The authors administered semi-structured interviews to 187 psychiatric inpatients to determine the role of anhedonia in schizophrenia. The interviews were taperecorded and then given blind ratings for anhedonia on a 7-point scale. Schizophrenic patients had significantly more anhedonia than nonschizophrenics, although many of the latter group had anhedonic tendencies. Most of the difference between the groups resulted from high anhedonia scores for chronic schizophrenics; less anhedonia was found in the acute schizophrenic patients. The data indicate that anhedonia is not necessary or unique t

One of the difficulties in defining schizophrenia is the possibility of its heterogeneity. Schizophrenia may be divided into organic and idiopathic on the basis of precipitating factors; the difference between the two is supported by evidence from family studies and differences in symptomatology. A further division of idiopathic schizophrenia may be made into typical and atypical schizophrenia based on differences in clinical features, family history, mode of inheritance, and prognosis. There is suggestive evidence that within typical schizophrenia, paranoid and nonparanoid subtypes may be ids

Anosognosia, defined as a lack of knowledge of the disease, was originally identified in neurological disorders and is common in schizophrenia. These deficits are commonly referred to as "lack of insight" or "unawareness of illness." They include challenges in accurate judgments of the reality of experience, as well as global and specific personal abilities. Related to inaccuracies in self-assessment are response biases when an incorrect self-assessment is made. We adopted a perspective focused on Introspective Accuracy (IA) and Introspective Bias (IB). IA is the ability to accurately judge se

Schizophrenia is a psychiatric disorder characterized by distortions of thinking and perception, with no strictly pathognomonic symptoms that can be divided into positive, negative, and cognitive symptom domains. People with schizophrenia have, between others, a reduced life expectancy and cardiorespiratory and muscular fitness and increased risk of cardiovascular disease, metabolic syndrome, obesity, hypertension, and hyperlipidemia compared to the general population. Furthermore, the economic burden of mental disorders including schizophrenia is evident and it is expected to increase to more

Schizophrenia is a severe mental disorder characterized by a heterogeneous symptom profile which comprises a clinical platform for widespread use of polypharmacy even though antipsychotic monotherapy is the recommended treatment regimen. This narrative review provides a summary of the current gap between evidence and practice for use of antipsychotic combination therapy in patients with schizophrenia. Antipsychotic polypharmacy is frequently prescribed instead of following international consensus of clozapine monotherapy in treatment-resistant patients. Antipsychotic-benzodiazepine combination

Although visual processing impairments are common in schizophrenia, it is not clear to what extent these originate in the eye vs. the brain. This review highlights potential contributions, from the retina and other structures of the eye, tovisual processing impairments in schizophrenia and high-risk states. A second goal is to evaluate the status of retinal abnormalities as biomarkers for schizophrenia. The review was motivated by known retinal changes in other disorders (e.g., Parkinson's disease, multiple sclerosis), and their relationships to perceptual and cognitive impairments, and diseas

BACKGROUND: Perazine is an old phenothiazine derivative used for the treatment of people with schizophrenia and is reputed to have a low level of extrapyramidal adverse effects. As far as we are aware, its use is limited to Germany, Poland, the former Yugoslavia and the Netherlands.
OBJECTIVES: To examine the effects of perazine for those with schizophrenia or related psychoses in comparison with placebo, no treatment or other antipsychotic medications.

PURPOSE OF REVIEW: It is a well established fact that many serious mental illnesses, in particular psychoses such as schizophrenia, may have a significant hormonal aetiological component. This study aims to discuss the oestrogen protection hypothesis of schizophrenia in particular, with an emphasis on findings from the recent literature in support of this theory.
RECENT FINDINGS: Epidemiological and life-cycle data point to significant differences in the incidence and course of schizophrenia between men and women, suggesting a protective role of oestrogen. In-vitro and in-vivo preclinical res

Smoking is associated with the induction of the CYP 1A2 enzyme, and hence with diminished levels of drugs metabolized by this enzyme. This article presents a hypothetical patient, diagnosed with schizophrenia, who smokes about 10 cigarettes a day. The progress of the patient is examined across several follow up meetings, each of which presents a fresh clinical problem related to smoking and its interaction with the medication. Clinical guidance for each problem is presented along with a brief discussion on the subject. The article concludes with a discussion of other pharmacokinetic considerat

The impact of mental illness, comorbid substance abuse, and medication nonadherence, coupled with disjointed psychiatric and social services, conspires to a disproportionately high rate of psychiatric disorders among people who are homeless in the United States. This article reviews the prevalence of homeless among the mentally ill as well as the prevalence of mental illness among the homeless and details barriers in access to care and the solutions that have been attempted. The need and solutions to introduce a new generation of physicians and allied health care workers to the unique health c

Schizophrenia is a disorder of the brain with multicausality, chronicity, and various symptom manifestations within its course. The therapeutic interventions are determined by the actual psychiatric condition characterized by the three main syndromes (positive, negative and cognitive symptoms). Antipsychotics are essential drugs in the therapy of schizophrenia and anxiolytics, antidepressants, antiepileptics are only adjuvants with temporary use. The previous studies show no results in the therapy of the cognitive symptoms, and the new antipsychotics improve the negative symptoms moderately. T

BACKGROUND: Levomepromazine is an 'older' typical antipsychotic medication licensed for use in schizophrenia but sparingly prescribed in the United Kingdom.
OBJECTIVES: To determine the clinical effects and safety of levomepromazine compared with placebo or antipsychotic medications for schizophrenia and schizophreniform psychoses.
SEARCH STRATEGY: We searched the Cochrane Schizophrenia Group's Register (December 2008) which is based on regular searches of, amongst others, BIOSIS, CENTRAL CINAHL, EMBASE, MEDLINE and PsycINFO. References of all identified studies were inspected for further tr
