Optimal regimens using recent radiotherapy (RT) equipment for bleeding gastric cancer (GC) have not been fully investigated yet. We retrospectively reviewed the clinical data of 20 patients who received RT for bleeding GC in our institution between 2016 and 2021. Three-dimensional conformal RT was performed. The effectiveness of RT was evaluated by the mean serum hemoglobin (Hb) level and the number of transfused red blood cell (RBC) units 1 month before and after RT. The median first radiation dose was a BED of 39.9 Gy. The treatment success rate was 95% and the rebleeding rate was 10.5%. The

INTRODUCTION: Since 1980s, computerization has made improvements in radiation therapy delivery from conventional two-dimensional to three-dimensional conformal radiotherapy (2DCRT to 3DCRT) to intensity-modulated radiotherapy (IMRT) and its newer versions. This small study is aimed to compare the existing techniques for planning target volume (PTV) and organ at risk (OAR) dose distribution parameters in postoperative buccal mucosa cases.
MATERIALS AND METHODS: Ten post operative cases of early stage carcinoma buccal mucosa in whom only post operative bed irradiation was indicated was enrolled

BACKGROUND AND PURPOSE: Independent dosimetry audits improve quality and safety of radiation therapy. This work reports on design and findings of a comprehensive 3D conformal radiotherapy (3D-CRT) Level III audit.
MATERIALS AND METHODS: The audit was conducted as onsite audit using an anthropomorphic thorax phantom in an end-to-end test by the Australian Clinical Dosimetry Service (ACDS). Absolute dose point measurements were performed with Farmer-type ionization chambers. The audited treatment plans included open and half blocked fields, wedges and lung inhomogeneities. Audit results were de

INTRODUCTION: Langerhans' cell histiocytosis (LCH) is a proliferative disorder of Langerhans cells, which is seen extremely rarely in adults. Conventional imaging modalities, such as skeletal surveys and bone scans, were accepted to be standard methods for diagnosis; however, 18-fluorodeoxyglucose positron emission tomography (PET-CT) has been increasingly used.
METHODS: We report on a 33-year-old female patient with disseminated LCH treated with radiotherapy and systemic chemotherapy where PET-CT has been used for defining the extent of the disease, RT planning and assessment of treatment re

The objective of this study was to assess the efficacy of modern conformal fractionated radiotherapy (RT) in patients with uncured Cushing's disease (CD) after failed transsphenoidal surgery (TSS). In this retrospective analysis, we reviewed records of patients with CD who received modern conformal fractionated RT between 2001 and 2010. Records were evaluated for frequency and interval of remission post RT. The change in the tumour size, endocrine insufficiencies and complications developing post RT were noted. Remission was defined as 2 mg Low dose dexamethasone suppressed cortisol of <50 nmo

INTRODUCTION: Radiotherapy (RT) for bladder cancer is as an effective alternative of cystectomy. Although rapid cancer clonogen repopulation contributes to radio-resistance, accelerated RT schemes based on ≤ 2 Gy fractions have failed to improve results. We suggest that accelerated hypofractionation (HypoARC) may be more effective, as it targets both tumors with increased clonogenic activity and tumors with low radio-sensitivity.
PATIENTS AND METHODS: Eighty-two bladder cancer patients were treated with concomitant-boost conformal RT (14 × 2.7 Gy to the pelvis and 15 × 3.4 Gy to the bladder,

INTRODUCTION: With the publication of several trials demonstrating equivalence between partial breast irradiation and whole breast irradiation, the popularity of partial breast radiation has increased. Methods of delivery include interstitial brachytherapy, intracavitary brachytherapy, and 3D conformal irradiation, although the least amount of data exists on 3D conformal. Despite the relative paucity of data on 3D conformal, it is the most common method delivered on the National Surgical Adjuvant Breast and Bowel Project (NSABP) B-39 trial. To date, minimal toxicities and good cosmetic results

Traditionally, pretreatment detected patient-positioning errors have been corrected by repositioning the couch to align the patient to the treatment beam. We investigated an alternative strategy: aligning the beam to the patient by repositioning the dynamic multileaf collimator and adjusting the beam weights, termed dynamic compensation. The purpose of this study was to determine the geometric range of positioning errors for which the dynamic compensation method is valid in prostate cancer patients treated with three-dimensional conformal radiotherapy. Twenty-five previously treated prostate c

Fanconi's anemia (FA) is rare autosomal recessive disorder characterized by aplastic anemia, congenital anomalies, and cancer susceptibility. FA patients have deficiencies in DNA repair pathways that cause cellular sensitivity to ionizing radiation and cross-link agents such as mitomycin C and diepoxybutane (DEB). If these patients survive until early adulthood, they are at high risk for developing solid tumors, most commonly squamous cell carcinoma of the oropharynx, esophagus, and vulva. Treatment of these solid tumors with radiotherapy is complicated by the increased risk of normal tissue t

PURPOSE: The goal of three-dimensional (3-D) conformal radiation is to increase the dose delivered to tumor while minimizing dose to surrounding normal brain. Previously it has been shown that even escalated doses of 70 to 80 Gy have failure patterns that are predominantly local. This article describes the failure patterns and survival seen with high-grade gliomas given 90 Gy using a 3-D conformal intensity-modulated radiation technique.
PATIENTS AND METHODS: From April 1996 to April 1999, 34 patients with supratentorial high-grade gliomas were treated to 90 Gy. For those that recurred, failu

