Trunk-mediated spinal cord stimulation: A step toward clinical implementation after spinal cord injury.

In this issue of Med, Agnesi et al., show that residual trunk movements after spinal cord injury can modulate the effects of epidural spinal cord stimulation (SCS) to enable participants to volitionally initiate and control stepping.1 This trunk-mediated interface offers a practical strategy for bringing epidural SCS one step closer to widespread clinical adoption.

Spinal cord stimulation to manage autonomic dysfunction after spinal cord injury: a systematic review.

INTRODUCTION: Spinal cord injury (SCI) results in severe autonomic dysfunction, across cardiovascular, bladder, bowel and sexual domains, negatively affecting quality of life. Recently, there has been growing interest in the application of spinal cord stimulation (SCS) to restore or modulate autonomic functions in people with SCI. This review systematically evaluated the evidence for spinal cord and/or root stimulation, delivered via epidural (eSCS), transcutaneous (tSCS) spinal cord stimulation, or sacral (posterior) anterior root stimulation (SARS/SPARS) - in managing autonomic dysfunction a

Neurophysiological Effect of Transcutaneous Electrical Spinal Cord Stimulation in Chronic Complete Spinal Cord Injury.

BACKGROUND: Transcutaneous electrical spinal cord stimulation combined with activity-based therapy (TESCS-ABT) holds promising potential for motor rehabilitation of spinal cord injury (SCI). However, its effectiveness in individuals with chronic complete SCI remains largely unexplored, despite recent evidence suggesting that many of these individuals exhibit signs of neurological incompleteness. This study was a prospective, single-arm, open-label trial that investigated the neurophysiological effects of TESCS-ABT in chronic complete SCI and assessed whether any changes translate into function

Mapping Spinal Cord Stimulation-Evoked Muscle Responses in Patients With Chronic Spinal Cord Injury.

OBJECTIVES: Epidural spinal cord stimulation (eSCS) has shown promise for restoring some volitional motor control after spinal cord injury (SCI). Maximizing therapeutic response requires effective spatial stimulation generated through careful configuration of anodes and cathodes on the eSCS lead. By exploring the way the spatial distribution of low frequency stimulation affects muscle activation patterns, we investigated the spatial specificity of stimulation-evoked responses for targeted muscle groups for restoration after chronic SCI (cSCI) in participants in the Epidural Stimulation After N

Spinal Cord Stimulation Enhances Microglial Activation in the Spinal Cord of Nerve-Injured Rats.

Microglia can modulate spinal nociceptive transmission. Yet, their role in spinal cord stimulation (SCS)-induced pain inhibition is unclear. Here, we examined how SCS affects microglial activation in the lumbar cord of rats with chronic constriction injury (CCI) of the sciatic nerve. Male rats received conventional SCS (50 Hz, 80% motor threshold, 180 min, 2 sessions/day) or sham stimulation on days 18-20 post-CCI. SCS transiently attenuated the mechanical hypersensitivity in the ipsilateral hind paw and increased OX-42 immunoreactivity in the bilateral dorsal horns. SCS also upregulated the m

Spinal cord stimulation in a patient with spinal epidural lipomatosis.

BACKGROUND AND OBJECTIVE: Spinal cord stimulation is the most commonly used implantable neurostimulation modality for management of pain syndromes. For treatment of lower extremity pain, the spinal cord stimulator lead is typically placed in the thoracic epidural space, at the T10-T12 levels. Typically, satisfactory stimulation can be obtained relatively easily. Anatomical variability in the epidural space, such as epidural scarring, has been reported to prevent successful implantation of spinal cord stimulators. Spinal epidural lipomatosis describes an abnormal overgrowth of adipose tissue in

Spinal cord stimulation: An emerging strategy for chronic pain relief after spinal cord injury.

