BACKGROUND: The use of supply-side incentives to increase health service utilisation and enhance service quality is gaining momentum in many low- and middle-income countries. However, there is a paucity of evidence on the impact of such schemes, their cost-effectiveness, and the process of implementation and potential unintended consequences in these settings. A pay for performance (P4P) programme was introduced in Pwani region of Tanzania in 2011.
METHODS/DESIGN: An evaluation of the programme will be carried out to inform a potential national rollout. A controlled before and after study wil

The Bureau of National Health Insurance (BNHI) of Taiwan issued a new reimbursement regulation effective from 1 February 2001 forbidding the use of antimicrobials in ambulatory patients with upper respiratory infections (URI) without evidence of bacterial infection. We evaluated the effect of this regulation by analysing changes in the types of infections diagnosed and the amount of antibiotics prescribed in 1999, 2000 and 2001. Between 1999 and 2001, antimicrobials for respiratory infections decreased from 18.0 to 9.97 DDD/1000 per day or by 44.6% (P=0.0000+). Antimicrobials for URI decreased

IMPORTANCE: Quality metrics with financial incentives are widely used, but their impact on clinical care and patient health remains challenging to isolate.
OBJECTIVE: To evaluate the association of a physician-facing quality metric and financial incentive for hypertension control (blood pressure <140/90 mm Hg) with clinical decisions and health outcomes.
DESIGN, SETTING, AND PARTICIPANTS: This quasi-experimental difference-in-differences study in a large US health system compared changes in outcomes at practices that did vs those that did not adopt the financial incentive before (2021) vs af

Hospital-acquired infections (HAIs) are a common complication in inpatient care. We investigate the incentives to prevent HAIs under the German DRG-based reimbursement system. We analyze the relationship between resource use and reimbursements for HAI in 188,731 patient records from the University Medical Center Freiburg (2011-2014), comparing cases to appropriate non-HAI controls. Resource use is approximated using national standardized costing system data. Reimbursements are the actual payments to hospitals under the G-DRG system. Timing of HAI exposure, cost-clustering within main diagnoses

The Centers for Medicare & Medicaid Services (CMS) introduced the Severe Sepsis/Septic Shock Management Bundle (SEP-1) as a pay-for-reporting measure in 2015 and is now planning to make it a pay-for-performance measure by incorporating it into the Hospital Value-Based Purchasing Program. This joint IDSA/ACEP/PIDS/SHEA/SHM/SIPD position paper highlights concerns with this change. Multiple studies indicate that SEP-1 implementation was associated with increased broad-spectrum antibiotic use, lactate measurements, and aggressive fluid resuscitation for patients with suspected sepsis but not with

This study exploits policy reforms in Swedish primary care to examine the effect of pay-for-performance (P4P) on compliance with hypertension drug guidelines among public and private health care providers. Using provider-level outcome data for 2005-2013 from the Swedish Prescription Register, providers in regions using P4P were compared to providers in other regions in a difference-in-differences analysis. The results indicate that P4P improved guideline compliance regarding prescription of angiotensin converting enzyme inhibitors and angiotensin receptor blockers. The effect was mainly driven

Basic scientists and drug developers are accelerating innovations toward the goal of precision medicine. Regulators create pathways for timely patient access to precision medicines, including individualized therapies. Healthcare payors acknowledge the need for change but downstream innovation for coverage and reimbursement is only haltingly occurring. Performance uncertainty, high price-tags, payment timing, and actuarial risk issues associated with precision medicines present novel financial challenges for payors. With traditional drug reimbursement frameworks, payment is based on an assumed

UNLABELLED: A national reimbursement scheme aimed to enhance secondary fracture prevention after hip fracture. Although prevention processes improved substantially, early secondary fracture incidence did not clearly decline. Differential effects by hospital payment system suggest that financial incentives alone may be insufficient and that organizational readiness and care pathways may influence impact.
PURPOSE: In 2022, Japan introduced a reimbursement scheme to promote secondary fracture prevention (SFP) after hip fracture. We examined whether this implementation was associated with a reduc

BACKGROUND AND OBJECTIVES: The unmet demand for kidney transplantation has generated intense controversy about introducing incentives for living kidney donors to increase donation rates. Such debates may affect public perception and acceptance of living kidney donation. This study aims to describe the range and depth of public opinion on financial reimbursement, compensation, and incentives for living kidney donors.
DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Twelve focus groups were conducted with 113 participants recruited from the general public in three Australian states in February 20

