[Massive hemorrhage following multiple epidural punctures as a late complication in thrombocythemia].

We present a female patient in whom numerous lumbar punctures for epidural anaesthesia were followed by a delayed development of a large subcutaneous haematoma in the lumbar region. The outcome was fatal. The patient's basic disease was found to be chronic myeloic leukaemia that had caused an excessive elevation of platelet count. We, therefore, suggest that lumbar punctures for epidural anaesthesia should be approached with extreme care in cases of thrombocytaemia. If possible, repeated punctures for the placement of epidural catheters should be avoided.

Puncturing Plaques.

PURPOSE: To test and validate magnetic resonance imaging (MRI) sequences for peripheral artery lesion characterization and relate the MRI characteristics to the amount of force required for a guidewire to puncture peripheral chronic total occlusions (CTOs) as a surrogate for immediate failure of endovascular therapy. METHODS: Diseased superficial femoral, popliteal, and tibial artery segments containing 55 atherosclerotic lesions were excised from the amputated limbs of 7 patients with critical limb ischemia. The lesions were imaged at high resolution (75 μm3 voxels) with T2-weighted (T2W) an

Lumbar puncture.

Lumbar puncture is the original neurological procedure. The technique is learned by the student first through observation then performance under supervision by a physician who has mastered the technique. It is one of the more difficult procedures in medicine because success is not only dependent on the skill of the physician but also the size, anatomy, and comfort of the patient. The technique, complications, contraindications, and treatment of the complications are reviewed. A discussion of the composition of normal cerebrospinal fluid (CSF) is included, as well as tables for diagnostic CSF s

Puncture wounds.

Puncture wounds are common and challenging problems facing the physician. In general, puncture wounds can be classified into two distinct types. In one type a child usually sustains a puncture wound of the foot. The other group of puncture wounds occurs among hospital employees. These seemingly innocuous injuries may have serious sequelae that can be averted by prompt appropriate care of the wound. The fundamental trends of treatment of puncture wounds include cleansing the wound, debridement of any necrotic or devitalized tissue, and appropriate immunoprophylaxis.

Puncture technique and postural postdural puncture headache. A randomised, double-blind study comparing transverse and parallel puncture.

BACKGROUND: This clinical study was conducted in order to investigate the effect of two different orientations of the bevel during dural puncture on development of postural postdural puncture headache (PPDPH). METHODS: Two hundred and eighteen patients aged 18 to 50 years scheduled for minor non-obstetric surgery using spinal anaesthesia (SA) were included in this randomised, double-blind study. Dural puncture was performed using a 0.42 mm O.D. (27-g) Quincke spinal needle with the orientation of the bevel parallel or transverse relative to the longitudinal axis of the dural cylinder. All pat

Computed Tomography-Guided Microcoil Localization of Pulmonary Nodules: Effects of Multiple Punctures.

BACKGROUND: The aim of the study is to analyze the effect of multiple punctures in computed tomography (CT)-guided microcoil localization of pulmonary nodules with other risk factors for common complications. METHODS: Consecutive patients who underwent CT-guided microcoil localization and subsequent video-assisted thoracoscopic surgery (VATS) between January 2020 and February 2021 were enrolled. Nodules successfully located after only one puncture were defined as the single puncture group, and nodules requiring two or more punctures were defined as the multiple puncture group. Transcatheter creation of bidirectional cavopulmonary connections by needle punctures in two patients.

We report on two patients who received a transcatheter cavopulmonary connection by a needle puncture under deep conscious sedation. In both patients, the vessel-to-vessel connection was achieved by a venous access into the superior caval vein and direct needle puncture of the pulmonary artery. The two cavopulmonary anastomoses were held open by a covered stent and a bare-metal stent, respectively.

JOURNAL CLUB: Incidence of Complications Following Fluoroscopically Guided Lumbar Punctures and Myelograms.

