Congenital Malformations Associated With a Single Umbilical Artery in Twin Pregnancies.

A single umbilical artery (SUA) was identified in 1.5% of twin pregnancies. The presence of a SUA in a twin pregnancy was associated with a 50% incidence of fetal anomalies, many of them complex and severe. The embryology and pathophysiological mechanisms associated with a SUA are reviewed. Aneuploidy is relatively common and should be considered, particularly in the presence of associated anomalies. Fetal growth restriction is frequent and preterm delivery is common.

Prenatal assessment of Wharton's jelly in umbilical cords with single artery.

OBJECTIVE: To investigate whether the amount of Wharton's jelly in non-malformed fetuses with a single umbilical artery is different from that of fetuses with a normal umbilical cord. METHODS: We evaluated patients with singleton pregnancies, non-malformed fetuses and single umbilical artery undergoing sonographic evaluation at a gestational age ranging from 19 to 41 weeks' gestation. The cross-sectional areas of the umbilical cord and of the umbilical vessels were measured. The amount of Wharton's jelly was calculated by subtracting from the total cross-sectional area of the umbilical cord t

The impact of isolated single umbilical artery on labor and delivery outcome.

OBJECTIVE: Data regarding the association between isolated single umbilical artery (SUA) and pregnancy outcome are inconsistent and mainly address the risk of pregnancy complications. Thus, we aimed to focus on the association between isolated SUA, and labor and delivery. METHODS: We conducted a case-control study of all singleton pregnancies diagnosed with isolated SUA (no known chromosomal/structural anomalies/fetal growth restriction) attempting vaginal delivery. Obstetric and neonatal outcome was compared with that of a control group of pregnancies with a three-vessel cord in a 1:2 ratio

Aplasia of the trachea in a malformed human foetus with single umbilical artery.

Aplasia of the trachea associated with multiple congenital anomalies is described in a stillborn male foetus with single umbilical artery. The tracheal anomaly conformed to the usual anatomic pattern consisting of absence of the trachea from the cricoid to slightly above the main stem bronchi where lay a fistula with the oesophagus. The larynx was normal externally but had a deformed cricoid and a narrow, blind lumen. The single umbilical artery was possibly responsible for causing the cardiovascular and tracheo-oesophageal anomalies by disturbing the haemodynamics, and the remaining caudal de

Pregnancy prognosis associated with an isolated single umbilical artery in twin pregnancy.

To determine the prognosis of an isolated single umbilical artery (SUA) in a twin pregnancy, we selected twin pregnancies with a second trimester ultrasound diagnosing a SUA in at least one fetus at our tertiary hospital. This was confirmed by placental histopathology or by expert review of ultrasound images. Cases were identified by searching the hospital ultrasound database over a period of 7.5 years. Higher order multiples or coexistent aneuploidy or major anomalies were excluded. Each case of an isolated SUA was assigned three consecutive twin pregnancy controls paired for chorionicity and

Prenatal sonographic diagnosis of laryngeal atresia in association with single umbilical artery.

Congenital laryngeal atresia is an extremely rare anomaly that in most cases is incompatible with life. About 50% of cases involve other major malformations. Prenatal sonographic diagnosis of this condition has been described only 6 times. We present a case in which prenatal sonographic examination at 21 weeks' menstrual age revealed enlarged, hyperechoic lungs, dilated upper airways, ascites, and a single umbilical artery. To our knowledge, this is the first reported case of the prenatal diagnosis of laryngeal atresia in association with a single umbilical artery.

Association between isolated single umbilical artery and neurodevelopmental disorders: population-based study.

OBJECTIVE: A single umbilical artery (SUA) is associated with a risk of adverse perinatal outcome. Around 11% of fetuses with SUA present with an associated major anomaly, including those of the central nervous system. However, studies on the associations between SUA and childhood neurodevelopmental disorders (NDD), such as intellectual disability (ID), attention deficit hyperactivity disorder (ADHD), autism spectrum disorder, epilepsy, impaired hearing and impaired vision, are limited. We aimed to evaluate the risk of NDD in children presenting with an isolated SUA (iSUA) at birth and the pos

Screening infants with an isolated single umbilical artery for renal anomalies: nonsense?

BACKGROUND: Presently there is no consensus regarding the extent of workup required in newborns who present with an isolated single umbilical artery (SUA). Because of the association with silent renal anomalies, some studies advise to perform renal ultrasonography in infants born with an isolated SUA. AIMS: To decide whether screening (performing a renal ultrasound and in case of abnormalities on the ultrasound performing a micturating cystourethrogram) infants with an isolated SUA is justified by evaluating the prevalence of clinically relevant renal abnormalities. STUDY DESIGN: Retrospecti

Common iliac artery flow velocity waveforms in fetuses with a single umbilical artery: a longitudinal study.

OBJECTIVES: In fetuses with a single umbilical artery the entire blood flow to the placenta is transported through the common and internal iliac arteries from the side of the single artery, whereas the pelvic vessels from the side of the missing artery do not participate in the fetoplacental circulation. The aim of this study was to investigate the effect of gestational age on pelvic arterial blood flow in fetuses with single umbilical artery. DESIGN: In 15 fetuses with a single umbilical artery (SUA), common iliac artery flow velocity waveforms were studied longitudinally using high resoluti

Significance of the single umbilical artery. A clinical, radiological, chromosomal, and dermatoglyphic study.

A single umbilical artery is present in about 1% of all singletons. It is associated with a wide range of congenital malformations. Chromosomal analysis of our surviving cases gave normal results. Dermatoglyphic patterns were normal, except that the boys showed a three- to fourfold increase in the number of the radial loops on the fingers, and a lowering of the total finger ridge count. Evidence favouring a common non-genetic cause for both a single umbilical artery and the associated malformations is discussed.

The clinical significance of a single umbilical artery as an isolated finding on prenatal ultrasound.

OBJECTIVE: To evaluate the perinatal outcome in fetuses with single umbilical artery detected on targeted prenatal ultrasound without other anomalies. METHODS: During a 3.5-year period, an isolated single umbilical artery was suspected on prenatal ultrasound examination in 57 fetuses evaluated at two referral centers. Targeted imaging to rule out concurrent fetal anomalies was normal in all cases. Pregnancy and perinatal outcome data were retrieved by review of the medical records or from conversations with referring physicians. Complete follow-up was available in 50 cases. RESULTS: A two-ve

[Ultrasonic study of the single umbilical artery syndrome. A series of 80 cases].

The authors report on 80 cases of the syndrome of a single umbilical artery (SUA) and compare the anatomo-clinical features with the abnormalities found on ultrasound. The clinical results confirm the information given in the literature which is that there is a higher incidence of poor intra-uterine fetal growth (36.4%), of prematurity (16.3%), of associated congenital abnormalities (42%) and of perinatal mortality (21.3%). Ultrasound examination makes it possible to screen for associated major fetal malformations (26.6%) and of most cases of intra-uterine growth retardation (28.3%). The ultra
