Associations of sleep disturbance with ADHD: implications for treatment.

Attention-deficit/hyperactivity disorder (ADHD) is commonly associated with disordered or disturbed sleep. The relationships of ADHD with sleep problems, psychiatric comorbidities and medications are complex and multidirectional. Evidence from published studies comparing sleep in individuals with ADHD with typically developing controls is most concordant for associations of ADHD with: hypopnea/apnea and peripheral limb movements in sleep or nocturnal motricity in polysomnographic studies; increased sleep onset latency and shorter sleep time in actigraphic studies; and bedtime resistance, diffi

Effects of light on biological functions and human sleep.

The nonvisual effects of light in humans are mainly conveyed by a subset of retinal ganglion cells that contain the pigment melanopsin which renders them intrinsically photosensitive (= intrinsically photosensitive retinal ganglion cells, ipRGCs). They have direct connections to the main circadian clock in the suprachiasmatic nuclei (SCN) of the hypothalamus and modulate a variety of physiological processes, pineal melatonin secretion, autonomic functions, cognitive processes such as attention, and behavior, including sleep and wakefulness. This is because efferent projections from the SCN rea

Sleep and inflammation: psychoneuroimmunology in the context of cardiovascular disease.

BACKGROUND: Poor sleep is prospectively linked to all-cause and cardiovascular mortality. Inflammatory processes may be an important biological mechanism linking poor sleep to cardiovascular disease. Such processes involve active participation of signaling molecules called cytokines in development of atherosclerotic plaques. PURPOSE: I review evidence from experimental sleep deprivation and clinical observational studies suggesting a bidirectional relationship between sleep and inflammatory cytokines. RESULTS: Findings from sleep deprivation studies indicate that sleep loss is associated wit

Sleep disorders in Tourette's syndrome.

Sleep disorders have been reported in approximately 80% of Tourette's syndrome (TS) patients. Sleep studies in TS patients have demonstrated a 30% reduction in delta-sleep (slow-wave sleep) in nontreated subjects, decreased percentage of REM-sleep and the presence of tics during sleep. A subgroup of young TS patients was reported to have an increased percentage of delta sleep. Although these findings were initially thought to result from deranged dopaminergic and serotoninergic functions in TS, we suggest that abnormalities of hypothalamic-mediated control mechanisms involving the intrinsic op

Polysomnographic findings in children with headaches.

Although previous studies suggested a relationship between headache and sleep disturbances, polysomnographic findings in children with headache are rarely described. We investigated polysomnographic findings in children with headaches, and correlated them with headache type and severity, body mass index, and medical treatment. Analysis of polysomnographic findings of 90 children with migraine (60), chronic migraine (11), tension headache (6), and nonspecific headache (13) indicated that sleep-disordered breathing was more frequent among children with migraine (56.6%) and nonspecific headache (

[Evaluation and treatment of sleep problems in children diagnosed with attention deficit hyperactivity disorder: an update of the evidence].

INTRODUCTION: Attention deficit hyperactivity disorder (ADHD) affects approximately 5% of all children and adolescents, and these patients frequently suffer from sleep problems. The association between sleep disorders and ADHD, however, is multifaceted and complex. AIMS: To explore the relationship between sleep disorders and ADHD. DEVELOPMENT: Sleep problems in children with ADHD include altered sleep and specific disorders per se or that may be due to comorbid psychiatric disorders or to the stimulants they receive as treatment for their ADHD. Today, an evaluation of the sleep conditions i

Assessment of Multifractal-Multiscale Properties of Heart Rate Variability in Sleep Disorders.

Cardiovascular signals exhibit self-similar characteristics that can be altered by sleep disorders. This study evaluates the multifractal-multiscale structure of heart rate variability (HRV) components in some sleep disorders, such as sleep apnea and REM sleep behavior disorder (RBD), to describe possible alterations in cardiovascular control. For this purpose, the HRV signal was decomposed into intrinsic mode functions (IMFs) using the complete ensemble empirical mode decomposition with adaptive noise method. Then, the IMFs were analyzed by the multiscale-multifractal detrended fluctuation an

Sleep to Internalizing Pathway in Young Adolescents (SIPYA): A proposed neurodevelopmental model.

The prevalence of internalizing disorders, i.e., anxiety and depressive disorders, spikes in adolescence and has been increasing amongst adolescents despite the existence of evidence-based treatments, highlighting the need for advancing theories on how internalizing disorders emerge. The current review presents a theoretical model, called the Sleep to Internalizing Pathway in Young Adolescents (SIPYA) Model, to explain how risk factors, namely sleep-related problems (SRPs), are prospectively associated with internalizing disorders in adolescence. Specifically, SRPs during late childhood and ea

[Idiopathic insomnia].

Idiopathic insomnia (Childhood-onset insomnia) which belongs to Intrinsic Sleep Disorders in ICSD, has some dysfunction in sleep-wake center in brain. Sleep-wake center consists of two parts, one is sleep center which is present in the anterior parts of hypothalamus, and the other is wake center which is present in the ascending reticular activating system including posterior parts of hypothalamus. Idiopathic insomnia patients are said to have hyperactivity in wake center or hypoactivity in sleep center. And these patients have sleep disturbances such as inability to fall asleep, frequent awak

ipRGCs: possible causation accounts for the higher prevalence of sleep disorders in glaucoma patients.

