MRI in hyperparathyroidism requiring reoperation.

A 34-year-old woman presented with persistent hyperparathyroidism (HPT) following primary surgical removal of a hypertrophied parathyroid gland. Reevaluation ultrasound (US), computed tomography (CT), and parathyroid scintigraphy studies were negative for parathyroid adenoma. Magnetic resonance imaging (MRI) clearly identified an ectopic parathyroid adenoma within the lateral carotid sheath. Uncomplicated surgical extirpation of the adenoma was accomplished. We discuss the role of US, CT, parathyroid scintigraphy, and MRI in recurrent HPT.

Reoperation for small intestinal obstruction.

Today, intestinal adhesions represent the most frequent etiology for complete or partial intestinal obstruction. Although partial obstruction can be treated nonoperatively with a considerable likelihood of success, intestinal strangulation cannot uniformly be predicted or prevented. Complete intestinal obstruction is associated with a significant incidence of strangulation if not treated by a vigorous surgical approach. Consequently, complete intestinal obstruction secondary to adhesions is still a surgical disease. Attempts at control of the adhesion process include mechanical methods to prev

Reoperation in congenital aortic stenosis.

Over a 22-year period, 81 patients underwent initial operations for critical aortic stenosis at our institution. Their ages ranged from 3 days to 20 years (mean, 7.3 +/- 5.9 years). Fourteen (17%) were infants less than 1 year old. Three children died perioperatively (3.7%). We have followed the survivors and 3 children who underwent initial operations elsewhere for a mean of 9.0 +/- 6.8 years (range, 2 to 23 years). To the present, 27 patients have undergone one reoperation (24 of our initial survivors) at a mean interval of 7.3 years, with 2 perioperative deaths. Ten of these patients have r

Reoperation for recurrent peptic ulceration.

Thirty-eight patients undergoing reoperation for recurrent peptic ulceration have been reviewed after at least 3 years. All were considered to be in Visick grade IV before reoperation. There were no operative deaths. Thirty-three of the patients ended with both a vagotomy and antral resection. After reoperation 17 patients (45 per cent) remained in Visick grades III and IV. Eight patients have had further operations and, after 47 procedures in 38 patients, 14 (37 per cent) were still in Visick grades III and IV. Vagotomy and antrectomy has a high non-ulcer mobidity rate and we conclude that ot

Reoperation following patellectomy for chondromalacia.

When primary patellectomy is performed for chondromalacia of the patella and subsequent surgery is required, this indicates failure of the initial procedure. A review of 51 patients having patellectomy for chondromalacia revealed a failure rate of 39% as evidenced by reoperation. Analysis of these cases showed continued extensor mechanism malalignment and/or continued degeneration of the distal femur or soft tissue as the reason for additional surgery. We believe that a careful preand intraoperative evaluation of the extensor mechanism kinematics and articular cartilage status of the distal fe

Reoperative maxillofacial oncology.

Approximately 36,000 people in the United States will be diagnosed with oral or oropharyngeal cancer in 2010. In more than 90% of the cases of oral cavity and oropharyngeal cancers, histopathologic examination reveals squamous cell carcinoma (SCCA). Despite appropriate initial treatment, oral SCCA recurs in 25% to 48% of cases. Several studies have shown that most cancers recur within about 2 years after the initial treatment. Hence, close follow-up of patients is important for the timely detection of recurrences. This article discusses the types of recurrence and the surveillance and treatment

Reoperative midface trauma.

Reoperative midface surgery can be challenging. Although well-established surgical principles are still the basis of surgical approaches and techniques, the advent of new materials and technologies brings about opportunities to achieve the best possible outcomes with bony reconstruction and more precise results. Soft tissue deformities continue to be some of the most challenging, especially as they relate to the orbit, but continually evolving techniques offer improved results for volume corrections to treat enophthalmos and diplopia. Conventional orthognathic and reconstructive rhinoplasty te

Reoperative retroperitoneal surgery.

Although RPLND is both a diagnostic and therapeutic procedure, it must be performed with therapeutic intent. Adequacy of initial RPLND is a prognostic variable for clinical outcome. Effective cisplatin-based chemotherapy will not reliably compensate for suboptimal initial surgery. Many patients undergoing either primary RPLND or PC-RPLND will have unresected extratemplate disease if modified templates are used. Anatomic mapping studies, which provided the basis for modified templates, have significant limitations. Teratomatous elements are often found in the retroperitoneum of patients requiri

Reoperations in Hirschsprung disease.

