Risk of obstructive sleep apnea among taxi-motorbike drivers in Parakou city in West Africa and associated factors with road traffic accidents.

OBJECTIVES: Obstructive sleep apnea (OSA) is an underdiagnosed chronic respiratory disease, causing excessive daytime sleepiness (EDS) and road traffic accidents (RTA). This study aimed to determine the risk of OSA among taxi-motorbike drivers (TMD) and factors associated with RTA in Parakou, Benin. METHODS: A cross-sectional comparative study was carried out between July and September 2020, including 225 TMD and 450 motorbike drivers other than TMD (non-TMD). A multiple logistic regression was then performed to determine factors associated with RTA. RESULTS: The mean age of participants was

Urinary excretion profiles of the orexin receptor antagonist suvorexant and its metabolites.

PURPOSE: Suvorexant is an orexin receptor antagonist used in the treatment of insomnia. In this study, we investigated the urinary excretion profiles of suvorexant and its major metabolites, including conjugates, to obtain fundamental information for proving exposure to suvorexant in criminal cases. METHODS: Urine specimens were collected from three subjects for maximum 168 h after a single oral ingestion of suvorexant (10 mg), and suvorexant and its metabolites in urine were determined using liquid chromatography-tandem mass spectrometry with a C18 semi-micro column. RESULTS: The carboxylic

The Discovery of Suvorexant, the First Orexin Receptor Drug for Insomnia.

Historically, pharmacological therapies have used mechanisms such as γ-aminobutyric acid A (GABAA) receptor potentiation to drive sleep through broad suppression of central nervous system activity. With the discovery of orexin signaling loss as the etiology underlying narcolepsy, a disorder associated with hypersomnolence, orexin antagonism emerged as an alternative approach to attenuate orexin-induced wakefulness more selectively. Dual orexin receptor antagonists (DORAs) block the activity of orexin 1 and 2 receptors to both reduce the threshold to transition into sleep and attenuate orexin-m

Isolation and identification of the designer benzodiazepine cyclopropylflualprazolam in pressed candies marketed for sleep improvement.

A novel alprazolam analog was detected as an illegal additive in pressed candies marketed for sleep improvement using high-performance liquid chromatography (HPLC)-diode array detection (DAD). The compound was further isolated and purified via semi-preparative liquid chromatography, and its chemical structure was elucidated through high-resolution mass spectrometry (HRMS) and nuclear magnetic resonance (NMR) spectroscopy. The compound was identified as cyclopropylflualprazolam. Since alprazolam is a psychotropic controlled substance, this finding has garnered significant attention from regulat

Melatonin Vapes Contain Potential Contaminants and Alter the Human Bronchial Epithelial Transcriptome.

Melatonin vaping products, touted for their faster absorption than oral melatonin supplements, have been gaining popularity among adolescents as sleep aid. Here, we elucidated the response of human bronchial epithelial cells (hBECs) to high levels of melatonin from vaped aerosols, investigated the uptake of melatonin by hBECs in vitro, and characterized the chemical composition of three commercially available melatonin vapes. Melatonin vape exposure decreased the secretion of chemokines and produced an immunosuppressive gene expression signature. The tested devices contained potential contamin

Oveporexton: The first-in-class orexin receptor 2 (OX2R) agonist approved for treatment of narcolepsy type 1 (NT1).

Narcolepsy type 1 (NT1) is a chronic neurological disorder caused by selective loss of orexin-producing neurons, resulting in unstable sleep-wake regulation and characteristic symptoms including excessive daytime sleepiness (EDS), cataplexy, sleep paralysis, hypnagogic or hypnopompic hallucinations, and disrupted nocturnal sleep. Current treatments mainly provide partial symptomatic relief and do not correct the underlying orexin deficiency, highlighting the need for mechanism-based therapies. Oveporexton (TAK-861), an orally administered selective orexin receptor 2 (OX2R) agonist developed by

The mediating role of physical activity, morning wake-up time, and sleep-inducing medication use in the relationship between age and depression: a path analysis of a large kurdish cohort study in Iran.

BACKGROUND: Depression is a common and debilitating psychiatric disorder worldwide. Recognizing the relationships between depression-related factors can play a significant role in depression management. However, no study has yet used path analysis to examine the mediating role of physical activity, morning wake-up time, and sleep-inducing medication in the relationship between age and depression. METHODS: This path analysis study utilized data from the baseline phase of the Ravansar non-communicable disease cohort study. This study was conducted on people aged 35 to 65 years using sociodemogr

Paediatric narcolepsy: a review of diagnosis and management.

Narcolepsy is a chronic disabling neurological sleep disorder that requires lifelong treatment. We have outlined the clinical features of narcolepsy, the assessment and diagnosis process and have summarised the existing treatment options for children and adolescents with narcolepsy. In the future, the approach to management of paediatric narcolepsy should ideally be in a multidisciplinary setting, involving specialists in sleep medicine, sleep physiology, neurologists and psychologists/psychiatrists. A multidisciplinary approach will help to manage the potential impact of narcolepsy on childre

High Prevalence of Cannabidiol Use Within Male Professional Rugby Union and League Players: A Quest for Pain Relief and Enhanced Recovery.

Rugby is characterized by frequent high-intensity collisions, resulting in muscle soreness. Players consequently seek strategies to reduce soreness and accelerate recovery, with an emerging method being cannabidiol (CBD), despite anti-doping risks. The prevalence and rationale for CBD use in rugby has not been explored; therefore, we recruited professional male players to complete a survey on CBD. Goodness of fit chi-square (χ2) was used to assess CBD use between codes and player position. Effects of age on use were determined using χ2 tests of independence. Twenty-five teams provided 517 play

Integrative treatment of obstructive sleep apnea: principles and practice.

UNLABELLED: In the management of obstructive sleep apnea, the integrative concept of care is gathering momentum. This is an encouraging development to prevent deficiencies or errors due to a lack of professional collaboration since risks, benefits and alternatives of various techniques can be discussed candidly. However, while the deployment of a multidisciplinary team is encouraged, a scientific, evidence-based approach needs to be rigorously engendered to encompass the phenotypic heterogeneity of obstructive sleep apnea, including neurologic, metabolic, craniofacial, and myopathic endotypes.

Is bedtime use of kratom (Mitragyna speciosa) a sleep aid or disruptor? Examining its daily effects and individual differences.

Kratom (Mitragyna speciosa) is a psychoactive botanical native to Southeast Asia increasingly used in the United States for various self-reported benefits, including sleep improvement. However, empirical evidence concerning kratom's effects on sleep is limited. Here, we examined the association between bedtime kratom use and self-reported sleep outcomes in naturalistic settings across consecutive days, while also exploring sex and chronic pain status as potential moderators. A secondary analysis was conducted using the data from a 15-day ecological momentary assessment study of 357 U.S. adults
