USCAP Specialty Conference: case 1-type I pleuropulmonary blastoma.

Pleuropulmonary blastoma (PPB) was defined in 1988 by Manivel et al. in a series describing 11 intrathoracic pulmonary neoplasms in young children. The PPB is a unique peripheral pulmonary or pleural-based tumor of childhood that is characterized in its earliest form as a bland-appearing multiloculated cyst with small foci of tumor cells and in later forms as mixed and predominantly primitive, overtly malignant neoplasms. Prior to the introduction of the PPB as a distinct entity, this tumor had been reported in the literature as pulmonary blastoma, sarcoma arising in mesenchymal cystic hamarto

[Pleuropulmonary blastoma, case report].

Pleuropulmonary blastoma is a rare lung tumor of childhood that can occur with cystic or solid lesions, as a radiological finding with or without respiratory symptoms. We report the case of a 2 year old toddler in his first pulmonary obstructive episode with suspected toracic malformation of the left upper lobe in his chest x-ray and tomography. Surgery was performed showing cystic malformation of the left upper lobe. We received the pathology report with diagnosis of type I pleuropulmonary blastoma. He began follow-up with Oncology initiating treatment with cyclophosphamide and vincristine, w

[Adult pulmonary blastoma:a case report].

We reported a case of pulmonary blastoma treated with surgical resection, in which we analyzed the clinical features, pathological characteristics, diagnosis, differential diagnosis, treatment, and prognosis. The clinical features, histopathology, and immunohistochemical staining results were observed, along with perioperative adjuvant therapy. The patient, a 21-year-old female, was admitted with the chief complaint of "lung mass discovered during physical examination for over one year." PET-CT revealed a metabolically active soft tissue mass measuring 7.8 cm×8.4 cm in the right lower lobe, wi

Infant pleuropulmonary blastoma: report of a rare case and review of literature.

In infants, pleuropulmonary blastoma is a rare but aggressive tumor. The typical histopathological presentation includes the aggregation of malignant primitive small cells, usually observed in sheets. So as to provide proper and timely treatment, the differential diagnosis includes pulmonary blastoma, sarcomatoid mesothelioma, fetal rhabdomyoma, synovial sarcoma, and primitive neuroectodermal tumor. Herein, we will present one male pediatric patient with pleuropulmonary blastoma. The patient was a 4-month-old male infant, who had a prolonged cough and dyspnea for 4 months that was complicated

Pleuropulmonary blastoma: A report of three cases and review of literature.

Pleuropulmonary blastoma is a rare and highly aggressive pulmonary malignancy in children. Clinically, the malignancy is often mistaken for symptoms of respiratory tract infection or pneumothorax. The neoplasm is histologically characterized by primitive blastema and a malignant mesenchymal stroma that demonstrates multidirectional differentiation. The patients with PPB are managed by multimodal therapy. We present a report of 3 cases of histopathologically diagnosed pleuropulmonary blastoma. The patients presented with chief complaints of difficulty in breathing, cough, fever and chest pain.

Sarcomatoid neoplasms of the lung and pleura.

Sarcomatoid neoplasms of the lung and pleura are rare tumors that present a complex differential diagnosis, making them challenging for surgical pathologists. In the lung, the main tumors are the sarcomatoid carcinomas, including pleomorphic carcinoma, spindle cell carcinoma, giant cell carcinoma, carcinosarcoma, and pulmonary blastoma. They are characterized by histologic heterogeneity; molecular data support their origin from a pluripotent stem cell that undergoes neoplastic transformation with divergent epithelial and sarcomatous differentiation. Diagnosis is difficult in small biopsy speci

A Rare Case of Childhood Stiff Person Syndrome Associated With Pleuropulmonary Blastoma.

INTRODUCTION: Stiff person syndrome is a rare autoimmune, neurological disorder characterized by progressive rigidity and episodic painful spasms, predominantly affecting the proximal limbs and axial muscles, and leading to progressive disability. We report the case of a child who developed symptoms compatible with stiff person syndrome during treatment for pleuropulmonary blastoma. PATIENT DESCRIPTION: A 3-year, 5-month-old girl was admitted for gradually worsening postural tremor, painful spasms, and generalized stiffness. Since the age of 3 years, she had been on adjuvant chemotherapy for

Pleuropulmonary blastoma and the challenge of distinguishing the cystic form (type I) from other congenital lung malformations.

Pleuropulmonary blastoma is a rare pediatric lung tumor of mesenchymal origin, with prognosis varying by subtype. While types II and III are usually identifiable on imaging due to their solid tumor components, the purely cystic type I represents a particular diagnostic challenge. In imaging, it is often difficult to distinguish from benign congenital lung malformations such as congenital pulmonary airway malformation (CPAM, formerly congenital cystic adenomatoid malformation or CCAM), sequestration, or bronchogenic cyst. Accurate differentiation, however, is clinically relevant, as misclassifi

[Newborn with lung mass of uncommon etiology].

Thoracic masses in neonates usually respond to congenital anomalies of the respiratory system. They comprise a large number of diseases that can compromise the development of larynx, trachea, bronchi, pulmonary parenchyma and diaphragm or chest wall. Diagnosis is carried out during prenatal period by ultrasound in most cases. In others, respiratory distress is diagnosed during post-birth examination or later as a radiological finding. We present the case of a full term newborn with prenatal diagnosis of cystic "lung mass". Physical examination was unremarkable except for decreased breath sound

Imaging findings in pleuropulmonary blastoma.

