Intrasellar primitive neuroectodermal tumor (PNET) in familial retinoblastoma: a variant of "trilateral retinoblastoma".

An infant with bilateral familial retinoblastomas was found at age 6 months to have a large mass within the region of the sella turcica. The histology, immunoreactivity and ultrastructure of the tumor showed primitive neuroectodermal cells. A small number of cells showed positive immunostaining to retinal S antigen. This represents a rare form of so-called trilateral retinoblastoma. In most cases of trilateral retinoblastoma, the intracranial non-metastatic tumor is within the pineal gland. The occurrence of such a tumor in the region of sella turcica is very unusual and its possible significa

Benign pineal cysts in children with bilateral retinoblastoma: a new variant of trilateral retinoblastoma?

PURPOSE: Patients with hereditary retinoblastoma (Rb) develop in 4%-8% a malignant midline tumor called trilateral Rb (TRb). We report in this study on benign pineal cysts observed in patients investigated for TRb. PATIENTS AND METHODS: Between September 1990 and December 2001, 172 patients were screened for TRb. Ninty-five had bilateral, 77 unilateral disease. The median age at diagnosis of Rb was 7 months (range 1-26). Treatment included enucleation, local treatment with cryotherapy or photocoagulation, first-line chemotherapy (CT), thermo-chemotherapy (TCT), Ruthenium plaque, and, rarely,

Trilateral retinoblastoma variant indicative of the relevance of the retinoblastoma tumor-suppressor pathway to medulloblastomas in humans.

Results of recent studies have led investigators to suggest that the retinoblastoma tumor-suppressor (rb) gene plays an underappreciated role in the genesis of brain tumors. Such tumors cause significant rates of mortality in children suffering from hereditary retinoblastoma. It has been assumed that the pineal gland, which is ontogenetically related to the retina, accounts for the intracranial origin of these trilateral neoplasms. To address this issue, the authors describe an unusual trilateral retinoblastoma variant. The authors provide a detailed clinicopathological correlation by describi

Separation of retinoblastoma and esterase D loci in a patient with sporadic retinoblastoma and del(13)(q14.1q22.3).

A chromosome 13 deletion in a patient with sporadic retinoblastoma appears to have separated the loci for retinoblastoma and esterase D. This study indicates that: (1) the retinoblastoma locus is distinct from the esterase D locus; and (2) the linear order of these genes is centromere-esterase D-retinoblastoma.

Secondary Sinonasal Malignancies After Retinoblastoma.

PURPOSE: Retinoblastoma (RB) is the most common intraocular tumor in childhood. Survivors of RB are at risk of developing secondary malignancies. Prior radiotherapy substantially increases the risk of malignancy in the orbit, brain, and nasal cavities. The authors report 2 new cases of secondary sino-orbital malignancies in RB survivors who underwent radiotherapy during early childhood. A literature review on secondary sinonasal and orbital malignancies in RB is presented. METHODS A: case series and literature review of relevant publications on the topic was performed, with appropriate data e

Retinoblastoma: Advances in Genetic Testing.

Retinoblastoma is a genetic condition initiated by pathogenic variants causing biallelic loss of function of the RB1 gene. Genetic testing is a crucial component in the evaluation, treatment, and surveillance of retinoblastoma patients. A pathogenic germline mutation determines retinoblastoma heritability, a distinction that possesses significant implications for personal prognosis, risk assessments for family members, family planning, and clinical decision making. Any patient with a diagnosis of retinoblastoma with an uncertain RB1 status and first-degree relatives of probands with heritable

Animal models in retinoblastoma research.

Advances in animal models of retinoblastoma have accelerated research in this field, aiding in understanding tumor progression and assessing therapeutic modalities. The distinct pattern of mutations and specific location of this unique intraocular tumor have paved the way for two types of models- those based on genetic mutations, and xenograft models. Retinoblastoma gene knockouts with an additional loss of p107, p130, p53 and using promoters of Nestin, Chx10, and Pax6 genes show histological phenotypic changes close to the human form of retinoblastoma. Conditional knockout in specific layers

[Intraocular retinoblastoma: new therapeutic options].

Retinoblastoma is the most frequently occurring primary intraocular malignant tumour in children (12-15 new patients per year in the Netherlands). It occurs in one or two eyes. Bilateral retinoblastoma, which occurs in 40% of the cases, is always hereditary; unilateral retinoblastoma, which is found in 60% of cases, is hereditary in 10% of these cases. The presenting symptoms are: leucocoria, strabismus or a red, painful eye. Early detection of retinoblastoma is important for the chance of survival, the visual prognosis and preservation of the eye. The choice of treatment is based on the risk

Retinoblastoma protein in microphthalmic mice.

A microphthalmic strain of mice was used to study immunoresponse of the retinoblastoma protein. Comparing wild-type, heterozygote and homozygote microphthalmic eyes, we found an increasing labelling of phosphorylated retinoblastoma protein (pRb) in the retinal pigment epithelium. Additionally, microphthalmic eyes expressed pRb in the neuroepithelium. Especially rosettes were strongly labelled.

Trilateral retinoblastoma--a case report.

A sporadic case of trilateral retinoblastoma in a male child of 20 months is described using clinical and imaging approaches. Attempts were made to find out a tumour marker by analysing human chorionic gonadotrophin (hCG), alpha-feto protein (AFP), carcinoembryonic antigen (CEA), and lactic dehydrogenase (LDH) in blood and cerebrospinal fluid (CSF). Only LDH was elevated in CSF and the rest were normal. Retinoblastoma gene could not be isolated in this patient. It is proposed that LDH in CSF should be analysed in patients with bilateral retinoblastoma who had normal brain scan initially. If LD

Trilateral retinoblastoma with suprasellar involvement.

"Trilateral retinoblastoma" (TRB) is the association of a midline intracranial tumor with familial bilateral retinoblastoma. Classically, the intracranial tumor is a pineal region tumor (pineoblastoma) with histology similar to retinoblastoma. We present a 7-month-old child with bilateral orbital retinoblastoma with a third tumor arising in the chiasmatic cistern. The presentation and prognosis of patients with this association differ from those children with "classical" TRB. The presentation, treatment, and outcome are described, with a review of the literature.
