Characterizing Speech Errors Across Primary Progressive Apraxia of Speech Subtypes.

PURPOSE: Apraxia of speech (AOS) is a motor speech disorder affecting articulatory planning and speech programming. When AOS is the sole manifestation of neurodegeneration, it is termed primary progressive apraxia of speech (PPAOS). Recent work has shown that there are distinct PPAOS subtypes: phonetic, prosodic, and those that do not clearly align with either (mixed). PPAOS subtypes differ with respect to the predominating motor speech difficulties, as well as disease progression and underlying pathology. Because past studies have determined PPAOS subtype based on clinical impression, the goa

Orthognathic speech pathology: impacts of Class III malocclusion on speech.

INTRODUCTION: Patients with dentofacial disharmonies (DFDs) seek orthodontic care and orthognathic surgery to address issues with mastication, esthetics, and speech. Speech distortions are seen 18 times more frequently in Class III DFD patients than the general population, with unclear causality. We hypothesize there are significant differences in spectral properties of stop (/t/ or /k/), fricative (/s/ or /ʃ/), and affricate (/tʃ/) consonants and that severity of Class III disharmony correlates with the degree of speech abnormality. METHODS: To understand how jaw disharmonies influence speec

Reliability of speech naturalness ratings of stuttered speech during treatment.

This study evaluated the reliability with which relatively sophisticated and unsophisticated judges used a 9-point scale to rate the speech naturalness of speech samples from 10 clients in a treatment program for stuttering that employed prolonged speech. Judges rated repeated speech samples from different speakers during various phases of the program. Different groups of sophisticated and unsophisticated judges made ratings at either 15 sec, 30 sec, or 60 sec intervals while listening to the samples. Of the reliability indices, intraclass correlations were significantly higher for sophisticat

Speech Characteristics of Childhood Apraxia of Speech in Hebrew-Speaking Children.

Purpose: Childhood apraxia of speech (CAS) is a motor speech disorder characterized by deficits in speech planning and programming. However, current diagnostic frameworks have been developed primarily from studies of English-speaking children, raising questions regarding their applicability to other languages. This study examined the common and diagnostic speech characteristics of Hebrew-speaking children with CAS as perceived by Israeli speech-language pathologists (SLPs). Method: Two hundred Israeli SLPs with experience diagnosing and/or treating children with CAS completed an online questio

Speech-reading on the lips as a cognitive resource to understand speech in noise.

In challenging acoustic scenarios, speech processing is often linked to listening effort, which can be described as the balance between cognitive demands and motivation to understand speech. In such conditions, people usually rely on several behavioral strategies to support speech understanding and reduce listening effort (e.g., speech-reading behavior). Still, it is not clear what cognitive mechanisms underlie the use of behavioral strategies for listening. We hypothesized that the cognitive and motivational dimensions of listening effort may also drive speech-reading strategies spontaneously

Phonological and speech motor abilities in children with childhood apraxia of speech.

Childhood apraxia of speech (CAS) is a motor speech disorder in which the precision and consistency of speech sounds are impaired due to deficits in motor planning and programming. The literature on CAS suggests that the clinical features of CAS cannot be limited to one level of speech processing and that a more comprehensive understanding of how all levels involved in speech production are part of a complex system is needed. The aim of this study was to investigate the relationship between phonological and speech motor abilities in children with CAS and to determine the extent to which speech

Training on Childhood Apraxia of Speech: Experiences of Speech-Language Pathologists.

PURPOSE: The diagnosis and treatment of childhood apraxia of speech (CAS) remains a clinical challenge for many speech-language pathologists (SLPs). Given this, it is imperative to understand how SLPs learn about CAS during graduate school and throughout their careers. METHOD: In the current study, American Speech-Language-Hearing Association-certified SLPs who practice in the United States and work with young children completed a 32-item online survey. Survey items aimed to understand SLPs' experiences learning about CAS during graduate school and through continuing education (CE). Responden

Three-Dimensional Speech Profiles in Stroke Aphasia and Apraxia of Speech.

