Precise quantification of pressure flow waveforms of a pulsatile ventricular assist device.

Unreliable quantification of flow pulsatility has hampered many efforts to assess the importance of pulsatile perfusion. Generation of pulsatile flow depends upon an energy gradient. It is necessary to quantify pressure flow waveforms in terms of hemodynamic energy levels to make a valid comparison between perfusion modes during chronic support. The objective of this study was to quantify pressure flow waveforms in terms of energy equivalent pressure (EEP) and surplus hemodynamic energy (SHE) levels in an adult mock loop using a pulsatile ventricle assist system (VAD). Diurnal variation in pulsatile ocular blood flow in normal and glaucomatous eyes.

Ocular blood flow, in particular to the optic nerve head, is considered important in determining the extent of glaucomatous damage. The 24-hour variation in the pulsatile component of ocular blood flow (POBF) was measured using a pneumotonometer linked to the Langham Ocular Blood Flow System. Intraocular pressure (IOP), ocular pulse amplitude, POBF, systemic blood pressure and heart rate were recorded at three-hourly intervals over a 24-hour period in 10 ocular hypertensives, eight patients with primary open angle glaucoma (POAG) treated with timolol eyedrops (G timolol 0.25%), and eight ocula

Postural studies in pulsatile ocular blood flow: I. Ocular hypertension and normotension.

Measurements of pulsatile ocular blood flow (POBF) have been recorded in a group of healthy, ocular normotensive volunteers and ocular hypertensive patients recruited from outpatients. Use of a pneumotonometric probe linked to a Langham ocular blood flow system enabled readings of intraocular pressure and its variation with heart rate (ocular pulse) to be taken in erect and supine positions. Pulsatile ocular blood flow was calculated from these values by means of the pressure-volume relationship previously described for living human eyes. Assumption of the supine posture was accompanied by a s

A cardiac phantom and pulsatile flow pump for magnetic resonance imaging studies.

Fast scan magnetic resonance imaging (MRI) acquisitions are a rapid noninvasive means of evaluating the cardiovascular system. Because the appearance of flowing blood is highly variable, the interpretation of these images is sometimes difficult. A nonferromagnetic phantom that could generate lifelike pulsatile flow and also simulate the motions of the beating heart would facilitate image interpretation. This paper describes an MRI-compatible cardiovascular phantom that mimics the motions of the heart and also creates physiologic pulsatile flow. The phantom consists of a ventricle and an air pu

Is additional pulsatile pulmonary blood flow beneficial to patients with bidirectional Glenn?

OBJECTIVE: To compare the results of bidirectional Glenn when performed with or without pulsatile pulmonary blood flow in a cohort of patients with a single ventricle. METHODS: Records of 212 patients undergoing staged single ventricle palliation during a 10-year period were retrospectively reviewed. Of those, 103 (33 in pulsatile group A and 70 in nonpulsatile group B) were selected. RESULTS: Demographics and pre- and intraoperative variables were comparable for both groups. There was no difference in oxygen saturations immediately after the bidirectional Glenn in the 2 groups. The duration

Differential effects of pulsatile versus steady flow on coronary endothelial membrane potential.

Steady shear stress stimulates transient hyperpolarization coupled to calcium-sensitive potassium (KCa) channels and sustained depolarization linked to chloride-selective channels. Physiological flow is pulsatile not static, and whereas in vivo data suggest phasic shear stress may preferentially activate KCa channels, its differential effects on both currents remain largely unknown. To determine this interaction, coronary endothelial cells were cultured in glass capillary flow tubes, loaded with the voltage-sensitive dye bis-(1,3-dibutylbarbituric acid)trimethine oxonol, and exposed to constan

Postural studies in pulsatile ocular blood flow: II. Chronic open angle glaucoma.

The pulsatile ocular blood flow (POBF) has been recorded in 15 patients with chronic open angle glaucoma. Measurements were performed during regular treatment with timolol 0.25% eyedrops, two weeks after withdrawal of this treatment, and then a further two weeks after its reinstitution. Readings were taken with subjects in both the erect and supine positions by means of a pneumotonometric probe to measure intraocular pressure (IOP), linked to a Langham ocular blood flow system. Assumption of the supine posture was associated with a significant increase in IOP in all phases of the study. Treatm

Pulsatile flow pump based on an iterative controlled piston pump actuator as an in-vitro cardiovascular flow model.

In-vitro cardiovascular experiments provide an effective means for characterizing structural or hemodynamic features of medical devices before they are tested on animals or used in clinical practice. In-vitro experiments simulate complicated cardiovascular systems with blood pumps, vessels and valves, but without human or animal subjects. Therefore, such experiments are free from ethical issues and present large cost savings in comparison to in-vivo experiments. In this study, we aimed to design a fully programmable pulsatile flow pump that can consistently and accurately reproduce a wide rang

Development of in-vitro pulsatile flow generator for evaluating the performance of hemodialysis catheters.

Hemodialysis (HD) using an HD catheter is performed widely on renal failure patients. The catheter was evaluated using the recirculation ratio in pre-clinical status, which is a crucial index indicating its performance. However, pre-clinical in-vivo experiments have limitations: high cost, and ethical issues. Hence, computational and in-vitro methods have been developed as alternatives. However, computational methods require fluid dynamic knowledge, whereas in-vitro experiments are complicated and expensive. In this study, we developed a pulsatile flow generator to mimic blood flow achieving c

A model for estimating the blood flow of the POLVAD pulsatile ventricular assist device.

