Metachronous Liver Metastasis during Long-term Follow-up after Endoscopic Submucosal Dissection of a Small Rectal Neuroendocrine Neoplasm.

A 67-year-old woman underwent a screening colonoscopy, which revealed a 7-mm rectal subepithelial lesion. Endoscopic submucosal dissection (ESD) confirmed a grade 1 rectal neuroendocrine neoplasm (NEN), and no further treatment was administered. One year after ESD, she was diagnosed with lung cancer, which recurred 4 years later and required chemotherapy. Nine years after ESD, multiple liver metastases from the rectal NEN were found, which were well controlled with radiofrequency ablation. The patient died 13 years after ESD due to the progression of lung cancer. This case highlights the need

Lymph node metastasis of grade 1 neuroendocrine tumor from rectal mixed neuroendocrine non-neuroendocrine neoplasm: A case report.

Mixed neuroendocrine non-neuroendocrine neoplasms of the gastrointestinal tract are rare, with only a few cases documented in the literature. In particular, there are no reports of a rectal mixed neuroendocrine non-neuroendocrine neoplasm with regional lymph node metastasis of the neuroendocrine tumor component. Here, we present the case of a man in his early 70s with an incidental finding of multiple colonic polyps during a routine colonoscopy, including a 0.9-cm sessile polyp in the rectum. Repeat colonoscopy was performed for endoscopic mucosal resection. Histopathological examination revea

Single-port robotic versus laparoscopic transanal minimally invasive surgery (TAMIS) for rectal neoplasms: a first comparative analysis of surgical efficiency and clinical outcomes.

BACKGROUND: Single-port (SP) robotic platform has recently been applied to transanal minimally invasive surgery (TAMIS). However, comparative data with conventional TAMIS performed using laparoscopic instruments remain limited. This study aimed to evaluate the technical feasibility and short-term outcomes of SP robotic TAMIS compared to laparoscopic TAMIS. METHODS: Multi-institutional retrospective cohort study of 108 patients (SPR n = 34, LAP n = 74) undergoing TAMIS for rectal neoplasms from January 2016 to September 2025. Patients with benign rectal neoplasms, clinical stage T1N0 with well

Transanal minimally invasive surgery (TAMIS) for local excision of benign neoplasms and early-stage rectal cancer: efficacy and outcomes in the first 50 patients.

BACKGROUND: Since its inception in 2009, transanal minimally invasive surgery has been used increasingly in the United States and internationally as an alternative to local excision and transanal endoscopic microsurgery for local excision of neoplasms in the distal and mid rectum. Despite its increasing acceptance, the clinical benefits of transanal minimally invasive surgery have not yet been validated. OBJECTIVE: The aim of this study is to assess the adequacy of transanal minimally invasive surgery for the local excision of benign and malignant lesions of the rectum. DESIGN: This is a ret

Laparoscopic Low Anterior Resection and Eversion Technique Combined With a Nondog Ear Anastomosis for Mid- and Distal Rectal Neoplasms: A Preliminary and Feasibility Study.

The transanal eversion and prolapsing technique is a well-established procedure, and can ensure an adequate distal margin for patients with low rectal neoplasms. Potential leakage risks, however, are associated with bilateral dog ear formation, which results from traditional double-stapling anastomosis. The authors determined the feasibility of combining these techniques with a commercial stapling set to achieve a nondog ear (end-to-end) anastomosis for patients with mid- and distal rectal neoplasms. Patients with early-stage (c/ycT1-2N0), mid- to distal rectal neoplasms and good anal sphincte

Early-Stage Rectal Mixed Neuroendocrine-Non-neuroendocrine Neoplasm Treated With Local Excision in a Frail Elderly Patient: A Multidisciplinary Management Dilemma.

Mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) of the rectum are rare tumors that typically demonstrate aggressive behavior, largely driven by high-grade neuroendocrine carcinoma components. Evidence guiding management is limited, particularly for early-stage lesions and for patients unable to tolerate standard oncologic surgery. We report the case of an elderly man with extensive comorbidities who was found to have a 1.3 cm rectal MiNEN following transanal endoscopic microsurgery (TEMS) performed for rectal bleeding. Histopathology revealed a moderately differentiated adenocarcino

A case of rectal mixed neuroendocrine-non-neuroendocrine neoplasm diagnosed only after radical surgery: implications of diagnostic timing for therapeutic decision-making.

Colorectal mixed neuroendocrine-non-neuroendocrine neoplasms (MiNENs) are rare and aggressive tumors in which accurate preoperative diagnosis remains difficult because of marked intratumoral heterogeneity. A 75-year-old man presented with locally advanced rectal cancer with inguinal lymph node metastasis. Routine biopsy demonstrated moderately to poorly differentiated adenocarcinoma, while retrospective immunohistochemical analysis revealed focal neuroendocrine differentiation insufficient for a diagnosis of MiNEN. Initial systemic chemotherapy with FOLFOXIRI plus bevacizumab was administered

Early rectal neoplasm in dual-layer spectral detector computed tomography: dual-layer spectral computed tomography (CT) images improve tumor detection and staging.

BACKGROUND: Early rectal neoplasms can be treated endoscopically with good prognosis, yet usually present with unspecific or an absence of signs and symptoms and are detected largely by invasive endoscopy with less compliance to screening. The purpose of this cross-sectional study was to explore the diagnostic value of dual-layer spectral detector computed tomography (DSCT) imaging for early rectal neoplasm. METHODS: Patients who underwent DSCT for evaluation of rectal lesion or routine examination between September 2022 to September 2023 at West China Hospital were prospectively included and

[Integrated multidisciplinary treatment of colorectal neoplasms].