Sleep accounts for a third of one's lifetime, partial or complete deprivation of sleep could elicit sever disorders of body function. Previous studies have reported the higher prevalence of sleep disorders in glaucoma patients, but the definite mechanism for this phenomenon is unknown. On the other hand, it is well known by us that the intrinsically photosensitive retinal ganglion cells (ipRGCs) serve additional ocular functions, called non-image-forming (NIF) functions, in the regulation of circadian rhythm, melatonin secretion, sleep, mood and others. Specifically, ipRGCs can directly or ind

An EEMD-derived slow-wave activity metric improves association with obstructive sleep apnea severity beyond conventional N3 staging.

STUDY OBJECTIVES: Conventional sleep staging relies on visual criteria, overlooking the contiguous transitions of sleep configuration and individual variability of brainwaves. Consequently, whether conventional N3 sleep metrics sufficiently capture sleep architecture alterations associated with heterogeneous sleep disorders such as obstructive sleep apnea (OSA) remains uncertain. Our objective was to determine whether slow-wave sleep (SWS) derived from an elevated slow-wave activity (SWA)-based approach exhibits stronger associations with the apnea-hypopnea index (AHI) than conventional N3 sle

Irregular sleep-wake rhythm disorder: From the pathophysiologic perspective to the treatment.

Irregular sleep-wake rhythm disorder (ISWRD) is an intrinsic circadian rhythm disorder caused by loss of the brain's circadian regulation, through changes of the input and/or output to the suprachiasmatic nucleus (SCN), or of the SCN itself. Although there are limited prevalence data for this rare disease, ISWRD is associated with neurodegenerative disorders, including the Alzheimer disease (AD) and the Parkinson disease (PD), which will become increasingly prevalent in an aging population. It additionally presents in childhood developmental disorders, psychiatric disorders, and traumatic brai

Sleep disturbances and Parkinson's disease.

Although Parkinson's disease is defined by its motor symptoms, the symptoms that are most devastating to patients and caregivers are dementia and psychosis. In addition, sleep has a tremendous impact on patient well being and quality of life. Eighty percent to 90% of Parkinson's disease patients have a sleep disorder affecting their ability to fall asleep, ability to stay asleep, dreams, motor activity during sleep, post-sleep behavior, or daytime somnolence. Treatment plans for patients with Parkinson's disease who experience sleep disorders aim to improve nighttime sleep or daytime wakefulne

Analysis of sleep spindles in children with Asperger's syndrome.

Sleep spindles are an element of the sleep microstructure observed on the EEG during the NREM sleep phase. Sleep spindles are associated to sleep stability functions as well as memory consolidation and optimization of different cognitive processes. On the other hand, Asperger's syndrome (AS) is a generalized developmental disorder in which cognitive and sleep disturbances have been described. In this study we analyzed different characteristics of sleep spindles in a group of children with AS and compared them with sleep spindles of a group of children with typical development paired by age; bo

Effects of repetitive transcranial magnetic stimulation in subjects with sleep disorders.

In this review, we aimed at identifying the studies that have employed repetitive transcranial magnetic stimulation (rTMS) in patients with sleep disorders. Low-frequency (LF) rTMS stimulating the right dorsolateral prefrontal cortex (DLPFC) or the posterior parietal cortex (PPC) was found to be effective to reduce cortical hyperexcitability and improve the sleep quality in subjects with chronic primary insomnia (PI). Both high-frequency (HF) and LF rTMS applied over the primary motor cortex or the supplementary motor cortex seem to have transient beneficial effects in patients with restless l

Delayed Sleep-Wake Phase Disorder: Can Polysomnography Be Useful?

BACKGROUND: Delayed sleep-wake phase disorder (DSWPD) is a chronic condition with a multifactorial etiology that primarily affects adolescents, significantly influencing their quality of life. In clinical practice, the contribution of intrinsic and behavioral factors is difficult to determine. The aim of our study was to compare data from clinical interviews, sleep diaries, actigraphy, and nocturnal polysomnography (PSG) in a cohort of adolescents with DSWPD and to assess psychiatric/neurodevelopmental comorbidity. METHODS: Thirty-one patients (22 male; mean age 15.4 ± 2.2 years, range 12 to

Magnetoencephalography correlate of EEG POSTS (positive occipital sharp transients of sleep).

PURPOSE: In contrast to EEG, which has guidelines for interpretation and a plethora of textbooks, the full range of activity seen in magnetoencephalography (MEG) has not been fleshed out. Currently, magnetoencephalographers apply criteria for EEG waveforms to MEG signals based on an assumption that MEG activity should have morphology that is similar to EEG. The purpose of this article was to show the characteristic MEG profile of positive occipital sharp transients of sleep. METHODS: Simultaneous MEG-EEG recordings of two cases are shown. RESULTS: In both the cases, the morphologic features

Impaired sleep quality and restless legs syndrome in idiopathic focal dystonia: a controlled study.

While sleep disorders are common in Parkinson's disease and other basal ganglia disorders, information on sleep disturbances in dystonia is limited to generalized forms or Segawa disease. Although many patients with idiopathic cervical dystonia (CD) and blepharospasm (BL) report poor sleep, there are no data on frequency or interactions with well known symptoms like depression and pain. Standardized interviews and assessment instruments, clinical examinations, and self rating forms were applied in 221 patients with CD and BL, and in 93 neurologically healthy controls.
