Recovery of Mycobacterium tuberculosis from positive mycobacterium growth indicator tubes stored at room temperature for up to 6 years in low-income and High-Tuberculosis-Burden Country.

BACKGROUND: Biobanking of Mycobacterium tuberculosis (Mtb) sputum samples for future research activities recommends the use of -70°C or -80°C freezers. Infrastructure for biobanking is not readily available in the majority of low-income countries. This study aimed to assess the recovery rate of Mtb isolates stored at room temperature for more than 6 years in Zimbabwe. METHODS: Census samples of all confirmed rifampicin-resistant/multidrug-resistant tuberculosis isolates that were stored in mycobacterial growth indicator tubes (MGITs) at room temperature from 2011 to 2016 were identified and r

Predicting postoperative delirium assessed by the Nursing Screening Delirium Scale in the recovery room for non-cardiac surgeries without craniotomy: A retrospective study using a machine learning approach.

Postoperative delirium (POD) contributes to severe outcomes such as death or development of dementia. Thus, it is desirable to identify vulnerable patients in advance during the perioperative phase. Previous studies mainly investigated risk factors for delirium during hospitalization and further used a linear logistic regression (LR) approach with time-invariant data. Studies have not investigated patients' fluctuating conditions to support POD precautions. In this single-center study, we aimed to predict POD in a recovery room setting with a non-linear machine learning (ML) technique using pr

Effect of warmed ropivacaine versus room temperature ropivacaine administered spinal anesthesia on recovery of lower limb muscle strength following total knee or hip replacement: a prospective, double-blind, randomized controlled study.

BACKGROUND: With the aging of the population, the number of hip and knee replacements worldwide continues to increase. Postoperative enhanced recovery has become a clinical focus, and an increasing number of hip and knee replacement surgeries are completed on an outpatient basis. Spinal anesthesia is the most widely used anesthetic method in clinical practice, but its disadvantages, such as slow recovery of lower limb muscle strength after operation, have hindered the rapid recovery of such patients. We found that the duration of lower limb motor block under spinal anesthesia with warmed ropiv

Prevalence of hypothermia on admission to recovery room remains high despite a large use of forced-air warming devices: Findings of a non-randomized observational multicenter and pragmatic study on perioperative hypothermia prevalence in France.

BACKGROUND: Despite the availability of effective warming systems, the prevalence of hypothermia remains high in patients undergoing surgery. Occurrence of perioperative hypothermia may influence the rate of postoperative complications. Recommendations for the prevention of inadvertent perioperative hypothermia have been developed and are effective to reduce the frequency of perioperative hypothermia when professionals comply with. French Society of Anesthesiology (SFAR) decided to promote guidelines for the prevention of inadvertent hypothermia, and to conduct beforehand a pragmatic assessmen

The past, present and future of the postanesthesia care unit (PACU) in Japan.

The postanesthesia care unit (PACU), which is run and coordinated by anesthesiologists, delivers general medical supervision as well as close and constant care to patients who have just undergone a surgical procedure under anesthesia. Although PACU management has been considered a standard procedure in many developed countries since the 1940s, Japanese hospitals have tended to cease their management, and only 16.1% of hospitals in Japan currently have PACUs. In today's efficiency-required atmosphere in Japan, we need to consider a better postoperative management method, including facilities si

Respiratory depression in the post-anesthesia care unit: Mayo Clinic experience.

The anesthesia recovery is a complex physiologic process as systems recover from the effects of surgery and anesthesia. Inadequate recovery of respiratory physiology can lead to severe hypoxemia-induced end-organ damage and even death. Emerging evidence suggests that signs of respiratory depression during early anesthesia recovery may portend increased risk for future severe adverse events. This article briefly reviews the Mayo Clinic research experience and advances in clinical practice. From the implementation of a step-down model of discharge criteria in the post-anesthesia care unit (PACU)

Measures of Quality of Recovery and Discharge-readiness Tools in the Pediatric Postanesthesia Care Unit.

The postanesthesia care unit is a critical phase of recovery, yet routine assessment often focuses on discharge readiness rather than the child's recovery experience. This study reviewed measures used in pediatric postanesthesia care unit pathways to assess discharge readiness and early recovery quality and synthesized how they capture domains that matter to children, families, and clinicians. This is a structured literature review with narrative synthesis and identified eight tools used in children: Steward score, Modified Aldrete Score, physiologic criteria-based score, White and Song fast-t

Improving the Perioperative Outpatient Patient Experience: How Pre-op and PACU Made a Difference.

PURPOSE: Beginning in 2025, Outpatient and Ambulatory Surgery Consumer Assessment of Healthcare Providers and Systems (OAS CAHPS), became mandatory reporting for outpatient services. Facilities accepting Medicare and Medicaid payments, stand to lose 2% of reimbursement if they fail to report OAS CAHPS scores. January 2025, our community hospital OAS CAHPS score for the benchmark question "Likelihood to Recommend" was 93.7%, with a peer group rank of 13. This was near bottom-decile performance and considered below expectations. DESIGN: Process improvement. METHODS: A four-pillar patient exper

Sedation in PACU: the role of propofol.

In critically ill patients, adequate sedation increases comfort, minimizes stress response and facilitates diagnostic and therapeutic procedures. Propofol (2-, 6-diisopropylphenol) is an intravenous sedative-hypnotic agent popular for sedation in the Intensive Care Unit. The favorable propofol pharmacokinetic, characterized by a three compartment linear model, allows rapid onset and short duration of action. The emergence time from sedation with propofol varies with the depth and the duration of sedation and the patient's bodyweight. Propofol causes hypotension, particularly in volume depleted

Continuous positive airway pressure improves the immediate post-extubation airway patency.

BACKGROUND: Endotracheal tube extubation can cause laryngospasm, aspiration, upper airway obstruction and hypoxia. In addition, the risk of pulmonary complication increases during extubation for the patients with a difficult airway or a cervical spine injury. The aim of this study was to exam the effect of continuous positive airway pressure (CPAP) on the post-extubation airway patency and the recovery from anesthesia at the recovery room. METHODS: 30 adult patients who were scheduled for spine surgery were randomly allocated into 2 groups depending on the using of CPAP before extubation. Neu

Recovery from anaesthesia in children.

There are no published comprehensive surveys of paediatric recovery room experience and the incidence of complications. A prospective survey was made of 16,700 consecutive admissions to the recovery room at the Royal Manchester Children's Hospital during the years 1985-1988. The incidence of respiratory complications was low, with laryngospasm 0.85%. The incidence of hypotension was higher than that in adult studies; over 50% of children recorded a decrease in blood pressure in the recovery room of more than 20%, compared to values before operation. The incidence of vomiting in the recovery ro

Bloodborne pathogen exposures and prevention in the perioperative environment.

The fear of exposure to bloodborne pathogens and disease transmission from patients to health care professionals or health care professional to patients is real. Perioperative nurses and other surgical personnel should be aware of the dangers and risks of exposure to these viruses. Patients may be infected with one of the currently recognized bloodborne viruses including Hepatitis B (HBV), Hepatitis C (HCV) and the Human Immunodeficiency Virus (HIV). It is imperative that accidental exposure during surgical procedures be avoided. The most common means of exposure are percutaneous and mucous me

Analgesia in PACU: nonsteroidal anti-inflammatory drugs.

Control and abolition of pain during and after surgical procedures is crucial point in the practice of anaesthesiology. In the last years the concept of multimodal analgesia (the use of different analgesic and techniques to relieve pain), has developed and non-steroidal anti-inflammatory drugs (NSAIDs) have a major role in it, because they have an opioid sparing effect. Moreover, they are very effective on somatic and breakthrough pain. NSAIDs can be divided functionally in four classes: salicylates, acetaminophen, non-specific inhibitors (i.e. ibuprofen, ketorolac, etc.) and cyclooxygenase2-i

Malignant hyperthermia.

Although MH is a rare disease, occurring 1 in 7000 to 14,000 anesthetics in children and one in 50,000 to 200,000 anesthetics in adults, the critical care nurses must possess knowledge of its causes and treatment. MH can be potentially fatal. With aggressive intervention, both morbidity and mortality will be reduced.

A prospective study evaluating the feasibility and accuracy of very early postoperative translaryngeal ultrasonography in the assessment of vocal cord function after neck surgery.

BACKGROUND: Early recognition of postoperative vocal cord palsy enhances postoperative care. Translaryngeal ultrasonography can assess vocal cord function accurately and noninvasively, but it is unclear whether it is feasible or accurate when done immediately after extubation in the recovery room owing to possible interference from laryngeal swelling. This study assessed the feasibility and accuracy of translaryngeal ultrasonography in this setting. METHODS: Consecutive patients undergoing neck operations were subjected to translaryngeal ultrasonography and flexible direct laryngoscopy 1 day

Epidural analgesia in the Post-Anaesthesia Care Unit.

Epidural analgesia is widely used for postoperative pain in a variety of surgical operations and it is recognised to provide superior quality of analgesia when compared with systemic opioids. The combination of low doses of local anaesthetics and opioids appears to provide optimal analgesia with minimal motor blockade. However, side effects have been reported with epidural analgesia such as postoperative nausea and vomiting (PONV), respiratory depression and arterial hypotension. Although the incidence of these side effects is lower than those reported with the use of systemic opioids, they ca