In this retrospective study, the modality and advantages of the multidisciplinary diagnostic work-up and therapy regarding colorectal neoplasm were analysed. Over the period 2004-2008, 63 patients underwent multidisciplinary treatment for colorectal cancer. All patients underwent surgery (laparoscopic/open). Exeresis was supplemented by adjuvant chemotherapy in those cases beyond IIA stage; all cases of extraperitoneal rectal and anal canal neoplasms plus one case of carcinoma of the transverse colon, initially inoperable, underwent neoadjuvant radiotherapy plus chemotherapy. The treatment was

DMMR status and synchronous lesions predicts metachronous lesions after curative resection for rectal cancer.

BACKGROUND: There are no established standard colonoscopy surveillance protocols for patients after curative rectal cancer resection. We investigated the predictive factors for colorectal neoplasms during surveillance colonoscopies to identify patients who are at risk of developing metachronous neoplasms in the residual colorectum. METHODS: This was a single-center, retrospective study that included patients with diagnosis of rectal carcinoma who had undergone curative resection from October 2012 to June 2018. Clinicopathological variables were analyzed by logistic regression analysis to iden

Rectal neuroendocrine tumor with uncommon metastatic spread: A case report and review of literature.

Neuroendocrine tumors of the gastrointestinal tract are rare neoplasms. Rectal neuroendocrine tumors consist approximately the 5%-14% of all neuroendocrine neoplasms in Europe. These tumors are diagnosed in relatively young patients, with a mean age at diagnosis of 56 years. Distant metastases from rectal neuroendocrine tumors are not very common. Herein we describe a case of a rectal neuroendocrine tumor which metastasized to the lung, mediastinum and orbit. This case underscores the importance of early identification and optimal management to improve patient's prognosis. Therefore, the clini

The accuracy of endorectal ultrasound in staging rectal lesions in patients undergoing transanal endoscopic microsurgery.

BACKGROUND: Endorectal ultrasound (ERUS) is used to preoperatively assess locoregional stage in patients with rectal neoplasms. This study evaluates the accuracy of ERUS in determining the T stage of rectal neoplasms treated by transanal endoscopic microsurgery (TEM). METHODS: All patients in the St Paul's Hospital TEM database were evaluated and excluded if they had been treated with neoadjuvant therapy. ERUS results were compared with gold-standard postoperative histopathology reports. Tumor height from anal verge was measured by ERUS and endoscopic techniques. RESULTS: Fifty-three patient

[Rectal neuroendocrine tumors: surgical therapy].

The incidence of rectal neuroendocrine tumors (NET) has increased in recent years. Most of these neoplasms are asymptomatic and are diagnosed by colonoscopy screening, which could be one of the reasons for the increasing occurrence. As less than 1 % of rectal NET produce serotonin they are practically never discovered due to a carcinoid syndrome. The current guidelines of the European (ENETS) and North American (NANETS) Neuroendocrine Tumor Societies support clinicians with useful diagnostic and treatment algorithms. The most important criteria for therapy are tumor size and histopathological

[Rectal carcinoids on the rise - update].

Rectal carcinoids are becoming more common: in the USA they have increased in frequency by 800 - 1000 % in the past 35 years. This dramatic increase is probably related to the introduction of colonoscopic screening which also results in the "incidentally" detected neuroendocrine rectal tumours/carcinomas being smaller than in the pre-screening era. Endosonography is the method of choice for determining the size and depth of penetration of the tumours and for detecting lymph node metastases. Tumours of < 10 mm in size that have not infiltrated the muscularis propria can usually be removed endos

Using Materialise's interactive medical image control system to reconstruct a model of a patient with rectal cancer and situs inversus totalis: A case report.

BACKGROUND: Situs inversus totalis (SIT) is a rare congenital anomaly that refers to a completely reversed location of abdominal and thoracic organs. An extremely small number of patients with this condition, especially those with rectal neoplasms, have been reported. Surgery in these patients is technically challenging. Therefore, we reconstructed a three-dimensional (3D) digital model with the Materialise's interactive medical image control system (Mimics) as a guide for laparoscopic resection. CASE SUMMARY: We report the case of a 68-year-old woman with rectal neoplasms and SIT diagnosed b

[Anastomosis dehiscence in anterior resection of the rectum with total excision of the mesorectum].

Anterior rectal resection with total mesorectal excision is currently regarded as the operation of choice in patients with neoplasms of the extraperitoneal rectum. This operation is associated with a significant incidence of anastomotic dehiscence. Some authors, therefore, advise the execution of a protective stoma. From 1987 to 2000, 241 patients with rectal neoplasma were submitted to radical surgery: 183 to anterior rectal resection (extraperitoneal neoplasms in 129 cases and intraperitoneal neoplasms in 54) and 58 to a Miles operation. The total incidence of anastomotic complications was 8

Neuroendocrine neoplasms of the appendix, colon and rectum.

Neuroendocrine neoplasms of the appendix, colon and rectum are classified according to the most recent WHO classification as neuroendocrine tumors (NET), neuroendocrine carcinomas (NEC) and mixed neuroendocrine-non neuroendocrine neoplasms (MiNENs). NECs and MiNENs are aggressive neoplasms requiring multimodal treatment strategies. By contrast, NETs are, in most cases, indolent lesions occurring as incidental findings in the appendix or as polyps in the rectum. While most appendiceal and rectal NETs are considered relatively non-aggressive neoplasms, a few cases, may show a more aggressive cli
