Secondary prevention and estimation of fracture risk.

The key questions addressed in this chapter are: • How can individual risk of fracture be best estimated? • What is the best system to prevent a further fracture? • How to implement systems for preventing further fractures? Absolute fracture risk calculators (FRCs) provide a means to estimate an individual's future fracture risk. FRCs are widely available and provide clinicians and patients a platform to discuss the need for intervention to prevent fragility fractures. Despite availability of effective osteoporosis medicines for almost two decades, most patients presenting with new fragility f

Rheumatic fever - identification, management and secondary prevention.

BACKGROUND: Acute rheumatic fever is a rare multisystem disease caused by an immunological response to Group A streptococcus infection. Acute rheumatic fever usually has onset in childhood and is most prevalent in Aboriginal and Maori populations and other disadvantaged groups. OBJECTIVE: In this article we outline the clinical features of acute rheumatic fever and describe the important role of primary healthcare providers in its identification, management and secondary prevention. DISCUSSION: Recurrent episodes of acute rheumatic fever may lead to rheumatic heart disease. Early detection o

Treatment and prevention of secondary CNS lymphoma.

Central nervous system (CNS) involvement in non-Hodgkin lymphoma is a serious, potentially preventable complication that can occur in 5 to 10% of patients. Its occurrence is directly correlated with pathologic aggressiveness and ranges from less than 3% in the indolent, less-aggressive histologies to as high as 50% in the very aggressive ones such as Burkitt lymphoma. Aggressive treatment once detected can improve neurologic outcome, but because it is often associated with contemporaneous systemic relapse, is rarely associated with long-term survival. Preventing its occurrence, therefore, rema

Acute coronary syndromes: management and secondary prevention.

Acute coronary syndromes represent a broad spectrum of ischemic myocardial events including unstable angina, non-ST elevation myocardial infarction and acute ST elevation myocardial infarction, which are associated with high morbidity and mortality. They constitute the most frequent cause of hospital admission related to cardiac disease. Early diagnosis and risk stratification are essential for initiation of optimal medical and invasive management. Therapeutic measures comprise aggressive antiplatelet, antithrombotic, and anti-ischemic agents. In addition, patients with high-risk features, not

Antiplatelet agents for secondary prevention of stroke.

Patients with recent ischemic stroke or transient ischemic attack (TIA) face a high risk of recurrent stroke as well as an increased risk of myocardial infarction and sudden cardiac death. In the absence of a clearly established indication for long-term anticoagulation, such as atrial fibrillation, antiplatelet agents are the antithrombotic drugs of choice for preventing recurrent vascular events. For many years, aspirin (ASA) has been the first-line therapy for patients at high risk of vascular ischemic events. Two large clinical trials have established the superiority of the combination of A

[Primary, secondary and tertiary prevention of osteoporosis].

Preventive procedures of a disease can be classified into three levels. Primary prevention protects healthy people against the disease, secondary prevention screens people at high risk for the disease or with the disease of an early stage and treats them, and tertiary prevention rehabilitates patients with the disease through medical treatment. Many preventive measures for osteoporosis have been shown to have strong or fair evidence for their validity in each of the three levels. It is necessary to provide people with different risks of osteoporosis valid preventive measures which correspond t

[Secondary prevention of cytomegalovirus retinitis with ganciclovir].

The prophylaxis of cytomegalovirus retinitis reactivation is effective in reducing the risk of blindness and in prolonging the remission interval and time of survival if given daily throughout life. In this study, a newly developed therapeutic regimen with 3 infusions a week was compared to the conventional maintenance therapy of 5 infusions a week using the same total weekly dose. For this purpose, ten patients were given 10 mg ganciclovir/kg 3 times a week (group A), and 18 received 6 mg ganciclovir/kg once daily for 5 days a week (group B). Only patients with newly diagnosed retinitis were

Prevention of extravasation injuries secondary to doxorubicin.

