Real-world Clinical and Diagnostic Features of Patients With Isolated Anti-SSB Antibodies Compared With Those With Combined Anti-SSA and Anti-SSB Antibodies.

BACKGROUND/OBJECTIVE: Anti-SSA and/or anti-SSB antibodies are commonly seen in Sjögren disease but lack disease specificity. Isolated anti-SSB may have a limited association with Sjögren, while dual anti-SSA/SSB positivity may indicate more severe disease. This study compared clinical and serologic features of patients with isolated anti-SSB versus dual anti-SSA/SSB positivity in a real-world setting. METHODS: A retrospective medical records review study from January 1, 2018, to December 31, 2022, was conducted. Adults with positive serologic testing for anti-SSA and anti-SSB antibodies were

Epstein-Barr virus in the sublabial salivary gland in Sjögren's syndrome.

The proposed role of Epstein-Barr virus (EBV) in salivary gland destruction in Sjögren's syndrome (SS) prompted the authors to study the presence of EBV-DNA (hybridohistochemistry) and EBV-encoded proteins (immunohistochemistry) in sublabial salivary glands taken from eight patients with primary and five with secondary SS and from 16 controls. DNA probes and anti-EBV antibodies were controlled for activity by assessment of human blood B-lymphocytes after in vitro infection with EBV. None of the tissues investigated manifested the presence of EBV proteins (nuclear antigen, early antigen R, memb

A neonate with the rupture of mitral chordae tendinae associated with maternal-derived anti-SSA antibody.

Acute mitral insufficiency due to the rupture of chordae tendinae from papillary muscles is a rare but sometimes fatal condition in neonates and infants. Here, we report a 21-day-old neonate with sudden onset of mitral insufficiency due to rupture of the chordae tendinae. The baby was successfully rescued by appropriate diagnosis and emergency surgical operation with reconstruction of the ruptured chordae using autologous pericardium-reinforced mattress suture. Intraoperative findings revealed fibrous scar formation at the distal end of the posterior papillary muscle, indicating a remnant of a

Focal seizures as an unusual presentation of neonatal lupus erythematosus.

Neonatal lupus erythematosus is an uncommon passive autoimmune disease in which there is a transplacental passage of anti-Ro/SSA and/or anti-La/SSB maternal autoantibodies. Common clinical manifestations include cardiac disease, notably congenital heart block, cutaneous lupus lesions, hematologic disorders, and hepatobiliary disease. During the past decade, however, it has become clear that central nervous disease may also be a manifestation of neonatal lupus. We report a male neonate with the disease who had focal seizures in addition to cutaneous lupus, anemia, and thrombocytopenia. Brain ul

UVB light and 17-beta-estradiol have different effects on the mRNA expression of Ro/SSA and La/SSB autoantigens in HaCaT cells.

Antibodies produced against the Ro/SSA and La/SSB autoantigens are not only of diagnostic value but they may even play a role in the pathogenesis of several autoimmune diseases (Sjögren's syndrome, subacute cutaneous lupus erythematosus, neonatal lupus erythematosus and systemic lupus erythematosus). Among other factors, ultraviolet (UV) radiation and also the hormonal milieu are well-known cofactors in the pathogenesis of these autoimmune diseases. The goal of our research was to study the possible alterations in mRNA levels of three different Ro antigens and that of two La species produced b

Antinuclear antibodies in routine analysis: the relevance of putative clinical associations.

Defined antinuclear antibodies (ANA), such as antibodies to Ro/SS-A, La/SS-B, Sm, and nRNP, are often present in serum samples from patients with systemic lupus erythematosus (SLE) or other connective tissue diseases (CTD). Most data on associations between the presence of these antibodies and defined disease features have been obtained with the use of predefined groups of patients. In this work the issue of disease associations was approached from a different angle: patients suspected of having CTD were selected on the presence of these ANA in their serum samples and clinical data were subseq

A 52-kD protein is a novel component of the SS-A/Ro antigenic particle.

