Association Between Rewarming Rate and Survival and Neurologic Outcome of Accidental Hypothermia.

OBJECTIVES: Accidental hypothermia has high mortality. Rewarming is the initial primary management strategy. However, detailed evidence on rewarming management is limited, that is, rewarming rate is unclear, particularly with noncardiac arrest. Here, we evaluated the association between rewarming rate in the early phase of rewarming and survival and neurologic outcomes in patients with accidental hypothermia. DESIGN: A secondary analysis of a nationwide, multicenter, prospective, observational study-the Intensive Care with ExtraCorporeal membrane oxygenation Rewarming in Accidentally Severe H

[Design and application of a multifunctional infrared wind heating medical rewarming equipment].

Hypothermia can have adverse effects on various systems of trauma patients, and significantly increase the mortality. All of the current rewarming equipments are contact rewarming equipment, which have the shortcomings of single function and poor effect. The medical staff of the First People's Hospital of Chenzhou designed a multi-functional infrared heating medical rewarming equipment, and obtained the National Utility Model Patent of China (ZL 2018 2 1705172.9). By integrating the infrared heating lamp tube and the air heating device and controlling them independently, the equipment can not

Comparison Between Two Pharmacologic Strategies to Alleviate Rewarming Shock: Vasodilation vs. Inodilation.

Rewarming from hypothermia is often challenged by coexisting cardiac dysfunction, depressed organ blood flow (OBF), and increased systemic vascular resistance. Previous research shows cardiovascular inotropic support and vasodilation during rewarming to elevate cardiac output (CO). The present study aims to compare the effects of inodilatation by levosimendan (LS) and vasodilation by nitroprusside (SNP) on OBF and global oxygen transport during rewarming from hypothermia. We used an in vivo experimental rat model of 4 h 15°C hypothermia and rewarming. A stable isotope-labeled microsphere techn

Cardiac troponin-I phosphorylation underlies myocardial contractile dysfunction induced by hypothermia rewarming.

Rewarming the intact heart after a period of hypothermia is associated with reduced myocardial contractility, decreased Ca2+ sensitivity, and increased cardiac troponin-I (cTnI) phosphorylation. We hypothesized that hypothermia/rewarming (H/R) induces left ventricular (LV) contractile dysfunction due to phosphorylation of cTnI at Ser23/24. To test this hypothesis, the response of wild-type mice (n = 7) to H/R was compared with transgenic (TG) mice expressing slow skeletal TnI (TG-ssTnI; n = 7) that lacks the Ser23/24 phosphorylation sites. Hypothermia was induced by surface cooling and maintai

Concentrations of remifentanil, propofol, fentanyl, and midazolam during rewarming from therapeutic hypothermia.

BACKGROUND: The clearance of sedatives and analgesics may be reduced by therapeutic hypothermia. However, little is known about the concentrations of such drugs during rewarming. The aim of this study was to describe the serum concentrations of sedatives and analgesics during rewarming from therapeutic hypothermia. METHODS: Blood samples were collected for quantification of drug concentrations in 22 patients given analgesia/sedation with either remifentanil/propofol or fentanyl/midazolam during rewarming from therapeutic hypothermia (33-34°C) after cardiac arrest. Samples for were drawn befor

Finger anthropometrics may not be a primary influence on the thermal responses to cooling and rewarming.

The fingers have a large surface area to volume ratio (SA:V), minimal muscle mass, and potent vasoconstrictor capacity. These qualities make the fingers prone to heat loss and freezing injuries during whole-body or local cold exposure. Anthropologists have proposed that the large inter-individual variability in human finger anthropometrics may be an ecogeographic evolutionary adaptation, where shorter and thicker digits (i.e. smaller SA:V ratio) provide a favorable adaptation for cold climate natives. We hypothesized that the SA:V ratio of a digit has an inverse relationship with finger blood

Efficacy of Head and Torso Rewarming Using a Human Model for Severe Hypothermia.

INTRODUCTION: To evaluate the rewarming effectiveness of a similar amount of heat (from a charcoal heater) applied to either the head or torso in a human model for severe hypothermia in which shivering is pharmacologically inhibited in mildly hypothermic subjects. METHODS: Six male subjects were cooled on 3 different occasions, each in 8°C water for 60 min, or to a lowest core temperature of 35°C. Shivering was inhibited by intravenous meperidine (1.5 mg·kg-1), administered during the last 10 min of the cold-water immersion. Subjects then exited from the cold water, were dried, and were place

A novel approach to rapid rewarming of a frostbitten extremity: The sous vide method.

BACKGROUND: Rapid rewarming of an acutely frostbitten extremity has been the standard treatment for nearly 60 years, however, there are no existing practical recommendations to create a warm water bath. Our study describes a novel approach to rapid rewarming using a sous vide cooking device to create and maintain a circulating warm water bath at a desired set temperature. METHODS: A series of in vitro experiments were performed to assess the efficacy of different methods of maintaining constant water temperature while rapidly rewarming a simulated frostbitten extremity (frozen pig's foot). [Efficacy of propofol in controlling myoclonus during rewarming in a brain hypothermia patient].

