It is widely accepted that there is a finite failure rate when treating onychomycosis; a case report from the author serves as an example of multiple failures following treatments of varying duration and combinations, including oral-local, then oral-oral therapies. This article discusses the results obtained by authors who have varied the standard duration of therapy, increased the dosage of the oral antifungal agents, or have used two oral drugs in treating this condition.

We report a patient who began to have an unusual form of nail pigmentation after 8 months of receiving monthly pulses of dexamethasone-cyclophosphamide. A 74-year-old woman was diagnosed with invasive breast ductal carcinoma, cT2N0M1 (PUL), stage Ⅳ, and treated with abemaciclib and letrozole. The day after drug administration, the patient developed a fever of 38℃ and dyspnea upon exertion, and was diagnosed with drug-induced pneumonia. Steroid pulse therapy was administered during hospitalization, and the patient was discharged after the dose of prednisolone was gradually reduced. This case shows that, when abemaciclib is administered, drug-induced lung injury can occur within 1 week.

Recent reports demonstrate that failure of artemisinin-based antimalarial therapies is associated with an altered response of early blood stage Plasmodium falciparum. This has led to increased interest in the use of pulse assays that mimic clinical drug exposure for analysing artemisinin sensitivity of highly synchronised ring stage parasites. We report a methodology for the reliable execution of drug pulse assays and detail a synchronisation strategy that produces well-defined tightly synchronised ring stage cultures in a convenient time-frame.

Data on office blood pressure (BP) and pulse rate as well as information on adverse effects were collected from 148 patients with mild and moderate hypertension during 12 week treatment with long acting lipophilic selective beta-adrenoreceptor blocker betaxolol (20 mg/day).
