Sleep latency and duration estimates among sleep disorder patients: variability as a function of sleep disorder diagnosis, sleep history, and psychological characteristics.

STUDY OBJECTIVES: Insomnia patients often report greater sleep disturbance than found via polysomnography; yet the specific patient factors related to such sleep time misperceptions are poorly understood. We sought to characterize the extent to which a diverse group of patients complaining of insomnia (n=104) misperceive overnight total sleep time and sleep latency, and to identify patient factors associated with these variations. DESIGN: Cross-sectional. SETTING: University based sleep disorders center. PATIENTS: Sleep disorder groups consisted of patients with psychophysiological insomnia

Measurement of P300 and sleep characteristics in patients with hypersomnia: do P300 latencies, P300 amplitudes, and multiple sleep latency and maintenance of wakefulness tests measure different factors?

To explore further the relationship between ease of falling asleep, ability to maintain wakefulness, attention and information processing in sleep apnea and other sleep disorders, we conducted a thorough analysis of the similarities, differences and correlations between auditory and visual P300 amplitudes and latencies, and tests of sleepiness. The 283 consecutive patients presenting with hypersomnia were administered nocturnal polysomnography. Next day they underwent auditory and visual P300 recordings, Multiple Sleep Latency Test (MSLT) and Maintenance of Wakefulness Test (MWT). Correlation

A single nucleotide polymorphism in the HOMER1 gene is associated with sleep latency and theta power in sleep electroencephalogram.

Glutamate is the most excitatory neurotransmitter in the central nervous system and it is involved in the initiation and maintaining of waking and rapid-eye-movement (REM) sleep. Homer proteins act in the trafficking and/or clustering of metabotropic glutamate receptors, and polymorphisms in the HOMER1 gene have been associated with phenotypes related to glutamate signaling dysregulation. In this study, we report the association of a single nucleotide polymorphism (SNP) in the HOMER1 gene (rs3822568) with specific aspects of sleep in a sample of the Brazilian population. To accomplish this, 1,

A comparison between visual and computer assessment of sleep onset latency and their application in a pharmacological sleep study.

Sleep onset latency (SOL) is frequently defined as the time between lights-out and the first epoch of sleep stage 2. In practice, SOL can be quantified easily on the basis of visual examination. We have developed a computer algorithm allowing an automatic estimation of this parameter. The agreement between both strategies, visual and computer analysis, was tested using data from a pharmacological sleep study with 16 elderly insomniacs, which was aimed at comparing the effects of lormetazepam and zopiclone on polysomnography. A high correlation was found between the visual and the computer-base

Determination of sleep latency in polysomnographic evaluations of daytime somnolence in patients with sleep apnea and patients with narcolepsy.

Sleep onset during the multiple sleep latency test was scored by three criteria for 21 patients with narcolepsy and 21 patients with obstructive sleep apnea: a single epoch of stage 1, three consecutive epochs of stage 1, and a single epoch of stage 2 or REM. Mean sleep latency for both groups was predictably shortest using a single epoch of stage 1 and longest using a single epoch of stage 2 or REM. All estimates of sleep latency were highly correlated. It was concluded that a single epoch of any stage of sleep is an appropriate measure of sleep latency for patients with narcolepsy, although

Objective and subjective socioeconomic gradients exist for sleep quality, sleep latency, sleep duration, weekend oversleep, and daytime sleepiness in adults.

Socioeconomic gradients exist for multiple health outcomes. Lower objective socioeconomic position (SEP), whether measured by income, education, or occupation, is associated with inadequate sleep. Less is known about whether one's perceived ranking of their social status, or subjective SEP, affects sleep. This study examined whether a subjective socioeconomic gradient exists for sleep while controlling for objective SEP. Participants (N = 177; age, M = 45.3 years, SD = 6.3 years) completed the Pittsburgh Sleep Quality Index, Epworth Sleepiness Scale, MacArthur Ladder, and other self-report mea

[Quality of day time sleep in the multiple sleep latency tests in patients with narcolepsy, obstructive sleep apnea and psychogenic hypersomnia].

The multiple sleep latency test (MSLT) has proved to be a useful diagnostical tool for patients complaining of excessive daytime sleepiness (EDS). The intention of the present study was to investigate the structure of MSLT naps and in particular sleep spindle and k-complex density in three different groups of EDS patients. MSLT was performed at 8 a.m., 10 a.m. 12 a.m., 2. p.m. and 4 p.m.. Each recording lasted 20 minutes and was not stopped even if sleep occurred before 20 min. Sleep was scored visually. Spindle and k-complex density was determined per minute of S2 sleep. Statistical analysis

Sleep propensity in psychiatric hypersomnolence: A systematic review and meta-analysis of multiple sleep latency test findings.

Hypersomnolence plays a sizeable role in the course and morbidity of psychiatric disorders. Current sleep medicine nosology is reliant on the multiple sleep latency test (MSLT) to segregate hypersomnolence associated with psychiatric disorders from other central nervous system causes. However, the evidence base regarding sleep propensity in psychiatric hypersomnolence as measured by the MSLT has not been systematically evaluated, which is vital to clarify the utility and validity of current nosological schema. In this review, the use of sleep propensity assessed by the MSLT in patients with ps

[Multiple sleep latency test, maintenance of wakefulness test and suggestive immobilization test].

