Spinal cord involvement in adult-onset metabolic and genetic diseases.

In adulthood, spinal cord MRI abnormalities such as T2-weighted hyperintensities and atrophy are commonly associated with a large variety of causes (inflammation, infections, neoplasms, vascular and spondylotic diseases). Occasionally, they can be due to rare metabolic or genetic diseases, in which the spinal cord involvement can be a prominent or even predominant feature, or a secondary one. This review focuses on these rare diseases and associated spinal cord abnormalities, which can provide important but over-ridden clues for the diagnosis. The review was based on a PubMed search (search te

Risk management in posterior spinal endoscopic surgery in lumbar diseases.

BACKGROUND: Minimally invasive posterior spinal endoscopic surgery has increased in popularity. However, a steep learning curve may result in a high frequency of complications. Additionally, device failure may occur during routine use. We retrospectively investigated the incidence of such safety issues in a population of patients undergoing endoscopic spinal surgery at our institution. PATIENTS/MATERIALS AND METHODS: A total of 611 cases were included. Underlying diseases that required endoscopic surgery were lumbar disc herniation in 382 patients, lumbar spinal stenosis in 123 patients, lumb

Thoracoscopic surgical approaches for treating various thoracic spinal region diseases.

AIM: Minimally invasive surgery is currently a goal for surgical intervention in the spine. The effectiveness of endoscopic thoracic spine surgery and technological improvements are two factors that are always under consideration in the practice of spinal surgery. MATERIAL AND METHODS: We present twenty-five patients whose thoracoscopic spinal surgeries were performed between 2002 and 2008 for the treatment of various spinal diseases. Eleven patients with thoracic disc herniation, five patients with traumatic thoracic spinal compressive fracture, six patients with metastatic thoracic spinal t

Clinical approach and lesion localization in patients with spinal diseases.

Back or neck pain, reluctance to move or climb stairs, inability to walk, and abnormal gait or incoordination are frequent complaints associated with vertebral/spinal disease in dogs and cats. This article discusses the clinical approach and principles behind lesion localization in diseases affecting the vertebral column and its content (ie, the spinal cord and the cauda equina).

Dysfunctional elimination syndrome in children without obvious spinal cord diseases.

Elimination dysfunction is a functional disease that encompasses the urinary and GI tracts. Because the presentation is varied and ED is a disease of exclusion, a thorough history and physical examination are mandatory. Noninvasive management that includes the "fundamental four" and continuous engagement and encouragement of the patient and family are the pillars of treatment. For patients who are refractory to this approach, new research in children-directed biofeedback research shows promise.

The intervertebral disk: a landmark for spinal diseases in children.

The aim of this paper is to describe the diagnostic value of the different radiological modifications of the intervertebral disk in children. Usual disk modifications include: disk-space narrowing; disk space enlargement; intervertebral disk calcifications; intervertebral disc prolapse; and signal modification in MRI. The gamuts (which are provided in the paper) of these different images must take into account the association with bone changes and the possibility of several disks' involvement. Calcifications are usually related to benign disorders.

Diseases of the spinal cord in human immunodeficiency virus infection.

The most common disease of the spinal cord in human immunodeficiency virus (HIV) infection is vacuolar myelopathy. Pathology studies have demonstrated that vacuolization in the thoracic spinal cord is present in more than a third of patients with AIDS. The disease, however, manifests clinically only when the vacuolization in the spinal cord has become severe, with prominent myelin loss in the lateral and posterior columns. Vacuolar myelopathy presents usually with slowly progressing spastic paraparesis, accompanied by loss of vibratory and position sense and urinary frequency and urgency. In m

[Spinal diseases: vertebral complaints in relation to site and age].

Disorders of the vertebral column are one of the most common causes of pain in the human being. The nosological classification of various disorders is carried out according to many different criteria, e.g. topical pain descriptions such as cervical syndrome and lumbago in particular, but more often also according to purely speculative factors such as intervertebral disc lesions, muscular rheumatism etc. The analysis of each single structural disturbance which could come into question is a primary function of the anamnesis, and in particular of the clinical examination, which must seek, respect

Magnetic resonance imaging in the diagnosis of spinal cord diseases.

Experience with magnetic resonance imaging in 22 patients with diseases of the spinal cord is reported. Important additional diagnostic information as compared to conventional neuroradiological techniques (myelography, spinal CT) was gained especially in cases of hydrosyringomyelia, intraspinal tumour and multiple sclerosis. It is suggested that magnetic resonance imaging may become the method of choice in the diagnosis of structural spinal cord diseases.

[Osteitis pubis and spinal diseases : Differential diagnosis of groin pain].

Aseptic osteitis pubis can be a possible cause of groin pain in young physically active patients. As a correlation with femoroacetabular impingement has been demonstrated, this pathological condition should also be considered as a possible cause. In older patients aseptic osteitis pubis is usually a degenerative alteration of the symphysis, which can also lead to groin pain. It is important to distinguish this from septic osteitis pubis, which is often associated with infections in the region of the true pelvis or previous surgery in this area. Vertebrogenic causes of groin pain are attributab

[Pathogen-induced diseases of the spinal cord and adjacent structures].