BACKGROUND: We sought to identify causes of preventable complications related to operations for Hirschsprung disease. METHODS: We reviewed the cases of 51 patients with Hirschsprung disease who underwent a primary procedure elsewhere, had a complication, and were referred for reoperation. RESULTS: Thirty-five patients had 1 failed operation, 10 had 2, and 6 had 3. Initial operations were Soave (20), Duhamel (15), Swenson (5), transanal endorectal (4), myectomy (3), unknown (3), and laparoscopic Swenson (1). Thirty-one patients presented with a stoma. Patients without a stoma (20) had fecal i

Reoperations in anorectal malformations.

AIM OF STUDY: Significant advances have occurred in the management of anorectal malformations, yet many patients still have technical, frequently catastrophic, operative complications that are potentially avoidable. We chose to analyze our experience in patients who have previously undergone a repair which was unsuccessful and required a reoperation, to detect the technical problems that led to complications and to try to establish a set of recommendations to avoid them. METHODS: From a series of 1806 cases of anorectal malformations, 212 were reoperated on after a failed procedure done at an

[Reoperations on the biliary system].

Reoperations of the biliary apparatus is a branch of biliary surgery still not well enough clarified and difficult to cope with. A total of 169 patients are subjected to operation and investigation in the clinic of abdominal surgery. In 71 of them reoperation is undertaken over the period 1952-1973, and in 97--in the period 1974 through 1993, representing 8.7 per cent of all biliary operations done for benign diseases of the biliary apparatus. In 95 per cent of cases the primary operation is related to cholelithiasis (ChL). One-hundred fifty-five cases (92.6 per cent) are reoperated once, nine

Reoperative thyroid surgery.

BACKGROUND: Patients with thyroid cancer are sometimes denied repeat thyroid operations for fear of an increased risk of complications. METHODS: We therefore reviewed our experience in 114 patients with benign or malignant thyroid tumors who underwent 116 thyroid reoperations with or without other procedures. All patients had undergone at least one prior thyroid operation and 16 patients had undergone from two to four thyroid operations before referral. The initial histologic diagnosis before reoperation was thyroid carcinoma in 79 patients, papillary carcinoma in 47 patients, follicular carc

[Reoperations in lumbar discopathies].

In the years 1976-1990 at the Neurosurgery Centre in Rzeszów 938 patients were treated surgically for lumbar discopathy. Reoperation was necessary in 5.4% of cases. In 0.6% of them purulent wound infection was the cause, in the remaining 4.8% lacking improvement or recurrence of sciatic pain after 1 month to 12 years. In 2.2% of cases the cause of pain recurrence was recurrent hernia at the same or other level, and after its removal sciatic pain disappeared. Root compression by cicatricial tissue or osteophyte caused reoperation in 1% and surgical decompression of the root gave only minor impr

[Reoperations in primary hyperparathyroidism].

We report on 14 patients suffering from persistent (n = 13) and truly recurrent (n = 1) primary hyperparathyroidism. Reoperations were successful in 13 cases. Cervical reexploration was only required for 5 patients who had previous operations by unexperienced surgeons in other clinics. In 5 out of 7 patients operated by us, the causes of the persistent primary hyperparathyroidism were 4 adenomas and one hyperplastic gland in the mediastinum found by median sternotomy. The remaining cases were a rare ectopic location of the inferior glands and an inadequate resection in four-gland-hyperplasia.

Reoperative parathyroid surgery.

Reoperation for persistent or recurrent primary hyperparathyroidism immediately connotes a complex clinical management problem. Successful cure of hypercalcemia is less frequent whereas complications are more common compared to initial explorations. Of 212 patients operated on at the Mayo Clinic from 1978 through 1986, 189 (89%) were cured. Sporadic disease, multiple endocrine neoplasia, and familial hyperparathyroidism were found in 183 (87%), 20 (9%), and 9 (4%) patients, respectively. Prior to the most recent reoperation, these patients had undergone from one to five operations. Preoperativ

Fourteen patients underwent carotid reoperation for symptomatic recurrent carotid stenosis after previous ipsilateral carotid endarterectomy. Eight of these patients presented with focal transient ischemic attacks, two with strokes, and four with vertebrobasilar insufficiency. Recurrent symptoms in eight patients were similar to those prompting the initial carotid endarterectomy. Symptoms recurred early after previous carotid endarterectomy in 2 patients and late in 12 patients.
