Drug-induced hypersensitivity syndrome (DIHS): a reaction induced by a complex interplay among herpesviruses and antiviral and antidrug immune responses.

A relationship between viral infections and the simultaneous or subsequent development of allergic inflammation has often been observed in various clinical situations. Recent studies suggest an intimate relationship between reactivations of herpesviruses including human herpesvirus 6 (HHV-6) and the development of a severe systemic hypersensitivity reaction referred to as drug-induced hypersensitivity syndrome (DIHS). This syndrome has several important clinical features that cannot be solely explained by drug antigen-driven oligoclonal expansion of T cells: they include paradoxical worsening

Human herpes virus-like particles in pityriasis rosea lesions: an electron microscopy study.

BACKGROUND: In a previous study we detected virions with electron microscopy features of human herpes viruses in the supernatant of cocultured mononuclear cells from patients with acute pityriasis rosea. Because of their morphology and of polymerase chain reaction studies, we ascribed them to human herpes virus 7. OBJECTIVE: To find such virions in the lesional skin of pityriasis rosea patients. METHODS: Skin specimens from lesions of 21 patients with acute pityriasis rosea were examined by elecron microscopy. RESULTS: In 15 (71%) patients, human herpes virus particles in various stages of

Meta-analysis of sputum microbiome studies identifies airway disease-specific taxonomic and functional signatures.

Introduction. Studying taxonomic and functional signatures of respiratory microbiomes provide a better understanding of airway diseases.Gap Statement. Several human airway metagenomics studies have identified taxonomic and functional features restricted to a single disease condition and the findings are not comparable across airway diseases due to use of different samples, NGS platforms, and bioinformatics databases and tools.Aim. To study the microbial taxonomic and functional components of sputum microbiome across airway diseases and healthy smokers.Methodology. Here, 57 whole metagenome sho

Viral Encephalitis after Hematopoietic Cell Transplantation: A Systematic Review.

Viral encephalitis is a rare but serious complication after hematopoietic cell transplantation (HCT). The nonspecific early signs and symptoms and rapid progression can make it difficult to diagnose and treat in a timely fashion. To better inform clinical decision making in post-HCT viral encephalitis, a systematic review of prior studies of viral encephalitis was performed, with the goal of characterizing the frequency of various infectious etiologies and their clinical course, including treatments and outcomes. A systematic review of studies of viral encephalitis was performed. Studies were

Human Herpes Virus 7-related encephalopathy in children.

BACKGROUND: Primary HHV7 infection is almost ubiquitous, and it can present as exanthema subitem. Little is known on the clinical relevance of HHV7 neuroinvasion in immunocompetent children. METHODS: We describe 12 patients (median age 9.45 years, 50% males) with acute encephalopathy and active HHV7 infection. In all patients, HHV7-DNA was detected on cerebrospinal fluid (CSF) by RT-PCR. RESULTS: 7/12 patients had meningoencephalitis (two with ADEM and one with MOG antibody-associated CIS); 5/12 showed acute neuropsychiatric symptoms. EEG showed anomalies exclusively in patients with meningo

Human herpes virus-6 (HHV-6) pneumonitis and meningitis with viraemia in an immunocompetent adult patient.

Human herpes virus-6 (HHV-6) infection is a common infection in the paediatric population and is increasingly reported in immunosuppressed adult patients. It has been reported as the causative agent of disease in few case reports in immunocompetent adults. We report herein an unusual case of HHV-6-associated viraemia, pneumonitis and meningitis in a patient who presented with dyspnoea, hypoxia, dry cough and headache. She was treated for atypical pneumonia with no improvement. Meningitis was suspected as headache kept worsening. HHV-6B was detected by PCR in the cerebrospinal fluid, and subseq

Clinical and laboratory features of human herpesvirus 6 chromosomal integration.

Human herpesvirus 6 (HHV-6) comprises two separate viruses, HHV-6A and HHV-6B, although this distinction is not commonly made. HHV-6B is ubiquitous in the population with primary infection usually occurring in early childhood, and often resulting in febrile illness. HHV-6B is also recognized as a pathogen in the immunocompromised host, particularly in transplant recipients. HHV-6A is less well characterized and may have a more restricted prevalence. Both viruses are unique among the human herpesviruses in that the entire viral genome can be found integrated into the telomeric regions of host c

The long-term outcomes of patients transplanted due to acute liver failure with hepatic human herpesvirus-6 infection.

