Intraoperative neurophysiology (ION) plays a crucial role in dorsal rhizotomy, a procedure aimed at reducing spasticity while preserving neural function. ION encompasses monitoring and mapping, with the mapping of the root/rootlet constituting a significant component. Despite the acknowledged roles of ION, persistent controversies exist, and a universally accepted standard for integrating ION procedures into dorsal rhizotomy remains elusive. This chapter provides a detailed explanation of the author's approach to dorsal rhizotomy, with a primary focus on the decision-making process for root/ro

BACKGROUND: The effectiveness of pudendal afferents mapping in posterior sacral rhizotomies needs to be reviewed.
OBJECTIVE: To evaluate the effectiveness of pudendal afferents mapping for both the dorsal penile or clitoral nerve and the inferior anal nerve to decrease the risk of postoperative bowel and bladder dysfunction when the sacral nerve roots are candidates for rhizotomies.
METHODS: A retrospective review of 101 Asian children who underwent functional posterior rhizotomies with pudendal afferents mapping for spastic paresis was performed.
RESULTS: Pudendal mapping was successful in

PURPOSE: To raise the awareness of the rare complication of monocular blindness resulting from radiofrequency rhizotomy for trigeminal neuralgia.
METHODS: Case series from tertiary referral centers. Patients were referred after complaint of loss of vision.
RESULTS: We report three patients who had trigeminal neuralgia and underwent treatment by radiofrequency rhizotomy. Each developed complete loss of vision in one eye immediately after the procedure. One patient died of an unrelated cause, and pathologic analysis revealed changes compatible with acute trauma to the right optic nerve.

Selective posterior lumbar rhizotomy has recently become an alternative method for relieving spasticity in patients with spastic cerebral palsy. This procedure involves the selective sectioning of the lumbosacral posterior nerve rootlets. Because the whole nerve root is not cut, tactile and proprioceptive sensation remain intact. The present study measured the changes that occurred in 29 patients with spastic cerebral palsy 2 days before surgery and from 4 to 14 months after surgery. Each patient was used as his or her own control. In addition to the clinical evaluation of function, gait was a

INTRODUCTION: While selective dorsal rhizotomy (SDR) improves spasticity, the benefit in hereditary spastic paraplegia (HSP) remains unclear. This study describes a single institution's outcomes and experience with SDR in children with HSP.
METHODS: We conducted a retrospective chart review of pediatric patients (≤18 years) with HSP who underwent SDR at Monroe Carell Jr. Children's Hospital between July 2013 and January 2024. Patients were evaluated with standardized assessments, including the Modified Ashworth Scale (MAS), Gross Motor Function Classification System (GMFCS), Gross Motor Funct

BACKGROUND/PURPOSE: Chronic low back pain is a leading cause of disability worldwide, with lumbar facet joint syndrome accounting for a significant proportion of cases. While percutaneous radiofrequency ablation is the current standard of care for patients refractory to conservative therapy, endoscopic rhizotomy has emerged as a promising alternative with potential for improved precision and durability. This systematic review aims to evaluate the efficacy of endoscopic rhizotomy and compare the clinical outcomes with percutaneous ablation in the management of lumbar facet-mediated low back pai

Spasticity is a prevalent symptom of upper motor neuron syndrome, becoming debilitating when hindering voluntary movement and motor function and causing contractures and pain. Functional neurosurgery plays a crucial role in treating severe spasticity. Despite extensive literature on SDR for lower limb spasticity, there is a scarcity of papers regarding the procedure in the cervical region to alleviate upper limb spasticity. This case report details a cervical dorsal rhizotomy (CDR) performed for upper limb spasticity, resulting in significant improvement in spasticity with sustained outcomes a

OBJECTIVE: Internal neurolysis (IN) and intraoperative glycerin rhizotomy (ioGR) are emerging surgical options for patients with trigeminal neuralgia without neurovascular contact. The objective of this study was to compare the neurological outcomes of patients who underwent IN with those of patients who underwent ioGR.
METHODS: The authors retrospectively reviewed all patients who underwent IN or ioGR for trigeminal neuralgia at our institution. Patient demographic characteristics and immediate postoperative outcomes, as well as long-term neurological outcomes, were compared.
RESULTS: Of 10