Intravenously administered drugs with potentially devastating consequences should be given only by personnel highly knowledgeable regarding the side effects and skilled in intravenous cannulation. A strict protocol should be followed. The earliest signs heralding extravasation should be recognized and infusion discontinued immediately. If extravasation occurs, prompt surgical consultation is necessary. Injection into the volar wrist, dorsum of the hand, and antecubital fossa should always be avoided. Polyethylene catheters are preferable to butterfly needles for administering chemotherapeutic

Secondary prevention of cellulitis: A systematic review.

ObjectivesCellulitis is a common and often recurrent bacterial skin infection. Antibiotics are the first-line treatment for cellulitis and antibiotic prophylaxis is a known preventive measure for frequent recurrence. Evidence for other interventions targeting known risk factors (secondary prevention) remains unclear. This review evaluates the evidence for managing risk factors to prevent recurrent limb cellulitis.Study designA systematic review of studies on cellulitis risk factor management, excluding antibiotic prophylaxis, was conducted. Five databases were searched in February 2023 and Jun

Cardiac rehabilitation: the gateway for secondary prevention.

Cardiac rehabilitation (CR) is a multidisciplinary supervised programme which typically consists of tailored exercise and education on lifestyle management and risk factor modification in cardiac patients. Participation in CR reduces morbidity and mortality, while improving quality of life following major cardiovascular events. Despite the benefits of CR, it is underutilised, generally in the 20%-30% range for eligible patients. Participation and adherence rates are particularly suboptimal in vulnerable populations, such as those of lower socioeconomic status and women. Interventions such as a

Blood pressure management for secondary stroke prevention.

Hypertension is the most important vascular risk factor for stroke; therefore, optimal blood pressure (BP) management is essential for the prevention of recurrent stroke; lowering BP was shown to reduce the risk of recurrent stroke by 25-30%. A recent meta-analysis showed that intensive BP lowering to levels <130/80 mmHg significantly reduced the risk of recurrent stroke compared to standard management with BP levels <140/90 mmHg. The benefit of intensive BP management is evident with regard to a reduced risk of intracranial hemorrhage. Therefore, clinical practice guidelines have established

Primary and secondary prevention of breast cancer.

INTRODUCTION: Breast cancer is the most common cancer among women and is the second cancer frequently occurring worldwide of newly-diagnosed cancers. There is much evidence showing the influence of life style and environmental factors on the development of mammary gland cancer (high-fat diet, alcohol consumption, lack of physical exercise), the elimination of which (primary prevention) may contribute to a decrease in morbidity and mortality. Secondary prevention, comprising diagnostic tests (e.g. mammography, ultrasonography, magnetic resonance imaging, breast self-examination, as well as mode

Canakinumab for secondary prevention of atherosclerotic disease.

The widespread prevalence of cardiovascular disease (CVD) and its impact on morbidity and mortality requires effective secondary prevention measures. For years, inflammation has been advocated as a key mediator of atherosclerosis and its associated complications. Drugs for secondary prevention of CVD events include interventions aimed at risk factors control and antithrombotic management, but there is no drug currently recommended that specifically targets inflammation. Recently, the inflammatory hypothesis of atherosclerosis has been confirmed by a randomized clinical trial of canakinumab, a

Secondary Stroke Prevention in Nonvalvular Atrial Fibrillation.

Approximately 750,000 US adults per year experience a stroke. On average, the annual risk for future ischemic stroke (secondary stroke) after an initial ischemic stroke or transient ischemic attack is approximately 3% to 4%. Cardioembolic strokes account for 20% to 25% of all strokes, with nonvalvular atrial fibrillation (NVAF) considered one of the main sources of embolism; this explains up to half of all cardioembolic strokes. We present the risk factors for stroke in NVAF, risk stratification, a diagnosis of NVAF, and treatment and prevention of stroke in NVAF. We reviewed the literature by

[FLS - three letters alter secondary fracture prevention].

The increasing importance of preventive measures in the field of orthopedics and trauma surgery becomes apparent because of the demographic changes and the high risk for secondary fractures following osteoporotic fractures. Within the fracture treatment chain, orthopedics and trauma surgery are in the "pole position" to initiate these measures in geriatric patients. In the past orthopedists and trauma surgeons have constantly accused of neglecting secondary fracture prevention in fragility fracture patients. There are several reasons that speak in favor of us undertaking a role in secondary fr
