Radiologic findings in cases of attempted suicide and other self-injurious behavior.

Suicide is one of the leading causes of death in multiple age groups. Therefore, it is important that radiologists be aware of this commonly encountered entity. Attempted suicide may take a variety of forms; those discussed by the authors include sharp force injury, immolation and inhalation, ingestion, hanging, firearm use, jumping from a height, and drowning. Certain radiologic findings are commonly seen in cases of attempted suicide and other self-injurious behaviors. In some cases, especially when the patient is unconscious or is unwilling to provide a history, it may not be apparent that

Factors distinguishing youth who report self-injurious behavior: a population-based sample.

OBJECTIVE: To identify factors distinguishing adolescents across 3 groups: no self-harm, nonsuicidal self-injury (NSSI) only, and NSSI and suicide attempt (NSSI + SA). METHODS: Data were from the 2007 Minnesota Student Survey. The sample included 61,330 students in grades 9 and 12. Logistic regression analysis determined factors that best distinguished adolescents who reported NSSI from those who reported no self-harm, and adolescents who reported NSSI + SA. Final models were developed over 3 stages of analysis that tested the importance of variables within risk factor, protective factor, and

The structure and correlates of self-injurious behavior in an institutional setting.

The prevalence of self-injurious behavior (SIB) in an institution for people with mental retardation was investigated. The relationship between SIB and age, sex, level of retardation, length of institutionalization, adaptive behavior, and probable causes of mental retardation was examined. A factor analysis on the topographies of SIB indicated the existence of two forms of SIB, stereotyped and social. The results are discussed in terms of probable causes of SIB.

Self-injurious behavior and Prader-Willi syndrome: behavioral forms and body locations.

With few exceptions (e.g., Lesch-Nyhan syndrome), the specific nature of self-injury in relation to identified genetic syndromes associated with mental retardation is poorly understood. In the present study we surveyed the families of 62 persons with Prader-Willi syndrome to determine the prevalence, topographies, and specific body locations of self-injurious behavior. Self-injury was reported for 81% of the participants. Skin-picking was the most prevalent form, with the front of the legs and head being disproportionately targeted as preferred self-injury body sites. Individuals with the 15q1

Assessment of stimulus generalization gradients in the treatment of self-injurious behavior.

Descriptive and experimental analyses suggested that the self-injurious behavior (SIB) of a 10-year-old girl with severe mental retardation was maintained by attention. Additional analyses identified physical contact as the type of attention maintaining SIB; therefore, we hypothesized that physical proximity of an adult was a discriminative stimulus for SIB. Based on these findings, we systematically varied the distance between the participant and a therapist to assess stimulus generalization. Results showed that rates of SIB varied relative to the distance between the participant and therapis

Noncontingent escape as treatment for self-injurious behavior maintained by negative reinforcement.

We extended research on the role of noncontingent positive reinforcement following a functional analysis of attention-maintained self-injurious behavior to self-injury maintained by negative reinforcement in 2 young males with developmental disabilities. During a pretreatment functional analysis, each participant's self-injury was shown to be differentially sensitive to escape from instructional activities as negative reinforcement. During noncontingent escape, escape from learning activities was provided on a fixed-time schedule that was not influenced by the participant's behavior. One parti

Implicit cognitions on self-injurious and suicidal behavior in borderline personality disorder.

BACKGROUND AND OBJECTIVES: Performance on implicit measures of suicidality has been associated with suicidal and nonsuicidal self-injury. Despite the high prevalence of self-harm in patients with borderline personality disorder (BPD), no previous study has assessed implicit measures in this patient group. METHODS: Forty patients with BPD and 25 healthy controls completed three implicit association tests (IATs) (Death words - Me/Others words, Self-Harm pictures - Me/Others, and Self-Harm pictures - Good/Bad words) and a subliminal priming task (effect of the primes "dying"/"growing" on the cat

Functions of nonsuicidal self-injurious behavior in Russian patients with suicidal ideation.

INTRODUCTION: Nonsuicidal self-injurious behavior (NSSI) is an important risk factor for future suicide attempts. Previous research has identified a number of motivations for engaging in NSSI. The aim of the present study was to translate the Inventory of Statements About Self-Injury (ISAS) into Russian and then to evaluate its psychometric properties in a sample of patients with non-psychotic mental disorders and suicidal ideation (SI). Other aims were to determine the prevalence of specific NSSI functions in this population and to assess the relationship between different NSSI functions and

Self-injurious behavior and gambling-related attitudes, perceptions and behaviors in adolescents.

Gambling is prevalent among adolescents and adolescents are vulnerable to experiencing gambling-related problems. Although problem gambling and suicidal behavior have been linked in adults and self-injurious behaviors may predict future suicidality, prior studies have not investigated relationships between problem-gambling severity and self-injurious behavior in adolescents. Data from 2234 Connecticut high-school students were analyzed in chi-square tests and logistic regression models to examine self-injurious behaviors in relation to at-risk/problem gambling with respect to sociodemographic

Identifying and managing nonsuicidal self-injurious behavior in the primary care setting.

