Vestibulo-ocular reflexes in rabbits: reduction by intravenous injection of diazepam.

We have studied the influence of intravenously administered diazepam on the horizontal (HVOR) and vertical (VVOR) vestibulo-ocular reflexes of the rabbit. The HVOR and VVOR were evoked by sinusoidal oscillation of rabbits on a rate table (0.01 to 0.8 Hz, +/- 10 degrees), and eye movements were measured with an infrared light-projection technique. The gains of the HVOR and VVOR (evoked eye velocity/head velocity) were reduced by diazepam injections of 5 microgram/kg. The dose required to produce a 50% reduction in HVOR gain was 500 microgram/kg. The time required to reduce the HVOR gain to 50%

Enhanced vestibulo-ocular reflex to electrical vestibular stimulation in Meniere's disease.

Meniere's disease is characterized by sporadic episodes of vertigo, nystagmus, fluctuating sensorineural hearing loss, tinnitus and aural pressure. Since Meniere's disease can affect different regions of the vestibular labyrinth, we investigated if electrical vestibular stimulation (EVS) which excites the entire vestibular labyrinth may be useful to reveal patchy endorgan pathology. We recorded three-dimensional electrically evoked vestibulo-ocular reflex (eVOR) to transient EVS using bilateral, bipolar 100-ms current steps at intensities of 0.9, 2.5, 5.0, 7.5 and 10.0 mA with dual-search coil

The clinical utility of search coil horizontal vestibulo-ocular reflex testing.

CONCLUSION: Testing of the horizontal vestibulo-ocular reflex (VOR) with head rotations (including head impulses) using the magnetic scleral search coil technique (SCT HHI) provides valuable additional diagnostic information in patients with persistent dizziness, oscillopsia or imbalance. It identifies high and low frequency/acceleration vestibular abnormalities that are frequently missed using other methods. OBJECTIVES: To evaluate the diagnostic utility of SCT measurement of the horizontal VOR in the multidisciplinary neurotology clinic of a tertiary referral centre. PATIENTS AND METHODS:

The linear vestibulo-ocular reflex, locomotion and falls in neurological disorders.

PURPOSE: During locomotion, head perturbations, consisting of rotations and translations (linear movements), occur with predominant frequencies of 0.5-5.0 Hz. The vestibular reflexes act at short latency to safeguard clear vision and stable posture during locomotion. Much is known about the angular vestibulo-ocular reflex (aVOR) in response to head rotations, which depend on the semicircular canals of the vestibular labyrinth. However, the means to test reliably the linear or translational vestibulo-ocular reflex (tVOR), which depends on the otolithic organs, has only become available more rec

A reinterpretation of the purpose of the translational vestibulo-ocular reflex in human subjects.

In a prior study we reported that the human translational vestibulo-ocular reflex (tVOR) in response to vertical (bob) 2 Hz oscillations generated eye rotations of only 60% of those required to keep the eyes pointed at a stationary visual target, whether located at near (approximately 17 cm) or far (2 m). Best responses occurred in ambient illumination and we concluded that relative image motion between the target and background was an important determinant of tVOR behaviour. To investigate further how visual conditions influenced tVOR, we measured responses as subjects binocularly viewed the

Asymmetry of the pitch vestibulo-ocular reflex in patients with cerebellar disease.

Responses to pitch head impulses were measured in 15 patients with cerebellar degeneration and downbeat nystagmus and in 5 control subjects, using three-axis search coils. For each subject, response gains were calculated as (1) the ratio of instantaneous vertical eye velocity to pitch head velocity (at 30 ms and at 70 ms into the response), and (2) the ratio of peak eye velocity to peak head velocity. Gains varied more widely among patients. When calculated at 70 ms and using peak values, patients had higher gains for downward pitch, and normal subjects had symmetric gains. At 30 ms, gains wer

Transaural linear vestibulo-ocular reflexes from a single utricle.

