Development of the Test of Listening Difficulties-Universal and Australian Normative Data in Children and Adults.

PURPOSE: This research was carried out to create a new realistic speech-in-noise test designed to be sensitive to several causes of difficulty understanding speech in noise. The test, conducted under headphones, simulates listening in a typically reverberant classroom. It comprises a frontal target talker speaking high-context sentences and six competing talkers at different apparent locations. METHOD: The first experiment initially measured the degree of context in the sentences by presenting them in writing, with one or two words missing, to adult participants who were asked to guess the mi

Age standardization and time-of-day performance for the Oldenburg Sentence Test (OLSA): results from the population-based Gutenberg Health Study.

PURPOSE: The Oldenburg Sentence Test (OLSA) is a German matrix test designed to determine speech recognition thresholds (SRT). It is widely used for hearing-aids and cochlear implant fitting, but an age-adjusted standard is still lacking. In addition, knowing that the ability to concentrate is an important factor in OLSA performance, we hypothesized that OLSA performance would depend on the time of day it was administered. The aim of this study was to propose an age standardization for the OLSA and to determine its diurnal performance. METHODS: The Gutenberg Health Study is an ongoing populat

[Renewal of a hearing aid is a medical task].

A 67 year-old man was known to have a right-sided hearing loss with a normal discrimination score (DS) and was treated with a hearing aid (HA) in the right ear. An MRI-scan was normal. In 2006 and 2010 the HA was renewed at a private hearing aid clinic. No DS was performed. At the audiology clinic we found a masked DS of 0% and progression of the hearing loss. An MRI-scan revealed a vestibular schwannoma (VS) on the right side. The patient was fitted with an HA in the left ear and MRI-scans every six months. A masked DS could have revealed the VS and correct treatment thus could have been init

Unilateral crosstalk cancellation in normal hearing participants using bilateral bone transducers.

It is possible to psychophysically measure the phase and level of bone conducted sound at the cochleae using two bone transducers (BTs) [Mcleod and Culling (2019). J. Acoust Soc. Am. 146, 3295 - 3301]. The present work uses such measurements to improve masked thresholds by using the phase and level values to create a unilateral crosstalk cancellation system. To avoid changes in the coupling of the BT to the head, testing of tone and speech reception thresholds with and without crosstalk cancellation had to be performed immediately following the measurements without adjustment of the BT. To ach

The relationship between the intelligibility of time-compressed speech and speech in noise in young and elderly listeners.

A conventional measure to determine the ability to understand speech in noisy backgrounds is the so-called speech reception threshold (SRT) for sentences. It yields the signal-to-noise ratio (in dB) for which half of the sentences are correctly perceived. The SRT defines to what degree speech must be audible to a listener in order to become just intelligible. There are indications that elderly listeners have greater difficulty in understanding speech in adverse listening conditions than young listeners. This may be partly due to the differences in hearing sensitivity (presbycusis), hence audib

Multichannel (Ineraid) cochlear implant update.

From April 1984 to May 1991, 49 profoundly deaf patients received implantation of the Ineraid multichannel cochlear implant at the University of Utah. The auditory results of 48 patients indicated improvement in mean pure-tone performance to 500 Hz (32.9 +/- 10.1 dB), 1000 Hz (27.1 +/- 10.0 dB), and 2000 Hz (30.1 +/- 9.8 dB). This group of patients had mean performances of 61.2% on auditory-only environmental sounds (Minimum Auditory Capabilities [MAC] battery), 48.8% on auditory-only CID sentences, and 95.5% on auditory-plus-visual CID sentences. The percutaneous pedestal has been well-tolera

Audiometric results following endolymphatic sac surgery.

Decompression and drainage of the endolymphatic sac is a surgical procedure which was developed to relieve vertigo and hopefully to maintain or improve hearing in patients with Meniere's disease. In this study, 16 medically diagnosed Meniere's disease patients who underwent endolymphatic sac surgery were followed audiometrically for a period of up to three years. Pre- and postoperative audiometric and other symptomatic findings were reported. Results indicated that in the greatest majority of cases hearing remained essentially unchanged or was improved. Poorer hearing was found in 26.3% of the

Aging and contralateral suppression effects on transient evoked otoacoustic emissions.