OBJECTIVE: Headaches due to CSF leak are a well-described complication of dural puncture. It is uncertain how long patients should be observed after dural puncture to reduce the risk of headache. Most of the literature has focused on dural punctures performed without fluoroscopic guidance. The purpose of this study was to determine the incidence of complications from fluoroscopically guided dural punctures, with attention to predictive factors such as the length of bed rest after the procedure. MATERIALS AND METHODS: We retrospectively reviewed 2141 fluoroscopically guided dural punctures per

Use of the StarClose device for closure of inadvertent subclavian artery punctures.

PURPOSE: The placement of central catheters is a common procedure. It is also associated with multiple known complications. One of the potential complications that carry high morbidity and morality is arterial puncture and cannulation. Herein, we describe five case reports of a central line that was inadvertently placed in the subclavian artery and successfully removed using a StarClose device (Abbott Laboratories, Redwood CA). METHODS/RESULTS: A retrospective chart review of a prospectively maintained database was performed. We identified five cases of inadvertent subclavian artery cannulati

Term newborns who are at risk for sepsis: are lumbar punctures necessary?

OBJECTIVES: To determine: (1) whether a lumbar puncture (LP) is indicated in asymptomatic full-term newborns delivered by mothers at risk of intrapartum sepsis; and (2) whether gentamicin improves bacterial coverage for such newborns when used with ampicillin. DESIGN: A retrospective chart review from 1987 through 1993 of all newborns with positive blood and/or cerebrospinal fluid cultures in the first 7 days of life. METHODS: Pregnant women were screened in the second trimester for group B streptococci and given ampicillin during labor if two or more risk factors were present: group B strep

Evaluation of routine lumbar punctures in newborn infants with respiratory distress syndrome.

Infants with respiratory distress syndrome are routinely evaluated for infection which commonly includes a lumbar puncture. In this study cerebrospinal fluid (CSF) examination failed to elicit evidence for meningitis in 238 consecutively admitted infants with respiratory distress syndrome evaluated during the first 24 hours of life. Blood cultures were obtained in all; suprapubic or catheterized urine was obtained in 163 infants; CSF was collected successfully in 203 infants. Seventeen infants demonstrated positive blood cultures: 7 Streptococcus, 5 Staphylococcus, 3 Haemophilus influenzae, 1

Pulmonary and aortic punctures--complications of an attempt at internal jugular venipuncture.

A case of pulmonary and aortic punctures occurred during an attempted internal jugular venipuncture in a patient with a dilated aortic arch. Procedural errors contributing to the complications included an inexperienced operator, inappropriate medial and caudal angulation of the needle, and failure to use a "finder" needle or Seldinger technique. Use of proper procedural technique will reduce the risk of complications in performing this procedure.

[Embolisms in punctures and catheterization of the subclavian veins in young children].

Observations of thrombic and tissue (skin, muscle) embolism of small branches of the pulmonary artery in two infants of 2 1/2 months suffering from acute respiratory viral diseases and a child of 3 1/2 years who died of scarlet fever are presented. Complications of this kind due to insufficient observations of the rules of puncture and catheterization of central veins, particularly in young infants, are not infrequent. In some cases, they may aggravate the course of the main disease.

Thrombocytopenia and Adverse Bleeding Events in Neonatal Lumbar Punctures in the Emergency Department.

OBJECTIVE: Lumbar punctures are performed as part of a routine evaluation of neonates with fever in the emergency department. Current recommendations regarding performing a lumbar puncture in the presence of thrombocytopenia exist for children with leukemia and other cancers; however, there are no such recommendations for the general neonatal population. This study assesses the frequency of thrombocytopenia and adverse bleeding events in neonates who undergo a lumbar puncture to determine whether a complete blood count, and specifically the platelet count, should be reviewed before performing

Are C1-2 punctures for routine cervical myelography below the standard of care?

BACKGROUND AND PURPOSE: Recently, the performance of C1-2 punctures for cervical myelography was challenged in a medicolegal proceeding as being below the standard of care. We sought to examine current neuroradiologic practices and opinions on the technique. MATERIALS AND METHODS: An 11-question survey was sent to 120 program directors of neuroradiology via e-mail links regarding cervical myelography using a C1-2 puncture. Reminders were sent during a 2-month period before data were finalized. RESULTS: Eighty-five of 120 (71%) surveys were returned. In the previous year, 14.3% (12/85) of ins
