Hospital Addiction Medicine Consultation Service Orders and Outcomes by Patient Race and Ethnicity in an Urban, Safety-Net Hospital.

BACKGROUND: Hospital admissions involving substance use disorders are increasing and represent an opportunity to engage patients in substance use treatment. Addiction medicine consultation services improve access to medications for opioid use disorder (MOUD) and patient outcomes. However, as hospitals continue to adopt addiction medicine consultation services it is important to identify where disparities may emerge in the process of care. OBJECTIVE: To describe addiction medicine consultation service use by race and ethnicity as well as substance to identify opportunities to reduce substance

Brief Report: Financial Burden of Toxoplasmosis Encephalitis Treatment at a Safety Net Hospital.

BACKGROUND: Although the price increase of pyrimethamine in 2015 received heavy media coverage, there are little data regarding specific implications to hospitals and the total costs of treating inpatients with toxoplasmosis encephalitis (TE). METHODS: Using average drug wholesale costs, we estimated the inpatient drug costs of TE drugs 3 years prepyrimethamine and postpyrimethamine price increase in August 2015. The drug regimens and total doses were determined through retrospective chart review of patients living with HIV who received treatment for TE while inpatient during this period. RE

Influence of Social Determinants of Health on Treatment Completion for Pediatric Mild Traumatic Brain Injury in a Safety Net Hospital.

The negative impact of the Social Determinants of Health (SDOH) on health care outcomes in vulnerable populations, particularly pediatric patients, is a well-established phenomenon. The treatment of traumatic brain injury is not an exception. It is not clear, however, which specific social determinants of health factor is more influential on the current management of traumatic brain injury in these populations. This study addresses this gap by exploring how these factors influence the treatment of traumatic brain injuries in pediatric patients at Boston Medical Center, a safety net hospital in

Mapping 340B Funds Flow Through Contract Pharmacies.

The 340B Drug Pricing Program, established in 1992, was designed to help safety-net healthcare providers stretch scarce resources by allowing them to purchase outpatient medications at significant discounts. However, the program's dramatic expansion, particularly following the Affordable Care Act, and the rise of contract pharmacy arrangements have led to increasing scrutiny regarding its current function and impact. This study examines the flow of funds within the modern 340B ecosystem, with a particular focus on contract pharmacies, pharmacy benefit managers (PBMs), third-party administrator

Timing of initiation of therapeutic hypothermia in neonates with hypoxic ischemic encephalopathy born at a high-volume urban safety net hospital.

BACKGROUND: Ethnic and racial disparities in neonatal outcomes have been well-documented, including higher risk of hypoxic-ischemic encephalopathy (HIE). Therapeutic hypothermia (TH) is the only approved treatment for infants with moderate to severe HIE and previous studies have shown mixed results regarding the impact of timing of initiation of TH on outcomes. These studies often include both inborn and outborn neonates and few minority patients. STUDY DESIGN: This retrospective cohort study of exclusively inborn neonates from a high-volume, urban, safety net hospital (SNH) serving primarily

Disparities in presentation and management of chronic hepatitis B among Hispanics in a diverse safety net system.

BACKGROUND: Chronic hepatitis B (HBV) prevalence is highest in foreign-born Asian and African individuals in the US, though Hispanics make up the largest proportion of the immigrant population. Differences in the diagnosis and management of chronic HBV in Hispanics might exist due to the lower awareness of risk. We aim to examine racial/ethnic disparities in the diagnosis, presentation, and immediate management of chronic HBV in a diverse safety net system enriched for Hispanics. METHODS: In a large urban safety-net hospital system, we retrospectively identified patients with chronic HBV by s

Contributors to disparities in postpartum readmission rates between safety-net and non-safety-net hospitals: A decomposition analysis.

BACKGROUND: Safety-net hospitals (SNHs) treat more maternal patients with risk factors for postpartum readmission. OBJECTIVE: To assess how patient, hospital, and community characteristics explain the SNH/non-SNH disparity in postpartum readmission rates. DESIGN: A linear probability model assessed covariates associated with postpartum readmissions. Oaxaca-Blinder decomposition estimates quantified the contribution of covariates to the SNH/non-SNH disparity in postpartum readmission rates. SETTING: Healthcare Cost and Utilization Project 2016-2018 State Inpatient Databases from 25 states.

Feasibility of EHR-based prescreening for an adjuvant breast cancer trial in a safety-net health system.

INTRODUCTION: Racial and ethnic inequities persist in oncology clinical trial participation, while safety-net health systems may face resource and data limitations that complicate candidate identification. We evaluated the feasibility of an electronic health record (EHR)-assisted prescreening workflow for EMBER-4, a phase 3 adjuvant breast cancer trial, at a safety-net health system. METHODS: Structured Query Language (SQL) queries applied to Epic Clarity identified potential candidates using diagnosis codes, receptor-status codes, endocrine therapy medication records, recent oncology follow-

Implementation and evaluation of the 3 Wishes Project in safety-net hospitals: Protocol for a hybrid effectiveness-implementation study.

BACKGROUND: Palliative care interventions in the intensive care unit have been shown to improve communication and the quality of death and dying. However, few interventions have been implemented in safety-net hospitals (SNHs), which provide healthcare to low income and uninsured patients. The 3 Wishes Project (3WP) is a low-cost intervention that aims to enhance compassionate end-of-life (EOL) experiences by empowering the clinical team to elicit and fulfill small wishes for patients who are dying in the ICU. OBJECTIVE: To implement and evaluate the 3WP in three SNHs in Los Angeles. METHODS:
