Journal of clinical sleep medicine : JCSM : official publication of the American Academy of Sleep Medicine2007eng
The reliability and validity of EEG arousals and other types of arousal are reviewed. Brief arousals during sleep had been observed for many years, but the evolution of sleep medicine in the 1980s directed new attention to these events. Early studies at that time in animals and humans linked brief EEG arousals and associated fragmentation of sleep to daytime sleepiness and degraded performance. Increasing interest in scoring of EEG arousals led the ASDA to publish a scoring manual in 1992. The current review summarizes numerous studies that have examined scoring reliability for these EEG arous

The goal of the present study was to determine the effect of acute sleep deprivation on the ability to reappraise negative situations using a repeated-measures design. Seventy-six undergraduate students completed self-report measures of reappraisal use and the Script-Based Reappraisal Test (SBRT; Zeier et al., 2020) before and after randomization to a night of at-home sleep deprivation or 8 h of sleep opportunity. The Self-Assessment Manikin (SAM) indexed changes in valence and arousal, with more negative valence and higher arousal indicating greater difficulty reappraising the anger-inducing

Attempts to analyse the sleep structure of patients with eating disorders have so far led to conflicting results. Polygraphic findings suggest that patients with bulimia nervosa are not easily distinguishable from age-matched controls, whereas anorexic patients show some abnormalities in sleep efficiency and sleep architecture. Nevertheless, both bulimic and anorexic patients complain of poor quality sleep. The aim of this study was to evaluate the microstructure of sleep in anorexia and bulimia by analysing arousal (following the rules of the American Sleep Disorders Association) and the cycl

Studies in healthy young adults revealed that periodic arousals during the night result in increasing sleepiness as a function of the interval of time between arousals. When arousals are frequent, deficits similar to those seen after total sleep deprivation have been found. Observed decrements appear to be specifically related to EEG arousals and do not require complete awakening. In patients with fragmenting sleep disorders such as periodic leg movements and central sleep apnea, improved nocturnal sleep is related to increased alertness and daytime performance.

INTRODUCTION: Dissociated sleep-arousal states are clinical and experimental phenomena which represent mixed forms of the three stages of this cycle (arousal, NREM sleep and REM sleep).
CLINICAL CASE: We describe the case of a man who presented with a history of excessive diurnal somnolence for the previous 5 years. He also had symptoms of sleep paralysis, hypnagogic hallucinations, automatic behavior and excessive movements during sleep. He had had no episodes of loss of muscle tone; MR was normal; HL DR2 and DQwl antigens were negative; two polisomnographic and a Multiple Latency Sleep test

BACKGROUND: Patients with primary headache disorders including migraine and cluster headache often report poor sleep quality. While subjective reports of disturbed sleep are well established, the objective alterations in sleep architecture which occur in primary headache have not been systematically characterised. This systematic review aims to determine whether previously identified macrostructure changes in migraine occur across trigeminal autonomic cephalalgias, how sleep microstructure is altered, and how sleep architecture differs between the headache-free and headache attack phase. The category of common sleep disorders known as parasomnias includes disorders of arousal, rapid eye movement (REM) sleep behaviour disorder (RBD), nocturnal seizures, rhythmic movement disorder, and tooth grinding or 'bruxism'. Parasomnias are all characterised as undesirable physical or behavioural phenomena occurring during the sleep period. Although these conditions can be distressing and, in some cases, hazardous to the sleeper and his or her bed partner, it is important to recognise that parasomnias are diagnosable and treatable in the vast majority of patients. Evaluation begins with a

RATIONALE: A total of 20-30% of respiratory events in obstructive sleep apnea are terminated without clear arousal. Arousals are thought to predispose to further events by promoting hyperventilation, hypocapnia, and upper-airway dilator muscle hypotonia. Therefore, events terminated without arousal may promote stable breathing.
OBJECTIVES: To compare physiologic changes at respiratory event termination with American Sleep Disorders Association (ASDA) Arousal to No Arousal, and determine whether secondary respiratory events are less common and have higher dilator muscle activity after No Arous

INTRODUCTION: For many, sleep and sex are crucial for physical, emotional, and mental well-being. Poor sleep quality is linked to a myriad of ailments from coronary artery disease to major depressive disorder. Likewise, a decrease in the frequency of sexual activity is associated with a decrease in self-rated health status. Kleine-Levin syndrome (KLS) is a rare sleep disorder that provides a unique lens to examine the intricate interplay between sleep and sex as it is one of the few sleep disorders defined by concomitant sexual dysfunction.
OBJECTIVES: This study reviews the literature on lin

In this article the most frequent and interesting treatments for parasomnias are reviewed. Arousal disorders, REM sleep behavior disorders and nightmares, bruxism (which can occur in any sleep state) are considered. The pathophysiological mechanisms of parasomnias are unknown, explaining why there is rarely a specific treatment precipitating. Management depends on the patient's age, the frequency and intensity of the episodes, the familial structure, the presence of precipitating and risk factors, psychological disorders and the parasomnia itself. Different therapeutic approaches are described

New insights into the physiopathological correlates of arousal and sleep fragmentation have recently been gained through experimental and clinical studies in healthy individuals and in patients with sleep disorders. The development of new analyses of autonomic system during sleep, has enriched the knowledge of sleep fragmentation derived from electroencephalographic analysis and has made possible the characterization of other phasic events arising from sleep, such as autonomic arousals. All of these studies provide evidence in support of the hypothesis that autonomic activations without cortic

PURPOSE: Sleep enuresis (SE) is the second most common sleep complaint in childhood. It has been associated with bladder hyperactivity, excessive urine production and deeper sleep. Several sleep disorders have been described in association with SE like parasomnias and sleep apnea. The aim of this study was to analyze the presence of sleep disturbances in children with SE through the use of Sleep Disturbance Scale for Children (SDSC) compared to normal children matched for age and sex.
METHODS: A questionnaire evaluation was performed in 76 enuretic and 112 normal children through the Sleep Di

PURPOSE: Arousals may contribute to the pathogenesis of sleep-disordered breathing (SDB) and central sleep apnea (CSA). We aimed to determine the effect of the nonbenzodiazepine hypnotic zolpidem on the frequency of respiratory-related arousals and central apnea in patients with moderate-to-severe SDB. We hypothesized that zolpidem decreases the severity of SDB by decreasing the frequency of respiratory-related arousals.
METHODS: Patients with apnea-hypopnea index ≥ 15 events/hour and central apnea-hypopnea index ≥ 5 events/hour underwent a sleep study on zolpidem 5 mg and a sleep study with

A concept of generalized arousal of the CNS is presented and given an operational definition that leads to quantitative physical measures. Because this primitive arousal function underlies all motivated behavioral responses, cognitive functions, and emotional expression, disorders of generalized arousal can be associated with a large number of problems in medicine and public health, including vegetative states, attentional disorders, depression, occupational hazards, and problems with sleep and anesthesia. Some of its known mechanisms are briefly reviewed, at the levels of neuroanatomy, neurop

The neurobehavioral deficits of obstructive sleep apnea syndrome (OSAS) are often attributed to the rate of respiratory disturbance or rate of arousals during sleep. However, sleep disordered breathing is also associated with other changes in sleep infrastructure that may account for cumulative waking deficits. This was illustrated in polysomnographic data from 1,521 patients with OSAS where increasing arousal indices were associated with increased duration of stage 1 sleep and concomitant reduction in total sleep time. Similar results have been found in paradigms in which sleep was experiment

Periodic limb movement disorder (PLMD) is a condition in which patients experience frequent periodic limb movements of sleep (PLMS). Synchronized arousal responses cause sleep fragmentation, resulting in insomnia, daytime sleepiness, and fatigue. A 59-year-old man was identified as having intense sleep-talking and body movements, suggesting rapid eye movement (REM) sleep behavior disorder (RBD). Attended video-polysomnography (PSG) revealed that sleep-talking and body movements occurred only during non-REM sleep and were associated with PLMS-induced arousals (periodic leg movement arousal inde

BACKGROUND: Children with disorders of gut-brain interaction (DGBI) often report poor sleep. However, the presence of sleep disorders and polysomnography (PSG) characteristics have not been previously described. Thus, we aimed to compare PSG changes in children with DGBI compared to healthy controls and describe sleep disorders in DGBI patients.
METHODS: We conducted retrospective review of patients ages 7-19 y who met Rome 4 criteria for DGBI without organic GI, systemic conditions and underwent PSG at Cincinnati Children's Sleep Center within one year of the GI clinic visit. We collected de

The sleep apnea syndrome is often associated with the syndromes of daytime exhaustion and involuntary daytime sleeping fits. The cause is assumed to be fragmentary sleep resulting from night-time arousal. The central nervous activation reactions caused by apnea or hypopnea, respectively, and not the movement arousal determine the sleep structure. We have examined 10 male patients in the age range 40-55 years (48 +/- 6 SD) before and during the first 3 nights of CPAP therapy. Cardiorespiratory polysomnography was performed in all four nights. Sleep way analyzed visually and differentiation was

The measurement of arousals during sleep is useful to quantify sleep fragmentation. The criteria for electroencephalography (EEG) arousals defined by the American Sleep Disorders Association (ASDA) have recently been criticized because of lack of interobserver agreement. The authors have adopted a scoring method that associates the increase in chin electromyography (EMG) with the occurrence of an alpha-rhythm in all sleep stages (Université Catholique de Louvain (UCL) definition of arousals). The aim of the present study was to compare the two scoring definitions in terms of agreement and repe