BACKGROUND: Pleuropulmonary blastoma (PPB) is a rare tumor of the chest seen in young children and recently recognized as distinct from the pulmonary blastoma typically encountered in adults. OBJECTIVE: The purpose of this study is to review and describe the findings of PPB on radiography and CT in four patients. METHODS: Radiographs and CT findings were reviewed in four patients with pathologically proven PPB. RESULTS: All four cases demonstrated large masses in the right hemithorax with heterogeneous low attenuation, pleural effusion, contralateral mediastinal shift, and lack of chest wal

Fetal lung interstitial tumor (FLIT): a case report and review of prenatal imaging features.

Fetal lung interstitial tumor (FLIT) is a rare benign mesenchymal lung tumor that is often mistaken for more common congenital lung lesions such as congenital pulmonary airway malformation (CPAM). Prenatal detection is exceedingly uncommon. Here, we report a case of prenatally detected FLIT presenting in the third trimester with rapid interval growth, mediastinal shift, and fetal hydrops, raising concern for a malignancy, including pleuropulmonary blastoma. Prenatal ultrasonography and fetal magnetic resonance imaging (MRI) demonstrated a rapidly enlarging right lung mass first identified late

[Pneumoblastomas. Apropos of 2 cases].

Pulmonary blastomas, also referred to as embryonal carcinosarcomas or, in the past, embryomas, are rare tumors of which thirty cases have been described in English, German, and Italian publications. We present two typical pulmonary blastomas one in a 43-year-old man and the other in an infant of six months, the youngest case yet reported. Several arguments exist in favor of the embryonic origin of these neoplams: their histogenesis, their resemblance to nephroblastomas, their bifocal or multifocal presence in certain cases, and in about one third of all cases their discovery in patients under

Congenital pulmonary airway malformations: state-of-the-art review for pediatrician's use.

UNLABELLED: Congenital pulmonary airway malformations or CPAM are rare developmental lung malformations, leading to cystic and/or adenomatous pulmonary areas. Nowadays, CPAM are diagnosed prenatally, improving the prenatal and immediate postnatal care and ultimately the knowledge on CPAM pathophysiology. CPAM natural evolution can lead to infections or malignancies, whose exact prevalence is still difficult to assess. The aim of this "state-of-the-art" review is to cover the recently published literature on CPAM management whether the pulmonary lesion was detected during pregnancy or after bir

Congenital anomalies of the large intrathoracic airways.

Congenital lung lesions are numerous but rare in individual clinical practice. They do require close multidisciplinary collaboration between health care professionals. This educational review will focus on the pathophysiology, clinical manifestations, surgical approaches, and anesthetic management of congenital anomalies of the large intrathoracic airways: congenital tracheal stenosis, tracheal agenesis, tracheal diverticulum, bronchial anomalies (tracheal, esophageal, or bridging bronchus), congenital lung malformations, lung sequestrations and Scimitar syndrome, lobar emphysema, Williams-Cam

Pulmonary lipoblastoma in an 18-month-old child: a unique tumor in children.

Lipoblastoma, a rare benign tumor of adipose tissue, typically occurs in the superficial or deep layers of soft tissue on the trunk or extremities. Other sites of occurrence include the head, neck, and retroperitoneum. Lipoblastoma of the chest wall and parietal pleural have been reported, but occurrence within the lung has not been previously described. We report a case of pulmonary lipoblastoma in a young child presenting with complete opacification of the left hemithorax and mediastinal shift on chest radiograph. A lobectomy was performed, and the diagnosis was made by histological examinat

Selective dicer suppression in the kidney alters GSK3β/β-catenin pathways promoting a glomerulocystic disease.

Dicer is a crucial enzyme for the maturation of miRNAs. Mutations in the Dicer gene are highly associated with Pleuro Pulmonary Blastoma-Family Dysplasia Syndrome (PPB-FDS, OMIM 601200), recently proposed to be renamed Dicer syndrome. Aside from the pulmonary phenotype (blastoma), renal nephroma and thyroid goiter are frequently part of Dicer syndrome. To investigate the renal phenotype, conditional knockout (cKO) mice for Dicer in Pax8 expressing cells were generated. Dicer cKO mice progressively develop a glomerulocystic phenotype coupled with urinary concentration impairment, proteinuria an

Pleuropulmonary blastoma: a rare pathology with an even rarer presentation.

BACKGROUND: Pleuropulmonary blastoma is among the rarest tumors of childhood. Three types have been described: cystic, solid, and mixed. To date, bilateral disease has not been documented. METHODS AND RESULTS: A 5-week-old girl presented with a history of fever. Chest x-ray showed bilateral diffuse cystic lesions. Bowel obstruction developed that required laparotomy. Multiple small bowel polyps were resected. The patient was readmitted 4 months later with deteriorating respiratory status. She underwent sequential thoracotomies for resection of multiple bullae under high-frequency oscillatory

Cystic lung disease in infants and children.

Cysts within the lung are one of the more common pulmonary pathological findings seen in an active pediatric surgical pathology service. Of both developmental and acquired origin, they may present as respiratory distress in infants or may be asymptomatic lesions incidentally discovered by images studies of the thoracic for "non-pulmonary" reasons. The most frequently seen developmental cysts of the lung are those of Congenital Pulmonary Airway Malformations, types 1, 2 & 4. Other congenital cystic lesions include bronchogenic cysts (usually in older patients) and some of the enteric duplicatio
