Pure red cell aplasia (PRCA) is a syndrome characterized by a severe normocytic anaemia, reticulocytopenia, and absence of erythroblasts from an otherwise normal bone marrow. Primary PRCA, or secondary PRCA which has not responded to treatment of the underlying disease, is treated as an immunologically-mediated disease. Although vigorous immunosuppressive treatments induce and maintain remissions in a majority of patients, they carry an increased risk of serious complications. Corticosteroids were used in the treatment of PRCA and this has been considered the treatment of first choice although

We report the case of a 34-year-old woman with recurrent pure red cell aplasia and evidence of hepatitis B and C infection. Review of the English literature identified 19 prior cases in which pure red cell aplasia was associated with hepatitis. This case is the first in which serologic evidence of hepatitis C infection was documented. This patient also had porphyria cutanea tarda and marked hepatic siderosis but no active hepatitis or cirrhosis. Treatment with cyclophosphamide and prednisone produced complete remission of the pure red cell aplasia. Erythroid colony formation (colony-forming un

Pure red cell aplasia developed in a 9-year-old girl receiving sodium valproate therapy. Infection with measles and chickenpox had occurred two months previously. Regeneration of bone marrow erythroid precursors was demonstrated one month after drug withdrawal. Readministration of sodium valproate resulted in a second episode of red cell aplasia that again resolved promptly after drug withdrawal. The respective roles of infection and drugs in the etiology of red cell aplasia are discussed.

Among the patients with idiopathic pure red cell aplasia (PRCA) who do not meet the diagnostic criteria of chronic lymphocytic leukemia, there are some cases which suggest an association with clonal T lymphocytic proliferation. The morphological characteristics and responses to treatment revealed two distinct groups among the present 13 patients. The lymphocytes in one group were typical granular lymphocytes of T cell phenotype which were treated effectively with cyclophosphamide rather than cyclosporine. The response to therapy in this group occurred after a perceptible reduction in lymphocyt

A 42-year-old woman was admitted to our hospital because of easy fatigability in Jan. 1976. Laboratory examination revealed severe macrocytic anemia and slight lymphocytosis. She was diagnosed as having pure red cell aplasia (PRCA). She went into hematological remission 6 weeks following 40 mg/day treatment with prednisolone, but the anemia relapsed frequently when the dosage was lessened. She was then treated with 50 mg/day of cyclophosphamide, 50 mg/day of azathioprine, splenectomy, and methylprednisolone pulse therapy, but the recovery from anemia was temporary after each treatment. Since 1

A thymoma with pure red cell aplasia (PRCA) is relatively rare. A 71-year-old woman complainted of dizziness and her blood cell count showed a severe anemia. She was diagnosed as PRCA by bone marrow aspiration biopsy, which showed marked decrease in number of erythroblasts. In addition, the chest computed tomography revealed a solid tumor in the anterior mediastinum. She underwent extended thymothymectomy through median sternotomy. The resected specimen was 10.5×9.7 cm in diameter. The pathological diagnosis was type AB thymoma of the World Health Organization classification, and Masaoka stage

A 78-year-old woman has suffered from pure red-cell aplasia (PRCA) associated with generalized myasthenia gravis and thymoma. Cyclosporin A (CyA) with corticosteroid increased numbers of erythroid cells in her bone marrow cells but she required monthly blood transfusions. Administration of tacrolimus as a substitution for CyA inhibited progression of anemia without the need for further blood transfusion. No serious side effects were observed. This case demonstrates that tacrolimus is another option of treatment for PRCA in patients who fail to respond to CyA.

