Proposing a sphenoid bone quality score: a novel concept for transsphenoidal surgery.

Transsphenoid surgery is a common procedure for removing pituitary and other sellar tumors. The quality and density of the sphenoid bone, which serves as the access route to the sellar region, can affect the surgical outcomes and complications. However, there is no standardized method to assess sphenoid bone quality. I propose a sphenoid bone quality score, based on criteria and parameters derived from preoperative imaging techniques. This score could provide information on the bone characteristics and challenges of each case, and help to select the optimal surgical approach, instruments, graf

Changes in the Sphenoid Bone Encountered During the Endoscopic Endonasal Transsphenoidal Approach.

OBJECTIVES: We estimated volume changes in the posterior bony wall of the sphenoid sinus, as well as alterations in nasal function (including olfactory function and subjective symptoms), after sphenoid mucosal repositioning using the endoscopic endonasal transsphenoidal approach (EETSA). METHODS: During 2010 and 2021, 13 patients underwent sphenoid mucosal repositioning during EETSA, while 24 patients (the control group) did not. Pre- and postoperative paranasal sinus computed tomography and the Mimics program were used to evaluate three-dimensional changes in the posterior wall of the spheno

Acute cavernous sinus syndrome from metastasis of lung cancer to sphenoid bone.

Cavernous sinus syndrome is a rare entity in oncology reported only in occasional case reports. Optimal therapy is thus poorly defined with rapidly progressive disease dominating the picture. Management includes prompt diagnosis, attempts at stabilization of cranial nerve function, and aggressive control of central pain syndrome. Here, we report cavernous sinus syndrome secondary to the original squamous cell carcinoma of the lung. With common presenting causes of this syndrome being infection, thrombosis or tumor, it might seem that metastatic tumor would be expected in a patient with a cance

Aneurysmal bone cyst of the sphenoid bone extending into the ethmoid sinus, nasal cavity and orbita in a child.

An aneurysmal bone cyst (ABC) typically involves the long bones of the extremities, thorax, pelvis, or vertebrae. Skull base involvement is rare. We describe the case of a 9-year-old girl with ABC of the skull base. The patient had presented with nasal obstruction and headache over a period of approximately 8 months. The patient had no history of trauma or surgery. Physical and neurological examination findings normal. Magnetic resonance imaging (MRI) showed a multicystic lesion arising from the sphenoid sinus and extending into ethmoid sinus, superior nasal cavity and medial walls of the orbi

Suture-like pattern formation in the pterygospinous bridge of the human sphenoid bone.

The paper presents an anatomical study involving rare variations in the pterygospinous bridges found in Mongolian skulls. These structures extend between the lateral pterygoid plate and the sphenoid spine. Particularly interesting is the division of these bridges into two distinct parts of the similar length. The junctions within these structures resemble morphological patterns characteristic for the plain and zigzag sutures, which articulate the cranial bones.

A rare case of odontogenic keratocyst extending into the sphenoid bone from the maxilla.

BACKGROUND: Odontogenic keratocyst (OKC) is the third most common odontogenic cyst which arises from cell rests of dental lamina, and usually observed in the jaws. Because OKC is noted for its high rate of recurrence, there are various treatment strategies. Here, we present a rare case of OKC which occupied the entire maxillary sinus and pterygoid process of the sphenoid bone extending nearly to the skull base. CASE PRESENTATION: The patient was a 21-year-old male and underwent surgical removal of the cyst using the Caldwell-Luc procedure which in this case extended the surgical approach to t

Characterization and implications of the lingual process of the sphenoid bone: a cadaveric and radiographic study.

BACKGROUND: The surgical significance of the lingual process of the sphenoid bone (LPSB) has not been sufficiently addressed. The purpose of this study was to describe the anatomical details of the LPSB in relation to the quadrangular space. Moreover, the incidence of the LPSB and its correlation with the pneumatization of the sphenoid sinus and the development of the lateral recess of the sphenoid sinus (LRSS) were also evaluated. METHODS: A dissection and exposure of the LPSB and the quadrangular space was performed on 10 cadaveric specimens (20 sides). The incidence, length, and height of

Monostotic fibrous dysplasia of the sphenoid sinus: a serendipitous finding on a bone scan.