PURPOSE: Behaviorally, acquired apraxia of speech (AOS) is a multidimensional syndrome that the experienced clinician recognizes based on impaired articulation and abnormal temporal prosody. We conducted this study to determine the extent to which three core features of AOS-when defined quantitatively-distinguish categorically among aphasia with no or minimal speech sound involvement, aphasia with AOS, and aphasia with phonemic paraphasia (APP). METHOD: The study involved retrospective analysis of 195 participants with stroke-induced aphasia. We used three quantitative measures (phonemic erro

Progressive apraxia of speech as a window into the study of speech planning processes.

We present a 3-year follow-up study of a patient with progressive apraxia of speech (PAoS), aimed at investigating whether the theoretical organization of phonetic encoding is reflected in the progressive disruption of speech. As decreased speech rate was the most striking pattern of disruption during the first 2 years, durational analyses were carried out longitudinally on syllables excised from spontaneous, repetition and reading speech samples. The crucial result of the present study is the demonstration of an effect of syllable frequency on duration: the progressive disruption of articulat

Apraxia of speech: how reliable are speech and language therapists' diagnoses?

OBJECTIVE: To discover how reliably speech and language therapists could diagnose apraxia of speech using their clinical judgement, by measuring whether they were consistent (intra-rater reliability), and whether their diagnoses agreed (inter-rater reliability). DESIGN: Video clips of people with communication difficulties following stroke were rated by four speech and language therapists who were given no definition of apraxia of speech, no training, and no opportunity for conferring. SETTINGS: Videos were made of people following stroke in their homes. Ratings of the videos were carried ou

Efficacy of speech and language therapy for particular speech sounds in children.

At the present time there is a demand for research into the efficacy of speech and language therapy. This study aims to assess the efficacy of therapy provided in South Tees for children who do not use word-initial velar plosives. The study is in progress at the time of writing and it is expected that therapy will be completed by July 1998. Subjects are split randomly into an 'immediate treatment' and a 'deferred treatment' group. All subjects are reassessed after treatment of each group. It is of course possible that analysis of the results will not show a significant difference between pre-

Nonspeech and speech processing skills in patients with aphasia and apraxia of speech.

Nonspeech and speech auditory processing skills as well as internal speech processing skills were assessed among four patients with acquired "pure" apraxia of speech, 10 with acquired aphasia, 10 with aphasia plus apraxia of speech, and 11 neurologically normal adults. Fourteen tasks were administered and performances on 68 variables were examined using both nonparametric and parametric analyses controlling for the effects of advancing age and associated hearing loss. In all cases, the "pure" apractic patients performed as normal subjects. Few differences were noted among the performances of t

From acoustic to linguistic analysis of temporal speech structure: Acousto-linguistic transformation during speech perception using speech quilts.

Speech perception entails the mapping of the acoustic waveform to linguistic representations. For this transformation to succeed, the speech signal needs to be tracked over various temporal windows at high temporal precision in order to decode linguistic units ranging from phonemes (tens of milliseconds) to sentences (seconds). Here, we tested the hypothesis that cortical processing of speech-specific temporal structure is modulated by higher-level linguistic analysis. Using fMRI, we measured BOLD signal changes to 4 s long speech quilts with variable temporal structure (30, 120, 480, 960 ms s

Surgical speech disorders.

Most speech disorders of childhood are treated with speech therapy. However, two conditions, ankyloglossia and velopharyngeal dysfunction, may be amenable to surgical intervention. It is important for surgeons to work with experienced speech language pathologists to diagnose the speech disorder. Children with articulation disorders related to ankyloglossia may benefit from frenuloplasty. Children with velopharyngeal dysfunction should have standardized clinical evaluation and instrumental asseessment of velopharyngeal function. Surgeons should develop a treatment protocol to optimize speech ou