Pathogen-induced myelitis is an inflammatory disease of the spinal cord that can be caused by various pathogens including viruses, bacteria, fungi or parasites. The most frequent viral pathogens include herpes and enteroviruses, while bacterial myelitis can be caused by, e.g., Mycobacterium tuberculosis, Borrelia burgdorferi and Treponema pallidum. Fungi such as Candida and Aspergillus and parasites such as Toxoplasma gondii and schistosomes can also cause myelitis. The main symptom is subacute paraplegia with motor, sensory and autonomic deficits to varying degrees, often accompanied by fever

What's around the spinal cord? Imaging features of extramedullary diseases.

Diagnosis of extramedullary spinal diseases is often complex, firstly requiring a good anatomic knowledge for a precise localization of pathologies. The spinal canal, a tubular space delimited by vertebral bodies and neural arches, contains the spinal cord, nerve roots and cauda equina. Neural structures are surrounded and supported, from outer to inner, by meninges: dura, arachnoid and pia mater; meningeal layers divide extramedullary spaces in epidural, subdural and subarachnoid. Extramedullary diseases may be broadly classified in degenerative, neoplastic, traumatic, infective or miscellane

In vivo imaging of spinal cord atrophy in neuroinflammatory diseases.

OBJECTIVE: Spinal cord atrophy is prominent in chronic progressive neurologic diseases such as human T-cell lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and multiple sclerosis (MS). Here we compared the spinal cord cross-sectional area (SCCSA) in HAM/TSP and MS patients to that of healthy volunteers (HVs). METHODS: SCCSA and clinical disability scores were measured in 18 HAM/TSP patients, 4 asymptomatic carriers (ACs) of HTLV-1, 18 MS patients, and 10 HVs from a 3T magnetic resonance imaging. SCCSA measured in patients and ACs were compared t

[Imaging of demyelinating and neoplastic diseases of the spinal cord].

The clinical symptoms of myelopathy are variable and non-specific. Demyelinating as well as neoplastic spinal cord diseases can cause paresthesia, progressive sensomotoric deficits and bowel and bladder dysfunction. Imaging of the spine, especially with magnetic resonance imaging (MRI), is an essential component in the diagnostic assessment of myelopathy and makes a substantial contribution to achieving the correct diagnosis. Although intramedullary neoplasms are far less common than demyelinating spinal cord diseases, radiologists should be familiar with the three most common entities, astroc

[Specific features of urethral diseases in spinal cord injury patients].

This article discusses all of the urethral diseases observed in patients with neurogenic bladder. The frequency of these disorders has decreased considerably with progress in intermittent catheterization and bladder retraining. However, urologists may occasionally be required to manage an urethral diverticulum, or an ulcer of the urethra over an indwelling catheter and must be familiar with the specific features in this setting.

[Therapeutic use of "biologics" in inflammatory joint and spinal diseases].

At present, two principles of biologic treatment for rheumatoid arthritis and other chronic inflammatory joint and spine diseases (juvenile polyarticular idiopathic arthritis, spondarthritides) are available for use in clinical practice. These are the TNF-alpha antagonists etanercept and infliximab as well as the human recombinant IL-1 receptor antagonist anakinra. All three biologics have significant short and long-term therapeutic effects on clinical and humoral inflammatory activity compared to placebo treatment in controlled clinical trials and even radiological progression of rheumatoid a

[Spinal diseases in the nursing profession--a cross-sectional study].

A cross-sectional study on the profession-specific prevalence rates of back pain symptoms was conducted including N = 3332 nurses as the exposed group and N = 1720 office clerks as a control group. Only participants who had never changed from an exposed to an unexposed profession or vice versa, were included for the results presented here. Severity of symptoms was assessed, dividing into low back pain on the one hand, and lumbago-sciatica/sciatica on the other; frequency of pain episodes was measured using lifetime- and point-prevalence, in all these four combinations of severity and frequency

[Lumbar spinal stenosis in systemic diseases and in hereditary syndromes].

In the general category of congenital spinal stenosis, there is a group of patients with particular features: patients suffering from a hereditary or systemic disease with congenital narrowing of the spinal canal. Evaluation of their neurological symptoms is influenced by the peculiarities of the underlying disease. The tendency toward paralysis and paraplegia is much higher in those patients. Attention must especially be given to technical details when operating on achondroplastic dwarfs. Early, most extensive surgery is recommended. We report our experience between 1987 and 1992 with operati

[Verografin epidurography in the diagnosis of spinal and spondylogenic diseases].

In order to elaborate an epidurographic technique, the water-soluble contrast substance (verografin), oil solution of iodine, iodolipol and air were used in experimental studies on animals and in cadavers. The studies demonstrated that oil and gaslike contrast substances penetrate into the vein and create a threat of embolia formation. For epidurography the most acceptable are water-soluble contrast substances. The method of an ascending verografin epidurography was elaborated and used in clinical practice. Some indications and contraindications for epidurography are outlined. A group of 396 p
