Umbilical cord myiasis in a preterm neonate: a rare sepsis aetiology uncovered by root cause analysis - case report.

Myiasis is the infestation of living human or animal tissue by fly larvae and is most common in wounds, body orifices or necrotic tissue in tropical and subtropical regions where hygiene is poor. Of the various forms of human myiasis, umbilical cord myiasis is exceedingly rare, with most reported cases occurring after the 5th day of life in infants cared for at home. A preterm neonate born at 30 weeks gestation developed umbilical cord myiasis with early-onset sepsis by 36 hours of life in a neonatal intensive care unit. This is one of the earliest and rarest presentations to be reported. Live

Investigating Serious Adverse Drug Reactions in Patients Receiving Erythropoiesis-Stimulating Agents: A Root Cause Analysis Using the "ANTICIPATE" Framework.

BACKGROUND: Unexpected serious adverse drug reactions (sADRs) affecting patients with chronic kidney disease (CKD) who received erythropoiesis-stimulating agents were identified by study co-authors. These included pure red cell aplasia (PRCA) after administration of the Eprex formulation of epoetin or the epoetin biosimilar HX575 and fatal anaphylaxis associated with peginesatide, an erythropoietin receptor agonist. We developed and applied a structured framework to describe these sADRs, including root cause analyses and eradication efforts. METHODS: A 10-step framework termed "ANTICIPATE," f

Surgical site infection related to use of elastomeric pumps in pectus excavatum repair. Lessons learned from root cause analysis.

BACKGROUND: Pectus excavatum repair (PEX) is among the most painful thoracic procedures performed. Continuous peripheral nerve blockade (CPNB) is known to be efficacious in optimizing pain control while limiting narcotic use in adult thoracic procedures. It was introduced in May 2015 as a bridge to oral pain control in children undergoing PEX. Consequently, the surgical site infection (SSI) rate increased from 2.7% to 27.7%. METHODS: SSI surveillance followed national guidelines. The abrupt increase prompted root cause analysis and cessation of CPNB use. A dynamic systems model of SSI in PEX

Trial and error - Outcome of breech presentation depending on birth mode and root cause analysis of severe adverse events.

OBJECTIVES: The debate about the safest birth mode for breech presentation at term remains unresolved. The comparison of a vaginal breech birth (VBB) with an elective caesarean section (CS) regarding fetal outcomes favors the CS. However, the question of whether attempting a VBB is associated with poorer fetal outcomes is examined in this study. Additionally, the study evaluates factors contributing to a successful VBB and illustrates possible errors in VBB management. STUDY DESIGN: We performed a retrospective analysis of term breech births over 15 years in a Perinatal Center Level I regardi

A root-cause analysis of maternal deaths in Botswana: towards developing a culture of patient safety and quality improvement.

BACKGROUND: In 2007, 95% of women in Botswana delivered in health facilities with 73% attending at least 4 antenatal care visits. HIV-prevalence in pregnant women was 28.7%. The maternal mortality ratio in 2010 was 163 deaths per 100,000 live births versus the government target of 130 for that year, indicating that the Millennium Development Goal 5 was unlikely to be met. A root-cause analysis was carried out with the aim of determining the underlying causes of maternal deaths reported in 2010, to categorise contributory factors and to prioritise appropriate interventions based on the identifi

Drill-related root injury caused by intraoperative intermaxillary fixation: an analysis of 1067 screw applications.

BACKGROUND/AIMS: Intermaxillary fixation is a standard procedure for the treatment of mandibular fractures or in orthognathic surgery. Predrilling for screws poses the risk of accidental tooth root injury, potentially leading to further pathological processes. Limited evidence about accidental tooth injury during intermaxillary fixation is available due to heterogenous study designs. The aim of this study was to evaluate the risk of root trauma using predrilled transgingival fixation screws and the clinical consequences for the affected teeth. MATERIALS AND METHODS: In this retrospective stud

Root cause analysis and solution to the degradation of wavelength division multiplexing (WDM) couplers in high power fiber amplifier system.

In the evaluation a fused biconical taper 1480/1580 nm WDM's ability to handle high power cascaded Raman laser throughput (>100 W) a significant degradation in performance was observed. A systematic root cause investigation was conducted and it is experimentally confirmed that the WDM degradation was caused by an interaction between the high power 1480 nm line, an out-of-band Stokes line, and the -OH content of the glass optical fiber. Slanted fiber Bragg grating (SFBG) was introduced to filter out the 1390 nm out-of-band Stokes line in an attempt to avoid this interaction. Ultimately a series

Delay in diagnosis of cancer as a patient safety issue - a root cause analysis based on a representative case report.

BACKGROUND: It is well known in the literature that imaging has almost no value for diagnosis of superficial bladder cancer. However, wide gap exists between knowledge on diagnosis of bladder cancer and actual clinical practice. CASE PRESENTATION: Delay in diagnosis of bladder cancer in a male person with tetraplegia occurred because of reliance on negative flexible cystoscopy and single biopsy, negative ultrasound examination of urinary bladder, and computerised tomography of pelvis. Difficulties in scheduling cystoscopy also contributed to a delay of nearly ten months between the onset of h

Augmenting patient safety surveillance in radiation oncology with large language model-based root cause analysis: A proof-of-concept study.

We investigated the potential utility of large language models (LLMs) in supporting patient safety efforts. Specifically, we evaluated the reasoning capabilities of LLMs in performing root cause analysis (RCA) of radiation oncology incidents using narrative reports from the Radiation Oncology Incident Learning System (RO-ILS). We prompted four state-of-the-art LLMs, Gemini 2.5 Pro, GPT-4o, o3, and Grok 3, with the "Background and Incident Overview" sections from 19 publicly available RO-ILS cases. Each model was instructed to perform RCA and generate root causes, lessons learned, and suggested