To determine the utility of marrow culture in defining the natural history and therapeutic response of pure red cell aplasia we have studied 37 patients. Patients were evaluated at the University of Washington before specific therapies (n = 21) or at the time of treatment failure in = 16). Evaluation included a medical and drug exposure history, a physical examination, a chest x-ray or computed tomography to rule out thymoma, lymphocyte immunophenotype studies, anti-nuclear antibody and rheumatoid factor determinations, marrow cytogenetics, and marrow progenitor cell cultures. Retrospective So

A 14-year-old, castrated male mixed-breed cat presented with weight loss and rhinitis for several months and was referred due to anemia and thrombocytopenia. A complete blood cell count (CBC) revealed severe non-regenerative anemia (hematocrit (HCT), 9.7%) and thrombocytopenia (0 /μL). Serology was positive for feline immunodeficiency virus (FIV) antibodies. Bone marrow aspiration showed moderate myeloid hyperplasia, severe erythroid hypoplasia, and megakaryocytic hypoplasia. The cat was diagnosed with pure red cell aplasia (PRCA) and amegakaryocytic thrombocytopenia (AMT) and treated with cor

Both pure red cell aplasia (PRCA) and hypogammaglobulinemia are rarer conditions than myasthenia gravis (MG) in thymoma patients. Several articles have discussed the relation between PRCA and thymoma or hypogammaglobulinemia and thymoma, and their proper treatments. Instances of both PRCA and hypogammaglobulinemia in a thymoma patient are few and reported sporadically in the literature. We discuss a 46-year-old woman with thymoma and simultaneous PRCA and hypogammaglobulinemia who achieved complete remission from PRCA after perioperative steroid administration and extended thymectomy, and revi

We report a rare case who developed pure red cell aplasia (PRCA) and pseudothrombocytopenia associated with hepatitis. A 50-year-old woman was admitted to the hospital because of acute hepatitis A. On the 22nd hospital day, normocytic normochromic anemia without reticulocytosis was developed. A bone marrow aspirate revealed erythroid hypoplasia with a small percentage of proerythroblasts and basophilic erythroblasts, but almost complete absence of polychromatophilic and orthochromic erythroblasts. This case was diagnosed as PRCA characterized by the maturation arrest of erythropoiesis. Anemia

A 55-year-old male, former smoker, presented with exertional dyspnea associated with an anterior mediastinal mass. Biopsy findings were consistent with a type B2 thymoma. The tumor was deemed unresectable, and curative concurrent chemoradiotherapy was not feasible after three cycles of cyclophosphamide, adriamycin (doxorubicin), and platinum (cisplatin) (CAP) chemotherapy. Treatment was therefore continued to six cycles. Following radiological progression, second-line chemotherapy with paclitaxel and carboplatin was initiated. After two cycles, the patient developed superior vena cava syndrome

Pure red cell aplasia (PRCA) is increasingly recognised as a T-cell-mediated bone marrow failure syndrome, yet its immunogenetic drivers remain poorly defined. In their paper, Yamashita et al. integrate human leucocyte antigen (HLA) typing, T-cell receptor repertoire analysis and mutational profiling to reveal enriched HLA alleles, signal transducer and activator of transcription 3 (STAT3)-mutated T-cell clones and a shared T cell receptor beta (TCRβ) motif in PRCA patients. These findings suggest that antigen-driven cytotoxic T-cell responses represent a central mechanism underlying erythroid

BACKGROUND: Pure red cell aplasia (PRCA) is a hematological disorder characterized by anemia with severe reticulocytopenia caused by a marked reduction in erythroid precursors in the bone marrow. PRCA is known to be associated with pregnancy, but thymoma-associated PRCA during pregnancy is very rare, and its successful management has not been reported.
CASE PRESENTATION: A 37-year-old primiparous woman with severe anemia was referred to our center at 27 weeks' gestation. She was diagnosed with PRCA based on bone aspiration findings at 33 weeks' gestation. Magnetic resonance imaging (MRI) reve

Aggressive systemic mastocytosis (ASM) is characterized by mast cell accumulation in systemic organs. Though ASM may be associated with other hematological disorders, the association with pure red cell aplasia (PRCA) is rare and has not been reported. Pure red cell aplasia (PRCA) is a syndrome, characterized by normochromic normocytic anemia, reticulocytopenia, and severe erythroid hypoplasia. The myeloid and megakaryocytic cell lines usually remain normal. Here, we report an unusual case of ASM, presenting in association with PRCA and the management challenges.
