Differential effects of chemical and physical restraint on carbohydrate tolerance testing in nonhuman primates.

Ketamine hydrochloride, used during glucose or lactose tolerance tests to aid restraint of rhesus and stumptail macaques unconditioned to handling, prevented the establishment of definitive baseline plasma glucose tolerance curves from which meaningful interpretations could be derived. Repetition of the tests after the animals were restraint-chair conditioned provided clear baseline control data. It is suggested that ketamine might invalidate those carbohydrate tolerance tests which are based on the absorption of glucose from the gastrointestinal tract.

Deaths in custody: Physical restraint, excited delirium and medico-legal investigation - A systematic review.

BACKGROUND/OBJECTIVES: Deaths in custody represent a critical intersection between health care, law enforcement and human rights. By depriving an individual of liberty, the State assumes an enhanced duty to protect life and ensure access to adequate medical care. Any death occurring in custodial settings must therefore be regarded as potentially suspicious until clarified through a prompt, independent and technically robust investigation. METHODS: This systematic review synthesizes and critically examines scientific evidence on deaths in police and prison custody, with particular emphasis on

ICU Nurse Perspectives on Influences of Minimal Physical Restraint Practices: A Mixed-Methods Study.

BACKGROUND: Physical restraint is still common in Chinese intensive care units, especially for mechanically ventilated patients and those with invasive devices. Inappropriate use causes considerable physical and psychological harm. Despite global efforts to minimise restraint, systematic research on standardised assessment and management frameworks for restraint reduction in domestic ICUs is lacking. AIMS: This study aimed to explore the influencing factors of nurses' implementation of minimising physical restraint and identify key pathways for optimising clinical restraint management practic

A review of the scientific literature related to the adverse impact of physical restraint: gaining a clearer understanding of the physiological factors involved in cases of restraint-related death.

Deaths occurring during and/or in close proximity to physical restraint have been attributed to positional asphyxia, a conclusion primarily based on opinion and reviews of case studies. This review sought to identify the current scientific evidence available in regard to the aetiology of adverse events or death occurring during or in close proximity to physical restraint. A systematic search of electronic databases (SPORTDiscus, AMED, CINAHL, MEDLINE, PsycINFO) for papers published in English, between 1980 and 2011, using keywords that related to restraint, restraint position and cardiovascula

Association between patient primary language, physical restraints, and intramuscular sedation in the emergency department.

BACKGROUND: Despite the importance of effective communication during verbal de-escalation, research regarding patient primary language during management of agitation symptoms is limited. We evaluated associations between patient primary language and use of physical restraints and intramuscular (IM) sedation in the emergency department (ED). METHODS: This was a retrospective cohort analysis evaluating physical restraint and IM sedation characteristics using electronic medical records from 13 EDs affiliated with a large regional health care network located in the northeast United States. Disparities in use of physical restraint and chemical sedation in the emergency department by patient housing status.

BACKGROUND: A growing body of research has found there to be disproportionate physical restraint and chemical sedation use for historically marginalized populations in the emergency department (ED). This association has been examined with regard to patient race, ethnicity, sex, and age. Preliminary research has highlighted the ways in which unhoused status may also relate to the use of physical restraint and chemical sedation in the ED. Given the adverse health outcomes associated with these methods in the ED, further research is needed to explore the relationship between patient housing statu

Outcomes of Prehospital Chemical Sedation With Ketamine Versus Haloperidol and Benzodiazepine or Physical Restraint Only.

OBJECTIVE: The goal of this study is to describe complications and outcomes of prehospital ketamine use for agitation as compared to other methods of physical or chemical restraint such as haloperidol plus benzodiazepine or physical restraint only. METHODS: We conducted a single-center retrospective review of patient encounters in which restraint was administered in the prehospital setting. At the beginning of our study window, only physical restraint was available to paramedics managing agitated patients but subsequently, haloperidol and benzodiazepines were introduced, followed by ketamine

Social validation data on three methods of physical restraint: views of consumers, staff and students.

The use and evaluation of restraint methods with people with mental retardation is a continuing area of concern. Twenty-four undergraduate students, 21 residential care staff and 18 service-users from community settings rated videotapes of three physical restraint procedures. Two of the methods involved restraining an individual on the floor and a third method involved restraining an individual in a chair. Participants answered two open-ended questions to rate the methods of restraint and rated the methods on a 5-point scale of satisfaction [J. Ment. Defic. Res. 30 (1986) 369]. Participants al

Sudden unexplained death in a psychiatric patient--a case report: the role of phenothiazines and physical restraint.

Psychotropic drug use has long been associated with sudden unexplained and unexpected death in psychiatric patients despite controversies surrounding the issue. Physical restraint following violent episodes in psychiatric in-patients is also associated with neurally mediated sudden cardiac death. A case where these two mechanisms have jointly resulted in sudden death is reported. The literature on the subject is reviewed and the measures which may be useful in reducing the incidence of such deaths are discussed. The need for accurate and detailed reporting of such cases is emphasized.

Experiences of Intensive Care Nurses in Their Practice of Physical Restraint Among Patients: A Qualitative Study.

INTRODUCTION: Although physical restraint causes mental and physical harm to patients, such practice occurs worldwide to ensure patient safety. It is non-standard practice for a number of reasons, including the lack of education, training, and protocol. OBJECTIVE: This study aims to determine the experiences of nurses who apply physical restraint to patients. METHODS: A descriptive phenomenological method was employed in this study, wherein semi-structured interviews were the data collection method. In this qualitative study, the data consisted of semi-structured individual interviews with n

Leadership as decision architecture: an organizational framework for understanding and eliminating physical restraint in human services.

Physical restraint persists at high rates in human services settings supporting individuals with intellectual and developmental disabilities (IDD), even within organizations that have formally adopted positive behavior support (PBS) frameworks and invested substantially in staff training. This perspective article introduces the Staff Decision Ecology Model, a decision ecology account of physical restraint, which conceptualizes restraint as a context-dependent, staff-mediated decision shaped by nested individual, environmental, and organizational variables, and argues that leadership functions

Interventions for preventing and reducing the use of physical restraints of older people in general hospital settings.

BACKGROUND: Physical restraints, such as bedrails, belts in chairs or beds, and fixed tables, are commonly used for older people in general hospital settings. Reasons given for using physical restraints are to prevent falls and fall-related injuries, to control challenging behavior (such as agitation or wandering), and to ensure the delivery of medical treatments. Clear evidence of their effectiveness is lacking, and potential harms are recognised, including injuries associated with the use of physical restraints and a negative impact on people's well-being. There are widespread recommendation

Nursing Skill Mix, Nurse Staffing Level, and Physical Restraint Use in US Hospitals: a Longitudinal Study.

BACKGROUND: Although it is plausible that nurse staffing is associated with use of physical restraints in hospitals, this has not been well established. This may be due to limitations in previous cross-sectional analyses lacking adequate control for unmeasured differences in patient-level variables among nursing units. OBJECTIVE: To conduct a longitudinal study, with units serving as their own control, examining whether nurse staffing relative to a unit's long-term average is associated with restraint use. DESIGN: We analyzed 17 quarters of longitudinal data using mixed logistic regression,
