[Mechanism for producing a neoglottal fricative [h] in tracheoesophageal speech].

BACKGROUND: Over the past 24 years, we have been performing the tracheoesophageal (TE) fistulization for voice restoration following a total laryngectomy. The principle of this technique is to divert the exhaled air through the TE fistula into the hypopharynx, where the thyropharyngeus muscle forms the retropharyngeal prominence on which the neoglottis is located. In TE speech, the electromyographic (EMG) activity of the thyropharyngeus muscle decreases for voiceless positive production. This neoglottic articulatory adjustment plays an important role in opening the neoglottis. It has been prev

Current trends in post-laryngectomy speech rehabilitation.

In recent years there has been considerable interest in exploration of surgical speech rehabilitation (SSR) for the laryngectomee as an alternative to conventional management. The history of the evolution of present pneumatic and electronic methods is traced. The details of the various methods are analyzed including the phonatory neoglottis method reported by Staffieri. Algorithms are offered for the patient who is about to undergo laryngectomy, for the patient who has had laryngectomy in the past, and for the patient facing total pharyngolaryngectomy. The prospects for future developments in

Identification of synthetic vowels based on selected vocal tract area functions.

The purpose of this study was to determine the degree to which synthetic vowel samples based on previously reported vocal tract area functions of eight speakers could be accurately identified by listeners. Vowels were synthesized with a wave-reflection type of vocal tract model coupled to a voice source. A particular vowel was generated by specifying an area function that had been derived from previous magnetic resonance imaging based measurements. The vowel samples were presented to ten listeners in a forced choice paradigm in which they were asked to identify the vowel. Results indicated tha

Neural Correlates of Esophageal Speech: An fMRI Pilot Study.

OBJECTIVES: The esophageal speech is one of the possible alaryngeal voices resulting after total laryngectomy. Its production is made by the regurgitation of the air coming from the esophagus, sonorized through the passage from the walls of the upper esophageal sphincter. The neural correlates of this voice have never been investigated, while the neural control of laryngeal voice has been already documented by different studies. METHODS: Four patients using esophageal speech after total laryngectomy and four healthy controls underwent functional magnetic resonance imaging. The fMRI experiment

Location of the vibratory segment in tracheoesophageal speakers.

The Singer-Blom tracheoesophageal puncture procedure for surgical-prosthetic voice restoration has proved to be a viable option for alaryngeal speech rehabilitation. Following tracheoesophageal puncture, occlusion of the tracheostoma shunts pulmonary air through the Blom-Singer prosthesis into the cervical esophagus. The pulmonary air passing through the cervical esophagus and into the hypopharynx causes a portion of the upper alimentary tract to vibrate in a manner similar to that of the pharyngoesophageal segment during the production of esophageal speech. To study the location and shape of

The INFVo perceptual rating scale for substitution voicing: development and reliability.

In this paper, an experimental study of inter-judge consistency for the different dimensions of a recently proposed new scale for the rating of substitution voices is presented. The IINFVo rating scale tries to score five parameters, namely impression, intelligibility, noise, fluency and voicing. Each parameter is scored between 0 (very good substitution voicing) and 10 (very deviant substitution voicing) on a visual analogue scale. Inter-judge consistencies were measured among semi-professional as well as among professional jury members. The consistencies among semi-professionals, expressed a

[The voice prosthesis. An underutilized procedure in rehabilitation?].

In the last 10 years nearly 4450 patients who had undergone laryngectomy have attended speech therapy as part of a resident rehabilitation programme. Most of them mastered the technique of esophageal speech. The most surprising finding was that only 1.5% achieved speech by way of a tracheo-esophageal fistula or voice prosthesis, whereas this was true of 15% of such patients seen in the first 3 months of 1993.

Improving Hands-Free Speech Rehabilitation in Laryngectomized Patients with a Moldable Adhesive.

OBJECTIVE: This study aims to assess the product performance of a new moldable peristomal adhesive with corresponding heating pad designed to facilitate and improve automatic speaking valve (ASV) fixation for hands-free speech in laryngectomized patients. METHODS: Twenty laryngectomized patients, all regular adhesive users with prior ASV experience, were included. Study-specific questionnaires were used for data collection at baseline and after two weeks of moldable adhesive use. The primary outcome parameters were adhesive lifetime during hands-free speech, use and duration of hands-free spe

Post laryngectomy rehabilitation the case for planned early speech therapy.

This study reflects the efficacy of planned early speech therapy on post laryngectomy rehabilitation. Not only do a larger number of laryngectomees acquire intelligible esophageal speech where therapy is instituted early but also the pace of development and quality of the speech is far superior when compared to those laryngectomees in whom speech therapy was delayed. This paper unequivocally supports the institution of planned early speech therapy in the successful rehabilitation of the laryngectomee. Such therapy can proceed simultaneously with the post operative radiation therapy sans delete

Robust Sequence-to-sequence Voice Conversion for Electrolaryngeal Speech Enhancement in Noisy and Reverberant Conditions.

Electrolaryngeal (EL) speech, an artificial speech produced by an electrolarynx for laryngectomees, lacks essential phonetic features, and differs in temporal structure from normal speech, resulting in poor naturalness and intelligibility. To address this deficiency, sequence-to-sequence (seq2seq) voice conversion (VC) models have been applied in converting EL speech to normal speech (EL2SP), showing some promising performances. However, previous studies mostly focus on converting clean EL speech, thereby restricting the further applicability in real-world scenarios, especially when the EL spe

Perceptions and practices of people with a total laryngectomy during COVID-19 pandemic: A mixed methods analysis.

PURPOSE: People with a total laryngectomy (PTL) confront safety threats related to altered airway anatomy and risk of adverse events is amplified during healthcare crises, as exemplified by COVID-19 pandemic. Understanding these challenges, how they are navigated by PTL, and what resources can be deployed to alleviate risk can improve interprofessional care by speech-language pathologists (SLPs), otolaryngologists, and other professionals. MATERIALS AND METHODS: An online survey was disseminated to PTL in the United States during the COVID-19 pandemic, querying participants about safety conce

Surgical and radiological effects upon the development of speech after total laryngectomy.

The data presented examine the relationship between postlaryngectomy communication method, and the extent of total laryngectomy and the use of radiation therapy. The expectations of speech therapy providers were also examined. The author interviewed 60 laryngectomy patients who were six months to 3 1/2 years postsurgery. Surgeries were grouped into four categories and correlated with communication method. The relationship was statistically significant with the most apparent deterrent effect exhibited only for the most extreme surgical excisions. There was no relationship with the use of radiat