PURPOSE: The aim of this study was to determine the outcomes for children who underwent selective dorsal rhizotomy (SDR) for the treatment of spasticity related to spinal pathology.
METHODS: We performed a retrospective review of all cases of SDR at our institution over the last 30 years and identified patients in whom spasticity was attributed to spinal rather than cerebral pathology. We gathered demographic information and recorded functional status and spasticity scores pre-operatively and over long-term follow-up.
RESULTS: We identified four patients who underwent SDR for spinal-related

INTRODUCTION: Given the large number of cerebral palsy patients who have undergone selective dorsal rhizotomy in the past two decades, it is clearly imperative that the clinical community be provided with objective and compelling evidence of the long-term sequelae of the procedure.
MATERIALS AND METHODS: In the early 1980s, Peacock in Cape Town shifted the site of the rhizotomy from the conus medullaris to the cauda equina, and in the past 25 years, more than 200 children have been operated on. We have studied the incidence of spinal deformities after multiple-level laminectomy and recorded a

Selective dorsal rhizotomy (SDR) followed by intensive postoperative therapy is commonly performed to reduce spasticity in carefully selected children with cerebral palsy. Reduction in spasticity and improvement in range and quality of movements are frequently reported. Functional gains have also been indicated but have not often been examined in a systematic manner. Twenty-nine children with spastic diplegia or quadriplegia received SDR and postoperative physical and occupational therapy. Upper-limb movement, self-care, and functional mobility were evaluated prospectively through preoperative

A series of 144 lesions made during 32 radiofrequency rhizotomies was reviewed. The parameters of each lesion were compared with the sensory change resulting from the lesion. One-half of the lesions resulted in no sensory change, but 16.7% produced numbness in a division not predicted by stimulation. This was a barely detectable sensory change in 6% of lesions and more dense in 10%. The first lesion in a series was particularly likely to result in sensory deficit, and accounted for half of the more dense lesions not predicted by stimulation. When the threshold at which stimulation was perceive

Spasticity, characterized by muscle hypertonia, in children poses long-term challenges, leading to motor dysfunction, joint contractures, and a decline in overall quality of life (QOL). This underscores the critical need for effective spasticity management in disabled children. Various interventions, including oral medications, neurorehabilitation, and surgical procedures, have been used in the management of childhood spasticity. Dorsal rhizotomy, a neurosurgical intervention, plays a vital role in this context, selectively and functionally severing roots/rootlets to manage spasticity. Treatme

From August 1981 to May 1993 a total of 1263 percutaneous retrogasserian glycerol rhizotomies after Hakanson were performed. The intervention was performed with X-ray monitoring under local anaesthesia and rarely lasted longer than 20 min. It achieved good results in the treatment of idiopathic trigeminal neuralgia (TN) and symptomatic trigeminal neuralgia due to multiple sclerosis (TNMS). Some 97% of the TN patients were completely free of pain after the intervention. The recurrence rate within 5 years was 12.8%. Some 94.7% of the TNMS patients were immediately free of pain, but within 2-5 ye

The patterns of sensory loss in 29 cases of Juxta pontine trigeminal rhizotomy are reported. The section of 70% of the portio major gave a similar impairment of tactile, thermal and pain sensibility in the facial area corresponding to the portio of the nerve section in all the patients. These findings do not support Jannetta and Rand's statement that partial to complete section of the porto major at the pons with complete preservation of the intermediate fibre does not affect the perception of light touch. The reported data suggest: 1--intermediate fibres are not mode specific fibres for light

Selective dorsal rhizotomy represents one of the major neurosurgical tools in treating pediatric spasticity. Intraoperative rootlet selection is the key step of the procedure in order to achieve good outcomes. Intraoperative monitoring (IOM) and anatomical selection of the rootlets are the gold standard in the aforementioned selection leading the surgeon during the procedure. The tendency toward minimally invasive procedures further increases IOM importance, warranting the use of electrophysiology in choosing the more suitable rootlets for cutting. Discharge pattern and percentage of resection

Selective dorsal rhizotomy (SDR) has been practiced for many years; however, standardization of processes such as the surgical technique and rehabilitation protocols are only relatively recent phenomenon. Comparing data from various studies can be limited due to temporal changes in practice, different selection criteria for SDR, the heterogeneity of cerebral palsy and variable rehabilitation protocols. This chapter reviews the available evidence in relation to best practice for rehabilitation following SDR and explores factors affecting the short- and long-term outcome following SDR. This incl
