Sodium bicarbonate and systemic hemodynamics in volunteers anesthetized with halothane.

The cardiovascular effects of acute metabolic alkalosis (NaHCO3) in normal male volunteers anesthetized with halothane were measured. Pure metabolic alkalosis was studied by maintaning the end-tidal carbon dioxide tension at 40 torr. In each subject, cardiac index increased and total peripheral resistance decreased after each dose of NaHCO3. The increased cardiac index was associated with increased central blood volume, left ventricular minute work index, stroke index, and heart rate. Systolic time intervals showed increased myocardial performance. NaHCO3 administered to volunteers whose heart

Sodium bicarbonate for "metabolic acidosis": an illustration of the Goodhart's law.

BACKGROUND: Correction of "metabolic acidosis" by sodium bicarbonate is a common practice in intensive care unit or operating room. The physiological rationale underlying its use relies on the potential harmful hemodynamics effects of prolonged low extracellular pH. MAIN TEXT: Evidence showed that the "artificial" and transient correction of blood pH has little if any effect on intracellular pH, the variable of interest, which drives enzymatic reactions and metabolic pathways. Conversely, infusion of sodium bicarbonate corresponds to administer a sodium load which may affect patient's osmolar

Effect of sodium bicarbonate administration on blood constituents of horses.

OBJECTIVE: To describe changes in blood constituents of horses after oral and i.v. administration of sodium bicarbonate (NaHCO3), and to determine whether the changes are dose dependent. ANIMALS: 6 adult Standardbred mares. PROCEDURE: 3 oral doses (1,500, 1,000, and 250 mg/kg of body weight) or 1 intravenous dose (250 mg/kg, 5% solution) of NaHCO3 in 3 L of water, or water (3 L orally), were given to the mares; then changes in blood constituents were measured. Access to food and water was denied during the experiment. Blood samples were collected immediately before treatment and at hourly in

Influence of sodium bicarbonate on sprint performance: relationship to dosage.

The purpose of this investigation was to determine the minimum oral dosage of bicarbonate needed to significantly elevate blood bicarbonate and the influence of induced alkalosis on performance in high-intensity, short-duration exercise. Nine endurance-trained cyclists performed four 2-min sprints on separate occasions using an isokinetic cycle ergometer (Fitron, Cybex, Inc.). One hour before each test, the cyclists consumed either a placebo (A), a solution of 0.10 g NaHCO3.kg-1 body weight (B), a solution of 0.15 g NaHCO3.kg-1 body weight (C), or a solution of 0.20 g NaHCO3.kg-1 body weight (

Role of sodium, bicarbonate, and plasma osmolality in biliary secretion.

To examine the effect of changes in biliary sodium and bicarbonate secretion on bile formation, experiments were performed on fasted, pentobarbital-anesthetized pigs. During continuous intravenous secretin infusion (2.7 CU X kg body wt-1 X h-1) sodium secretion was altered by increasing or reducing plasma sodium concentration. Bicarbonate secretion was altered by varying arterial plasma pH. At increased biliary sodium secretion, bile formation was depressed, but changes in bicarbonate secretion were accompanied by parallel alterations in bile formation. Bile acid secretion was increased during

Insulin and sodium bicarbonate treatment in diabetic ketoacidosis in children.

Nineteen patients, nine males and ten females had 24 episodes of diabetic ketoacidosis. Infection was the precipitating factor in the development of ketoacidosis. Recovery of ketoacidosis occurred with 0.7 units of crystalline insulin per kg of body weight. The insulin requirement is less than the generally recommended dose. Hypoglycemia, hypokalemia, and alkalosis were not developed in our children. Ketoacidosis was adequately corrected by 2.5 mEq of sodium bicarbonate per kg of body weight.

Does Sodium Citrate Cause the Same Ergogenic Effect As Sodium Bicarbonate on Swimming Performance?

The aim of this study was to investigate the effect of ingesting sodium bicarbonate (SB) and sodium citrate (SC) on 400 m high-intensity swimming performance and blood responses. Six nationally ranked male swimmers (20.7 ± 2.1 yrs; 184 ± 6 cm; 79.9 ± 3.9 kg; 10.6 ± 1% body fat) participated in a double blinded, placebo controlled crossover trial. Ninety minutes after consuming SB (0.3 g·kg-1), SC (0.3 g·kg-1) or a placebo (PL) participants completed a single 400-m freestyle maximal test on three consecutive days. The order of the supplementation was randomized. Capillary blood samples were col

Sodium bicarbonate to the ROSCue: a case report on how sodium bicarbonate successfully resuscitated a patient in cardiac arrest from suspected quetiapine and trazodone overdose.