Chronic pain following a spinal cord injury refers to pain that persists or recurs after the injury. This pain can manifest as burning, stinging, or sensations similar to electric shocks. Recent studies have shown that spinal cord stimulation is an effective way to treat chronic pain after spinal cord injury. The purpose of this review is to introduce the technique of spinal cord stimulation, the clinical manifestations of spinal cord injury, and the role of spinal cord stimulation in the treatment of spinal cord injury. The mechanism and clinical application of spinal cord stimulation in the

REPORT-SCS: minimum reporting standards for spinal cord stimulation studies in spinal cord injury.

Objective.Electrical spinal cord stimulation (SCS) has emerged as a promising therapy for recovery of motor and autonomic dysfunctions following spinal cord injury (SCI). Despite the rise in studies using SCS for SCI complications, there are no standard guidelines for reporting SCS parameters in research publications, making it challenging to compare, interpret or reproduce reported effects across experimental studies.Approach.To develop guidelines for minimum reporting standards for SCS parameters in pre-clinical and clinical SCI research, we gathered an international panel of expert clinicia

Effect of epidural spinal cord stimulation on female sexual function after spinal cord injury.

Sexual dysfunction is a common consequence for women with spinal cord injury (SCI); however, current treatments are ineffective, especially in the under-prioritized population of women with SCI. This case-series, a secondary analysis of the Epidural Stimulation After Neurologic Damage (E-STAND) clinical trial aimed to investigate the effect of epidural spinal cord stimulation (ESCS) on sexual function and distress in women with SCI. Three females, with chronic, thoracic, sensorimotor complete SCI received daily (24 h/day) tonic ESCS for 13 months. Questionnaires, including the Female Sexual Fu

Spinal cord stimulation for neuropathic pain following traumatic spinal cord injury: a case report.

INTRODUCTION: Neuropathic pain is a common complication of spinal cord injury (SCI), and is notoriously difficult to adequately treat. Gunshot wounds (GSW) near the spinal cord may cause intractable chronic pain through spinal/nerve root transection, or reactive tissue formation resulting in nerve root compression from retained bullet fragments (RBF). CASE PRESENTATION: This case report describes a 30-year-old man with a T12 AIS B incomplete spinal cord injury with paraplegia secondary to multiple GSW who presented with severe bilateral lower extremity dysesthesias and muscle spasms. High-frequency spinal cord stimulation in a subacute animal model of spinal cord injury.

High-frequency spinal cord stimulation (HF-SCS) applied at the T2 spinal level results in physiologic activation of the inspiratory muscles in C2 spinal-sectioned dogs. Although the bulbo-spinal fibers were cut, they likely survived the duration of acute experiments, and inspiratory muscle activation may have involved stimulation of these fibers. In two anesthetized, C2 paralyzed, intubated, and mechanically ventilated dogs, HF-SCS (300 Hz) was applied at the T2 level. The effectiveness of HF-SCS in generating inspired volume (V) and negative airway pressures (P) was evaluated over a period of

Normalization of Blood Pressure With Spinal Cord Epidural Stimulation After Severe Spinal Cord Injury.

Chronic low blood pressure and orthostatic hypotension remain challenging clinical issues after severe spinal cord injury (SCI), affecting health, rehabilitation, and quality of life. We previously reported that targeted lumbosacral spinal cord epidural stimulation (scES) could promote stand and step functions and restore voluntary movement in patients with chronic motor complete SCI. This study addresses the effects of targeted scES for cardiovascular function (CV-scES) in individuals with severe SCI who suffer from chronic hypotension. We tested the hypothesis that CV-scES can increase resti

Spinal Epidural Abscess During a Spinal Cord Stimulation Trial in Complete Spinal Cord Injury: A Case Report.

Chronic pain after spinal cord injury (SCI) is often refractory to medication, and spinal cord stimulation (SCS) may provide relief. We report a 48-year-old man with complete thoracic SCI who underwent an SCS trial for severe bilateral lower-extremity-pain. Although pain improved, low-grade fever, elevated inflammatory markers, and purulent drainage developed on day 7. Magnetic resonance imaging revealed a spinal epidural abscess (SEA). SCS leads were removed, and cultures grew methicillin-resistant Staphylococcus aureus. As the abscess was small without significant compression, conservative t

Stimulation-associated motor and functional improvement after epidural spinal cord stimulation in chronic spinal cord injury secondary to spinal ependymoma: A case report.

