Co-administration of pethidine and clonidine: a spinal anaesthetic technique for total hip replacement.

Co-administration of pethidine 0.75 mg kg-1 and clonidine 75 micrograms intrathecally provided good intraoperative anaesthesia for total hip replacement, similar to that obtained using 0.5% isobaric bupivacaine. Sensory and motor block were of shorter duration than that after 0.5% isobaric bupivacaine and 0.5% isobaric bupivacaine with morphine 0.5 mg (P < 0.001 sensory block, P < 0.001 motor block). Postoperative morphine consumption, measured using a patient-controlled system, was similar to that in patients in the bupivacaine only group (pethidine-clonidine: median 39 mg/24 h; bupivacaine:

Mononeuropathy multiplex associated with acute parvovirus B19 infection: characteristics, treatment and outcome.

To describe the characteristics of peripheral neuropathy related to acute parvovirus B19 (B19V) infection. We reviewed clinical, electrophysiological and histological data of three patients with peripheral neuropathy and positive B19V detection (IgG, IgM and PCR) compatible with acute infection. The neuropathy fulfilled criteria for mononeuropathy multiplex (MM). It could be preceded by or concurrent with a limited purpuric eruption, but systemic manifestations were absent. The first neurological symptoms were always sensory and localized in a hand. Neuropathy was initially limited to a restri

Flexible intentions: An Active Inference theory.

We present a normative computational theory of how the brain may support visually-guided goal-directed actions in dynamically changing environments. It extends the Active Inference theory of cortical processing according to which the brain maintains beliefs over the environmental state, and motor control signals try to fulfill the corresponding sensory predictions. We propose that the neural circuitry in the Posterior Parietal Cortex (PPC) compute flexible intentions-or motor plans from a belief over targets-to dynamically generate goal-directed actions, and we develop a computational formaliz

Rewiring the Brain Through the Gut: Insights into Microbiota-Nervous System Interactions.

The gut-brain axis (GBA) represents an operant acting in a two-direction communication system between the gastrointestinal tract and the central nervous system, mediated by the enteric nervous system (ENS), vagus nerve, immune pathways, and endocrine signaling. In recent years, evidence has highlighted the pivotal role of the gut microbiota in modulating this axis, forming the microbiota-gut-brain axis (MGBA). Our review synthesizes current knowledge on the anatomical and functional substrates of gut-brain communication, focusing on interoceptive signaling, the roles of intrinsic primary affer

Irreversibility and collapsibility of predictive systems in mental representation: a sensorimotor paradox theory.

In this article, we will discuss the idea that one system cannot be both the observer and the agent of motor action at the same time. In that sense, the model of sensory prediction is somewhat biased, as it would suggest that a system would be able to observe its own action from a fixed viewpoint. We will argue that on a perceptual level, efferent and afferent signals cannot coexist simultaneously, at least as far as the point of the body leading the action is concerned. Triggering motor action activates available sensorimotor memory which a system adheres to completely. This adherence constit

Role of Purinergic Signaling in Voiding Dysfunction.

Purinergic signaling is a term that relates to adenosine triphosphate binding to its receptor (purinergic receptors such as P2X and P2Y subtypes). This pathway has been implicated in bladder functional disorders related to interstitial cystitis/painful bladder syndrome, neurogenic bladder secondary to spinal cord injury, lower urinary tract symptoms, diabetes, and aging. Purinergic signaling occurs at multiple sites, including the central nervous system, peripheral motor and sensory nerves, detrusor smooth muscle, and bladder urothelium. Future pharmacologic agents to treat bladder functional

Age-dependent appearance of SARS-CoV-2 entry sites in mouse chemosensory systems reflects COVID-19 anosmia-ageusia symptoms.

COVID-19 pandemic has given rise to a collective scientific effort to study its viral causing agent SARS-CoV-2. Research is focusing in particular on its infection mechanisms and on the associated-disease symptoms. Interestingly, this environmental pathogen directly affects the human chemosensory systems leading to anosmia and ageusia. Evidence for the presence of the cellular entry sites of the virus, the ACE2/TMPRSS2 proteins, has been reported in non-chemosensory cells in the rodent's nose and mouth, missing a direct correlation between the symptoms reported in patients and the observed dir

Diseases of peripheral nerves as seen in the Nigerian African.

The anatomical and aetiological diagnoses of peripheral nerve disease excluding its primary benign and malignant disorders, as seen in 358 Nigerians are presented. There is a male preponderance and the peak incidence is in the fourth decade. Sensori-motor neuropathy was the commonest presentation (50%). Guillain-Barré syndrome was the commonest identifiable cause (15.6%), accounting for half of the cases with motor neuropathy. Peripheral neuropathy due to nutritional deficiency of thiamine and riboflavin was common (10.1%) and presented mainly as sensory and sensori-motor neuropathy. Diabetes

Characterisation of ultraviolet-B-induced inflammation as a model of hyperalgesia in the rat.

In humans, the acute inflammatory reaction caused by ultraviolet (UV) radiation is well studied and the sensory changes that are found have been used as a model of cutaneous hyperalgesia. Similar paradigms are now emerging as rodent models of inflammatory pain. Using a narrowband UVB source, we irradiated the plantar surface of rat hind paws. This produced the classical feature of inflammation, erythema, and a significant dose-dependent reduction in both thermal and mechanical paw withdrawal thresholds. These sensory changes peaked 48h after irradiation. At this time there is a graded facilita

Modulation of the release and activity of neuropeptides in the microcirculation.