Transient evoked otoacoustic emissions (TEOAEs) were recorded in 30 normal-hearing subjects to nonlinear clicks while continuous contralateral broadband noise (CBBN) was presented at 40, 50, 60, and 70 dB HL. Thirty subjects between 20 and 79 years were divided system-atically into six-decade age groups, five subjects per group. All subjects in each group had hearing thresholds of 20 dB HL or better for the test frequencies from 0.25 to 8.0 kHz and normal acoustic immittance findings. The results provide evidence that contralateral suppression at varying levels of CBBN is interactive with age.

Bilateral bone-anchored hearing aids (BAHAs): an audiometric evaluation.

OBJECTIVES: Since the technique to implant bone-anchored hearing aids (BAHAs) with the use of osseointegrated implants was developed in 1977, more than 15,000 patients have been fitted with BAHAs worldwide. Although the majority have bilateral hearing loss, they are primarily fitted unilaterally. The main objective of this study was to reveal benefits and drawbacks of bilateral fitting of BAHAs in patients with symmetric or slight asymmetric bone-conduction thresholds. The possible effects were divided into three categories: hearing thresholds, directional hearing, and binaural hearing. STUDY

[Usefulness of TAPS (Polish version) for evaluation of CI children's progress in speech rehabilitation--7 years of observation].

The speech rehabilitation of pre- and perilingually deafened children with cochlear implants it is a very long process. It should be individualized for needs of every young patient. It is possible only after correct evaluation of speech level. There are many tests for this evaluation. Among them same are more known like TAPS test. Test of Auditory Perception of Speech (TAPS) was elaborated in English at Basel University in Swiss. It was translated to Polish at Adam Mickiewicz University and Karol Marcinkowski University School of Medical Sciences in Poznań. The authors present their own 7-year

Speech Understanding and Sound Localization with a New Nonimplantable Wearing Option for Baha.

OBJECTIVE: To measure the audiological benefit of the Baha SoundArc, a recently introduced nonimplantable wearing option for bone conduction sound processor, and to compare it with the known softband wearing option in subjects with normal cochlear function and a purely conductive bilateral hearing loss. METHODS: Both ears of 15 normal hearing subjects were occluded for the time of the measurement, yielding an average unaided threshold of 49 dB HL (0.5 - 4 kHz). Soundfield thresholds, speech understanding in quiet and in noise, and sound localization were measured in unaided conditions and wit

Tinnitus impairs segregation of competing speech in normal-hearing listeners.

Many tinnitus patients report difficulties understanding speech in noise or competing talkers, despite having "normal" hearing in terms of audiometric thresholds. The interference caused by tinnitus is more likely central in origin. Release from informational masking (more central in origin) produced by competing speech may further illuminate central interference due to tinnitus. In the present study, masked speech understanding was measured in normal hearing listeners with or without tinnitus. Speech recognition thresholds were measured for target speech in the presence of multi-talker babble

Psychophysical suppression effects for tonal and speech signals.

This experiment assessed the benefits of suppression and the impact of reduced or absent suppression on speech recognition in noise. Psychophysical suppression was measured in forward masking using tonal maskers and suppressors and band limited noise maskers and suppressors. Subjects were 10 younger and 10 older adults with normal hearing, and 10 older adults with cochlear hearing loss. For younger subjects with normal hearing, suppression measured with noise maskers increased with masker level and was larger at 2.0 kHz than at 0.8 kHz. Less suppression was observed for older than younger subj

Development of Urdu Speech Audiometry Material for the Deaf and Hard of Hearing Community.

PURPOSE: This study aimed to develop standardized Urdu speech materials for assessing speech recognition threshold (SRT) and word recognition score (WRS) for clinical use in Pakistan. METHOD: The development of Urdu speech materials followed four key parameters: phonemic coverage, phonetic dissimilarity, familiarity with the participants, and homogeneity in terms of audibility. Bisyllabic words for SRT measurement and monosyllabic words for WRS measurement were selected. The most familiar 50 spondee words and 50 monosyllabic words were selected for the evaluation of SRT and WRS, respectively,

Perceptual integration between target speech and target-speech reflection reduces masking for target-speech recognition in younger adults and older adults.

This study evaluated unmasking functions of perceptual integration of target speech and simulated target-speech reflection, which were presented by two spatially separated loudspeakers. In both younger-adult listeners with normal hearing and older-adult listeners in the early stages of presbycusis, reducing the time interval between target speech and target-reflection simulation (inter-target interval, ITI) from 64 to 0ms not only progressively enhanced perceptual integration of target-speech signals, but also progressively released target speech from either speech masking or noise masking.
