Modulation of nerve fibers in the rat thyroarytenoid muscle following recurrent laryngeal nerve injury.

CONCLUSION: Regeneration of nerve fibers in the thyroarytenoid (TA) muscle occurred actively after damage on the recurrent laryngeal nerve (RLN) compared with the vagus nerve (VN). However, remyelination did not occur after damage on the RLN. OBJECTIVES: To determine the regeneration process of nerve fibers in the TA muscle following transection and immediate anastomosis of the RLN or VN. METHODS: Three types of animal model were prepared: an RLN anastomosis model (RLNa), a VN anastomosis model (VNa), and a peroneal nerve anastomosis model (PNa). Animals were sacrificed at five time points f

The value of intraoperative nerve monitoring against recurrent laryngeal nerve injury in thyroid reoperations.

Reoperative thyroidectomy is challenging for surgeons because of the higher incidence of recurrent laryngeal nerve (RLN) palsy. RLN identification is the gold standard during thyroidectomy; however, it is sometimes difficult to perform thyroid reoperations. In recent years, intraoperative nerve monitoring (IONM) has gained increased acceptance, and the use of IONM can be a valuable adjunct to visual identification. The aim of this study was to evaluate the value of IONM during thyroid reoperation.A total of 109 patients who met our criteria at the Affiliated Hospital of Hangzhou Normal Univers

Routine exposure of recurrent laryngeal nerve in thyroid surgery can prevent nerve injury.

To determine the value of dissecting the recurrent laryngeal nerve during thyroid surgery with respect to preventing recurrent laryngeal nerve injury, we retrospectively analyzed clinical data from 5 344 patients undergoing thyroidectomy. Among these cases, 548 underwent dissection of the recurrent laryngeal nerve, while 4 796 did not. There were 12 cases of recurrent laryngeal nerve injury following recurrent laryngeal nerve dissection (injury rate of 2.2%) and 512 cases of recurrent laryngeal nerve injury in those not undergoing nerve dissection (injury rate of 10.7%). This difference remain

Microarray Analysis Gene Expression Profiles in Laryngeal Muscle After Recurrent Laryngeal Nerve Injury.

OBJECTIVES: The pathophysiology of recurrent laryngeal nerve (RLN) transection injury is rare in that it is characteristically followed by a high degree of spontaneous reinnervation, with reinnervation of the laryngeal adductor complex (AC) preceding that of the abducting posterior cricoarytenoid (PCA) muscle. Here, we aim to elucidate the differentially expressed myogenic factors following RLN injury that may be at least partially responsible for the spontaneous reinnervation. METHODS: F344 male rats underwent RLN injury (n = 12) or sham surgery (n = 12). One week after RLN injury, larynges

Multidisciplinary Management of Recurrent Laryngeal Nerve Injury After Transhiatal Esophagectomy.

Recurrent laryngeal nerve (RLN) injury is a significant morbidity following transhiatal esophagectomy (THE). While surgical platforms have evolved, the clinical burden and longitudinal management of these injuries remain under-characterized. This study evaluates RLN injury incidence and the effectiveness of a multidisciplinary management pathway in a contemporary cohort. A retrospective review of 135 patients undergoing THE with cervical anastomosis between 2017 and 2024 was conducted. The primary outcome was confirmed RLN injury via laryngoscopy and postoperative hoarseness. Predictors of inj

Beyond Surgical Injury: Systemic Etiologies of Recurrent Laryngeal Nerve Palsy.

The recurrent laryngeal nerve (RLN), a branch of the vagus nerve, plays a crucial role in phonation, respiration, and airway protection. Dysfunction of the RLN can result from various factors, including trauma, surgery, endotracheal intubation, infection, and systemic diseases, leading to clinical symptoms such as hoarseness, stridor, dysphagia, and severe respiratory distress. This systematic review examines the impact of systemic diseases on RLN function. Through an analysis of cases from the English literature, the review demonstrates that RLN dysfunction in the context of systemic diseases

Observations of recurrent laryngeal nerve injury and recovery using a rat model.

OBJECTIVES/HYPOTHESIS: To evaluate standardized recurrent laryngeal nerve (RLN) injuries using a rat model via minimally invasive transoral electromyography (ToL EMG) and histologic studies. METHODS: Forty-two female Sprague Dawley rats weighing 200 g to 250 g underwent crush injury to the right RLN using a calibrated pressure clip (0.61 N or 1.19 N) for 60 seconds. Following injury, serial ToL EMGs were performed on abductor and adductor laryngeal muscles during respiratory cycles and spontaneous vocal fold abduction on day 4 and then weekly for 6 weeks. Vocal fold motion associated with spo

Recurrent laryngeal nerve injury following uncomplicated carotid angioplasty and stenting.

PURPOSE: To describe a unique instance of cranial nerve injury related to uncomplicated carotid artery stenting (CAS). CASE REPORT: A 74-year-old woman with a history of expressive aphasia and right upper/lower extremity weakness underwent staged CAS procedures under local anesthesia and conscious sedation. After routine predilation with a 4-mm balloon, a tapered 7 x 10 x 30-mm Acculink stent was placed and dilated with a 5-mm balloon. At 1 month after the second procedure, the carotid stents were patent bilaterally, but the patient reported voice fatigue and hoarseness along with dysphagia t

Voice changes after thyroidectomy without recurrent laryngeal nerve injury.

