Pythium insidiosum is a pathogen that causes disease in both animals and humans. Human infection is rare; however, when it does occur, most patients, especially those having underlying hemoglobinopathy syndromes, such as thalassemia, exhibit a severe form. We identified four isolates of P. insidiosum. Two were recovered from tissue biopsy specimens from thalassemic and leukemic patients, one was derived from brain tissue from a thalassemic patient, and another was isolated from a corneal ulcer from a fourth patient. Western blotting and an enzyme-linked immunosorbent assay (ELISA) were perform

INTRODUCTION: Pythium insidiosum keratitis is an aggressive, vision-threatening corneal infection that closely mimics fungal keratitis clinically and on smear microscopy, frequently delaying targeted therapy and appropriate surgical intervention. Rapid and reliable molecular diagnosis is therefore critical, particularly given the small-volume, low-biomass nature of corneal specimens encountered in routine ophthalmic practice worldwide, especially in endemic regions with limited diagnostic infrastructure.
AREAS COVERED: This review summarizes advances in molecular diagnostics for Pythium insid

PURPOSE: To report the molecular and microbiological diagnosis and clinical profile of 13 patients with Pythium insidiosum keratitis.
METHODS: Phase 1 of the study consisted of DNA sequencing of the ITS region of the rDNA of 162 stocked morphologically unidentified nonconsecutive fungal isolates from corneal scraping of patients with keratitis (2010-2012). Blast and phylogenetic analyses of the sequences showed 9 to be P. insidiosum. A retrospective review of archived photographs of colony and direct microscopy of corneal scrapings and clinical records of the cases were performed. Phase 2 beg

BACKGROUND: The fungus-like organism Pythium insidiosum is the causative agent of a life-threatening tropical infectious disease, pythiosis, which has high rates of morbidity and mortality. A lack of reliable diagnostic tools and effective treatments for pythiosis presents a major challenge to healthcare professionals. Unfortunately, surgical removal of infected organs remains the default treatment for pythiosis. P. insidiosum is an understudied organism. In-depth study of the pathogen at the molecular level could lead to better means of infection control High quality genomic DNA (gDNA) is nee

Pythiosis is a severe infectious disease caused by the oomycete Pythium insidiosum, affecting both humans and animals and often associated with poor therapeutic outcomes. Although triamcinolone acetonide has been used as an adjunctive therapy for equine pythiosis, its antimicrobial activity remains controversial. This study evaluated in vitro susceptibility of 15 clinical P. insidiosum isolates to triamcinolone acetonide, alone and in combination with azithromycin, itraconazole, or amorolfine hydrochloride, using zoospore-based broth microdilution assays. Triamcinolone acetonide and itraconazo

Pythiosis is a neglected infectious disease caused by the aquatic oomycete Pythium insidiosum and remains underrecognized in cattle, particularly in tropical regions. Here, we report the first molecularly confirmed outbreak of bovine pythiosis in the Amazon biome, affecting more than 400 animals raised under extensive production systems and areas with prolonged exposure to standing water. Clinically affected cattle presented ulcerative and exudative cutaneous lesions, predominantly involving the distal limbs. Given the diagnostic challenges associated with pythiosis, etiological confirmation w

PURPOSE: To describe in vivo confocal microscopic features of Pythium insidiosum in patients with Pythium keratitis and compare with those observed in fungal keratitis.
METHOD: We collected in vivo confocal images of the cornea from patients with microbiologically confirmed Pythium and fungal keratitis, analysing five putative distinguishing features: filament width (broad or thin), granularity within the filament (present or absent), filament continuity or traceability, the presence or absence of loops, and the double track sign. Three masked observers were shown images with concealed identi

Pythium insidiosum keratitis (PIK) is a devastating corneal infection resulting in blindness in a large number of cases. Clinically and morphologically, it closely mimics fungal keratitis, and hence is also labeled as "parafungus." Although many clinical studies have documented evidence regarding the virulence of microorganism, and anatomical and functional outcomes, it remains a clinical challenge and diagnostic dilemma for most clinicians. Till today, PIK is being diagnosed and treated with certainty at only limited centers across the globe. But the question is why this is so? Taking this as

A 30-year-old woman with a history of contact lens wear and exposure to swimming pool water in Thailand presented with a non-responsive, progressive corneal ulcer of the right eye. Confocal microscopy evidenced septate linear branching structures, raising suspicion of fungal keratitis. She was promptly treated with topical antibiotics and both topical and intravenous caspofungin plus voriconazole. Worsening of the clinical picture after 1 month of intensive medical therapy led to a large therapeutic penetrating keratoplasty being performed. Corneal cultures grew a mold-like organism, which was

Patients with β-thalassemia are highly susceptible to Pythium insidiosum, the causative agent of human pythiosis, a severe and often life-threatening disease in tropical regions. This vulnerability has been associated with iron overload and immune dysregulation. However, the underlying mechanisms of impaired host defense, especially in neutrophils, remain incompletely understood. This study investigated neutrophil function in response to P. insidiosum zoospores, using neutrophils isolated from β-thalassemia patients and comparing their responses with those from healthy controls. Ex vivo study,

The influence exerted by nonpathogenic Fusarium oxysporum strain Fo47 in triggering cucumber protection against infection by Pythium ultimum was investigated ultrastructurally. Macroscopic and microscopic observations of the pathogen colony in dual cultures revealed that reduction of Pythium growth was associated with marked disorders, including generalized disorganization of the host cytoplasm, retraction of the plasmalemma, and complete loss of the protoplasm. Cytochemical labeling of cellulose with an exoglucanase-gold complex showed that the cellulose component of the host cell walls was s

BACKGROUND Pythium insidiosum keratitis is a rare but sight-threatening disease with a high morbidity rate. It can be misdiagnosed as fungal keratitis in clinic settings. We report a case of severe Pythium insidiosum keratitis in a Chinese child, treated with combined approaches. CASE REPORT A 7-year-old boy from Hainan province in the south of China developed a suppurative corneal ulcer after being in a forest. A mass of hyphae was detected by confocal imaging in vivo, fungal smear test, and histochemical examination. Treatment with Natamycin, fluconazole, and Voriconazole for 1 month was uns

Pythium insidiosum keratitis is a vision-threatening corneal infection that often mimics fungal keratitis, yet it responds poorly to standard antifungals. Historically, approximately 80% of cases have required surgical excision of the cornea. Unlike fungi, P. insidiosum lacks ergosterol in its cell membrane and possesses a cellulose-rich wall with minimal β-1,3-glucan exposure, reducing Dectin-1-mediated detection. Surface hydroxyproline-rich glycoproteins structurally resemble host collagen, potentially engaging inhibitory lectin receptors and dampening early innate immune responses. Delayed
