[Reproducibility of sinus node recovery time in relation to the time of the day and the overdrive stimulation rate (author's transl)].

Corrected sinus node recovery time (CSRT) was tested in 9 patients with (CSRTI) and 6 patients without sick sinus syndrome = SSS (CSRTII) at 6 constant time intervals each, on 3 consecutive days using 5 rates of atrial overdrive stimulation (AST) via a stable loop mounted atrial electrode. From the results presented it can be concluded: CSRTI is reproducible with high rates when corresponding time periods and the same AST rate are used. A stepwise CRST increase by 60% is consistent up to AST 140 bpm only; up to AST 200 bpm the pattern varies within maximal pathological CSRT ranges for each of

Embolism in sinoatrial disease.

Among a series of consecutive patients treated with permanent pacemaker between 1965 and 1976, 43 had sinoatrial disease with paroxysmal tachycardia (group A), 30 sinoatrial disease without tachycardia (group B) and 165 atrioventricular block (group C). A retrospective study showed systemic vascular events compatible with embolism in 35%, 7% and 10% in groups A, B and C, respectively. The groups were comparable as regards age, other diseases and duration of pacemaker therapy. Because of the high incidence of embolism in group A, anticoagulant therapy should be evaluated in patients with the br

Decreased connexin 40 expression of the sinoatrial node mediates ischemic stroke-induced arrhythmia in mice.

BACKGROUND: Arrhythmia is the most common cardiac complication after ischemic stroke. Connexin 40 is the staple component of gap junctions, which influences the propagation of cardiac electrical signals in the sinoatrial node. However, the role of connexin 40 in post-stroke arrhythmia remains unclear. METHODS: In this study, a permanent middle cerebral artery occlusion model was used to simulate the occurrence of an ischemic stroke. Subsequently, an electrocardiogram was utilized to record and assess variations in electrocardiogram measures. In addition, optical tissue clearing and whole-moun

Genetic predictors of sick sinus syndrome.

Sick sinus syndrome (SSS) encompasses a group of conduction disorders characterized by the inability of sinoatrial node to perform its pacemaker function. Our aim was to identify genetic predictors of SSS in a prospective cohort of patients admitted to the clinic for pacemaker implantation using single-locus and multilocus approaches. We performed genotyping for polymorphic markers of CLCNKA (rs10927887), SCN10A (rs6795970), FNDC3B (rs9647379), MIR146A (rs2910164), SYT10 (rs7980799), MYH6 (rs365990), and KCNE1 (rs1805127) genes in the group of 284 patients with SSS and 243 healthy individuals.

Sinoatrial response to premature atrial stimulation during atrial pacing in aged patients with and without sinus node dysfunction.

Sinus node function was analysed in 15 aged patients, including 8 with sick sinus syndrome and 7 without apparent sinus node dysfunction, by rapid artial pacing and premature atrial stimulation during sinus rhythm and 5 beats of atrial pacing. Three patterns of sinoatrial response were identified by premature atrial stimulation during atrial pacing. Type I response (3 cases) represented approximately constant return cycles and type II (2 cases) represented a markedly prolonged return cycles following premature stimulation. Type III response was characterized by a progressive shortening of retu

Optical mapping of activation patterns in an animal model of congenital heart block.

Congenital heart block (CHB) is associated with high mortality and affects children of mothers with autoantibodies (IgG) to ribonucleoproteins SSB/La and SSA/Ro. IgG from mothers of children with CHB (positive IgG) was used to assess activation patterns in both the right atrium (RA) and right ventricle (RV) of Langendorff-perfused young rabbit hearts. Optical action potentials (AP) were obtained by using a 124-site photodiode array with 4-[-[2-(di-n-butylamino)-6-naphthyl]vinyl]pyridinium. Optical APs were recorded to simultaneously image activation patterns from the RA and RV. Perfusion of po

Comparative analysis of anti-arrhythmic effect of propranolol and metoprolol in patients with supraventricular tachyarrhythmias.

Eighty-seven patients with paroxysmal tachydysrhythmias were submitted to clinical and laboratory evaluation and electrophysiologic studies were performed during the 3rd, 6th and 12th months of the therapy with propranolol or metoprolol. Metoprolol did not induce any significant change in sinoatrial correct time, in sinoatrial recovery time or in corrected sinoatrial recovery time. It decreases significantly the effective and functional AV refractory periods as well as increases the effective refractory period of the additional pathway. The mechanism of tachycardia preventing an involvement of

[Effect of ethmozine on the automatic function of the sinus node and sinoatrial conduction in patients with normal and impaired function of the sinus node].