BACKGROUND: Per ACLS algorithms (Class III, LOE B), sodium bicarbonate (SB) use is indicated for cardiac arrest with underlying hyperkalaemia, tricyclic antidepressant (TCA) overdose, or pre-existing severe metabolic acidosis, with routine use of SB being discouraged due to concerns raised in animal studies. However, SB is a potent antidote for sodium channel blockade (SCB) in drug toxicities beyond TCA toxicity and should be considered more broadly. CASE SUMMARY: We present a 50-year-old man who suffered an out-of-hospital cardiac arrest with a wide-complex bradycardia (QRS 184 ms) due to su

Comparative effects of sodium bicarbonate and sodium chloride on reversing cocaine-induced changes in the electrocardiogram.

Cocaine abuse is associated with a number of cardiovascular complications that include arrhythmias and sudden cardiac death. Although the mechanism(s) remain unclear, cocaine-induced block of sodium channels resulting in slowed cardiac conduction is thought to play an important role. Several reports suggest that the effects of cocaine effects on cardiac sodium channels can be reversed by administration of sodium bicarbonate. Whether the beneficial effects of sodium bicarbonate are due to sodium ions or an increase in blood pH is unknown. Therefore the purpose of this study was to compare the e

When heat strikes: Investigating unexpected particles in sodium bicarbonate injections.

The presence of particles in injectable formulations presents significant risks, such as embolism, phlebitis, and thrombophlebitis. Moreover, visible particles violate pharmacopeial standards and may lead to regulatory actions. Therefore, it is crucial to identify the underlying cause and eliminate visible particles to ensure both patient safety and regulatory compliance. Sodium Bicarbonate Injection, a straightforward formulation containing only the active pharmaceutical ingredient and water for injection, is commonly packaged in glass vials for commercial use. This study reports an unexpecte

Antidotal Sodium Bicarbonate Therapy: Delayed QTc Prolongation and Cardiovascular Events.

BACKGROUND: Sodium bicarbonate therapy (SBT) is currently indicated for the management of a variety of acute drug poisonings. However, SBT effects on serum potassium concentrations may lead to delayed QTc prolongation (DQTP), and subsequent risk of adverse cardiovascular events (ACVE), including death. Emergency department (ED)-based studies evaluating associations between SBT and ACVE are limited; thus, we aimed to investigate the association between antidotal SBT, ECG changes, and ACVE. METHODS: This was a secondary data analysis of a consecutive cohort of ED patients with acute drug overdo

Prevalence of sodium bicarbonate-induced alkalemia in cardiopulmonary arrest patients.

BACKGROUND: Intravenous sodium bicarbonate (SB) administration during cardiopulmonary arrest (CPA) is intended to counteract lactic acidosis due to hypoxia, poor perfusion, and anaerobic metabolism. Despite a lack of documented efficacy and a level III recommendation from the American Heart Association, SB is widely used during resuscitation events. SB has both theoretical and measurable adverse effects. Excess or poorly timed administration during a CPA may elevate a patient's pH, inducing alkalemia. Despite decades of controversy surrounding use of this drug, the prevalence of SB-induced alk

Treatment of acute chlorine gas inhalation with nebulized sodium bicarbonate.

Three male patients, 19 to 20 years old, were exposed to chlorine gas secondary to a leak in the chlorination system of an indoor pool. All of the patients were symptomatic with cough, chest pain, and shortness of breath. Physical examinations, arterial blood gases, and chest radiographs were normal. All patients were given a nebulized solution of 3.75% sodium bicarbonate which resulted in prompt relief of their symptoms. None of the patients suffered from prolonged symptomatology. This therapy appears to be useful in treating chlorine gas inhalation; however, it cannot be routinely recommende

Reservations and recommendations regarding sodium bicarbonate administration in cardiac arrest.

The American Heart Association's (AHA) revised "Standards and Guidelines for Cardiopulmonary Resuscitation (CPR) and Emergency Cardiac Care (ECC)" recommend that bicarbonate be used ". . . only at the discretion of the physician directing the resuscitation." Reliance upon arterial blood gases is suggested for bicarbonate administration to the patient in cardiac arrest. However, recent literature suggests that arterial blood gases may not reflect the severe cellular acidosis that occurs at the tissue level during cardiac arrest. This article reviews the literature support for this recommendatio