To clarify the directional sensitivity of a single utricle with respect to the transaural linear vestibulo-ocular reflex (L-VOR) we studied seven patients before and after vestibular neurectomy. Patients were seated upright in an electrically powered car running on a linear track. Transaural acceleration steps of 0.24 g were applied randomly to the left and right in the dark. The slow phase velocity of the L-VOR was measured from the average of the induced compensatory eye movements. L-VOR asymmetry was calculated as (R-L/R + L x 100) and is < 13% in normals. Before surgery, responses were mos

Autorotation test of the horizontal vestibulo-ocular reflex in Menière's disease.

Sixty-four patients with the diagnosis of Menière's disease were tested at the House Ear Clinic with an active head-rotation test system, the Vestibular Autorotation Test (VAT). The VAT is a portable, computerized test that measures the horizontal vestibulo-ocular reflex (VOR) with the use of high-frequency (2 to 6 Hz) active head movements to obtain gain and phase. The purpose of this study was to characterize the horizontal VOR at high frequencies in patients with Menière's disease. At frequencies from 5 to 6 Hz, all patients demonstrated horizontal phase greater than 180 degrees and 85% sho

Effects of fixation and optokinetic stimulation on vestibulo-ocular reflex suppression.

Suppression of the vestibulo-ocular reflex was assessed in normal subjects and patients with neurological disorders to determine the relative effects on suppression of a single fixation target and an optokinetic field. Subjects were rotated sinusoidally in yaw at varying frequencies of up to 0.5 Hz whilst seated in a Barany chair. A comparison was made between eye movements in darkness, those produced during fixation on a central target mounted to the chair, and eye movements during fixation on the target plus an "earth-fixed" or "chair-fixed" visual background. The behavior of the vestibulo-ocular reflex at high velocities of head rotation.

The behavior of the vestibulo-ocular reflex (VOR) during both passive and active, high-velocity head movements was recorded in three normal subjects. We found that the VOR is compensatory for head velocities up to at least 350 deg . s-1, during both active and passive head rotation provided there is an attempt to visualize a real (or imagined) stationary object in space. Slow phase velocities, however, could reach values as high as 500 deg . s-1. Furthermore, during passive rotation at high velocities, the quick phase trajectory is modified--indicating an interaction between the slow phase and

Recovery of the high-acceleration vestibulo-ocular reflex after vestibular neuritis.

Vestibular neuritis (VN) usually leads to a sudden gain asymmetry of the high-acceleration horizontal vestibulo-ocular reflex (VOR). We asked whether this asymmetry decreases over time indicating peripheral recovery and/or central compensation. The horizontal VOR during rapid rotational head impulses to both sides was recorded with search coils in 37 patients at different time periods (1-240 weeks) after the onset of VN. In ten patients, sequential measurements were performed. Gains of the VOR during head impulses toward the ipsilesional side significantly increased after the initial drop (ave

Long-term vestibulo-ocular reflex changes in patients with vestibular ablation.

Long-term changes in the parameters of the vestibulo-ocular reflex (VOR) were evaluated using recently developed multilevel statistical modelling techniques in a group of patients who had undergone ablative inner ear procedures. Data from 84 patients were available for study. The most dramatic recovery in the VOR occurred during the first 60 postoperative days, however, slow changes continued, levelling off at about 120 days. While frequency was a highly significant predictor of outcomes in all parameters, age showed significant predictive value only to phase measures (p = 0.006) and gender ha

Short vestibulo-ocular reflex time-constant in complete unilateral vestibular lesions.

The time-constant of the vestibulo-ocular reflex (VORtc) in response to short accelerations was studied in 18 patients with complete unilateral vestibular lesions: 12 patients with incomplete lesions and 9 healthy controls. The marked group differences in VORtc, which was short in patients with complete lesions and long (i.e., > or = 10 s) in the controls and some patients with incomplete lesions, suggests the finding of a long VORtc in a patient with a unilateral vestibular lesion to indicate the presence of an incomplete lesion. Moreover, one analytic method (where the initial phase of decay