Anti-SS-A/Ro autoantibodies are found in the sera of patients with Sjogren's syndrome (SS) and SLE. In the course of analyzing 61 SS patients for their autoantibody profiles, we found that 42 were positive for anti-SS-A by double diffusion in agarose and demonstrated precipitin lines identical to that produced by a prototype anti-SS-A serum. Further analysis of these SS-A antibody-positive sera by Western blotting of cell extracts revealed that 21 sera reacted with two proteins of 60 and 52 kD, 13 sera reacted with 52-kD protein, two detected only 60 kD, while six were nonreactive. [Antinuclear antibodies in Sjögren's disease and syndrome].

A study was made of the presence of antinuclear antibodies to Ro, La, nDNA, RNP and Sm in 18 patients with Sjögren's disease (SD) and in 13 patients with Sjögren's syndrome (SS). Anti-Ro and/or anti-La were revealed in 12 SD patients (67%) and in 3 SS patients (23%, P less than 0.05) only. Anti-nDNA and anti-RNP were detected in 39 and 15% of the SS patients respectively and were undetectable in the SD patients. Anti-Ro and/or anti-La were detected in all SD patients, III degree activity, and associated with the presence of Raynaud's syndrome, recurring nonerosive arthritis and the absence of

Differential recognition of the 52-kd Ro(SS-A) antigen by sera from patients with primary biliary cirrhosis and primary Sjögren's syndrome.

Antibodies against the 52-kd Ro(SS-A) protein are significantly associated with the primary Sjogren's syndrome (pSS). A small proportion of patients suffering from primary biliary cirrhosis (PBC) with secondary Sjogren's syndrome (PBC/SS) who are serologically characterized by antimitochondrial type 2 antibodies also express anti-52-kd Ro(SS-A) antibodies. The primary B-cell-derived antigenic responses by autoimmune sera were analyzed in both entities using truncated recombinant proteins to examine whether different epitopes are associated with these diseases. Sera were collected from 25 patie

Outline of Salivary Gland Pathogenesis of Sjögren's Syndrome and Current Therapeutic Approaches.

Sjögren's syndrome (SS) is an autoimmune disease characterized by the involvement of exocrine glands such as the salivary and lacrimal glands. The minor salivary glands, from which tissue samples may be obtained, are important for the diagnosis, evaluation of therapeutic efficacy, and genetic analyses of SS. In the onset of SS, autoantigens derived from the salivary glands are recognized by antigen-presenting dendritic cells, leading to the activation of T and B cells, cytokine production, autoantibody production by plasma cells, the formation of ectopic germinal centers, and the destruction o

Definition of a discontinuous immunodominant epitope at the NH2 terminus of the La/SS-B ribonucleoprotein autoantigen.

High-titer IgG autoantibodies to the La/SS-B ribonucleoprotein (RNP) are a hallmark of patients with primary Sjogren's syndrome. Anti-La/SS-B-positive human sera bind to multiple epitopes on recombinant La/SS-B, although the initial response is against an immunodominant epitope within the first 107 NH2-terminal amino acids (aa). Sequence analysis has identified a striking homology between aa 88-101 in this NH2-terminal region of La/SS-B and a feline retroviral gag polypeptide suggesting the anti-La/SS-B response may be initiated by cross-reactivity with an exogenous agent. In the present study

Primary Sjögren's syndrome is characterized by distinct phenotypic and transcriptional profiles of IgD+ unswitched memory B cells.

OBJECTIVE: The significance of distinct B cell abnormalities in primary Sjögren's syndrome (SS) remains to be established. We undertook this study to analyze the phenotype and messenger RNA (mRNA) transcript profiles of B cell subsets in patients with primary SS and to compare them with those in sicca syndrome patients and healthy controls. METHODS: CD19+ B cells from 26 patients with primary SS, 27 sicca syndrome patients, and 22 healthy controls were analyzed by flow cytometry. Gene expression profiles of purified B cell subsets (from 3-5 subjects per group per test) were analyzed using Aff

Circulating auto-antibodies against nuclear and non-nuclear antigens in primary Sjögren's syndrome: prevalence and clinical significance in 335 patients.