In clinical practice, assessment usually involves self-report; however, objective measures are available. The multiple sleep latency test(MSLT) is performed during the main period of wakefulness and is designed to determine a patient's propensity to fall asleep. To be valid, the MSLT should be performed the day after nocturnal polysomnography (PSG). It is the standard test for the assessment of objective sleepiness and diagnosis of narcolepsy, that is a mean sleep latency equal to or under 8 minutes and equal to or greater than two sleep onset REM periods (SOREMPs). As opposed to the MSLT, the

What does the multiple sleep latency test measure in a community sample?

Multiple sleep latency test (MSLT)-defined daytime sleepiness and its relationships with nocturnal and daytime psychophysiological activation were investigated in a random community sample of 77 subjects aged 35-55 years. The correlation structure between all study variables was explained by a simple model of daytime sleepiness. The model suggested that indicators of psychophysiological arousal (psychological distress, nocturnal motor activity and serum thyrotropin level) and daytime reported tiredness, body mass index (BMI) and age were related significantly and independently to MSLT-defined

The influence of age and sex on sleep latency in the MSLT-30--a normative study.

STUDY OBJECTIVES: The Multiple Sleep Latency Test-30 (MSLT-30) is a variation of the Multiple Sleep Latency Test with a fixed duration of 30 minutes for each of 5 test sessions. It requires less effort for reliable recording and is not susceptible to on-line scoring errors. The aim of the study was to provide normative data for the clinical use of the MSLT-30 and to evaluate the influence of age, sex, and other sociodemographic variables. DESIGN: An MSLT-30, along with measures of mood, objective, and subjective sleepiness was performed in a sample of healthy subjects in a balanced quota desi

Comparison of the results of the Epworth Sleepiness Scale and the Multiple Sleep Latency Test.

The Epworth Sleepiness Scale (ESS), which asks patients to estimate the likelihood that they would doze off or fall asleep in sedentary situations, has been proposed to be a quick, inexpensive way to assess sleepiness. We analyzed relations among ESS scores, mean sleep latencies on the Multiple Sleep Latency Test (MSLT), and subjective assessments of severity of sleepiness in 60 patients (34 women) with suspected excessive daytime sleepiness. Mean scores were 14.2 +/- 5.9 on the ESS and 8.3 +/- 5.2 minutes on the MSLT. ESS scores correlated negatively, but not strongly, with MSLT scores (rho =

REM sleep latency as an independent risk for cardiovascular events in hemodialysis patients.

BACKGROUND: Clinical significance of objectively measured poor sleep quality (SQ) as a risk for cardiovascular disease (CVD) events is not well known in hemodialysis (HD) patients, independently of sleep-related breathing disorders (SRBDs) and sleep-related metabolic abnormality. METHODS: The present study investigated baseline levels of objective sleep architecture together with obstructive sleep apnea (OSA) and central sleep apnea (CSA) using polysomnography in 88 HD study participants (M/F, 56/32; age 68.4 ± 9.3). Then, HD study participants were monitored for the occurrence of new-onset C

Multiple sleep latency test and polysomnography in patients with central disorders of hypersomnolence.

A multiple sleep latency test (MSLT) with occurrence of sleep onset REM periods (SOREMP) is considered one of the central diagnostic criteria for narcolepsy according to the International Classification of Sleep Disorders, but its sensitivity and specificity have been questioned. This study aims to describe MSLT and polysomnography (PSG) findings, including frequency and distribution of SOREMP during the day, in a large cohort of patients with central disorders of hypersomnolence (CDH). We retrospectively analyzed electrophysiological data from MSLT and PSG in 370 consecutive patients with nar

Influence of a Post-Test Factor on the Results of the Multiple Sleep Latency Test.

STUDY OBJECTIVES: The multiple sleep latency test (MSLT) is the standard method for the objective measurement of excessive daytime sleepiness. It is well established that preceding factors may influence the MSLT result. Aim of this study was to evaluate the potential influence of a post-test factor on the MSLT outcome. METHODS: For this retrospective study we reviewed polysomnography (PSG) reports conducted between 2012 and 2013. PSG reports including MSLT with at least four runs were included. Patients were stratified into (1) those discharged immediately after the MSLT or (2) those discharg

Diurnal variations in the waking EEG: comparisons with sleep latencies and subjective alertness.

Daytime measures of sleep latency and subjective alertness do not correlate with one another, suggesting that they assess different aspects of alertness. In addition, their typical diurnal variations show very different time courses. Quantitative analysis of the waking electroencephalogram (EEG) has been proposed as an objective measure of alertness, but it is not clear how it compares with other measures. In this study, the waking EEG was measured in the daytime to determine the presence of diurnal variations in the activity of standard frequency bands and to compare these variations with the

The impact of music upon sleep tendency as measured by the multiple sleep latency test and maintenance of wakefulness test.

Previous work has shown that background noise or music has a small positive impact on performance during sleep deprivation. The current study examined the effect of background music on the ability to fall asleep or remain awake. Twelve normal-sleeping young adults took multiple sleep latency tests (MSLT) and maintenance of wakefulness tests (MWT) after baseline sleep and one night of total sleep deprivation either with background music or under standard (quiet) conditions. It was hypothesized that the music would help maintain wakefulness both under baseline and sleep deprivation conditions. T

Maintenance of wakefulness test and multiple sleep latency test. Measurement of different abilities in patients with sleep disorders.

The multiple sleep latency test and the maintenance of wakefulness test were administered on the same day to 258 consecutive patients whose clinical presentation required evaluation for excessive sleepiness. While the MSLT is the standard test for assessing excessive daytime sleepiness, the MWT may have some clinical advantage over the MSLT when the assessment of daytime alertness is the primary goal. To explore further the relationship between alertness and sleepiness, we have conducted a thorough analysis of the similarities, differences, and correlations between MWT and MSLT. The results of
