Case report: Serotoninergic and cholinergic syndromes induced by self-medication.

Self-medication is a part of the self-care practices carried out by the elderly in their environment. The aim of this case report is to show how the self-medication of fluoxetine and dimenhydrinate in an older adult can induce serotoninergic and cholinergic syndromes, showing symptoms such as nausea, tachycardia, tremor, loss of appetite, memory loss, decreased vision, falls, and increased urination. An older adult who has been diagnosed with arterial hypertension, dyslipidemia, diabetes mellitus, and a recent diagnosis of essential thrombosis is the subject of this case report. After the anal

Ruminant self-medication against gastrointestinal nematodes: evidence, mechanism, and origins.

Gastrointestinal helminths challenge ruminants in ways that reduce their fitness. In turn, ruminants have evolved physiological and behavioral adaptations that counteract this challenge. Ruminants display anorexia and avoidance behaviors, which tend to reduce the incidence of parasitism. In addition, ruminants appear to learn to self-medicate against gastrointestinal parasites by increasing consumption of plant secondary compounds with antiparasitic actions. This selective feeding improves health and fitness. Here, we review the evidence for self-medication in ruminants, propose a hypothesis t

Pathways to heroin dependence: time to re-appraise self-medication.

The self-medication hypothesis emphasizes the role of distressing affect as the primary motivator for the compulsive use that leads to substance dependence. The model also postulates that there will be psychopharmacological specificity between symptom presentation and the primary drug of dependence. In this review, the self-medication hypothesis is examined in relation to the development and chronicity of heroin dependence. It is argued that if self-medication has a role in engendering and extending substance dependence, it should be apparent in the use of a drug that carries such overwhelming

[Self-medication with local anaesthetics by glaucoma patients using teletonometry].

BACKGROUND: Self-tonometry, a supplementary measurement of the intraocular pressure in ophthalmology by glaucoma patients using an automatic tonometer, will become more and more important in the future. As long as the self-tonometry has to work in the contact modus with the ocular surface, home application of a topical anaesthetic by the glaucoma patient will be a requirement for a successful measurement. So far no severe problems within this controlled self-medication have been seen. Nevertheless, public health authorities believe patient health is put at high risk by the application of local

[Schizophrenia and addiction: An evaluation of the self-medication hypothesis].

Despite the fact that most researchers acknowledge the high prevalence of comorbid substance abuse among schizophrenic patients, there is no common agreement regarding the etiology of this serious public health problem. At the center of this debate though, Khantzian's self-medication hypothesis has captured most of the attention. In the present literature review, the authors evaluate this hypothesis in the light of our current knowledge. Formulated in a clinical context, in reaction to the psychoanalytic interpretation of addiction as a pleasure seeking pathology, Khantzian's hypothesis holds

Allergie acute contact dermatitis due to Arnica tincture self-medication.

After repeated intermittent use of Arnica tincture for rosacea a 66-year-old patient developed acute allergic contact dermatitis with blistering upon the single application of Arnica tincture to the dorsum of the right hand for minor swelling. Contact allergy of the delayed type to Arnica tincture could be proven by patch testing. Early diagnosis and treatment prevented exacerbation and spreading of the disease. In view of the wide use of Arnica preparations, particularly for self-medication, and the notable sensitizing potential, Arnica tincture should be a regular component of patch test ser

[Dependence on anticholinergics among schizophrenics. Iatrogenic disease or self-medication?].

This report concerns two schizophrenic patients who had ingested a biperiden overdose between 8 and 14 mg per day. This drug had been prescribed to treat extrapyramidal side effects of neuroleptics. Once the ingestion was suppressed, both patients complained about anxiety, discomfort and depression. These symptoms disappeared when reintroducing the biperiden. Both patients had a previous record of alcohol and cannabis abuse. One of them had also a recorder of sporadic consumption of other drugs such as heroin, amphetamines and hallucinogens. The predominant psychopathology was of the negative

Cigarette Smoking as Sleep Aid: Self-Medication Practices in Adolescents.

BACKGROUND: While smoking has been associated with alertness, it has also been reported as a stress relief and sleep aid. However, evidence on tobacco self-medication to improve sleep remains limited, particularly among adolescents. Therefore, the aim of this study was to explore the prevalence of tobacco self-medication in adolescents and analyze the association with sleep quality. METHODS: A cross-sectional descriptive study was conducted with 1,121 Spanish adolescents (mean age = 16.2 ± 0.7; 57.1% female) from public secondary schools. Participants completed assessments on their sleep qual

A case of sudden cardiac death following Domperidone self-medication.

The phenomenon of sudden cardiac death is usually related to the worsening of existing heart conditions leading to ventricular arrhythmia (VA). One of the well-known triggers of SCD is drug-induced prolongation of the QT interval, such as that caused by Domperidone (D). Despite its risk to prolong the QT interval and associated narrow therapeutic index, D is available as an over-the-counter (OTC) drug in many countries such as Italy, Ireland, Netherlands, China, South Africa, Mexico, New Zealand and Chile to treat gastroesophageal reflux and functional dyspepsia. The present paper reports a ca

[Analysis of the self-medication pattern among Brazilian elderly women].

The elderly consume more medications than other age groups, being susceptible to irrational use of therapeutic drugs. This work describes a cross-sectional study that evaluated self-medication practiced by elderly women, who were assisted at the Elderly Care Facility of the hospital of the Catholic University of Brasilia. The socio-economical variables analyzed were age group, educational level and monthly family income. The pharmaceuticals were evaluated according to prevalence, suitability for the age group and possible drug interactions. Among the 218 patients interviewed, 26% declared self

Self-medication hypothesis of substance use: testing Khantzian's updated theory.

Substance use research has been a salient focus for mental health professionals in recent years. Several organizations, including the American Psychological Association, have been pressing for more substance use research, particularly clinically relevant, theory-based investigations. However, there are few theories of substance use, and even fewer with scientific support. One theory is the Self Medication Hypothesis by Khantzian (1977, 1974), a theory with 30 years of research. However, recent modifications in Khantzian's theory (1999) have not been properly tested. Specifically, two areas req

Self-management for medication reduction in chronic low back pain.

It has been demonstrated that pain relief is seldom produced by medication or surgical methods where there is evidence of emotional disturbance, as indicated by the MMPI. A program that attempts to engender a high level of patient responsibility in a population of chronic low back pain patients is described. Self-managed reduction of drug dependence is a major component of this program. The data indicate that the program produces a significant reduction in dependence on opiates, derivatives, synthetic opiates, hypnotics, sedatives, tranquilizers, and analgesics. Follow-up data (with attrition