The aim of this study was to analyze the prevalence and clinical significance of circulating auto-antibodies against nuclear and non-nuclear antigens in a large cohort of Spanish patients with primary Sjögren's syndrome (SS). We studied 335 patients diagnosed with primary SS seen consecutively in our department since 1994 and tested for anti-nuclear antibodies (ANA), anti-Ro/SS-A, anti-La/SS-B, anti-Sm, anti-ribonucleoprotein (anti-RNP), anti-smooth muscle antibodies (anti-SMA), anti-parietal cell antibodies (anti-PCA), anti-liver-kidney microsome type-1 (anti-LKM-1) antibodies and anti-mitoch

The significance of CD5+ B cells in rheumatic diseases.

The presence of autoantibodies in rheumatoid arthritis (RA) and primary Sjögren' syndrome (SS) is a feature of these diseases. Because of the probable role of autoantibodies in the pathogenesis of RA and primary SS, the study of the origin and regulation of B lymphocytes producing autoantibodies is of special importance. In mice, B lymphocytes can be divided into two populations of distinct lineages, the conventional bone marrow-derived B subset and the self-renewing peripheral subset. B lymphocytes of the latter express an antigen initially described as T cell specific, the Ly-1 or CD5 antige

Can B cell epitopes of 60 kDa Ro distinguish systemic lupus erythematosus from Sjögren's syndrome?

Antibodies binding components of the Ro/La (or SSA/SSB) ribonucleoprotein particle are found in the sera of patients with systemic lupus erythematosus (SLE) and Sjögren's syndrome (SS) as well as mothers who give birth to babies with neonatal lupus. Anti-La occurs in a subset of sera that contain anti-Ro, and anti-La is found more commonly in sera of patients with SS than in sera from SLE patients. The fine specificity of autoantibodies binding 60 kDa has been studied extensively. Recent data have suggested that there are disease-specific epitopes which identify patients with either SLE or SS.

Nonbullous neutrophilic dermatosis: Sweet's syndrome, neonatal lupus erythematosus, or both?

We describe a 5-day-old infant who fulfilled the diagnostic criteria for Sweet's syndrome, and the concurrent histologic and autoantibody features supporting the diagnosis of neonatal lupus. To our knowledge, this is the youngest case of Sweet's syndrome reported in the literature. Importantly, our findings further support the hypothesis that lupus erythematosus should be considered in the differential diagnosis of a nonbullous neutrophilic dermatosis, as it may represent the initial manifestation of the disease.

[Single-cell RNA sequencing of B cells reveals molecular typing in Sjögren syndrome].

OBJECTIVE: To establish a molecular classification framework for Sjögren syndrome (SS) by stratifying patients into distinct subtypes through unsupervised clustering of B cell single-cell RNA sequencing (scRNA-seq). This study characterizes subtype-specific gene signatures to construct protein-protein interaction (PPI) networks, thereby elucidating core regulatory mechanisms and potential therapeutic targets. Concurrently, it defines the clinical heterogeneity of SS by profiling autoantibodies and B-cell subset distributions across subtypes. METHODS: The scRNA-seq data from 24 SS patients and

Anti-centromere antibodies target centromere-kinetochore macrocomplex: a comprehensive autoantigen profiling.

OBJECTIVES: Anti-centromere antibodies (ACAs) are detected in patients with various autoimmune diseases such as Sjögren's syndrome (SS), systemic sclerosis (SSc) and primary biliary cholangitis (PBC). However, the targeted antigens of ACAs are not fully elucidated despite the accumulating understanding of the molecular structure of the centromere. The aim of this study was to comprehensively reveal the autoantigenicity of centromere proteins. METHODS: A centromere antigen library including 16 principal subcomplexes composed of 41 centromere proteins was constructed. Centromere protein/complex

Interferon-α induction and detection of anti-ro, anti-la, anti-sm, and anti-rnp autoantibodies by autoantigen microarray analysis in juvenile dermatomyositis.

OBJECTIVE: To evaluate serum interferon-α (IFNα) activity in the context of autoantibody profiles in patients with juvenile dermatomyositis (JDM). METHODS: Sera from 36 patients with JDM were analyzed. Autoantibody profiles were determined by probing microarrays, which were fabricated with ∼80 distinct autoantigens, with serum and a Cy3-conjugated secondary antibody. Arrays were scanned and analyzed to determine antigen reactivity. Serum IFNα activity was measured using a functional reporter cell assay. Sera were assayed alone or in combination with cellular material released from necrotic U9
