Macular cysts in retinal dystrophy.

PURPOSE OF REVIEW: To describe the entity of macular cysts in retinal dystrophy, differentiate it from cystoid macular edema (CME), and review the role of carbonic anhydrase inhibitors in management. RECENT FINDINGS: Macular cysts in retinal dystrophy are seen in retinopathies caused by mutations in the NR2E3 gene, juvenile X-linked retinoschisis (XLRS), and some other retinal dystrophies. These must be distinguished from CME. Optical coherence tomography can clearly demonstrate intraretinal cysts which may not be clinically detectable. Intravenous fluorescein angiography (IVFA) does not show

Retinitis pigmentosa and other dystrophies.

Retinitis pigmentosa (RP) is an inherited retinal degeneration that affects predominantly peripheral visual fields. Macular edema may cause additional central visual acuity decrease. Fluorescein angiography and/or optical coherence tomography detect the presence of macular edema in 10-20% of RP patients. Macular edema can manifest at any stage of the disease and may be unilateral or bilateral. In X-linked forms, macular edema is very rare. The origin of macular edema in RP patients still remains poorly understood. The possible pathophysiological role of autoantibodies has been suggested (retin

Genetic landscape of inherited retinal dystrophies in a Welsh tertiary referral centre.

BACKGROUND/AIMS: Inherited retinal dystrophies (IRD) are a group of predominantly monogenic disorders which have genetically heterogeneous origins and display wide clinical phenotypic heterogeneity. The current study describes the clinical diagnoses, demographics and genetic aetiology of a cohort of Welsh patients to provide insight into the rates of genetic diagnosis of IRD. This will help inform patient prognosis and counselling in future clinical encounters. METHODS: A comprehensive database of patients attending the tertiary referral clinic from January 2011 to December 2023 was compiled

Mitochondrial Macular Dystrophy-A Case Report and Mini Review of Retinal Dystrophies.

Background: Retinal dystrophies are often challenging to diagnose. At early stages, they may resemble benign retinal pigment epithelium alterations and drusen present in otherwise healthy individuals. With the increased incidence of autoimmunity-related disorders and new treatments for retinal dystrophies on the horizon, thorough investigations and making the correct diagnosis in time are particularly important for these patients. Case report: A 44-year-old myopic female was admitted to the Ophthalmology Department with a 3-week history of painless blurred vision in her right eye. Fundoscopic

[Genetic diagnostics of retinal dystrophies : Breakthrough with new methods of DNA sequencing].

Until the mid-2000s, knowledge about the genetic causes of retinal dystrophies was not adequately translated into molecular diagnostics and genetic counselling offered to the patients. Although many genes whose mutations underlie retinal degeneration, e.g., retinitis pigmentosa, Leber congenital amaurosis and cone-rod dystrophies were known, they could not be analyzed on a routine diagnostic basis because DNA sequencing was too expensive and time-consuming. New methods summarized under the term next-generation sequencing (NGS) procedures for high-throughput sequencing have changed this complet

Single-Cell Transcriptomics in Inherited Retinal Dystrophies: Current Findings and Emerging Perspectives.

Inherited retinal dystrophies (IRDs) represent a diverse group of disorders caused by mutations in genes essential for retinal function and maintenance. Traditional bulk RNA sequencing techniques provide valuable information for deciphering disease pathogenesis but lack the resolution to capture variation among specific cell clusters during disease progression. In contrast, single-cell transcriptomics, including single-cell RNA sequencing (scRNA-seq), enables detailed examination of distinct retinal clusters in both healthy and diseased states, uncovering unique gene expression signatures and

Functional imaging of mitochondria in genetically confirmed retinal dystrophies using flavoprotein fluorescence.

BACKGROUND: Whether by indirect oxidative stress or direct genetic defect, various genetic retinal dystrophies involve mitochondrial stress. Mitochondrial flavoprotein fluorescence (FPF), reported as either average signal intensity or variability (heterogeneity), may serve as a direct, quantifiable marker of oxidative stress. MATERIALS AND METHODS: This observational study enrolled patients with genetically confirmed retinal dystrophies between January and December 2021. Patients with concomitant maculopathy and ocular hypertension were excluded. Patients were FPF imaged with OcuMet Beacon® t

Precise CRISPR/Cas9 and Cas12 Correction Using Lipoplexes in Retinal Models Derived from Patients with Inherited Retinal Dystrophies.

Gene editing, particularly CRISPR/Cas technology, represents a promising approach for the treatment of rare genetic diseases, including inherited retinal dystrophies, for which effective therapies are largely unavailable. Despite extensive research investigating gene editing across a wide range of cell types, transient delivery of CRISPR/Cas components and efficient homology-directed repair (HDR) in differentiated cells remain challenging. In this study, we employed hiPSCs derived from patients with Stargardt disease or Best disease, carrying pathogenic variants in ABCA4 or BEST1, respectively

Tapetal-like reflex in RPGR-associated retinal dystrophies: two cases with distinct phenotypes.

INTRODUCTION: Retinitis pigmentosa GTPase regulator (RPGR) mutations are associated with X-linked inherited retinal diseases (IRDs), with variable phenotypes including retinitis pigmentosa (RP), cone-rod dystrophy (CRD), and cone dystrophy (CD). Tapetal-like reflex (TLR) is commonly observed in female carriers and has more recently been reported in hemizygous male patients with cone-dominant or cone-rod phenotypes. Here, we report two male patients with RPGR mutations and TLR. MATERIALS AND METHODS: Clinical history, ophthalmic examination, multimodal retinal imaging, and genetic testing were