Human herpesvirus (HHV)-6, comprised of HHV-6A and HHV-6B, belongs to the betaherpesviruses that infect 95%-100% of humans. Primary infection, known as exanthema subitum, occurs in early childhood. Reactivations of latent HHV-6, mostly HHV-6B, are common after liver transplantation. The vast majority of them are asymptomatic; in a minority of cases, the virus may infect the liver transplant, causing graft dysfunction or hepatitis. An association between hepatic HHV-6 infection and indeterminate acute liver failure (ALF) has been shown, but the causality is not clear because of the ubiquitous n

Human herpesvirus 6 (HHV-6) infection and risk of Alzheimer's disease: a systematic review and meta-analysis.

BACKGROUND: Alzheimer's disease (AD) is the most common form of dementia worldwide. Human Herpesvirus 6 (HHV-6), a neurotropic virus capable of establishing lifelong latency in the CNS, has been suggested as a potential risk factor in the pathogenesis of AD. However, studies investigating this association have reported inconsistent findings. This systematic review and meta-analysis aimed to explore this association further and shed light on the matter. OBJECTIVE: This study aimed to systematically review and meta-analyze existing literature to assess the association between HHV-6 infection an

Choreoathetosis in the Setting of Human Herpesvirus-6 Infection in a Transplant Recipient.

BACKGROUND: Human herpesvirus-6 (HHV-6) has been associated with various neurologic disorders, but movement disorders are rare. This case describes a patient who developed a choreoathetotic movement disorder in the setting of HHV-6 infection. CASE REPORT: A 72-year-old woman with AML and recent HHV-6 encephalitis following stem cell transplant presented with involuntary movements. Neurologic examination demonstrated motor impersistence and irregular non-stereotyped writhing movements consistent with a choreoathetotic movement disorder secondary to HHV-6 infection. DISCUSSION: This is the fir

[Human herpesvirus 7].

Human herpesvirus 7 known since 1990 is closely related to herpesvirus 6B. It replicates in human cells only after binding CD4 receptor. It establishes lifelong latency in infected cells, and its frequent reactivations result in asymptomatic virus shedding through saliva. Most children acquire infection by age 3 and 4, but in any later age group seronegative individuals are at risk of infection. Rarely, exanthema subitum or convulsions with fever in children, pityriasis rosea in young adults, lethal complications in immunocompromised persons with concomitant herpesvirus 6B and cytomegalovirus

Different expression of human herpesvirus-6 (HHV-6) load in whole blood may have a significant impact on the diagnosis of active infection.

BACKGROUND: Viral load in whole blood is the main virological marker for assessing HHV-6 infection and is used as an indication to begin antiviral therapy. Results are usually expressed as the number of genomic equivalent copies (gec) per mL of blood, although HHV-6 DNA in blood is mainly localized in lymphocytes and polymorphonuclear leukocytes. OBJECTIVES: Since leukocyte counts vary in immunocompromised patients, especially in stem cell transplant recipients, the aim of this study was to compare HHV-6 load expressed as gec per mL with load expressed as gec per million cells (mc), using qua

[Is antiviral therapy in gastroduodenal pathology necessary?].

Mucous membrane of the gastroduodenal region of 104 patients with different pathologies (gastritis and erosive ulcerative alterations) was investigated by polymerase chain reaction (PCR). Examination of the biopsy material revealed frequent occurrence of pathogenic strains of Helicobacter pylori (up to 80%) and Herpesviridae (EBV, HHV 6, HHV 7, HHV 8) in various combinations. In such cases it is necessary to use adequate antibacterial and antiviral therapy with individual choice of immunomodulators.

A 1-year-old boy presenting with roseola accompanied by bilateral eyelid oedema and acute infectious mononucleosis syndrome caused by human herpesuvirus-6B: A case report.

Bilateral eyelid oedema, also known as Hoagland sign, is a specific physical sign of infectious mononucleosis (IM) syndrome caused by Epstein-Barr virus (EBV) infection in older children and adolescents. IM syndrome, which is mainly caused by EBV or cytomegalovirus infection, has milder symptoms in young children than in older children and adolescents. The human herpesvirus 6B (HHV-6B) causes roseola infantum (sixth disease) and various complications in children younger than 2 years; however, bilateral eyelid oedema and IM syndrome are rarely reported. We report the case of a 1-year-old boy wi

[Three-day fever-associated liver failure and encephalitis in an infant].

Human herpesvirus 6 (HHV-6) infection typically presents as a benign and self-limiting viral illness. However, in rare cases, it can lead to life-threatening complications. In this case a previously healthy four-month-old infant was admitted in circulatory collapse, experiencing seizures and rapid cerebral deterioration. The cause was identified as HHV-6-induced acute necrotising encephalopathy (ANE) and acute liver failure. Despite intensive medical efforts, the condition was unresponsive to treatment, and the infant was transitioned to palliative care.