BACKGROUND: In a retrospective analysis we compared conventional radiotherapy and stereotactically guided conformal radiotherapy (SCRT) in patients with esthesioneuroblastoma.
PATIENTS AND METHODS: Between 1991 and 1999 14 patients with esthesioneuroblastoma underwent radiotherapy at our institution. Median follow-up was 30 months (range 12-107 months). Treatment included adjuvant radiotherapy (9), adjuvant radiochemotherapy (3) or radiotherapy alone (2). Eight patients received SCRT with 3-D treatment planning. For comparison a standard three-field plan for these patients and dose-volume his

PURPOSE: To describe rectal mucosal damage in an endoscopic study after conformal radiotherapy of prostate cancer and to correlate this with clinical outcome.
MATERIALS AND METHODS: Flexible rectosigmoidoscopy was performed on 44 patients who voluntarily accepted the examination. The median follow-up was 29 months (20-41 months) after 3-D-planned conformal radiotherapy of prostate cancer (66 Gy at the ICRU Reference point, 2 Gy per fraction). To enable a systematic topographic description of endoscopic findings the rectum was divided into four sections. Additionally we differentiated between

A retrospective treatment planning study was carried out in five patients to assess the effectiveness of conformal radiotherapy of the oesophagus. A two-phase conventional treatment plan was created for each patient, with a prescribed dose of 55 Gy. This plan was compared with a single-phase conformal plan consisting of the same field arrangement as the second phase of the conventional treatment, but with conformal blocks shaped to the beam's eye view of the planning target volume. A further comparison was made between the conventional plan and a two-phase plan using the same beam angles and w

Background and Purpose: Based on the distortion of the current conformity index (CI) formula in handling multi-target plans, the VTV parameter in the current CI formula has been redefined to more accurately calculate the CI value of multi-target plans, providing a reference for clinical applications. Methods and Materials: Considering the limitations of the current VTV calculation formula in CI proposed by van't Riet and Paddick, a new VTV has been defined to better reflect the true conformity of the target volume in multi-target planning. We selected 15 breast cancer (BC) plans with PTVsc and

Objective. We aim to simplify the traditional method of patient-specific skin collimation and to improve the precision of electron radiotherapy treatment with a new surface-conforming collimation system.Method.A surface-conforming collimation device for electron radiotherapy, consisting of parallel steel cables, was created. The cables move freely relative to each other and can be fixed and reshaped with a clamp system along the length of the device. The cables do not completely fill the space; thus, their packing fraction was determined to characterize dosimetric effects. The cable collimator

Bladder dose constraints in case of conformational radiotherapy/intensity-modulated radiotherapy and stereotactic radiotherapy are reported from the literature, in particular from the French radiotherapy society RECORAD recommendations, according to the treated pelvic tumor sites. The dose-volume effect on urinary toxicity is not clearly demonstrated, making difficult to establish absolute dose constraints for the bladder. In case of high-dose prostate cancer radiotherapy, the bladder dose constraints are: V60Gy<50% and maximum dose<80Gy for standard fractionation and V60Gy<5%, V48Gy<25% and V

Introduction and aim Fast-forward radiotherapy (26 Gy in five fractions) has gained widespread acceptance for early-stage breast cancer due to its convenience and efficacy. However, in left-sided cases, there is a risk of radiation-induced cardiac and pulmonary toxicity due to the proximity of the heart and lungs. The voluntary deep inspiration breath-hold (vDIBH) technique increases thoracic volume and heart-chest wall distance, offering a non-invasive method to minimize organ-at-risk exposure. This study aimed to evaluate the dosimetric benefits of 3D-conformal radiotherapy (3D-CRT) combined

PURPOSE: This study evaluates a possible advantage of intensity-modulated stereotactic radiotherapy (IMSRT) over stereotactic conformal radiotherapy (SCRT) in the treatment of lesions in the base of the skull.
METHODS AND MATERIALS: Ten patients (7 with a skull base meningioma) planned for routine SCRT were replanned for IMSRT. The criteria for comparison were the same for both methods: optimal dose to the planning target volume (PTV) and optimal sparing of the organs at risk (OAR). For SCRT, sparing of OAR was achieved by conformal avoidance using 5-6 fields. The IMSRT inverse planning proce

BACKGROUND/AIM: Hippocampal-sparing whole-brain radiotherapy (HS-WBRT) is increasingly used for multiple brain metastases. However, most studies have not reported dose conformity indices (CI). In the only study indicating the CI, conformity was low (CI=0.7). We developed a new technique to achieve a significantly higher CI and better dose coverage.
PATIENTS AND METHODS: Ten patients received 30 Gy of HS-WBRT for brain metastases. Three variants of treatment plans (VAR1, VAR2, VAR3) were investigated. Volumetric modulated arc therapy plans with two (2ROT) or three rotations (3ROT) were created

PURPOSE: Clinical translation of ultra-high dose rate (UHDR) delivery to harness potential FLASH effect requires a treatment planning system (TPS) to optimize and calculate dose and dose rate in patients. Proton conformal FLASH treatment aims to deliver pencil beam scanning (PBS) Bragg Peaks to the tumor region with UHDR. In this work, we conducted a treatment planning study for head and neck (H&N) re-irradiation patients using a research-version of a commercial TPS paired with conformal FLASH hardware integrated into a nozzle of a clinical cyclotron-based system.
METHODS: Fifteen H&N patient