OBJECTIVE: The aim of this work was to model the blood flow rate of the POLVAD-MEV pulsatile ventricular assist device (VAD). An adequate flow rate is crucial to restore physiological cardiac output. Unfortunately, during clinical heart support, neither blood flows nor pressures can be measured within the device. In general, the flow rate depends on the control parameters and patient conditions. However, the patient's hemodynamic parameters are not constantly monitored. Therefore, blood flow must be evaluated based on the standard measurements from the device control unit. METHODS: The model

Change in Pulsatile Cerebral Arterial Pressure and Flow Waves as a Therapeutic Strategy?

While intracranial pressure (ICP), arterial pressure and transcranial middle cerebral artery flow velocity (MCAFV) are often monitored in unconscious patients following stroke or head injury, the value of waveform indices has not been fully established. We retrospectively analysed the data of eight adults (aged 19-36 years) with closed head injury who had spontaneous and repeated episodes of elevated ICP (i.e. "plateau waves"). MCAFV was measured using transcranial Doppler, ICP using a Codman catheter and radial artery pressure using cannulation. Ascending aortic pressure (AAP) was generated f

Defining pulsatility during continuous-flow ventricular assist device support.

Continuous-flow ventricular assist devices (CVADs) have gained widespread use as an effective clinical therapy for patients with advanced-stage heart failure. Axial and centrifugal CVADs have been successfully used as bridge-to-transplant and destination therapy. CVADs are smaller, more reliable, and less complex than the first-generation pulsatile-flow ventricular assist devices. Despite their recent clinical success, arteriovenous malformations, gastrointestinal bleeding, hemorrhagic strokes, aortic valve insufficiency, and valve fusion have been reported in heart failure patients supported

Sex differences in cerebral pulsatility and damping: A 4D flow MRI study.

Cerebral pulsatility is a potential marker of cerebrovascular health, yet little is understood about sex differences in cerebral pulsatility with age, especially within different cerebral arteries. Additionally, cerebral damping can blunt cerebral pulsatility and might decline with age. Therefore, we aimed to identify sex differences in cerebral pulsatility and damping across the adult lifespan. Forty-three young, 67 middle-aged and 54 older adults had cerebral haemodynamics measured in the internal carotid arteries (ICAs), middle cerebral arteries (MCAs) and basilar artery using 4D flow MRI.

Measuring global cerebrovascular pulsatility transmission using 4D flow MRI.

Pulse wave encephalopathy (PWE) is hypothesised to initiate many forms of dementia, motivating its identification and risk assessment. As candidate pulsatility based biomarkers for PWE, pulsatility index and pulsatility damping have been studied and, currently, do not adequately stratify risk due to variability in pulsatility and spatial bias. Here, we propose a locus-independent pulsatility transmission coefficient computed by spatially tracking pulsatility along vessels to characterise the brain pulse dynamics at a whole-organ level. Our preliminary analyses in a cohort of 20 subjects indica

Dynamic modeling of the outlet of a pulsatile pump incorporating a flow-dependent resistance.

Outlet tube models incorporating a linearly flow-dependent resistance are widely used in pulsatile and rotary pump studies. The resistance is made up of a flow-proportional term and a constant term. Previous studies often focused on the steady state properties of the model. In this paper, a dynamic modeling procedure was presented. Model parameters were estimated by an unscented Kalman filter (UKF). The subspace model identification (SMI) algorithm was proposed to initialize the UKF. Model order and structure were also validated by SMI. A mock circulatory loop driven by a pneumatic pulsatile p

LES of additive and non-additive pulsatile flows in a model arterial stenosis.

Transition of additive and non-additive pulsatile flows through a simple 3D model of arterial stenosis is investigated by using a large eddy simulation (LES) technique. We find in both the pulsatile cases that the interaction of the two shear layers, one of which separates from the nose of the stenosis and the another one from its opposite wall, causes recirculation in the flow downstream of the stenosis where the nature of the transient flow becomes turbulent. The strength of this recirculation is found to be quite high from the non-additive pulsations when the flow Reynolds numbers, Re>or=15

Blood Flow Velocity: a Decision Tool for Stenting Indication in Venous Pulsatile Tinnitus.

BACKGROUND AND PURPOSE: Lateral sinus stenosis is the most common cause of venous pulsatile tinnitus (VPT). Stenting is an effective treatment after demonstration of a trans-stenotic pressure gradient; however, pressure measurement has many technical limitations. In 2018, a study showed that a combined approach with intravascular velocity measurement could be effective in identifying most appropriate candidates for stenting. The aim of the present study was to evaluate a new strategy using this biomarker for the indication of stenting even without a significant pressure gradient. MATERIAL AND

Numerical simulation of pulsatile blood flow characteristics in a multi stenosed coronary artery.

BACKGROUND: Coronary artery disease is reported as one of the most common sources of death all over the world. The presence of stenosis (plaque) in the coronary arteries results in the restriction of blood supply, which leads to myocardial infarction. OBJECTIVE: The aim of this study was to investigate the effect of multi stenosis on hemodynamics parameters in idealized coronary artery models with varying degrees of stenosis and interspace distance between the stenosis. METHODS: A finite volume-based software package (Ansys CFX version 17.2) was employed to model the blood flow. The hemodyna
