OBJECTIVE: To determine the effectiveness of Neodymium-YAG (Nd-YAG) laser for hysteroscopic transection of the septate uterus to improve pregnancy outcome.
DESIGN: Patients treated for recurrent pregnancy loss and/or infertility were evaluated for before versus after treatment pregnancy outcomes.
SETTING: All patients were referred to a University Reproductive Endocrine and Infertility practice from 1986 through 1990.

Metroplasties were performed on twenty three septate uterus associated with recurrent reproductive wastage (19 cases) or primary infertility (4 cases). The surgical procedure and ultrasound monitoring of pregnancies conceived postoperatively are reported. Eighteen pregnancies occurred in sixteen patients; fourteen ended in live birth and four spontaneous abortions, including two blighted ova and one mole occurred. It appears from the results that uterine anomalies are rarely the sole cause of primary infertility; therefore, selection of patients is emphasized.

Mullerian anomalies occur as a result of errors during embryogenesis. We present the case of a 19-year-old nulligravida woman with severe dysmenorrhea since menarche; she was found to have a longitudinal vaginal septum, cervical duplication and two endometrial cavities, separated by a complete septum. A woman had an unusual müllerian anomaly with a septate uterus, cervical duplication, and longitudinal vaginal septum. Gynecologists should be aware of the possibility of cervical duplication associated with uterine septum and not didelphic uterus, as this disorder in a patient with infertility or recurrent miscarriages can be treated surgically by resection of the uterine and vaginal septum. Embryologic explanations may account for the occurrence of the anomaly.

Between April 1982 and April 1986, 91 patients underwent hysteroscopy for treatment of septate uteri. In 88 patients simultaneous laparoscopy was performed and one patient had a simultaneous laparotomy. Uterus septus is the most frequent congenital uterine anomalies caused by insufficient resorption of the müllerian ducts. Retrospective statistical analysis shows that the uterine septa have the worst reproductive outcome with a high incidence of early abortions. We report a rare case of uro-genital malformation in 18 years old girl. The malformation associate total septus uteri, hemivagina atretica dextra and homolateral dysplastic kidney. Dysmenorrhea, purulent discharges from cervix and vaginal tumor evocated the diagnosis, which was confirmed by ultrasonography, hysteroscopy and coelioscopy. Therapeutic attitude was total resection of vaginal septus and right nephrectomy. At today, seven cases were published in international literature. In all cases, renal aplasia was reported. Embryologic acknowlegements can't explain this malformation.
