participant data (IPD) aimed at identifying predictors and moderators of STPP for depression efficacy. METHOD AND ANALYSIS: We will conduct a systematic literature search in multiple bibliographic databases (PubMed, PsycINFO, Embase.com, Web of Science

Psychotherapy versus usual care in pediatric migraine and tension-type headache: a single-blind controlled pilot study.

BACKGROUND: Despite growing interest in psychotherapy in child and adolescent headache, efficacy studies in this research field have focused mainly on cognitive-behavioral therapies. Whereas relaxation and cognitive-behavioral techniques, in particular, have been found to reduce the intensity and frequency of headache in children and adolescents, data on psychodynamic psychotherapy in this population are lacking. Our aim was to explore the effectiveness of a brief psychodynamic psychotherapy program in the treatment of idiopathic headache in childhood and adolescence. METHODS: Thirty-three new

Racialized Health Care Dynamics in Time-Limited Psychodynamic Psychotherapy: A Case of a Patient Awaiting Heart Transplantation.

Racial inequities within both heart transplantation and psychotherapy access have been well documented. In this report, we describe the case of a patient from a racially/ethnically minoritized background awaiting heart transplantation for whom panic attacks significantly impacted his quality of life and presented a barrier to transplantation. This case illustrates the novel utilization of time-limited psychodynamic psychotherapy exploring racialized health care dynamics as a precipitant to psychological symptoms. We highlight how racialized treatment experiences can contribute to symptoms of p

Professionally delivered interventions for gambling disorder.

PURPOSE OF REVIEW: To assess recent developments in professionally delivered interventions for gambling disorder. RECENT FINDINGS: During the past 24 months a number of reviews and individual studies have been published. Collectively they assess the efficacy and effectiveness of a range of interventions and identify methodological and other shortcomings. These studies advance knowledge with respect to pharmacological and psychological treatments including brief interventions delivered face-to-face and in other ways. Increased attention has been given to patient diversity, comorbidity, relapse

Prognostic factors in bulimia nervosa.

Factors predictive of outcome were sought from the data from a controlled study of the treatment of bulimia nervosa. Four different indices of outcome were used, and outcome was assessed at four points during a 12-month, treatment-free follow-up period. It was found that the pre-treatment level of self-esteem was a consistent predictor of outcome with those patients having low self-esteem responding least well to treatment. There were no other consistent predictors of outcome.

Intensive Short-Term Dynamic Psychotherapy (ISTDP) associated with healthcare reductions in patients with functional seizures.

Intensive Short-Term Psychodynamic Therapy (ISTDP) has demonstrated promising evidence for the treatment of Functional Neurological Disorders (FND) including functional seizures. This paper aimed to further examine the therapeutic effects of a 3-session course of this treatment focusing on its potential to maintain reduced healthcare utility within a group of patients with complex difficulties, across an extended time period, post-therapy. The original study followed a mixed methods case series design and recruited 18 patients from secondary adult mental health care and specialist neurology se

Structured group treatment of couples experiencing sexual dysfunctions.

This paper describes a structured, time-limited method for treating couples in which both people are experiencing sexual dysfunctions. In our previous research, we found that group treatment was as effective as single-couple treatment for treating men experiencing premature ejaculation and women experiencing secondary orgasmic dysfunction. In this article we have described our procedures for evaluating and selecting group participants, our 12-week structured group treatment format, the group process, common problems and resistances, and our methods for assessing treatment outcome. Our structur

From traumatic attachment to somatization disorder.

OBJECTIVE: This paper emphasizes attachment trauma as an important etiological factor in somatization disorder and psychoanalytic psychotherapy as an effective treatment. METHOD: Weekly psychotherapy with an 18 year old with somatization disorder, using the Conversational Model, is described. CONCLUSION: Early cumulative trauma has far-reaching health consequences. Brief psychotherapy produces symptom relief, and long-term analytic therapy is necessary for sustained improvement.