BACKGROUND: Injury of the inferior laryngeal nerve is not the only cause of voice alteration after thyroidectomy; many patients notice minimal changes immediately after operation, without evidence of inferior laryngeal nerve damage. We hypothesized that there may be other causes for voice modification, such as injuries of the superior laryngeal nerve, prethyroid strap muscles, and cricothyroid muscles. We describe voice changes after total thyroidectomy, without inferior laryngeal nerve injury, using a computer program to objectively compare different patterns of voice. STUDY DESIGN: Forty-si

Ultrasonic Vocalization Acoustics After Recurrent Laryngeal Nerve Injury and Recovery.

OBJECTIVES: The lack of well-defined functional acoustic correlates of vocal fold paralysis in animal models creates challenges for translating basic science studies of recurrent laryngeal nerve (RLN) injury to patients with vocal fold paralysis. Capitalizing on natural communicative rodent ultrasonic vocalizations (USVs), this study aimed to (1) investigate the impact of RLN injury on USV acoustics in both acute and chronic stages of RLN injury and (2) to identify USV parameters with the greatest ecological validity specific to RLN injury. METHODS: USVs were recorded over 2 weeks (acute) in

Tissue-Engineered Implant for Hemilaryngectomy Reconstruction with Recurrent Laryngeal Nerve Injury.

OBJECTIVE: Laryngeal cancer resections often require excision of portions of the larynx along with sacrifice of the ipsilateral recurrent laryngeal nerve (RLN). In such cases, there are no reconstructive options that reliably restore laryngeal function, rendering patients with severe functional impairment. To address this unmet clinical need, we extend our evaluation of a 3-implant mucosal, muscle, cartilage reconstruction approach aimed at promoting functional laryngeal restoration in a porcine hemilaryngectomy model with ipsilateral RLN transection. METHODS: Six Yucatan mini-pigs underwent

Augmentation and vocal fold biomechanics in a recurrent laryngeal nerve injury model.

OBJECTIVES/HYPOTHESIS: Composite vocal fold (VF) biomechanical data are lacking for augmentation after recurrent laryngeal nerve (RLN) injury. We hypothesize resulting atrophy decreases VF stiffness and augmentation restores native VF biomechanics. METHODS: Sixteen Yorkshire Crossbreed swine underwent left RLN transection and were observed or underwent carboxymethylcellulose (CMC) or calcium hydroxyapatite (CaHa) augmentation at 2 weeks. Biomechanical measurements (structural stiffness, displacement, and maximum load) were measured at 4 or 12 weeks. Thyroarytenoid (TA) muscle cross-sectional

Elucidating mechanisms of recurrent laryngeal nerve injury during thyroidectomy and parathyroidectomy.

BACKGROUND: Intraoperative nerve monitoring during thyroidectomy, parathyroidectomy, or related central neck procedures can elucidate actual or potential mechanisms of recurrent laryngeal nerve (RLN) injury, especially visually intact nerves, which were previously unknown to the endocrine surgeon. STUDY DESIGN: In this prospective evaluation study, 373 patients underwent 380 consecutive thyroidectomy- or parathyroidectomy-related operations using intraoperative nerve monitoring, with 666 RLNs at risk. The success of visual and functional identification of the RLN, persistent loss of RLN funct

Intraoperative Neuromonitoring and Optical Magnification in the Prevention of Recurrent Laryngeal Nerve Injuries during Total Thyroidectomy.

Background and Objectives: Recurrent laryngeal nerve (RLN) paralysis is a fearful complication during thyroidectomy. Intraoperative neuromonitoring (IONM) and optical magnification (OM) facilitate RLN identification and dissection. The purpose of our study was to evaluate the influence of the two techniques on the incidence of RLN paralysis and determine correlations regarding common outcomes in thyroid surgery. Materials and Methods: Two equally sized groups of 50 patients who underwent total thyroidectomies were examined. In the first group (OM), only surgical binocular loupes (2.5×−4.5×) we

Nerve injury: injury to the recurrent laryngeal, phrenic, vagus, long thoracic, and sympathetic nerves during thoracic surgery.

Nerve injury occurs in about 1% of patients undergoing thoracic surgery. Most commonly, it is the result of surgical trauma specific to the area of operation. Alternatively, it may be due to retraction on adjacent structures during the exposure of complex surgical procedures. This article reviews the most common types of nerve injury and discusses the pathophysiology, etiology, diagnosis, and treatment of each.

Extra-laryngeal branching of recurrent laryngeal nerve: a possibility thyroid surgeons need to keep in mind to prevent inadvertent nerve injury.

A 45-year-old woman presented with a neck swelling since the past 6 years. The swelling was painless but slowly and progressively increasing in size. On examination, a thyroid swelling of size approximately 6×4 cm was seen on the left side. Ultrasonography revealed the size to be 6×5 cm with microcalcifications. Fine needle aspiration cytology revealed the mass to be papillary carcinoma. She was planned for total thyroidectomy with central compartment clearance. Preoperative assessment including thyroid hormone profile was normal and the patient was taken for surgery after proper anaesthesia c

Comparison of Recurrent Laryngeal Nerve Injury during Thyroidectomy with and Without Routine Identification of the Nerve Peroperatively.

Thyroidectomy is one of the commonest operative procedures performed in the neck and injury to recurrent laryngeal nerve (RLN) is not uncommon. It results in hoarseness to serious respiratory distress depending on the extent of the injury. The incidence of RLN injury varies widely and is multifactorial depending on the extent of surgical procedures, experience and expertise of the surgeons, nature of the thyroid diseases and a wide range of anatomical variations. Peroperative routine identification of the nerve during thyroidectomy can be a way to prevent injury. Despite recommendation for ide