PURPOSE: To provide an overview of nonsuicidal self-injurious behavior (NSSIB) for nurse practitioners (NPs) who see patients with this complex phenomenon in primary care settings. DATA SOURCES: Selected research and clinical reviews. CONCLUSIONS: NSSIB is characterized as a low-lethality, repetitive act that can alter or cause damage to the body tissue. Among researchers and those who work directly with young people, there is a general consensus that NSSIB is increasing in prevalence. Unfortunately, NPs often view self-injuring patients negatively, likely because of a lack of knowledge as t

Self-injurious behavior in adolescent girls. Association with psychopathology and neuropsychological functions.

BACKGROUND: Self-injurious behavior (SIB) is increasingly popular in psychically ill adolescents, especially in girls with posttraumatic stress (PTSD) and personality disorders. Adolescents with SIB frequently exhibit neurofunctional and psychopathological deficits. We speculated that specific neuropsychological deficits and temperamental factors could predispose patients to SIB and prospectively explored adolescent psychiatric patients with and without SIB in order to find out differences in psychopathology, and neuropsychological or temperamental factors. SAMPLING AND METHODS: Ninety-nine p

Effects of a topical anesthetic on non-socially maintained self-injurious behavior.

In the present study we hypothesized that the self-injurious behavior (SIB) of a 12-year-old male diagnosed as having autism and severe mental retardation was maintained by automatic reinforcement. Further, we hypothesized SIB served a sensory reinforcement function related to sensation at the skin surface. The participant engaged in high rates of SIB in the form of self-slapping. A functional analysis resulted in undifferentiated findings. Additional observations indicated that SIB occurred only to exposed skin surfaces. To test for a sensory function, we applied a topical anesthetic to the p

Repetitive self-injurious behavior: a neuropsychiatric perspective and review of pharmacologic treatments.

The phenomenology, pathophysiology, and psychopharmacology of repetitive self-injurious behavior (rSIB) are reviewed. Although numerous neurotransmitter systems are thought to be involved in the initiation and maintenance of rSIB, the majority of clinical studies attend to the role of serotonin or endogenous opioids. This focus has emerged from a conceptualization of rSIB as a problem of impulse control (primarily mediated by serotonin) and/or as a maladaptive pain-related behavior (ultimately mediated by opioids). A developmental perspective of rSIB is emphasized, highlighting the biased prev

Paradoxical effects of serotonin and opioids in pemoline-induced self-injurious behavior.

Self-injurious behavior (SIB) is a symptom of various psychiatric disorders with differing etiologies. Although no generally effective pharmacological treatment of SIB is available, subsets of individuals exhibiting SIB have been found to respond to opioid antagonists and selective serotonin reuptake inhibitors (SSRIs). The present study evaluated the efficacy of these two treatments in the pemoline-induced model of self-biting behavior (SBB) in rats. Using a factorial design, adult rats receiving daily pemoline at 100 mg/kg or the peanut oil vehicle were pretreated with either distilled water

Risperidone for aggression and self-injurious behavior in adults with mental retardation.

Risperidone has proven efficacy with reduced likelihood of causing extrapyramidal symptoms in the treatment of schizophrenia. Initial work suggests its utility in the management of aggression and self injury in patients with mental retardation. The use of risperidone in eight adult patients with moderate to profound mental retardation is described. Risperidone in these individuals was associated with significant reduction in aggression and self injurious behavior. Side effects were primarily those of sedation and restlessness. These cases illustrate the possible utility of risperidone in the t

Assessing the functions of self-injurious behavior: a case of assessment failure.

Seven attempts to assess the function of self-injurious behavior in a person with profound mental retardation are described. Although repeated correlational and experimental methods were implemented to determine the functional role of this behavior no clear functions were identified. A clinical implication of this case and that this methodology is not all-embracing. Different assumptions should be explored after substantial efforts have failed.

Plasma B-endorphin levels in patients with self-injurious behavior and stereotypy.

B-endorphin and cortisol concentrations were examined in the plasma of mentally retarded patients who displayed symptoms of self-injurious behavior (SIB) (n = 9), stereotypy (n = 17), or SIB plus stereotypy (n = 14). Morning and evening samples were compared with a matched patient control group (n = 13) and a group of nonretarded controls (n = 17). Compared with nonretarded controls, the retarded control group had significantly lower b-endorphin concentrations for the morning sample and nonsignificantly lower concentrations for the evening sample. The combined patient groups had lower b-endorp

Effects and side effects of DRO as treatment for self-injurious behavior.

A three-part controlled case study is presented in which severe and longstanding self-injurious behavior exhibited by a 9-year-old-boy was treated successfully with differential reinforcement of other behavior. In Phase 1, an experimental analysis demonstrated that the boy's scratching was not maintained by environmental contingencies; instead, it appeared that the self-injurious behavior was a stereotypic (automatically reinforced) response. In Phase 2, the effects of an escalating differential-reinforcement-of-other-behavior schedule mediated through token reinforcement (pennies) were evalua

Self-injurious behavior produced in rats by daily caffeine and continuous amphetamine.

Self-biting (SB) is an unusual behavioral effect of high doses of certain amphetamine-like drugs in rats. This bizarre behavior has received little attention, perhaps because the high doses of drug required and the dramatic disturbance of the animals' behavioral repertoire have raised the possibility that SB is a high dose phenomenon. However, we have found that continuous administration of very low amounts of amphetamine reliably produces SB in rats, and that this behavioral change can be very selective. We compared SB produced by continuous amphetamine to SB produced by daily caffeine; the l