Healthcare-associated infections (HAIs) result in substantial patient harm and avoidable costs. Pay-for-performance programs (PFP) through the Centers for Medicare and Medicaid Services (CMS) have resulted in reductions of HAIs like central line-associated bloodstream infections (CLABSI) and methicillin-resistant Staphylococcus aureus bacteremia, through robust infection prevention programs and practices. Hospital Onset Bacteremia and Fungemia (HOB) is proposed as an alternative quality measure for public reporting and PFP, and was endorsed by the National Quality Forum in 2022. This broad mea

The health financing system in the Democratic Republic of Congo (DRC) presents an extreme example of low government investment, high dependency on user fees and poor harmonization across donors. Within this context, performance-based financing mechanisms are being implemented by various donors in the expectation that they will improve health worker motivation and service delivery performance. Drawing on qualitative and quantitative data at different levels of the health system, this study focuses on one such programme in Katanga Province, which combines paying for performance (P4P) with a redu

Non-adherence to immune modulating agents is the single most common cause of renal graft rejection and failure with not only devastating consequences for patients, but also increased dialysis and transplant organ demands causing substantial medical expenses. Financial incentives used to reward and promote patient compliance with immune modulating therapy and post transplantation management could constitute a motivation that might increase renal graft survival, and thereby improve individual patient outcome as well as alleviate public health spending for renal replacement therapy.

OBJECTIVE: This study examines the impact of Australia's pay-for-performance (P4P) program for general practitioners (GPs). The voluntary program pays GPs A$40 and A$100 in addition to fee-for-service payment for providing patients recommended diabetes and asthma treatment over a year, and A$35 for screening women for cervical cancer who have not been screened in 4 years.
DESIGN: Three approaches were used to triangulate the program's impact: (1) analysis of trends in national claims for incentivized services pre- and postprogram implementation; (2) fixed effects panel regression models exami

Payment and financial incentive models in primary healthcare have undergone substantial transformation over the past two decades, shifting from volume-based reimbursement towards value-oriented approaches designed to improve healthcare quality and strengthen preventive care delivery. Despite their widespread implementation, evidence regarding their effectiveness remains inconsistent. This systematic review aimed to evaluate the effects of payment and financial incentive models on healthcare provider behavior, preventive care delivery, and related quality-of-care outcomes in primary healthcare

OBJECTIVE: To elucidate disparities in physician compensation and workforce distribution across gender, subspecialties, affluence, age, payers, and geographies.
METHODS: The NPI registry was linked with the U.S. Census small-area income estimates to evaluate associations between median household income and urologist density, stratified by gender. Commercial reimbursement was gathered across national payers. Reimbursements were compared between female and male urologists using Mann-Whitney U tests stratified by CPT code, subspecialty, and years in practice. Logistic regression modeled the like

BACKGROUND: Hip fractures represent a significant public health issue in an aging society. Early surgical intervention for hip fractures in older adults is associated with fewer complications and higher survival rates, and recent reimbursement incentives in Japan have aimed to encourage surgery within 48 h of injury. However, information on the determinants of delayed surgery, including reimbursement incentives, remains limited. This study aimed to investigate the number of days from admission to surgery and identify factors associated with delayed surgery using data from two acute care hospit

BACKGROUND: The 2003 revision of the UK GMS contract rewards general practices for performance against clinical quality indicators. Practices can exempt patients from treatment, and can receive maximum payment for less than full coverage of eligible patients. This paper aims to estimate the gap between the percentage of maximum incentive gained and the percentage of patients receiving indicated care (the pay-performance gap), and to estimate how much of the gap is attributable respectively to thresholds and to exception reporting.
METHODS: Analysis of Quality Outcomes Framework data in the Na

Environmental impacts of medicines are increasingly recognised but rarely embedded in their pricing and reimbursement (P&R) and related assessments. This scoping review mapped how environmental aspects, including antimicrobial resistance (AMR) stewardship, are or could be considered in P&R of medicines and identified challenges for their implementation. Electronic searches from PubMed, Scopus, Web of Science, Edilex, HeinOnline, Westlaw, Google Scholar and official agencies' websites were conducted. Eligible items covered developed country settings and described existing or proposed integratio

BACKGROUND: We aim to examine the association between primary care physicians' billing of Q050A, a pay-for-performance heart failure (HF) management incentive fee code, and the composite outcome of mortality, hospitalization, and emergency department visits.
METHODS AND RESULTS: This population-based cohort study linked administrative health databases in Ontario, Canada, for patients with HF aged >66 years between January 1, 2008, and March 31, 2020. Cases were patients with HF who had a Q050A fee code billed. Cases and controls were matched 1:1 on age, sex, patient status on being rostered t