Propofol is an intravenous anaesthetic agent having anticonvulsant property. We report here a case in which propofol was effective in controlling myoclonus during rewarming in brain hypothermia patient. A 35-year-old male patient was admitted in a comatose state with right-sided hemiparesis, anisocoria and absence of bilateral light reflex. On admission, a head CT showed traumatic subarachnoid hemorrhage, left subdural hematoma, 10 mm midline shift and tentorial herniation with massive brain swelling together with extensive hypodensity in the frontal, temporal and occipital lobes bilaterally.

Seizures after initiation of rewarming in cooled infants with hypoxic ischaemic encephalopathy.

BACKGROUND: Seizures after initiation of rewarming from therapeutic hypothermia for neonatal encephalopathy are well recognised but not easy to predict. METHODS: A secondary analysis was performed of NEOLEV2 trial data, a multicentre randomised trial of levetiracetam versus phenobarbital for neonatal seizures. Enrolled infants underwent continuous video EEG (cEEG) monitoring. The trial data were reviewed for 42 infants with seizures during therapeutic hypothermia and 118 infants who received therapeutic hypothermia but had no seizures on cEEG. RESULTS: Overall, 112 of 160 (70%) had cEEG moni

Clinical utilization of a sous vide device in the acute rewarming of frostbitten extremities.

The current standard of care for acute frostbite rewarming is the use of a circulating warm water bath at a temperature of 37 °C to 39 °C. There is no standardized method to achieve this. Manual management of a warm water bath can be inefficient and time consuming. This case describes the clinical use of a sous vide cooking device to create and maintain a circulating warm water bath to rewarm acute frostbite. A 34 year-old male presented to the emergency department with acute frostbite. Each of the patient's feet were placed in a water bath with a sous vide device attached to the side of the b

Rewarming curves and derived parameters in the diagnosis of hand-arm vibration syndrome.

BACKGROUND: Exposure to hand-arm vibrations is a known cause of the Hand-arm vibration syndrome (HAVS), a progressive syndrome beginning with sensory loss and leading to gangrene, making timely diagnosis essential. OBJECTIVES: Assessment of the usefulness of 9 diagnostic parameters claimed as being of greatest value in the diagnosis of HAVS, and examination of the complementary diagnostic value of the curve shapes. METHODS: Three groups of subjects (HAVS cases, exposed workers without irreversible changes, and controls) were examined by cold provocation followed by thermographic imaging, obt

Human medullary responses to cooling and rewarming the skin: a functional MRI study.

A fall in skin temperature precipitates a repertoire of thermoregulatory responses that reduce the likelihood of a decrease in core temperature. Studies in animals suggest that medullary raphé neurons are essential for cold-defense, mediating both the cutaneous vasoconstrictor and thermogenic responses to ambient cooling; however, the involvement of raphé neurons in human thermoregulation has not been investigated. This study used functional MRI with an anatomically guided region of interest (ROI) approach to characterize changes in the blood oxygen level-dependent (BOLD) signal within the hum

Airway cooling and rewarming. The second reaction sequence in exercise-induced asthma.

To determine if a relationship exists among the magnitude and rate of airway rewarming, and the severity of bronchial obstruction in thermally induced asthma, we had seven subjects perform three- to four-point stimulus response curves with isocapnic hyperventilation of frigid air with and without pretreatment with inhaled norepinephrine. The latter was employed to alter the heat supplied to the airway walls by producing vasoconstriction. 1-s forced expiratory volume (FEV1) was measured before and 5 min after the cessation of each bout of hyperpnea and before and after norepinephrine. On a sepa

Effect of Exercise Intensity on Active Rewarming From Outdoor Cold-Water Immersion.

INTRODUCTION: Cold-water immersion (CWI) is a threat to warfighters operating in the cold. Even after removal from the cold, core temperature continues to decrease because of the afterdrop effect. The purpose of this study is to evaluate the effect of exercise-intensity on rewarming from CWI. MATERIALS AND METHODS: Following a randomized match-paired design, 28 military personnel (27 Male, 1 Female; mean ± SD age: 29 ± 7year; ht: 175 ± 11 cm; wt: 86.9 ± 12.5 kg; body fat: 22 ± 9%) were immersed in an outdoor pond (air: -4.2 °C; water: 1.3 °C) for 10 minutes and subsequently rewarmed for 40 mi

Role of erythrocytes in short-term rewarming kidney perfusion after cold storage.

Short term normothermic reconditioning by machine perfusion after cold storage has shown beneficial effects in renal transplantation models. Systematic investigations concerning the inclusion of washed erythrocytes as oxygen carriers are lacking in this context. Porcine kidneys were subjected to 20 h of static cold storage. Prior to reperfusion, grafts were put on a machine for 2 h of oxygenated (95% O2 ; 5% CO2 ) rewarming perfusion. In one group (n = 6) washed erythrocytes were added to the perfusate after temperature has reached 20°C; the other group (n = 6) was run without additives. Contr