CONTEXT: Chronic spinal cord injury (SCI) secondary to spinal tumors may cause persistent motor and sensory dysfunction despite surgical treatment and rehabilitation. Ependymomas are the most common intramedullary spinal tumors in adults, but long-standing neurological deficits after tumor surgery are difficult to treat. FINDINGS: We report a 51-year-old man with chronic SCI after thoracic spinal ependymoma surgery performed in 2000. He had persistent right-predominant lower-extremity weakness, sensory loss below T10, limited walking endurance, and cane-dependent ambulation. EES was performed

Electrical epidural stimulation of the cervical spinal cord: implications for spinal respiratory neuroplasticity after spinal cord injury.

Traumatic cervical spinal cord injury (cSCI) can lead to damage of bulbospinal pathways to the respiratory motor nuclei and consequent life-threatening respiratory insufficiency due to respiratory muscle paralysis/paresis. Reports of electrical epidural stimulation (EES) of the lumbosacral spinal cord to enable locomotor function after SCI are encouraging, with some evidence of facilitating neural plasticity. Here, we detail the development and success of EES in recovering locomotor function, with consideration of stimulation parameters and safety measures to develop effective EES protocols. E

Spinal Cord Stimulation 50 Years Later: Clinical Outcomes of Spinal Cord Stimulation Based on Randomized Clinical Trials-A Systematic Review.

To assess the efficacy of spinal cord stimulation (SCS) for each indication, one must critically assess each specific clinical outcome to identify outcomes that benefit from SCS therapy. To date, a comprehensive review of clinically relevant outcome-specific evidence regarding SCS has not been published. We aimed to assess all randomized controlled trials from the world literature for the purpose of evaluating the clinical outcome-specific efficacy of SCS for the following outcomes: perceived pain relief or change pain score, quality of life, functional status, psychological impact, analgesic

Transcutaneous spinal cord stimulation of the cervical cord modulates lumbar networks.

It has been established that coordinated arm and leg (A&L) cycling facilitates corticospinal drive and modulation of cervico-lumbar connectivity and ultimately improves overground walking in people with incomplete spinal cord injury or stroke. This study examined the effect of noninvasive transcutaneous spinal cord stimulation (tSCS) on the modulation of cervico-lumbar connectivity. Thirteen neurologically intact adults participated in the study. The excitability of the Hoffmann (H) reflex elicited in the soleus muscle was examined under multiple conditions involving either the arms held in a

Spinal Cord Stimulation Attenuates Below-Level Mechanical Hypersensitivity in Rats After Thoracic Spinal Cord Injury.

OBJECTIVES: The burden of pain after spinal cord injury (SCI), which may occur above, at, or below injury level, is high worldwide. Spinal cord stimulation (SCS) is an important neuromodulation pain therapy, but its efficacy in SCI pain remains unclear. In SCI rats, we tested whether conventional SCS (50 Hz, 80% motor threshold [MoT]) and 1200 Hz, low-intensity SCS (40% MoT) inhibit hind paw mechanical hypersensitivity, and whether conventional SCS attenuates evoked responses of wide-dynamic range (WDR) neurons in lumbar spinal cord. MATERIALS AND METHODS: Male rats underwent a moderate contu

Transcutaneous Spinal Cord Stimulation Induces Temporary Attenuation of Spasticity in Individuals with Spinal Cord Injury.

Epidural spinal cord stimulation (SCS) is currently regarded as a breakthrough procedure for enabling movement after spinal cord injury (SCI), yet one of its original applications was for spinal spasticity. An emergent method that activates similar target neural structures non-invasively is transcutaneous SCS. Its clinical value for spasticity control would depend on inducing carry-over effects, because the surface-electrode-based approach cannot be applied chronically. We evaluated single-session effects of transcutaneous lumbar SCS in 12 individuals with SCI by a test-battery approach, befor
