We report a case of renal artery thrombosis resulting from a stent fracture in a patient with a solitary functional kidney. It was successfully revascularized by surgical repair despite renal ischemia lasting more than 48 hours. This article illustrates the danger of generalizing endovascular stenting in renal artery disease regardless of the etiology. Renal artery entrapment must be kept in mind as a possible cause of renal artery stenosis. Treatment of compressive pathologies with stenting can lead to stent failure. Surgery remains the best approach for the treatment of this type of lesion.

BACKGROUND: Isolated renal artery involvement, whether inflammatory or dissecting, is an uncommon and challenging presentation in clinical practice. Its nonspecific symptoms and overlap with non-inflammatory vasculopathies complicate the distinction between localized vasculitis and isolated renal artery dissection (IRAD), two entities requiring fundamentally different therapeutic approaches.
CASE PRESENTATION: We report two cases of unilateral renal artery pathology initially suspected to represent vasculitis. Case 1: A 47-year-old man presented with acute flank pain. CT and MRA revealed peri…

BACKGROUND: Renal artery stenosis (RAS) impacts the pathogenesis and control of heart failure (HF) and may further contribute to increased cardiovascular morbidity and mortality in HF patients. However, the long-term effects of renal artery revascularization on cardiovascular outcomes in HF patients are not well studied.
METHODS: The prevalence of HF and its effects on all-cause mortality were studied in 163 consecutive patients with systemic hypertension and chronic kidney disease (serum creatinine >2 mg/dL) who underwent percutaneous transluminal renal angioplasty (PTRA) with stenting for a…

Both captopril renal scintigraphy (CRS) and conventional arteriography were retrospectively evaluated in 64 patients to study CRS efficacy i … the CRS diagnosis was falsely positive. In the presence of MRA, CRS proved effective in identifying renal artery stenosis of > 50%, involving either one or all the MRA. This study shows that the presence of MRA is not a drawback in the evaluation of renal artery stenosis by means of CRS.

OBJECTIVE: To assess the outcome of ex vivo renal artery repair with orthotopic renal autotransplantation for patients with complex renal artery disease.
METHODS: The single-center study collected and analyzed the data from patients with complex renal artery disease undergoing ex vivo renal artery repair with orthotopic renal autotransplantation, retrospectively.
RESULTS: A total of 21 complex renal artery lesions from 19 patients were included. The mean blood pressure showed a significant decrease from the preoperative to the postoperative period (P < .05). Renal function kept stable for th…

A young female who underwent nephrectomy for renovascular hypertension was diagnosed on histology to have tuberculosis of the renal artery. This was an isolated finding as there was no tuberculous infection elsewhere including tissues in the vicinity of the vessels. A survey of literature did not yield any reports of tuberculous renal arteritis, making this the first such case.

Atherosclerotic disease of the abdominal aorta and its visceral branches poses significant health risks due to its potential to cause severe complications. Atherosclerosis affects renal and mesenteric vessels, contributing to renal artery stenosis and mesenteric ischemia, respectively, both of which can result in organ damage and ischemic bowel disease if left untreated. Therapy with guideline-directed medical therapy is advised in all patients. Chronic mesenteric ischemia (CMI) is infrequent and difficult to diagnose illness. Current evidence suggests that compared with open surgical repair, …

BACKGROUND: Renal artery aneurysm (RAA) is a rare condition, traditionally managed with endovascular or open surgical techniques.
OBJECTIVE: To report our experience with robotic RAA repair.
DESIGN, SETTING, AND PARTICIPANTS: Nine consecutive patients underwent intracorporeal robotic surgery for 10 RAAs.
SURGICAL PROCEDURE: Two patients underwent concomitant robotic partial nephrectomy. One patient had RAA in a solitary kidney. Median RAA diameter was 2.2 (1.8-3)cm. Intracorporeal transarterial hypothermic renal perfusion was performed in five patients. Robotic techniques included tailored …

Renal artery stenosis (RAS) is a common form of peripheral arterial disease. The most common cause of RAS is atherosclerosis. It is predominantly unilateral. The pathophysiologic mechanism stems from renal underperfusion resulting in the activation of the renin- angiotensin-aldosterone pathway. Even though the majority of patients with RAS are asymptomatic, it can clinically present with hypertension, nephropathy and congestive heart failure. This progressive disease can lead to resistant hypertension and end stage kidney failure. Screening patients for RAS with either Doppler ultrasonography,…

Three patients with renovascular hypertension due to complex and branch lesions of the renal artery are reported, successfully managed by a technique consisting in the "ex-vivo" repair of the lesions followed by kidney autotransplantation. The indications and technical details of the procedure are subjected to a description and discussion.

BACKGROUND: Treatment of renal artery stenosis with angioplasty may be performed in patients with renovascular hypertension, ischaemic renal failure, or to preserve renal function.
MATERIAL AND METHODS: From 1982 to 1993 Rikshospitalet performed 591 renal angioplasties in 419 patients with significant renal artery stenoses. Clinical and angiographic follow-ups were performed up until 1996.
RESULTS: In patients with atherosclerotic disease, the acute success rate was 94%, primary patency 60%, and secondary patency 74%. The results were better for fibromuscular dysplasia. Patients with the hig…

A 42-year-old man presented with left hydronephrosis incidentally discovered on abdominal echogram during a routine health examination. Color Doppler ultrasonography, intravenous pyelography and angiography revealed a non-calcified renal artery aneurysm of 30 mm in size compressing the pyeroureteral junction and causing hydronephrosis. Three-dimensional computed tomography (3-D CT) using spiral CT clearly displayed the aneurysm located at the first bifurcation of the left renal artery and involving the anterior segmental artery. Decompression was successfully obtained via in situ revasculariza…

Management of renovascular disease can be based on surgical revascularization, medical therapy or interventional radiology. Results obtained with transluminal angioplasty, as compared to surgery, indicate that interventional radiology should be used as first treatment in patients with renovascular hypertension without renal failure. The best indications are treatments of fibrous dysplasia, and atherosclerotic stenosis complicated with pulmonary oedema. In case of restenosis, observed mainly with atherosclerotic stenosis, a second angioplasty eventually combined with stent implantation can be p

Five patients were treated for renal artery occlusion, as a result of embolism (2 patients), thrombosis of a stenosed vessel (1 patient) or acute occlusion during percutaneous transluminal angioplasty (2 patients). Three patients had poorly controlled hypertension. One patient was anuric. Patients were treated operatively in 4 cases and non operatively in 1 case. There were no death and no renal failure that necessitated chronic hemodialysis. All bypasses except one remained patent. Blood pressure increased in the patient with the bypass occlusion and was reduced in all four other patients. Re…