A 22-year-old woman had a Tc-99m MDP whole-body scan for low back pain. A focal area of increased activity was seen in the skull base in the region of the sella turcica. A computed tomographic examination showed ground-glass opacification of the sphenoid sinus and bony sclerosis along its walls, characteristic of fibrous dysplasia. Monostotic fibrous dysplasia, the more common form compared with the polyostotic variety, occurs in 70% to 80% of all patients with fibrous dysplasia. Monostotic lesions usually involve the ribs, femur, tibia, cranium, maxilla, and mandible. The frontal and sphenoid

Microsurgical anatomy and the importance of the petrosal process of the sphenoid bone in endonasal surgery.

OBJECTIVE: The petrosal process of the sphenoid bone (PPsb) is a relevant skull base osseous prominence present bilaterally that can be used as a key surgical landmark, especially for identifying the abducens nerve. The authors investigated the surgical anatomy of the PPsb, its relationship with adjacent neurovascular structures, and its practical application in endoscopic endonasal surgery. METHODS: Twenty-one dried skulls were used to analyze the osseous anatomy of the PPsb. A total of 16 fixed silicone-injected postmortem heads were used to expose the PPsb through both endonasal and transc

Aspergillus Sphenoiditis Associated with an Artificial Bone Graft Used for Transsphenoidal Surgery.

Cerebrospinal fluid (CSF) leakage is a major complication during and after transsphenoidal surgery (TSS) for intra- and suprasellar tumors. To prevent postoperative CSF leakage, various surgical techniques have been used, including sellar floor reconstruction with artificial bone grafts. However, some authors have recently reported infections associated with artificial bone grafts. Most cases are associated with bacterial infection, and fungal infection is extremely rare. We present the case of a 53-year-old woman with sphenoiditis caused by Aspergillus infection that developed 8 years after T

Asymmetry of the sphenoid bone and its suitability as a reference for analyzing craniofacial asymmetry.

The purposes of this study were to evaluate the asymmetry of the sphenoid bone and to determine its suitability as a reference for analyzing asymmetry of the skull. Thirty-seven dry skulls from India were divided into group A (n = 18), with a right-left length discrepancy of less than 2 mm for both the external acoustic meatus-frontozygomatic suture and external acoustic meatus-subspinale, and group B (n = 19), with a right-left length discrepancy of more than 2 mm for either of these 2 parameters. The skulls were then examined with regard to the percentage of asymmetry of the sphenoid bone, t

[Morphology of the sphenoid bone in individuals with syndromes which affect the craniofacial complex].

Anatomically, the sphenoid bone can be characterized as the center of the skull. It represents the crossroads where various factors which contribute--each in its own way--to the craniofacial complex, are combined. The morphology of the sphenoid bone is changeable and the opinion that it serves the functional needs for the viability of the individual was formulated in the literature. The findings from the study of 20 patients exhibiting various syndromes that affect the craniofacial complex lead to the conclusion that there is an admirable adaptability and mutual support of the elements which c

Giant cell tumour of the sphenoid bone with coincidental galactorrhoea--a case report.

A case of the rare giant cell tumour involving the sphenoid bone is reported. The usual presentation of these tumours is headache and cranial nerve deficits. This 25-year-old lady presented with oligomenorrhoea and galactorrhoea. Investigations demonstrated an asymptomatic erosive mass lesion in the sphenoid with suprasellar extension and extension into the nasopharynx. The tumour was partially resected via the transphenoidal route. The differential diagnosis and treatment of these lesions are discussed.

Intraosseous lipoma of the sphenoid bone causing optic nerve compression: A case report.

BACKGROUND: Intraosseous lipomas are rare developmental anomalies that present with visual impairment. Due to their infrequency, they often pose diagnostic challenges, particularly in differentiating them from more common optic nerve pathologies. Magnetic resonance imaging (MRI) plays a crucial role in the early identification and management of these lesions. CASE SUMMARY: We present the case of a 41-year-old male with a history of dyslipidemia who developed sudden onset headaches and decreased vision in his left eye. MRI revealed a T1-hyperintense fat-containing lesion within the left anteri

Sphenoid Bone Structure and Its Influence on the Cranium in Syndromic Versus Nonsyndromic Craniosynostosis.

BACKGROUND: Little is known about the detailed growth of the sphenoidal and temporal bones, even though they contribute significantly to the cranial base and cranial fossa skeletons. They also serve to connect the cranial vault with facial structure. This study details their morphologic development in isolated bicoronal synostosis and associated syndromes. METHODS: Eighty-one CT scans were included (nonsyndromic bicoronal synostosis, n = 28; Apert syndrome associated with bicoronal synostosis, n = 19; Crouzon syndrome associated with bicoronal synostosis, n = 8; and controls, n = 26), and mea