Electrical stimulation of the sensory saphenous nerve leads to neurogenic edema formation in the innervated area of the paw of the anesthetized rat. Evidence suggests that the edema formation is the result of increased microvascular permeability mediated via neurokinin NK1 receptors and increased blood flow mediated via calcitonin gene related peptide CGRP1 receptors. Results indicate that selective receptor antagonists will only inhibit the response mediated by the specific receptor they antagonise. In the case of neurogenic inflammation, where it is common for more than one biologically acti

Characterization of the peptide and sensory neurotoxic effects of capsaicin in the guinea pig.

The effects of capsaicin, the major pungent ingredient of hot peppers, were assessed on sensory neuron neuropeptide levels and on sensory function in the adult guinea pig. Systemic doses of capsaicin as low as 2.5 mg/kg depleted substance P (SP) in dorsal roots plus ganglia (DRG) whereas a 10-mg/kg dose depleted the peptide maximally in DRG and in the dorsal spinal cord. High doses of capsaicin had no consistent effects on levels of cholecystokinin (CCK), vasoactive intestinal polypeptide, or somatostatin, although a transient decrease in CCK levels was observed 4 days after dosing. Convergence of need technology and culture driving the era of physical AI in medicine.

Medicine is a predominantly physical profession, yet most medical artificial intelligence (AI) remains screen bound. Physical artificial intelligence (PAI) extends AI's capabilities and can directly address many of the healthcare system challenges and unmet needs such as workforce shortages, unsustainable healthcare costs, heightened expectations, aging population and need for pandemic preparedness. PAI relies on systems that autonomously perceive, decide, and actuate in real-time by synthesizing diverse environmental data from multiple sensory sources. These data undergo rapid processing and

Modeling of the oral malodor system and methods of analysis.

Bacterial putrefaction is the central metabolic process involved in oral malodor and can easily be modeled in the salivary sediment system developed from centrifuged whole saliva. Methods used in this system for examining malodor have included measurement of (1) head space (gaseous-phase) and liquid-phase odors, organoleptically; (2) volatile sulfur compounds, by means of an electrochemical sensory instrument; (3) pH, Po2, and oxidation-reduction potential with appropriate respective electrodes; (4) the malodorous compounds indole and skatole, colorimetrically; (5) substrate levels and their e

The Molecular and Pharmacological Mechanisms of HIV-Related Neuropathic Pain.

Infection of the nervous system with the human immunodeficiency virus (HIV-1) can lead to cognitive, motor and sensory disorders. HIV-related sensory neuropathy (HIV-SN) mainly contains the HIV infection-related distal sensory polyneuropathy (DSP) and antiretroviral toxic neuropathies (ATN). The main pathological features that characterize DSP and ATN include retrograde ("dying back") axonal degeneration of long axons in distal regions of legs or arms, loss of unmyelinated fibers, and variable degree of macrophage infiltration in peripheral nerves and dorsal root ganglia (DRG). One of the most

Insights Into Stria Vascular Function, Stria Immunity, and Age-Related Hearing Loss.

The microvasculature plays a crucial role in maintaining auditory health by delivering essential nutrients such as glucose, ions, growth factors, and hormones, while also facilitating the elimination of metabolic waste. Simultaneously, the innate immune system acts as a protective barrier against cochlear damage caused by infections, toxic substances, and foreign agents. The normal functioning of these two systems creates an appropriate microenvironment that supports the health of sensory hair cells and spiral ganglion neurons. Disruptions in blood flow or inappropriate activation of the immun

The role of Triclosan in dentifrice formulations, with particular reference to a new 0.3% Triclosan calcium carbonate-based system.

To discuss the role of Triclosan in dentifrice systems and demonstrate the enhanced efficacy of Triclosan in calcium carbonate-based systems when the level of the antimicrobial agent is raised from 0.2% to 0.3%. Triclosan is the most commonly used antimicrobial agent in oral care products, being compatible with a wide range of ingredients found in toothpaste formulations, whilst having no negative sensory features (e.g. taste, staining) that are associated with some other antimicrobial/anti-plaque agents. Triclosan is a broad spectrum antimicrobial agent, with additional anti-metabolic and ant

Modulation of sensory nerve function and the cough reflex: understanding disease pathogenesis.

To cough is a protective defence mechanism that is vital to remove foreign material and secretions from the airways and which in the normal state serves its function appropriately. Modulation of the cough reflex pathway in disease can lead to inappropriate chronic coughing and an augmented cough response. Chronic cough is a symptom that can present in conjunction with a number of diseases including chronic obstructive pulmonary disease (COPD) and asthma, although often the cause of chronic cough may be unknown. As current treatments for cough have proved to exhibit little efficacy and are larg

Cardioprotection via the skin: nociceptor-induced conditioning against cardiac MI in the NIC of time.

Timely reperfusion is still the most effective approach to limit infarct size in humans. Yet, despite advances in care and reduction in door-to-balloon times, nearly 25% of patients develop heart failure postmyocardial infarction, with its attendant morbidity and mortality. We previously showed that cardioprotection results from a skin incision through the umbilicus in a murine model of myocardial infarction. In the present study, we show that an electrical stimulus or topical capsaicin applied to the skin in the same region induces significantly reduced infarct size in a murine model. We defi

Pharmacological reversal of a pain phenotype in iPSC-derived sensory neurons and patients with inherited erythromelalgia.

In common with other chronic pain conditions, there is an unmet clinical need in the treatment of inherited erythromelalgia (IEM). TheSCN9Agene encoding the sodium channel Nav1.7 expressed in the peripheral nervous system plays a critical role in IEM. A gain-of-function mutation in this sodium channel leads to aberrant sensory neuronal activity and extreme pain, particularly in response to heat. Five patients with IEM were treated with a new potent and selective compound that blocked the Nav1.7 sodium channel resulting in a decrease in heat-induced pain in most of the patients. We derived indu