In this study involving 45 patients with the normal function of the sinus node and 10 patients showing signs of its damage (n-10), intravenous administration of ethmozine in a dose of 150 mg (1.5-2 mg/kg) exerted atropine like action. In four cases with dysfunction of the sinus node the drug significantly increased the time of the restoration of sinus node function. Ethmozine induced a Stage II sinoatrial block in 3 out of 10 patients with sinus node dysfunction. The above data suggest that ethmozine produces an atropine-like effect on the sinus node. In some patients with sinus node dysfuncti

Bradyarrhythmias: clinical significance and management.

Clinicians have long recognized the potentially serious manifestations of extreme bradycardia. However, even marked bradycardia can often be physiologic, and in the presence of impaired ventricular function may offer important compensatory hemodynamic effects. Disorders of the sinoatrial node producing bradycardia include failure of impulse formation, sinoatrial conduction block, concealed sinus-perinodal reentry, carotid sinus hypersensitivity and the constellation of brady- and tachyarrhythmias that compose the "sick sinus syndrome." Bradycardia can also result from intraatrial block, atriov

[Direct recording of electrical activity of human sinoatrial node].

The authors developed a method for recording the electrical activity directly from the human sinus node through the right subclavian vein. Electrical activity of the sinoatrial node (SA) was studied in 179 patients with CHD (angina pectoris, myocardial infarction). The sick-sinus syndrome (SSS) was revealed in 86 patients. The duration of slow deflection Sd, Ss -- P interval (from the onset of Ss to the onset of P wave), P -- A1 interval (from the onset of P wave to the first atrial spike) was prolonged in cases with SSS. The duration of Sd, Ss -- P and P -- A1 decreased after nitroglycerine a

[Suicide attempt with diltiazem].

We report a case of a patient, with no conduction abnormalities, who had took 1,800 mg of diltiazem in an attempted suicide. We haven't come across a similar case, which is not associated with other drugs, in bibliography we have consulted. She had conduction abnormalities at different levels, such as atrioventricular block and sinoatrial block, but after a few hours sinus rhythm returned without more complicated treatment. Serial electrocardiograms showed the different types of block. Although we didn't make an electrophysiologic study, we made vagal tests which were normal.

Holter monitoring in the complex analysis of sinus node dysfunction.

To study the diagnostic possibility and the mechanisms involved in sinus node dysfunction, 23 patients with sick sinus syndrome were evaluated by the basic electrophysiological method (recovery times, secondary postpacing phases, sinoatrial conduction times) before and after pharmacologic autonomic blockade with i.v. propranolol 0.2 mg/kg and atropine 0.04 mg/kg, and by continuous rhythm monitoring. Patient groups of normal (I) and pathological (II) intrinsic heart rate (IHR) were compared. In group I (no. 15) prolonged recovery time (2/15), postpacing sinoatrial-block (1/15) and chaotic poste

[Sinus node function test--diagnostic value and methodologic limits].

By means of atrial stimulation methods for the establishment of the sinus node recovery time and for the indirect calculation of the so-called sinuatrial conduction time under certain methodical conditions disturbances of the sinus node automatism and of the sinuatrial conduction may be differentiated and moreover latent dysfunctions of the sinus node may be unmasked. Methodical problems in the recognition of the sinus node recovery time by higher-frequent atrial stimulation and the calculation of the sinuatrial conduction time by programmed atrial single or double manifold stimulation result

Staged operation to Fontan increases the incidence of sinoatrial node dysfunction.

Morbidity and mortality of total cavopulmonary connection (modified Fontan procedure) may be decreased in many patients with single ventricle in whom the risk of surgery is high by performing the operations in a staged fashion. Each operative intervention, however, exposes the sinoatrial node region to risk of injury, and a multistaged approach may increase the risk of altered sinoatrial node function in these patients. The purpose of this study was to compare the prevalence of perioperative arrhythmias in patients undergoing either a primary or staged approach to the Fontan operation. Records

The Subtle Pattern of Sinoatrial Wenckebach.

INTRODUCTION: Second-degree sinoatrial (SA) exit block type I, or SA Wenckebach, is a rare arrhythmia marked by progressively shortening P-P intervals followed by a dropped P wave and a noncompensatory pause, producing a grouped beating pattern on electrocardiogram (ECG). CASE PRESENTATION: A 64-year-old male with multiple comorbidities presented after a mechanical fall with bradycardia. ECG revealed a new SA exit block type I. Echocardiography showed preserved ejection fraction and grade II diastolic dysfunction. DISCUSSION: Nodal blockers were discontinued, and pacemaker implantation was d
